위암환자에서 임상적 양상 및 치료에 따른 혈청 CEA치의 변화에 관한 연구 (Serum CEA Related to Clinical Status and Outcome of Treatment in Stomach Cancer)

  • Serum carcinoembryonic antigen (CEA) levels were measured by radioimmunoassay in 82 patients with stomach cancer and were related to their clinical status and to the response to therapeutic modalities. Serum CEA in patients over 40 years of age was higher (p < 0.05) than that of younger age group and was higher (p 앞 0.0025) in patients with advanced stages (IIII and IV) than in patients with earlier stages (EGC and stage II). No significant differences in serum CEA levels were seen between the tumor cell types. In 9 patients who underwent successful radical resection of the cancer, serum CEA decreased significantly (p < 0.05) after surgery, whereas the changes in serum CEA levels after palliative surgery in other 7 patients were statistically insignificant (p > 0.01). Eleven patients who were given chemotherapy were followed up until death, the changes in their serum CEA levels were significant reverse-correlation (r=-0.72) with duration of survival.

  • The serum CEA levels were measured by radioimmunoassay technique in 15 patients with chronic renal failure, who were not treated with hemodialysis, in 39 patients under hemodialysis and in 23 patients who received renal transplantation. The results were compared with those in 65 normal adults and the following results were obtained. 1) Serum CEA concentrations.in 65 normal adults were in the range of 1.0 to 4.3ng/ml with a mean value of $1.6{\pm}0.66ng/ml$. 2) Serum CEA concentrations in 15 chronic renal failure patients who were not treated with hemodialysis, were in the range of 0.3 to 8.3ng/ml with a mean value of $3.6{\pm}2.10ng/ml$ which was significantly higher than those of normal controls(P<0.001). 3) Serum CEA concentrations in 39 chronic renal failure patients under hemodialysis were also much higher than normal controls(P<0.001), but not significantly different from those of the patients who were not under hemodialysis(P>0.05), 4) In 23 patients who received renal transplantation, serum CEA levels were significantly higher than normal controls(P<0.001), but not significantly different from those of chronic renal failure patients.

Clinical Utility of Haptoglobin in Combination with CEA, NSE and CYFRA21-1 for Diagnosis of Lung Cancer

  • Purpose: To investigate the clinical value in lung cancer of a combination of four serum tumor markers, haptoglobin (Hp), carcinoembryonic antigen (CEA), neuron specific enolase (NSE) as well as the cytokeratin 19 fragment (CYFRA21-1). Materials and Methods: Serum Hp (with immune-turbidimetric method), CEA, NSE, CYFRA21-1 (with chemiluminescence method) level were assessed in 193 patients with lung cancer, 87 patients with benign lung disease and 150 healthy controls. Differences of expression were compared among groups, and joint effects of these tumor markers for the diagnosis of lung cancer were analyzed. Results: Serum tumor marker levels in patients with lung cancer were obviously higher than those with benign lung disease and normal controls (p<0.01). The sensitivities of Hp, CEA, NSE and CYFRA21-1 were 43.5%, 40.9%, 23.3% and 41.5%, with specificities of 90.7%, 99.2%, 97.9% and 97.9%. Four tumor markers combined together could produce a positive detection rate of 85.0%, significantly higher than that of any single test. With squamous carcinomas, the positive detection rates with Hp and CYFRA21-1 were higher than that of other markers. In the adenocarcinoma case, the positive detection rate of CEA was higher than that of other markers. For small cell carcinomas, the positive detection rate of NSE was highest. The area under receiver operating characteristic curve ($AUC^{ROC}$) of Hp in squamous carcinoma (0.805) was higher than in adenocarcinoma (0.664) and small cell carcinoma (0.665). Conclusions: Hp can be used as a new serum tumor marker for lung cancer. Combination detection of Hp, CEA, NSE and CYFRA21-1 could significantly improve the sensitivity and specificity in diagnosis of lung cancer, and could be useful for pathological typing.

