화상치료제의 조직학적 수복효과 (A Study on Histological Recuperative Effect of Burn Remedies)

  • This study was carried out to test the recuperative effect of 2 types of sample drugs for 3 degree burn. The burn injury was made by iron plate heated in the boiling water. The Sprague Dawley rats were shaven with a razor preliminarily and burned by direct contact method for 10 seconds. The experimental groups were classified with 5 each-normal, control, MEBO ointment, sample A, sample B. The effect of the sample drugs were decided by histological results after 3 week application. The results were as following. The 3 treatment groups recovered the burn injury faster than control group. The recuperative effect precedes about 4-5 days at the time of 15th day and 1 week at the time of 3rd week. The therapeutical procedure of 3 treatment groups was similar with naked eye and with microscopic histology in the 1 st, 2nd and 3rd specimens. So there weren't significant differences in curative effect in 3 treatment groups of this experiment. But a regeneration of hair follicle was noted in Sample B uniquely. These results suggested that 3 burn remedies have similar effect of therpy, but sample B containing yolk sac oil has slightly better effect in part of hair regeneration.

화상환아 어머니의 돌봄 경험에 대한 내용분석 (Content Analysis on Caring Experiences of Mothers of Children with Burns)

  • Purpose: This study was conducted to identify the caring experience of the mothers of childhood burn patients. Methods: Data were collected from 28 mothers of childhood burn patients, through semi-structured in-depth interviews. Data were categorized and coded by using content analysis. Results: 27 categories and 102 statements were drawn from 4 domains. The analyzed domains were psychological sufferings, confronted harsh reality, coping method, and future concerns. Psychological sufferings were categorized heartache, sense of guilt, confusion, regret, depression, sorriness, getting hurt, frustration and upset feeling. Confronted harsh nature were categorized economic difficulty, physical burn-out, lack of caring other children and family troubles. Coping method were categorized positive thinking, having hope, ventilating feelings, accepting the situations, demanding help from family members, changing patterns of the burned child rearing, collecting information for burn treatment, refusing accept the condition of burned child, avoiding personal and social relationship, and reliance on religion. Concerns were categorized concerns of growth and adaptation of the burned child, anxiety for scar, concerns of adapting school life and vague future concerns. Conclusion: The nursing interventions for early assessing psychological problems and providing social supports for caring both burn patients and other siblings should be provided to the mothers of childhood burn patients.

A new burn-up module for application in fuel performance calculations targeting the helium production rate in (U,Pu)O2 for fast reactors

  • In light of the importance of helium production in influencing the behaviour of fast reactor fuels, in this work we present a burn-up module with the objective to calculate the production of helium in both in-pile and out-of-pile conditions tracking the evolution of 23 alpha-decaying actinides. This burn-up module relies on average microscopic cross-section look-up tables generated via SERPENT high-fidelity calculations and involves the solution of the system of Bateman equations for the selected set of actinide nuclides. The results of the burn-up module are verified in terms of evolution of actinide and helium concentrations by comparing them with the high-fidelity ones from SERPENT, considering two representative test cases of (U,Pu)O2 fuel in fast reactor conditions. In addition, a code-to-code comparison is made with the independent state-of-the-art module TUBRNP (implemented in the TRANSURANUS fuel performance code) for the same test cases. The herein presented burn-up module is available in the SCIANTIX code, designed for coupling with fuel performance codes.

어린이 화상사고의 실태와 예방대책 프로그램 개발에 대한 연구 (A Study of Burn Accidents in Children towards Developing a Program for Prevention)

