행정구역상 읍/면/동 단위의 소지역 (small area)별로 질병위험의 차이에 대한 분석을 위해, 2005년 기준 서울 행정동을 기준으로 2005년부터 2008년까지 질병, 사고, 암 사망자료에 대한 표준화 사망률 (SMR; standardized mortality rate)을 고려하였다. 소지역 단위로 질병사망률을 직접 추정하는 것은 소지역 내 표본수가 작아, 개발 소지역 단위에서의 직접 계산된 SMR은 그 추정치의 정도 (precision) 확보가 어려운 문제점이 발생한다. 따라서, 본 연구에서는 각 소지역간 효과 추정을 위해 공간적 상관성 (spatial correlation)을 가지는 다단계 일반화 선형모형 (HGLM; hierarchical generalized linear models)을 고려하였다. 이를 통해, 서울지역 동별 주요 사망원인에 따른 공변량의 효과 및 추정된 SMR을 근거로 질병지도 결과를 제시하였다.
Purpose: The purpose of this study was to develop a Clinical Ladder System (CLS) model for staff nurses working in inpatient units of tertiary care hospitals in Korea. Methods: The study was carried out in 2 steps. First, a nursing competence evaluation scale was developed. Second, evaluation of the nursing competences, qualifications, and professional activities of 230 nurses from five tertiary care hospitals was done by 49 head nurses between Feb. and Dec., 2014. Nurses were selected by head nurses according to their clinical experience and expected behavioral characteristics at each level of the ladder. Results: A nursing competence scale was developed consisting of 5 subcategories (clinical practice, ethical practice, education, leadership, and professional development) and 11 elements, and 5 levels of behavioral indicators for each element. Cronbach's alphas for the entire tool and subcategories were over .853 and stability of the scale was confirmed. There were significant differences in nursing competence according to the 5 levels of the ladder. Conclusion: The findings indicate that the proposed CLS model with a standard score for nursing competence, recommended or obligatory criteria for qualifications and professional activities provides a good tool for developing nurses' competences and retaining excellent nurses in clinical practice.
지난 10년 동안 암 관리법에 의한 국가 정책에 따라 일본에서는 호스피스 및 완화의료가 급속히 발전해왔다. 완화의료 시설 및 입원 환자의 호스피스, 병원의 완화 의료팀, 가정 호스피스 기능을 갖춘 진료소의 수는 꾸준히 증가하고 있으며 국가 협회로부터 완화의료 전문가로 공인된 의사, 간호사, 약사의 수도 증가하고 있다. 학부, 대학원 및 계속적인 의학 교육을 통해서 교육프로그램 및 교육의 기회를 표준화하고 보급하기 위한 공동 노력이 이루어졌다. 일본의 연구 활동은 말기 섬망, 말기 탈수증, 완화적 진정, 임종 환자의 의료, 예후, 의사 소통, 정신 종양학, 지역적인 완화의료 프로그램의 분야에서 현저하게 기여하였다. 이 보고서에서는 일본에서의 주요 완화의료 환경, 특성, 국가 협회, 교육, 완화의료 연구에 초점을 맞추었다.
Objective : Hypernatremia is a common complication encountered during the treatment of neurocritically ill patients. However, it is unclear whether clinical outcomes correlate with the severity of hypernatremia in such patients. Therefore, we investigated the impact of hypernatremia on mortality of these patients, depending on the degree of hypernatremia. Methods : Among neurosurgical patients admitted to the intensive care unit (ICU) in a tertiary hospital from January 2013 to December 2019, patients who were hospitalized in the ICU for more than 5 days and whose serum sodium levels were obtained during ICU admission were included. Hypernatremia was defined as the highest serum sodium level exceeding 150 mEq/L observed. We classified the patients into four subgroups according to the severity of hypernatremia and performed propensity score matching analysis. Results : Among 1146 patients, 353 patients (30.8%) showed hypernatremia. Based on propensity score matching, 290 pairs were included in the analysis. The hypernatremia group had higher rates of in-hospital mortality and 28-day mortality in both overall and matched population (both p<0.001 and p=0.001, respectively). In multivariable analysis of propensity score-matched population, moderate and severe hypernatremia were significantly associated with in-hospital mortality (adjusted odds ratio [OR], 4.58; 95% confidence interval [CI], 2.15-9.75 and adjusted OR, 6.93; 95% CI, 3.46-13.90, respectively) and 28-day mortality (adjusted OR, 3.51; 95% CI, 1.54-7.98 and adjusted OR, 10.60; 95% CI, 5.10-21.90, respectively) compared with the absence of hypernatremia. However, clinical outcomes, including in-hospital mortality and 28-day mortality, were not significantly different between the group without hypernatremia and the group with mild hypernatremia (p=0.720 and p=0.690, respectively). The mortality rates of patients with moderate and severe hypernatremia were significantly higher in both overall and matched population. Interestingly, the mild hypernatremia group of matched population showed the best survival rate. Conclusion : Moderate and severe hypernatremia were associated with poor clinical outcomes in neurocritically ill patients. However, the prognosis of patients with mild hypernatremia was similar with that of patients without hypernatremia. Therefore, mild hypernatremia may be allowed during treatment of intracranial hypertension using hyperosmolar therapy.
