석회화 치원성 낭종의 치험례 (A REPORT OF CALCITYING ODONTOGENIC CYST)

  • The calcifying odontogenic cyst was identified as a pathological entity by Gorlin & his associates in 1962. This lesion is one of the rarest and most disputable cysts in the oral region. The calcifying odontogenic cyst has variable clinical and radiological features. We review the previous literatures and report 2 cases of calcifying odontogenic cyst at Department of Oral and Maxillofacial Surgery, Kyung-Hee University. The 1st case was as follows. The patient vas 22 year old female. The past dental history revealed extraction of prolonged retained #73 tooth about 15days ago. She complained a painful swelling on the lower anterior teeth area. There were chin and vestibular swelling on the lower anterior teeth area, tenderness and missing of #33 tooth. The radiograph revealed well-demarcated unilocular radiolucency containing radiopaque calcific flecks around impacted #33 tooth. The clinical diagnosis was COC, so surgical enucleation was done. There was no recurrence and COC was confirmed by pathologist. The second case was as follows. The patient was 72 year old male. The past history revealed inactive tuberculosis, bronchial asthma and denture construction. The chief complaint was rapidly growing mass on the lower left anterior edentulous area. The clinical findings were chin swelling protruding mass with surface ulceration, fluctuation and a few bloody fluid in aspiration. The radiograph revealed well-demarcated radiolucency mimiking the residual cyst. The biopsy result was COC. The surgical excision was done, but the lesion was recurred 10 months later. The treatment was surgical excision with aggressive peripheral bone grinding and FTSG form groin area. There was no problem during the postoperative period.

석회화 건막 섬유종 - 증례 보고 - (Calcifying Aponeurotic Fibroma - A Case Report -)

  • 목적: 석회화 건막 섬유종은 소아 및 청소년층에서 수장부를 가장 많이 침범하는 양성이나 공격적 성향을 띈 섬유성 종양이다. 저자들은 최근에 15세 소년의 수부에 발생한 석회화 건막 섬유종의 치료를 경험하였기에 이를 문헌 고찰과 함께 보고하고자 한다. 증례: 15세 소년이 좌수의 소지구 부위에 재발하는 종물을 주소로 내원하였으며 이 소년은 7년전에 타 병원에서 국소 마취 하에 동일 부위에 발생한 종물을 제거한 기왕력을 가지고 있었다. 종물은 단단하였으며 압통이 있었다. 단순 방사선 촬영상에는 종물 내에 다소성으로 산재된 미세 석회 침착이 보였다. 수술 소견상 종물은 주변과 명확히 구분되지 않았으며 단단하고 옅은 회색빛의 석회화를 동반한 침습성의 결절성 형태를 보였으며 가이언 관내의 척골 신경과 척골 동맥이 바로 인접하고 있었다. 수술 후 제거한 종물을 현미경적으로 관찰한 결과 종물은 많은 석회 침착이 되어 있으면서 이형성 연골양 분화를 보이는 조직이 중간 중간에 섞여 있는 빽빽한 섬유 조직의 증식으로 이루어져 있었고 주변부는 명확히 경계 지어지지 않으며 분엽상을 보이고 있었다. 결론: 이 질환에 있어서 가장 바람직한 치료법은 기능적인 면과 주요 조직을 보존할 수 있는 한도내에서 광범위 절제를 시행하는 것이며 이렇게 치료하더라도 많게는 50 %이상의 재발율이 보고되고 있는 실정이다.

