  • Purpose: Squamous cell carcinoma(SCC) of the lower lip is the most common malignant tumor comprising 90% of all lip SCC. The typical picture of SCC of the lower lip is of an ulcerated lesion with raised margins. Surgery is the treatment of choice for SCC of lower lip. Depending on the location and size of the tumor, different types of flaps are used. We used new method - 'both buccal mucosa transposition flap' for the reconstruction of the near total mucosal defect of the lower lip. Methods: This 67 - year - old men presented with the crusted $1cm{\times}1cm$ sized ulceration of the lower lip that was arised 30 years ago. There were no size and color change, except the bleeding and ulceration. At first, We diagnosed the SCC through the incisinal biopsy. Then We performed the wide excision of the tumor and reconstruction of the lower lip. After the excision of the whole tumor, the defect was measured at $8cm{\times}3.5cm$. We designed the buccal mucosa transposition flap taking care to avoid the parotid duct. The flap was made in a triangular shape for the reconstruction of defected lower lip. The donor site defect can be sutured primarily. Results: A patient in this study had no postoperative complications such as necrosis, dehiscence, infection of the flap or donor site. Reconstructed lower lip is relatively close to that of the natural lip; More satisfactory aesthetic and functional results can be obtained by using this technique rather than other techniques. Conclusion: 'Both buccal mucosa transposition flap' is reliable method for the reconstruction of the large lower lip mucosal defect. The operation is simple and performed in one stage, with no postoperative complications. This technique can offer consistently good functional and esthetic outcomes after reconstruction of lower lip mucosal defect.

Anti-cell Proliferative and Anti-angiogenic Potential of Andrographolide During 7,12-Dimethylbenz(a)anthracene Induced Hamster Buccal Pouch Carcinogenesis

  • Our aim was to explore anti-cell proliferative and anti-angiogenic potential of andrographolide by analyzing the expression pattern of cell proliferative (PCNA, Cyclin D1) and angiogenic (VEGF) markers during 7, 12-dimethylbenz(a)anthracene (DMBA) induced hamster buccal pouch carcinogenesis. DMBA painting three times a week for 14 weeks in the buccal pouch of golden Syrian hamsters resulted in oral tumors which were histopathologically diagnosed as well differentiated squamous cell carcinoma. Immunohistochemical (PCNA, VEGF) and RT-PCR (Cyclin D1) studies revealed over expression of PCNA, VEGF and Cyclin D1 in the buccal mucosa of hamsters treated with DMBA alone. Oral administration of andrographolide at a dose of 50 mg/kg bw to hamsters treated with DMBA not only suppressed the histological abnormalities but also down regulated the expression of PCNA, VEGF and Cyclin D1. The results of the present study suggest that andrographolide suppressed tumor formation in the buccal mucosa of hamsters treated with DMBA through its anti-cell proliferative and anti-angiogenic potential.

Buccal Mucosal Ulcer Healing Effect of rhEGF/Eudispert hv Hydrogel

  • We have studied the effect of rhEGF on the buccal mucosal ulcer healing. rhEGF was rapidly degraded upon incubation with the hamster buccal mucosal homogenates; The degradation of rhEGF was significantly inhibited by sodium lauryl sulfate (SLS). Eudispert hv hydrogel and Polycarbophil 974P hydrogel were prepared for rhEGF delivery and their mucoadhesiveness was measured by the $Instron^R$ method. The mucoadhesive force of Eudispert hv was significantly greater than that of Polycarbophil 974P. Moreover, rhEGF in Eudispert hv hydrogel remained stable for about 2 months. To evaluate the ulcer healing effect of rhEGF, the buccal mucosal ulcer was induced in golden hamsters using acetic acid. At 24 h after administration of rhEGF/Eudispert hv hydrogel, the ulcerous area was decreased compared with rhEGF solution and, as a result, the curative ratio was $36.8\pm5.68$%. By the addition of SLS (0.5%) to Eudispert hv hydrogel, the curative ratio increased 1.5 times. The mechanism of the action was probably due to a combination of protection of the drug against proteases present in mucosa and prolongation of the release of rhEGF from the formulation at the site of action.