Purpose: The present study aimed to study the immunogenicity of the ChAdOx1 nCoV-19 vaccine in patients with hematologic malignancies. Materials and Methods: This prospective cohort study of hematology patients aimed to evaluate their antibody levels against the receptor-binding domain of the severe acute respiratory syndrome coronavirus 2 spike protein and seroconversion rates following two doses of the ChAdOx1 nCoV-19 vaccine. Between June and July 2021, we enrolled 61 patients and included 44 patients in our analysis. Antibody levels were assessed 8 and 4 weeks after the first and second injections, respectively, and compared with those of a healthy group. Results: Eight weeks after the first dose, the geometric mean antibody level was 1.02 binding antibody units (BAU)/mL in the patient group and 37.91 BAU/mL in the healthy volunteer group (p<0.01). Four weeks after the second dose, the geometric mean antibody level was 9.44 BAU/mL in patients and 641.6 BAU/mL in healthy volunteers (p<0.01). The seroconversion rates 8 weeks after the first dose were 27.27% and 98.86% in the patient and healthy volunteer groups, respectively (p<0.001). The seroconversion rate 4 weeks after the second dose was 47.73% in patients and 100% in healthy volunteers. Factors leading to lower seroconversion rates were rituximab therapy (p=0.002), steroid therapy (p<0.001), and ongoing chemotherapy (p=0.048). Factors that decreased antibody levels were hematologic cancer (p<0.001), ongoing chemotherapy (p=0.004), rituximab (p<0.001), steroid use (p<0.001), and absolute lymphocyte count <1,000/mm3 (p=0.009). Conclusion: Immune responses were impaired in individuals with hematologic malignancies, particularly patients undergoing ongoing therapy and B-cell-depleting therapy. Additional vaccinations should be considered for these patients, and further investigated.

Purpose: In this study, the dosimetric characteristics of lung stereotactic body radiotherapy (SBRT) plans using the new Halcyon system were analyzed to assess its suitability. Methods: We compared the key dosimetric parameters calculated for the Halcyon SBRT plans with those of a conventional C-arm linear accelerator (LINAC) equipped with a high-definition multileaf collimator (HD-MLC)-Trilogy Tx. A total of 10 patients with non-small-cell lung cancer were selected, and all SBRT plans were generated using the RapidArc technique. Results: Trilogy Tx exhibited significant superiority over Halcyon in terms of target dose coverage (conformity index, homogeneity index, D0.1 cc, and D95%) and dose spillage (gradient). Trilogy Tx was more efficient than Halcyon in the lung SBRT beam delivery process in terms of the total number of monitor units, modulation factor, and beam-on time. However, it was feasible to achieve a dose distribution that met SBRT plan requirements using Halcyon, with no significant differences in satisfying organs at risk dose constraints between both plans. Conclusions: Results confirm that Halcyon is a viable alternative for performing lung SBRT in the absence of a LINAC equipped with HD-MLC. However, extra consideration should be taken in determining whether to use Halcyon when the planning target volume setting is enormous, as in the case of significant tumor motions.