치아종을 동반한 석회화 치성낭에 관한 증례 보고 (CALCIFYING ODONTOGENIC CYST ASSOCIATED WITH ODONTOMA : CASE REPORT)

  • 석회화 치성낭은 상악 전치부에 호발하는 발육성 치성낭이나 조직학적 소견이 다양하고 따라서 분류 체계도 다양하다. 악골 내에서 느리게 진행되는 무통성 종창이 일반적이며 24-30%에서 매복치 및 치아종과 연관되어 나타난다. 피복 상피에 나타나는 유령세포는 결합 조직 에 노출되어 석회화 조직을 형성한다. 이 증례는 14세 여아의 상악 견치부에 발생한 치아종을 동반한 석회화 치성낭으로, 매복된 상악 좌측 견치 치관 주위로 경계가 명확한 방사선 투과성 병소와 다발성의 방사선 불투과성 석회화 물질이 혼재된 병소였다. 조직병리학적으로 중층 편평상피로 이장되어있고 상피 내에 유령세포가 관찰되었다. 석회화된 물질은 복합 치아종의 소견을 보여 Preatorius의 조직학적 분류에 의한 석회화 치성낭 Type IB로 진단되었다. 병소의 적출 및 병소에 이환된 견치 발치를 시행하였다. 현재 염증 소견이 관찰되어 병소에 이환된 상악 좌측 제1소구치는 치수치료를 시행하였으며 환아 및 보호자가 포괄적인 교정 치료를 원하여 현재 공간 유지 장치를 장착 중이다.

성인에서 발생한 비석회화 모기질종 2예 (Two Cases of Non-Calcifying Pilomatrixoma in Elderly Individuals)

  • Pilomatrixoma is an unusual benign cutaneous tumor, originating from precursor cells that normally differentiate into hair matrix cells. The majority of pilomatrixoma is seen in the first two decades of life, and they are extremely rare in older patients. Thus, pilomatrixoma is often not considered in the clinical differential diagnosis of solitary, firm nodules in older adults. Most present as firm, solid nodules in the head and neck region. We present two cases of a non-calcifying pilomatrixoma in older patients. One lesion arose from the upper neck in 54 year-old male and the other from the cavum concha of left ear in 35 year-old male.

Jaw lesions associated with impacted tooth: A radiographic diagnostic guide

  • This review article aimed to introduce a category of jaw lesions associated with impacted tooth. General search engines and specialized databases such as Google Scholar, PubMed, PubMed Central, MedLine Plus, Science Direct, Scopus, and well-recognized textbooks were used to find relevant studies using keywords such as "jaw lesion", "jaw disease", "impacted tooth", and "unerupted tooth". More than 250 articles were found, of which approximately 80 were broadly relevant to the topic. We ultimately included 47 articles that were closely related to the topic of interest. When the relevant data were compiled, the following 10 lesions were identified as having a relationship with impacted tooth: dentigerous cysts, calcifying odontogenic cysts, unicystic (mural) ameloblastomas, ameloblastomas, ameloblastic fibromas, adenomatoid odontogenic tumors, keratocystic odontogenic tumors, calcifying epithelial odontogenic tumors, ameloblastic fibro-odontomas, and odontomas. When clinicians encounter a lesion associated with an impacted tooth, they should first consider these entities in the differential diagnosis. This will help dental practitioners make more accurate diagnoses and develop better treatment plans based on patients' radiographs.

소아의 Malherbe씨 석회화 상피종(Pilomatricoma) (Calcifying Epithelioma of Malherbe(Pilomatricoma) in Children)

  • A clinical analysis was made of 42 pilomatricoma in 34 children treated, from January 1980 to August 1996 at the Department of Pediatric Surgery, Hanyang University. The gender ratio was 13 to 21 with female preponderance. The average age at presentation was 7 years 3 months(range 8 months to 19 years old). The time average interval from onset to excision was 9.8 months. The predilection site was neck, upper extremities, face, trunk and lower extremities in the decreasing order of frequency. The masses were less than 3cm in diameter and there was no correlation between age at presentation and size of the mass. Of the six children with multiple lesions, two presented with multiple lesions initially and four presented with second lesion metachronously after excision of the primary lesions. Two girls had cysticercosis and von Recklinghausen's disease respectively in association with a pilomatricoma. All lesions were encapsulated subcutaneous masses which were clearly distinguished from surrounding tissue, and 4 of them had pigmentation, ulceration, skin disruption and hemorrhagic changes in the overlying skin. Microscopically the mass was composed of basophilic cells and eosinophilic shadow cells. Calcification, foreign body giant cell, ossification, focal cystic, inflammatory or necrotic changes were also seen.