  • This study is a descriptive study which was done to examine the circumstances surrounding burn accidents in children in order to develop a burn prevention program. The subjects for the study were the mothers of 107 children admitted to two burn specialty hospitals in seoul and the mothers of 172 healthy children attending day care or kindergarten. The tool used in the study was developed by the researchers, pretested and refined. Cronbach's alpha was 0.72. Data collection was done between November 1995 and April 1996. Statistically analysis was done using SAS Version 6.04 and the data was analyzed using test, chi square, regression and logistic analysis. A suumary of the results shows the following : Eighty percent of the children in both groups lived in nuclear families but comparatively more of the children with burn accidents lived in apartments. More of the parents in the group of healthy children had university education and held white collar or professional jobs, while the parents of the children with burn accidents were slightly younger than the parents of the healthy children. but not significantly younger. A significantly greater number of families of children with burn accidents had more than one child and the characteristics of the children with burn accidents that they were more frequently boys, either first or last born and very active. Most of the burn accidents occurred in fall or winter. sometime between noon and midnight and the majority occurred in the home and were caused by hot liquid. In 86% of the cases emergency care was not given at home but the child was brought to the hospital emergency unit. Most of the burns were second degree or greater. involved the arms or legs. required admission, and 41.5% occurred when the parents were preparing or eating meals. In a test of knowledge of preventive measures, the mothers of children who had suffered burn accidents scored slightly higher than the mothers who had no experience with burn accidents, but this was not statistically significant. As to what to do in the case of a fire, the mothers of the healthy children had more knowledge about who to notify. but 70% of the mothers in both groups did not know how to escape from the house if it was of fire. 80 to 96% did not know what do to in case of a fire, such as rolling if clothes were on fire. 80% did not know what emergency care to give for a burn, and 74 to 87% did not know when it is appropriate to try to put out a fire. It can be concluded from this study that. since the results show that the most frequent situation which resulted in burn accidents was when the parents were not aware of the danger of burns or were not paying attention to the child. and when there were no protective devices to prevent burns. and that accidents most frequently occurred at home. the most effective way of preventing accidents is to develop in the parents a "safety mind", Since burn accidents in children are usually more severe than in adults. the physical. emotional. psychological and social damage is greater. In order to develop and awareness of this in the general public. the use of mass media. books and pamphlets and other educational media that provide education on prevention of burn accidents need to be utilized.

보건진료 전담공무원의 전문직 삶의 질 관련요인 (Factors related to Professional Quality of Life among Community Health Practitioners in Korea)

  • Purpose: The aims of this study were to examine the work, client, and personal environments of community health practitioners, including the level of their professional quality of life (ProQOL), and identify the factors affecting their ProQOL. Methods: Data were collected using a web-survey questionnaire, which was completed by a sample of 308 community health practitioners currently working in Korea. The questionnaire included items on ProQOL; three dimensions labeled compassion satisfaction, burn out, and secondary traumatic stress; job stress; job satisfaction; sense of community; and general characteristics. Results: The mean scores for the three dimensions were $39.2{\pm}6.44$ (compassion satisfaction), $31.9{\pm}3.59$ (burn out), and $26.4{\pm}5.05$ (secondary traumatic stress). A multiple linear regression revealed that compassion satisfaction varied significantly according to the satisfaction with job choice, sense of community, job stress, and job satisfaction. The factors affecting burn out were sense of community and work load, while the factors affecting secondary traumatic stress were education, job stress, and job satisfaction. Conclusion: The findings of this study suggest that comprehensive intervention focusing on improving the sense of community and job satisfaction and reducing job stress is essential to promote community health practitioners' ProQOL.

미분탄과 목재 바이오매스 혼합 연료의 연소 및 열분해 특성에 관한 연구 (A Study on Characteristics of Combustion and Thermo Pyrolysis in Co-firing with Pulverized Coal and Wood Biomass)

  • The effect of co-firing with pulverized coal and wood biomass on ignition and burn-out temperature was investigated at air and oxy-fuel conditions by thermo gravimetric analyzer(TGA). Three kinds of coal(shenhua, adaro, wira) were selected and mixing ratios of coal and wood biomass was set to 1, 0.5, and 0.8. The ignition temperature depended on the amount of volatile matter of blended fuel, while the burn-out temperature was dominated by the oxidant ingredients. The oxy-fuel condition with an oxygen ratio(Ofr,o) of 0.3 showed similar tendency with air condition in the heat flow measurement. Volatile matter reaction, however, became dominant when oxygen ratio exceeded 0.8 for co-firing combustion of wood biomass and pulverized coal.