목적: 말기암환자 가족 간병인의 간병 부담을 줄이는 것은 가족뿐 아니라 환자를 위해서도 중요하다. 본 연구에서는 말기암환자 가족 간병인의 간병 부담과 관련된 요인에 대해 살펴보았다. 방법: 국내 7개 기관의 완화의료병동에서 입원치료를 받는 말기암환자의 가족 간병인 289명의 자료를 분석하였다. 간병인이 느끼는 주관적 간병 부담은 암환자 가족 돌봄 경험 평가도구를 사용하였고, 5가지 차원에서 단계적 변수 선정을 사용한 로지스틱 회귀 모형을 통해 유의한 인자를 확인하였다. 결과: 간병부담의 각 차원에서 다양한 인자들과의 관련성이 확인되었다. 감정적 요인은 가장 폭넓은 영향을 미쳤는데, 감정적 스트레스를 가진 군은 그렇지 않은 군에 비해, 생활패턴이 변할 가능성이 2.54배(95% confidence interval, 1.29~5.02), 가족의 협조가 부족할 가능성이 2.27배(1.04~4.97), 그리고 신체적 부담이 커질 가능성이 5.44배(2.50~11.88)였다. 가족기능은 가족의 협조부족을 매우 잘 반영하였으며, 심한 가족기능 장애를 보일 경우 경제적 부담과도 관련이 있었다. 종교를 가진 군과 동반질환이 없는 군에서 오히려 간병 부담이 더 높은 것으로 나타났으며, 간병기간과 하루 중 간병시간은 생활패턴의 변화와 신체적 부담을 유의하게 예측하였다. 직업을 가지고 있거나, 사회적 지지가 부족하거나, 자주 방문하지 못하는 가족 간병인은 낮은 자아 존중감을 보였다. 결론: 본 연구 결과에 의하면, 가족 간병인의 간병 부담을 파악하기 위해서는 그들의 정서상태와 가족기능을 파악하는 것이 도움이 되며, 사회적 지지체계를 포함하여 경제적 부담을 완화시키는 노력이 필요하겠다.
Background: CT based brachytherapy allows 3-dimensional (3D) assessment of organs at risk (OAR) doses with dose volume histograms (DVHs). The purpose of this study was to compare computed tomography (CT) based volumetric calculations and International Commission on Radiation Units and Measurements (ICRU) reference-point estimates of radiation doses to the bladder and rectum in patients with carcinoma of the cervix treated with high-dose-rate (HDR) intracavitary brachytherapy (ICBT). Materials and Methods: Between March 2011 and May 2012, 20 patients were treated with 55 fractions of brachytherapy using tandem and ovoids and underwent post-implant CT scans. The external beam radiotherapy (EBRT) dose was 48.6Gy in 27 fractions. HDR brachytherapy was delivered to a dose of 21 Gy in three fractions. The ICRU bladder and rectum point doses along with 4 additional rectal points were recorded. The maximum dose ($D_{Max}$) to rectum was the highest recorded dose at one of these five points. Using the HDRplus 2.6 brachyhtherapy treatment planning system, the bladder and rectum were retrospectively contoured on the 55 CT datasets. The DVHs for rectum and bladder were calculated and the minimum doses to the highest irradiated 2cc area of rectum and bladder were recorded ($D_{2cc}$) for all individual fractions. The mean $D_{2cc}$ of rectum was compared to the means of ICRU rectal point and rectal $D_{Max}$ using the Student's t-test. The mean $D_{2cc}$ of bladder was compared with the mean ICRU bladder point using the same statistical test. The total dose, combining EBRT and HDR brachytherapy, were biologically normalized to the conventional 2 Gy/fraction using the linear-quadratic model. (${\alpha}/{\beta}$ value of 10 Gy for target, 3 Gy for organs at risk). Results: The total prescribed dose was $77.5Gy{\alpha}/{\beta}10$. The mean dose to the rectum was $4.58{\pm}1.22Gy$ for $D_{2cc}$, $3.76{\pm}0.65Gy$ at $D_{ICRU}$ and $4.75{\pm}1.01Gy$ at $D_{Max}$. The mean rectal $D_{2cc}$ dose differed significantly from the mean dose calculated at the ICRU reference point (p<0.005); the mean difference was 0.82 Gy (0.48-1.19Gy). The mean EQD2 was $68.52{\pm}7.24Gy_{{\alpha}/{\beta}3}$ for $D_{2cc}$, $61.71{\pm}2.77Gy_{{\alpha}/{\beta}3}$ at $D_{ICRU}$ and $69.24{\pm}6.02Gy_{{\alpha}/{\beta}3}$ at $D_{Max}$. The mean ratio of $D_{2cc}$ rectum to $D_{ICRU}$ rectum was 1.25 and the mean ratio of $D_{2cc}$ rectum to $D_{Max}$ rectum was 0.98 for all individual fractions. The mean dose to the bladder was $6.00{\pm}1.90Gy$ for $D_{2cc}$ and $5.10{\pm}2.03Gy$ at $D_{ICRU}$. However, the mean $D_{2cc}$ dose did not differ significantly from the mean dose calculated at the ICRU reference point (p=0.307); the mean difference was 0.90 Gy (0.49-1.25Gy). The mean EQD2 was $81.85{\pm}13.03Gy_{{\alpha}/{\beta}3}$ for $D_{2cc}$ and $74.11{\pm}19.39Gy_{{\alpha}/{\beta}3}$ at $D_{ICRU}$. The mean ratio of $D_{2cc}$ bladder to $D_{ICRU}$ bladder was 1.24. In the majority of applications, the maximum dose point was not the ICRU point. On average, the rectum received 77% and bladder received 92% of the prescribed dose. Conclusions: OARs doses assessed by DVH criteria were higher than ICRU point doses. Our data suggest that the estimated dose to the ICRU bladder point may be a reasonable surrogate for the $D_{2cc}$ and rectal $D_{Max}$ for $D_{2cc}$. However, the dose to the ICRU rectal point does not appear to be a reasonable surrogate for the $D_{2cc}$.
목적: Siemens사 선형가속기에 장착된 가상쐐기를 이용하여 반대측 유방에 흡수되는 선량을 기존쐐기와 비교 연구하고자 하였다. 대상 및 방법: 반대측 유방선량을 인체모형에서 이극진공관을 사용하여 측정하였다. 이극진공관을 조사영역의 내측 경계선으로부터 반대쪽 외측방향으로 5.5 cm (1번 위치), 9.5 cm (2번 위치), 14 cm (3번 위치) 떨어진 곳에 위치하였다. 6 MV X-선을 이용하여 50도와 230도에서 $17{\pm}10cm$의 비대칭조사영역을 사용하여 접면 조사를 실시하였다. 첫번째 실험은 4가지의 치료방법을 시도하였다; (i) 개방 내측조사와 30도 기존쐐기를 사용한 외측조사; (ii) 15도 기존쐐기를 사용한 내측 및 외측조사; (iii) 개방 내측조사와 30도 가상쐐기를 사용한 외측조사; (iv) 15도 가상쐐기를 사용한 내측 및 외측조사. 두번째 실험은 개방조사, 15도 및 60도 기존쐐기 및 가상쐐기 모두를 사용하여 내측조사를 시행하였으며, 이때 동일한 모니터단위로 조사하였다. 모든 실험은 3회 반복되었다. 결과: 첫번째 실험은 반대측 유방선량은 1번 위치, 2번 위치, 3번 위치의 순으로 감소한다. 또한 기존쐐기 및 가상쐐기와 무관하게 내측에 쐐기를 사용한 경우($3.25{\pm}1.59%$)보다는 사용하지 않은 경우(2.70{\pm}1.45%$) 선량이 낮았고, 이러한 차이는 가상쐐기($0.10{\pm}0.01%$)보다 기존쐐기($0.99{\pm}0.18%$)의 경우 더 컸다. 가상쐐기의 사용은 같은 기법의 기존쐐기를 사용한 것에 비해 처방선량 대비 $0.12{\sim}1.20%$의 반대측 유방선량을 감소시켰다. 두번째 실험시 1번 위치에서는 개방빔, 가상쐐기, 기존쐐기 순으로 선량이 높았으며, 2, 3번 위치에서는 기존쐐기, 개방빔, 가상쐐기 순으로 선량이 높았다. 결론: Siemens사 선형가속기에 장착된 가상쐐기를 사용할 경우 반대측 유방선량을 줄일 수 있으며, 위치에 따른 선량분포는 Varian사 것과 차이가 있었다.