Application of Tumor Markers SCC-Ag, CEA, and TPA in Patients with Cervical Precancerous Lesions

  • Background: To determine the potential clinical utility of tumor markers CEA, TPA, and SCC-Ag for early detection of cervical precancerous lesions. Materials and Methods: A case-control study was carried out on 120 women (46 patients with histologically confirmed cervical precancerous lesions and 74 healthy controls). The significance of serum selected tumor markers in early detection of cervical intraepithelial neoplasia (CIN) were assessed. Results: Of the case group, the rates of CIN I, II, III, was 69.6%, 23.9%, and 6.5%, respectively. According to the manufacturer's cut-off values of 2ng/ml, 5ng/ml, and 70 U/ml for SCC-Ag, CEA and TPA tests, in that order, SCC-Ag test had a sensitivity of 13%, but CEA and TPA tests could not distinguish between case and control groups. The diagnostic sensitivities were highest at cut-off values of 0.55 ng/ml for SCC-Ag, 2.6ng/ml for CEA, and 25.5 U/ml for TPA which were 93%, 61%, and 50%, respectively. However, the area under the receiver operating characteristic curve was the largest for SCC-Ag (0.95 vs. 0.61 and 0.60 for CEA and TPA, respectively). Moreover, there was a highly significant direct correlation between SCC-Ag concentration and the degree of cervical precancerous lesions (r=0.847, p<0.001). Conclusions: The new cutoff of 0.5 for SCC-Ag test might be useful as a tumor marker in Iranian patients with CIN and it needs to be more evaluated by studies with larger populationa.

악성 종양 표지자가 상승된 만성 흉수를 동반한 종격동 기형종 1예 (A Case of Mediastinal Teratoma Associated with Elevated Tumor Marker in Chronic Empyema)

  • 기형종은 주로 전 종격동에서 발생하는 양성 종양으로 대부분 무증상이며 검사에서 우연히 발견된다. 드물게 기형종이 파열하여 여러 합병증을 일으키는 것으로 알려져 있으며 합병증은 급성 증상을 동반한다. 저자들은 급성 증상없이 만성적인 경과를 보인 기형종 파열 1예를 경험 하였다. 환자는 왼쪽 폐에 다량의 흉수가 있었는데 배액관 삽입으로 증상이 호전되었으며 지속적으로 흉수의 carcinoembryonic antigen (CEA) 및 carbohydrate antigen 19-9 (CA19-9)이 증가되어서 악성 흉수를 동반하는 종양과 구별이 어려웠다. 본 증례는 이차 감염에 의한 농흉을 치료하기 위해 흉강경으로 확인하기 전까지는 확진이 어려웠고, 매우 높은 흉수 내 악성 종양 표지자 수치를 보인 증례가 국내에는 보고되지 않았으므로 유사 증례의 문헌 고찰과 함께 보고한다.

국산 항 CEA 항체의 I-131, Tc-99m 표지법 확립 및 면역학적 특성 분석 (Establishment of I-131, Tc-99m Labeling Methods to In-house Anti-CEA Antibodies and Evaluation of the Immunological Characteristics)

  • 서울대학교 의과대학에서 개발한 항 CEA항체에 대한 방사성옥소 및 $^{99m}Tc$의 표지법을 개발하고 이런 표지항체의 면역학적 특성을 분석하였다. 1) 8가지의 항 CEA의 단세포군 항체중 CEA-79항체와 CEA-92항체가 가장 좋은 면역학적 특성을 보였다. 표지항체의 면역반응성은 60% 정도이었고, 항원/항체 친화상수는 $1\sim2\times10^9l/m$ 이었다. 2) CEA-79항체는 chloramine-T법으로, CEA-92항체는 iodogen 법으로 방사성옥소를 표지 하는 것이 가장 좋은 표지효율 및 면역 반응성을 보였다. 3) CEA-79항체는 0.5 mCi/mg에서부터 100mCi/mg까지 비방사능을 변화시켰을 때 표지효율이나 면역 반응성에 유의한 차이는 발견할 수 없었다. 4) Glucarate를 이용한 pretargeting transchelation법으로 CEA-79항체를 $^{99m}Tc$로 표지하는 조건을 확립하였다. 표지효율은 $60\sim70%$ 이었다. 본 실험을 통하여 얻은 적절한 조건의 $^{131}I$$^{99m}Tc$표지방법의 확립은 앞으로 방사면역신티그라피와 방사면역치료법에 유용하게 사용될 수 있을 것으로 기대된다.

Clinical Application of Serum Tumor Associated Material (TAM) from Non-small Cell Lung Cancer Patients

  • Objective: To explore the associations of serum tumor associated material (TAM) with other common tumor markers like carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125), carbohydrate antigen19-9 (CA19-9) and its clinical application in non-small cell lung cancer (NSCLC) patients. Methods: A total of 87 patients were enrolled into this study, all with histologically or cytologically confirmed NSCLC. With the method of chemical colorimetry, the level of TAM was determined and compared, while chemiluminescence was used to measure the levels of common tumor markers. Results: The level of TAM decreased after chemotherapy compared with before chemotherapy when CT or MRI scans showed disease control. Furthermore, it increased when disease progessed and there was no statistically significant difference in monitoring of TAM and common tumor markers (P>0.05). Conclusions: Detecting TAM in NSCLC patients has a higher sensitivity and specificity, so it can be used as an indicator for clinical monitoring of lung cancer chemotherapy.