화상감염 소아환자에서 분리된 주요 균종에 대한 항생제의 내성률 (Antimicrobial Resistance of Clinically Important Bacteria Isolated from Burn Wound Infections in Children)

  • Background & Objectives: Burn injury mortality and septic complication are frequent and well-known in burned pediatric patients. The overuse of antibiotics is the base for development of wound infection by resistant microorganisms as well as opportunist agents. Methods: We have carried out a study of the bacterial profile and antimicrobial resistance clinically important bacteria isolated from burn wound infections in children patients. The most common isolate from burn wound cultures was Pseudomonas aeruginosa (26.8%), followed by Staphylococcus aureus (25.4%), Acinetobacter baumannii (12.7%), coagulase negative staphylococcus (12.0%), Enterococcus faecium (7.7%), Escherichia coli (4.9%), Enterococcus faecalis (3.7%), Burkholderia cepacia (3.0%), Enterobacter cloacae (2.3%) and Klebsiella pneumonia (2.3%). Colistin was very significantly effective drug in gram negative organism, such as Pseudomonas aeruginosa and Acinetobacter baumannii. Results & Conclusion: The resistance rates were 65% and 98% to piperacillin, 63% and 97% to ceftazidime, 28% and 50% to levofloxacin. The most effective antibiotic in gram positive organism, such as Staphylococcus aureus, coagulase negative staphylococcus were moxifloxacin. The resistance rates were 83% and 64% to ciprofloxacin, 80% and 17% to clindamycin.

Implant system의 매몰조건과 소환온도에 대한 조사 (Research for Implant system according to investment condition and Bum out temperature)

  • This study was investigated on effect of implant system about investment condition and burn out temperature. Posterior bridges were fabricated by using plastic sleeve and gold sylinder. The results were as follows; In plastic sleeve test and fitting cast copings. the phosphate bonded investment showed better gypsum bonded investment. The pure special liquid showed better margin fitness compared with mixed liquid (special liquid + distilled water). In gold cylinder test. the good burn out temperature of Ceramic PFG and Crown Gold was evaluated $0^{\circ}C$ to $820^{\circ}C$ and was decreased on $780^{\circ}C$ for 30 minutes. $0^{\circ}C$ to $750^{\circ}C$ and was decreased on $650^{\circ}C$ for 30 minutes.

Evidence to Support the Therapeutic Potential of Bacteriophage Kpn5 in Burn Wound Infection Caused by Klebsiella pneumoniae in BALB/c Mice

  • The emergence of antibiotic-resistant bacterial strains is one of the most critical problems of modern medicine. Bacteriophages have been suggested as an alternative therapeutic agent for such bacterial infections. In the present study, we examined the therapeutic potential of phage Kpn5 in the treatment of Klebsiella pneumoniae B5055-induced burn wound infection in a mouse model. An experimental model of contact burn wound infection was established in mice employing K. pneumoniae B5055 to assess the efficacy of phage Kpn5 in vivo. Survival and stability of phage Kpn5 were evaluated in mice and the maximum phage count in various organs was obtained at 6 h and persisted until 36 h. The Kpn5 phage was found to be effective in the treatment of Klebsiella-induced burn wound infection in mice when phage was administered immediately after bacterial challange. Even when treatment was delayed up to 18 h post infection, when all animals were moribund, approximately 26.66% of the mice could be rescued by a single injection of this phage preparation. The ability of this phage to protect bacteremic mice was demonstrated to be due to the functional capabilities of the phage and not due to a nonspecific immune effect. The levels of pro-inflammatory cytokines (IL-$1{\beta}$ and TNF-${\alpha}$) and anti-inflammatory cytokines (IL-10) were significantly lower in sera and lungs of phage-treated mice than phage untreated control mice. The results of the present study bring out the potential of bacteriophage therapy as an alternate preventive approach to treat K. pneumoniae B5055-induced burn wound infections. This approach not only helps in the clearance of bacteria from the host but also protects against the ensuing inflammatory damage due to the exaggerated response seen in any infectious process.