목 적 : 전립선암의 방사선치료는 치료기법에 따라 주요장기와 정상조직의 선량분포에 차이가 나타난다. 본 연구에서는 전립선암 환자의 수술 후 방사선치료 시 직장풍선을 삽입한 환자에게 회전수를 달리한 용적변조회전치료(volumetric modulated arc therapy, VMAT)계획과 세기변조방사선치료(intensity modulated radiation therapy, IMRT)계획을 각각 수립하여 선량분포 특성을 비교하고 치료의 효율성을 평가하고자 한다. 대상 및 방법 : 본원에서 전립선암 수술 후 방사선 치료를 시행한 10명의 환자를 대상으로 하였다. 직장풍선을 삽입한 환자의 CT영상을 3 mm 두께로 획득하고 Eclipse (Ver 11.0, Varian, Palo Alto, USA)를 사용하여 HD120MLC가 장착된 Truebeam STx (Varian, Palo Alto, USA)의 10 MV 에너지를 적용하였다. 환자마다 1 Arc, 2 Arc VMAT계획과 7조사면의 IMRT계획을 수립하였으며 각 치료계획의 선량체적제한과 plan normalization 값은 동일하게 적용하였다. 수립된 세 가지 치료계획을 평가하기위해 PTV의 coverage, conformity index (CI), homogeneity index (HI)를 비교하였고, PTV주변 정상조직의 선량퍼짐정도를 알아보기 위해 50% 등선량체적과 PTV체적의 비($R_{50%}$)를 산출하였다. 결정장기 (organ at risk, OAR)에서는 직장의 $D_{25%}$와 방광의 $D_{mean}$을 비교하였고 치료의 효율성을 평가하기 위해서 총 MU와 조사시간을 측정하였으며, 각 평가항목별 결과는 환자 10명의 평균값으로 비교분석하였다. 추가적으로 선량전달 정확도를 검증하기 위해 EPID를 이용한 portal dosimetry를 진행하였다. 결 과 : 각 치료계획에서 나타난 PTV coverage와 HI의 차이는 크지 않았지만 CI는 1A-VMAT, 2A-VMAT, 7F-IMRT에서 각각 1.036, 1.035, 1.230으로 VMAT에 비해 7F-IMRT가 높았고(p=0.00), $R_{50%}$는 3.083, 3.054, 3.991로 2A-VMAT이 제일 낮았고 7F-IMRT에서 가장 높게 나타났다(p=0.00). 치료계획에 따른 직장의 $D_{25%}$는 VMAT에서 비슷했지만 7F-IMRT가 약 7% 높게 나타났고(p=0.02), 방광의 $D_{mean}$은 큰 차이가 없었다(P>0.05). 총 MU는 1A-VMAT, 2A-VMAT, 7F-IMRT에서 각각 494.7, 479.7, 757.9로 7F-IMRT가 가장 많았고(p=0.00), 조사시간은 65.2초, 133.1초, 145.5초로 1A-VMAT이 확연히 짧았다(p=0.00). portal dosimetry 검증에서는 모든 치료계획에서 99.5% 이상의 gamma pass rate(2 mm, 2%)을 보였다(P=0.00). 결 론 : 본 연구결과 직장풍선을 삽입한 전립선암 환자의 수술 후 방사선치료 시, 치료기법에 따른 PTV coverage의 차이는 크지 않았지만 1A, 2A-VMAT이 7F-IMRT에 비해 정상조직과 직장선량을 낮추는데 효과적이었다. VMAT중에서는 $R_{50%}$와 MU가 2A-VMAT에서 다소 낮았지만 조사시간이 짧은 1A-VMAT이 치료에 더 효율적이며 환자의 치료 중 움직임 또한 줄일 수 있을 것으로 사료된다.

제 11 조 (회원의 의무)

제 4 장 서비스의 이용

② 당 사이트는 서비스를 특정범위로 분할하여 각 범위별로 이용가능시간을 별도로 지정할 수 있습니다. 다만

① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
