치아종을 동반한 석회화 치성낭의 치험례 (CALCIFYING ODONTOGENIC CYST ASSOCIATED WITH COMPLEX ODONTOMA : CASE REPORT)

  • 석회화 치성낭은 비교적 드문 낭으로 하나의 독립된 유형의 치성 병소로 인정받고 있지만, 조직학적 소견이 다양하고 분류체계도 학자들마다 다르다. 석회화 치성낭의 이장상피는 중층편평상피로 되어 있으며, 상피층 내에 특징 세포인 유령세포가 출현한다. 유령세포들은 인접한 결합조직 에 치성조직을 유도할 수 있는 능력을 가지고 있어 법랑모세포종, 법랑모세포 치아종, 법랑모세포 섬유치아종, 치아종 등의 치성종양이 합병되어 발생하기도 한다. 외과적 제거 후 재발율은 적지만 치성종양이 병발될 경우가 있기 때문에 반드시 조직병리학적 검사를 수행하여야 한다. 7세 남아가 상악 좌측 중절치의 미맹출을 주소로 원광대학교 치과병원 소아치과에 내원하였다. 방사선 사진에서 맹출되지 않은 상악 좌측 중절치 및 측절치 치관 상방에 방사선 불투과성 물질이 보였다. 초기 임상적 방사선학적 검사 결과 치아종으로 진단하였으며, 외과적 적출 및 생검을 시행하였다. 적출물은 불규칙적인 석회화 물질로 구성된 복잡 치아종 양상을 보였으며 상피조직으로 싸여 있었다. 조직병리학적 검사 결과, 이장상피에서 유령세포의 군집이 관찰되었고, 석회화 조직은 법랑질, 상아질 형태의 조직들이 혼합된 양상을 보여, 복잡 치아종을 동반한 석회화 치성낭으로 진단하였다. 본 증례에서와 같이 매복치 상방의 치아종 양상의 병소가 관찰되었을 때 석회화 치성낭의 가능성을 염두에 두어야 할 필요가 있다.

발생 치배와 치성 종양에서 Osteonectin발현에 관한 연구 (EXPRESSION OF OSTEONECTIN IN DEVELOPING TOOTH GERM AND ODONTOGENIC TUMORS)

  • The osteonectin is a sort of glycoprotein which is secreted in human tissues. The osteonectin is generally detected in number of normal or neoplastic human tissues in vivo, but hasn't been studied the role of osteonectin in developing human teeth and odontogenic tumors. We evaluated degree of the expression of osteonectin immunohistochemically in 20 cases of developing tooth germ which growth from fetus 5 to 38 weeks, and total 51 odontogenic tumors whitch has taken from routine biopsy, such as 10 ameloblastomas, 5 cases of adenomatoid odontogenic tumors and odontomas and odontogenic fibromas, 4 cases of cementomas and calcifying epithelial odontogenic cyst and odontogenic keratocyst and dentigerous cysts and periapical cysts, and 3 cases of ameloblastic fibromas and myxomas. The results were as follows: 1. The osteonectin on the bud stage of tooth germ was strongly expressed in the epithelial dental lamina and in the outer dental epithelium on the early bell stage, and also strongly expressed in the inner dental epithelium on the late bell stage of tooth germs. 2. In ameloblastoma, the osteonectin was strongly expressed in the epithelial tumor component and especially in the acanthomatous types. 3. In both of calcifying epithelial odontogenic tumor and adenomatoid odontogenic tumors, the osteonectin was moderately expressed on the duct like spindle cells and epithelial tumor cells around calcification areas. 4. In odontogenic tumors originated from epithelial-mesenchymal tissues, the osteonectin was moderately expressed on the epithelial tumor components and in odontogenic cysts, it was expressed in ghost cells and calcification areas only. These were summaried the osteonectin may be strongly related to the developing tooth germ and odontogenic tumors and could be regulated hard tissue of human tooth in morphogenesis and involved with calcification mechanism in development odontogenic tumors.

  • Florid cemento-osseous dysplasia (FCOD) is a benign, non-neoplastic lesion characterized by multiple sclerosing masses only within jawbones. It is frequently confused with chronic diffuse sclerosing osteomyelitis (CDSO) in previous literatures. In our study, two cases of FCOD were examined to know the characteristics of their calcifying tissues. The first case was non-infected, while the second case was severely infected, displaying the typical features of CDSO in clinico-radiologic findings. The infected FCOD case showed a lot of bacterial colonies in the main lesion with relatively rare inflammatory reaction. The globular cementum-like materials of FCOD showed woven bone pattern and was positive for Alcian blue stain, and also positive for the antibodies of ameloblastin, bone morphogenetic protein (BMP) -2 and -4. On the other hands, in the immunostains of matrix metalloproteinase (MMP) -3, -9, -10, and $TNF-{\alpha}$, macrophage infiltrated in the FCOD lesion was rarely observed. These data suggest that the cementum-like materials of FCOD contain various matrix proteins, and that the cementum-like materials are relevant to the overgrowth of the bacterial colonies by inhibition of the regional inflammatory reactions.