Clinical Application of Serum Tumor Abnormal Protein from Patients with Gastric Cancer

  • Background: To verify whether serum tumor abnormal protein (TAP) would correlate with the responsiveness of palliative chemotherapy in patients with advanced gastric cancer, and the variation of conventional serum tumor markers e.g., carcinoembryonic antigen (CEA), antigen 125 (CA125),carbohydrate antigen19-9 (CA19-9) of adjuvant chemotherapy in patients with early gastric cancer. Materials and Methods: Patients with histologically confirmed gastric cancer and treated with chemotherapy were enrolled into this study. TAP values of these patients were determined by detecting abnormal sugar chain glycoprotein in serum, combined with the area of agglomerated particles. For patients with advanced gastric cancer, responsiveness of palliative chemotherapy was compared with variation of TAP and the relation between variation of TAP and tumor markers in patients with early gastric cancer was analyzed. Results: Totally 82 gastric cancer patients were enrolled into this study. The value of TAP is more closely related to responsiveness of palliative chemotherapy for patients with advanced gastric cancer. The correlation between TAP and responsiveness to palliative chemotherapy is stronger than the correlation between several conventional serum tumor markers (CEA, CA125 and CA199). The variation of TAP was also positively correlated with the trend of CA125 in adjuvant chemotherapy. Conclusions: TAP is sensitive in monitoring the responsiveness to palliative chemotherapy in patients with advanced gastric cancer. But this result should be confirmed by randomized clinical trials for patients with gastric cancer.

Comparison of Serum Tumor Associated Material (TAM) with Conventional Biomarkers in Cancer Patients

  • Objective: To compare expression level of serum tumor associated materials (TAM) with several conventional serum tumor biomarkers, eg., carcinoembryonic antigen (CEA), carbohydrate antigen19-9 (CA19-9), carbohydrate antigen 15-3 (CA15-3), alpha-fetoprotein(AFP), in selected solid tumors. Methods: Patients diagnosed histologically or cytologically with liver, breast, esophageal, gastric, colorectal or pancreatic cancers were enrolled into this study. After diagnosis, the level of TAM was determined by chemical colorimetry, and levels of conventional tumor markers was measured by chemiluminescence methods. Results: A total of 560 patients were enrolled into this study. No statistically significant difference was detected in TAM and the above mentioned tumor biomarkers in terms of their positivity and negativity ( P>0. 05). Conclusions: Detection of TAM in liver, breast, esophageal, gastric, colorectal, and pancreatic cancer patients demonstrates a good accordance with CEA, CA199, CA153, and AFP, thus suggesting that further study is warranted to verify whether TAM could be a surrogate for these conventional biomarkers.

Application of MMP-7 and MMP-10 in Assisting the Diagnosis of Malignant Pleural Effusion

  • Background: Matrix metalloproteinases (MMP) are proteolytic enzymes that are essentially involved in turnover of the extracellular matrix (ECM). The aim was to investigate the diagnostic value of MMP-7 and MMP-10 as tumor markers in pleural effusion (PE) and evaluate the value of combining MMP-7, MMP-10 and carcinoembryonic antigen (CEA) assays as diagnostic aids for malignant cells. Materials and Methods: A total of 179 patients with PE (87 malignant and 92 benign) were included in this study. The levels of MMP-7 and MMP-10 were measured using ELISA. Results: Values for MMP-7 and MMP-10 were significantly higher in malignant PE than those in benign PE (P<0.01). Among all variables evaluated, logistic regression found that MMP-7 and MMP-10 were significantly correlated with the presence of malignant disease (P<0.01). Analysis of receiver operating characteristics (ROC) curves showed that the area under the curve of MMP-10 (0.806) was significantly larger than that of MMP-7 (0.771) and CEA (0.789) (P<0.01). With parallel interpretation, the combination of MMP-10 and CEA achieved the higher sensitivity of 94.6%. The combination of MMP-7 and CEA in serial interpretation was able to boost the specificity to 95.7%. The combination of MMP-7, MMP-10 and CEA produced better sensitivity, specificity, PPV and NPV than MMP-7 and MMP-10 alone. Conclusion: MMP-7 and MMP-10 in PE may represent helpful adjuncts to conventional diagnostic tools in ruling out malignancy as a probable diagnosis, thus guiding the selection of patients who might benefit from further invasive procedures.