Ghost cell odontogenic carcinoma on right mandible and its respective surgical reconstruction: a case report

  • Calcifying cystic odontogenic tumor (CCOT) is defined as an odontogenic cyst-like benign neoplasm that characteristically contains several ghost cells, ameloblastoma-like epithelium, and occasional calcification. Ghost cell odontogenic carcinoma (GCOC), a malignant form of CCOT, is an exceptionally rare malignant tumor. In this report, we present a case of a 53-year-old man whose chief complaint was a solitary mass on the right mandible area. The mass was completely removed through an extraoral surgical approach and reconstructive surgery was performed in two phases.

  • The calcifying odontogenic cyst (COC) is a rare disorder of the jaws and shows various radiographic features. The purpose of this study is to describe the different radiographic appearances of 2 cases of COC. Case 1 was located in the posterior maxilla extending into maxillary sinus, showing unilocular radiolucency with a well-defined margin. Cortical bone expansion and thinning were prominent. Root resorption of adjacent teeth was apparent. Case 2 showed unilocular radiolucency with a calcified material. Calcification was supposed to be dystrophic dental hard structures, detected at the periphery of the lesion. Ghost cell and proliferation of ameloblastoma-like tissues were common features for these two lesions on histopathological findings. This reports presented common and atypical radiographic features of the COC.

  • Nine cases presented with a calcifying odontogenic cyst to the Dental Hospital of the College of Dentistry, Yonsei University. from January 1987 to September June, 1996, Clinical or histopathological findings were observed according to each radiologic criteria. The results obtained are as follows : 1. The male to female ratio was 2 : 1 with a mean age of 25 years. 2. The radiographic appearences of nine cases were well-defined radiolucent lesions in which eight cases were shown unilocular lesions and only one lesion was seen multilocular lesion. 3. Radiologically. three of nine cases were pure radiolucent lesions and others, six cases were radiolucent contained a variable amount of radiopaque material. 4. Histologically, two cases were classified simple cyst, five were cyst associated odonoma, two were neoplastic type. 5. Histological findings according to the radiological classification, two of three radiolucenct lesions were simple cysts; another was a cyst associated with odontoma. While, four of six radiologic mixed lesions turned out to be a cyst associated with odontoma, two was the neoplastic type.

  • 석회화 치성낭은 하나의 독립된 유형의 치성 병소로 인정하고 있지만, 조직학적 소견이 다양하고 분류 체계도 학자들마다 다르다. 또한 석회화 치성낭의 피복상피는 인접한 결합조직에 치성조직을 유도할 수 있는 능력을 갖고 있어 다른 치성 종양과 흔히 동반하여 발생한다. 이 증례는 5세 여아의 상악 전치부에 발생한 치아종을 동반한 석회화 치성낭으로, 매복된 상악 좌측 중절치 및 측절치 치관 주위로 경계가 잘 된 방사선 투과성 병소와 다발성의 방사성 불투과성 석회화 물질이 혼재된 병소였다. 병리조직학적으로 피복상피는 법랑모세포와 유사하였고 상피 내에 유령세포의 군집이 관찰되었다. 석회화된 종괴는 일부 잔존한 범랑기질이 상아질로 둘러싸여 있었고, 퇴축 법랑상피 및 유령세포의 군집이 관찰되어 복잡 치아종이 동반된 석회화 치성낭으로 진단하였다. 이 증례에서와 같이 소아에서도 매복치와 동반하여 방사선 투과 및 불투과상의 혼합 병소가 관찰되면 석회화 치성낭의 가능성을 항상 염두에 두어야 할 필요가 있다. 환자는 수술 1년이 경과한 현재까지 재발의 소견은 없었고, 현재 혼합치열기의 간격조절을 시행 받고 있는 중이다.