성장 중인 흰쥐 악관절의 basic fibroblast growth factor(bFGF) 분포에 관한 면역조직화학적 연구 (IMMUNOHISTOCHEMICAL PROFILE OF BASIC FIBROBLAST GROWTH FACTOR(bFGF) IN GROWING RAT T-M JOINT)

  • This study was designed to localize the distribution of basic fibroblast growth factor(bFGF) in the developing rat condylar region and to elucidate the associated function of bFGF in the condyle development. The condyles of temporomandibular joint of Sprague-Dawley rats (27g of weight) were used. The tissues were examined with electron microscope and immunohistochemical method. The results were as follows: 1. The developing condylar region are divided in to 5 zones apparently: proliferative, maturation, hypertrophic, calcifying, and ossification zones. 2. The cells in the proliferative zone are condensed and have under-developed cell organells in the cytoplasm. This zone shows a strong immunoreactivity of bFGF. 3. The cells in the maturation zone are typical chondroblasts showing well-developed cell organells and round nucleus. The cartilaginous matrix does not show the immunoreactivity of bFGF, while the chondroblasts show the immunoreactivity. 4. The cells in the hypertrophic zone show hypertrophic change having the degenerated cell organelles and small nucleus. There are no immunoreactivity of bFGF in this zone except the nucleus and endoplasmic region showing mild immunoreactivity. 5, The cells in the calcifying zone show hypertrophic change and cell organelles are disappeared. The cells are surrounded by the calcified cartilaginous matrix. There are no immunoreactivity of bFGF in this zone except the endoplasmic region showing mild immunoreactivity. 6. In the zone of bone formation, chondroblasts are disappeared. Newly differentiated osteoblasts secreting osteoid around the calcified cartilaginous matrix. The bone marrow shows the immunoreactivity of bFGF, while the bone matrix does not show the immunoreactivity of bFGF.

  • 치성유령세포암종은 석회화치성낭종의 악성 부분으로 치성낭종, 치성종양 모두에서 기인할 수 있다. 다형성 상피 세포와 유령세포가 nucleated cell들과 혼재된 침윤성 종양의 형태로 나타나는 것이 특징이고, 국소 재발 및 원격 전이된 증례도 보고된 바있다. 저자들은 드문 치성종양인 치성유령세포암종을 상악골에서 경험하였기에 문헌 고찰과 함께 보고하였으며, 본 증례의 경우 향후 재발 및 전이 여부 파악을 위해 주기적인 경과 관찰이 필요하리라 사료된다. 또한 악성 변화의 요인으로 이종골 이식의 영향에 대한 연구도 필요하리라 사료된다.

해양 산성화 및 수온 상승 환경에서의 전복치패(Haliotis discus hannai)의 생존 및 성장 (Survival and Growth in Juvenile Abalone Haliotis discus hannai to Ocean Acidification and Elevated Temperature)

  • The increasing of atmospheric $CO_2$ are changing the pH (ocean acidification) and temperature of the sea. Although the effects of ocean acidification on calcifying organisms have well-documented, only a few studies have examined the combined effects of ocean acidification and elevated temperature. This study investigated the effects of ocean acidification and elevated temperature for 2100 on survival and growth of juvenile abalone, Haliotis discus hannai. Ocean acidification was simulated by bubbling $CO_2$ into seawater at concentrations of 1,000 and 1,500 ppm, and temperature was set at room temperature $+2^{\circ}C$. Neither $CO_2$ nor temperature had a significant effect on survival of abalone, while both significantly affected growth. There was no significant interaction between the two factors. Shell length can be used as a growth index of abalone to access the impacts of ocean acidification and elevated temperature.

Characterization of Two Urease-Producing and Calcifying Bacillus spp. Isolated from Cement

  • Two bacterial strains designated as CT2 and CT5 were isolated from highly alkaline cement samples using the enrichment culture technique. On the basis of various physiological tests and 16S rRNA sequence analysis, the bacteria were identified as Bacillus species. The urease production was 575.87 U/ml and 670.71 U/ml for CT2 and CT5, respectively. Calcite constituted 27.6% and 31% of the total weight of sand samples plugged by CT2 and CT5, respectively. Scanning electron micrography analysis revealed the direct involvement of these isolates in calcite precipitation. This is the first report of the isolation and identification of Bacillus species from cement. Based on the ability of these bacteria to tolerate the extreme environment of cement, they have potential to be used in remediating the cracks and fissures in various building or concrete structures.

  • Pilomatricoma, calcifying epithelioma of Malherbe, is a benign skin neoplasm of hair matrix origin that is typically occurred in the head and neck region. It usually presents as a superficial, firm, solitary, slow-growing, and painless mass of less than 3 cm in diameter and most often occurs in young age under 20 years. The tumor does not express an aggressive behavior and surgical excision is the treatment of choice. The purpose of this article is to present a case of pilomatricoma of the preauricular region and to review the literature regarding pilomatricomas of the head and neck region.

이소매복된 치아의 자가치아이식 증례 (Autotransplantation of Ectopically Impacted Teeth : Two Case Reports)

  • 매복치의 치료방법으로 교정적 견인, 발치 후 임플란트, 자가치아이식 등 다양한 치료 방법이 있다. 그 중에서도 자가치아이식은 성장하는 환자들에게서 이식치 고유의 치주인대세포의 보존 및 치조골 성장이 가능하다는 점에서 좋은 치료방법이라고 생각된다. 이 증례에서는 성장중인 청소년의 이소 매복 치아를 자가치아이식으로 치료한 두 개의 증례를 소개하고자 한다. 첫 번째 증례에서 이소 매복된 좌측 하악 제2소구치를 발거 후 정위치로 자가치아이식하였고 혈소판 농축 피브린(PRF)과 mineral trioxide aggregate (MTA)를 사용하여 재생 근관 치료를 하였다. 석회화 치성낭으로 인해 이소매복된 좌측 하악 제2대구치를 지닌 두 번째 증례에서 병소의 적출술을 시행하였다. Obturator를 3개월 간 장착하여 매복치아의 자발적 맹출을 기대하였으나 맹출 양상이 없어 자가치아이식한 후 MTA를 이용한 근관치료를 하였다. 두 증례 모두 자가치아이식술로 이소매복치를 간단하고 빠르게 치료하였다. 성장하는 환자에게서 이소매복치의 자가치아 이식술은 임플란트나 보철물 수복 대신 좋은 치료 방법이 될 것이다.

양성 치성종양의 임상적 분석 (A CLINICAL ANALYSIS OF BENIGN ODONTOGENIC TUMOR)

  • The author studied on the 128 cases of benign odontogenic tumors which had been diagnosed with biopsy during the period of Jan. 1989 to Dec. 1998 at the Kyungpook National University Hospital, Yeungnam University Medical Center, Keimyung University Dongsan Medical Center, and Taegu Catholic Medical Center. This study contained the clinicostatistical analysis of the frequency in relation to sex, age, locations, chief complaints, duration, radiographic findings, recurrence, teeth, and treatment methods. The results were as follow : 1. Of a total of 128 benign odontogenic tumors, ameloblastomas(57 cases; 44.5%) and odontomas (44 cases ; 34.4%) mostly occupied. The other types of lesions were 8 calcifying odontogenic cysts, 7 benign cementoblastomas, 4 myxomas, 3 adenomatoid odontogenic tumors, 2 calcifying epithelial odontogenic tumors, 2 ameloblastic fibro-odontomas, and 1 odontogenic fibroma. 2. In age and sex distribution, benign odontogenic tumors occured slightly more often in males(53.9%) than females(46.1%) and the majority of cases(79.7%) were found during 2nd, 3rd, and 4th decade. 3. There was a predilection for mandibular lesions(mandible-maxilla ratio, 2.6 : 1). 4. The most common chief complaint was swelling(29.7%) and in respect to duration, the cases less than 1 year(50.0%) mainly appeared. 5. There were 7 cases(13.0%) of recurrence on ameloblastoma and there was no recurrence in the others. 6. In Ameloblastoma It commonly occured during 3rd and 4th decade(59.6%) and mean age was 30.2 years. The majority of cases were occurred in mandible(96.5%) , especially mandibular molar and angle area(71.9%). The most common chief complaint was swelling(47.4%) and in respect to duration, the cases less than 1 year(52.6%) mainly appeared. In relation to teeth, there were resorption of root(52.6%), displacement of teeth(31.6%), and in relation to impacted teeth(43.9%). There was higher recurrence rate in the cases by conservative treatment(14.7%) than radical treatment(10.0%). As regards radiographic findings, conservative treatments were prevalent in the cases of unilocular type(85.7%) as compared with multilocular type(48.5%). and there was higherrecurrence rate in the cases of multilocular type(18.2%) than unilocular type(4.8%). As regards the type of treatment in relation to age, conservative treatments were prevalent in patients younger than 20 years of age. 7. In Odontomas It commonly occured during 2nd decade(50.0%) and in maxillary anterior teeth(40.9%). The most common chief complaint was delayed retention and permanent impaction of teeth(72.7%), and most frequently associated with impacted teeth(79.5%).