The level of buccal gingival margin around single and two adjacent implant restorations: a preliminary result

  • STATEMENT OF PROBLEM. Little information is available about the buccal gingival level of multiple implant restorations. PURPOSE. This study was aimed to evaluate the relationship between width and height of buccal soft tissue around single and 2 adjacent implant restorations. MATERIAL AND METHODS. Four implant restoration groups (first and second molars, single second molars, posterior single restorations between teeth, and anterior single restorations between teeth) were randomly chosen from one dental institute. Each group comprised of 6 patients. After 6 months of function, silicone impressions were taken and stone models were fabricated for each restoration group. The stone models were cut in bucco-lingual direction at the most apical point of buccal gingival margin. The height and width of buccal supra-implant soft tissue were measured. One way ANOVA and Tukey HSD post hoc tests were performed to analyze the data obtained (P < .05). RESULTS. The most unfavorable width-height ratio was noted for the group, which was comprised of the second molar in the multiple adjacent (first and second molar) implant-supported restorations. The group also resulted in the shorter height of buccal supra-implant mucosa rather than that of anterior single implant restorations between natural teeth. CONCLUSION. To achieve a favorable level of buccal gingival margin, greater thickness of buccal supra-implant mucosa is required for the implant restorations without a neighboring natural tooth compared to the implant restorations next to a natural tooth.

  • 다형성 선종은 남성보다는 여성에게서 호발하고 발생연령은 40-60대의 중년에서 주로 발생하며 발생부위로는 소타액선의 경우 구개부와 상하순부에서 많이 발생하는 것으로 알려져 있다. 저자는 다형성 선종이 13세의 소아에서 그리고 협부에서 발생한 임상적으로 매우 드문 경우를 경험하였고, 외과적 절제술로 잘 치유되었으며, 소아의 협부에서 발생할 수 있는 여러 다른 종양과의 감별진단이 중요하리라 사료되어 이에 보고하는 바이다.

협점막판을 이용한 외상성 상순 결손부의 외과적 재건 (Reconstruction of Avulsed Upper Lip with Buccal Mucosal Flap)

  • The buccal mucosa is an effevtive and versatile intraoral grafting material. For adequate reconst ruction of avulsed lip, restoration of natural appearance and adequate lip function should be done. We treated avulsed upper lip defect which had been affected underlying orbicularis oris muscle as well as vermilion border. We chose cheek mucosa for upper lip reconstruction and performed surgical correction of avulsed upper lip with pedicled buccal mucosal flap. The result was very good because of its superior color and texture matching to remaining yermilion tissue.

  • There are major and minor salivary glands that aid in the digestive process. Major glands are discrete and exist in predictable locations; minor salivary glands are more widespread and usually found dispersed in the mucosa of the mouth. Glands have their own contractile abilities, which allow them to secrete products without the assistance of vasculature or skeletal, or smooth muscle. This study will describe a cadaveric histological specimen in which an ectopic buccal gland was embedded within bucinator muscle fibers. Potential causes and explanations for this finding will be discussed, as well.

흡연이 구강보건에 미치는 영향에 관한 연구 (A Study on the Oral Health of Smokers)

  • Few changes were found in the mouths of 52 smokers were examined as a part of program to evaluate the oral health of Korean smokers. 52 smokers and 30 non-smokers were performed careful oral examination. In many of smokers, heavy, black deposits are found on the buccal and lingual surfaces of the teeth and leukoplakia were obsreved in 2 subjects and varied merely in the severity of the involvement. Cytologic studies were carried out on 60 oral smears from the hard palate, buccal mucosa, and gingiva. Smears were taken wit a wood spatula scraped over the mucosal surface. After fixation the slides were stained using the papanicolau cells without nuclei. The cornified cells were smaller than the noncornified cells and stained a deep brown or orange as compared with the blue or red of the large, round or polygonal cells in nonkeratinized regions of oral mucosa. Abnormal cell forms, suggestive of malignancy were not found in smears from the mucosa of smokers. Cells with large hyperchromatic nuclei and elongated, spindle-type cells with large nuclei were not observed.

N-Nitrosamine과 단순포진성 바이러스가 햄스터의 구강암발생에 미치는 영향에 관한 실험적 연구 (Combined Effect of N-Nitrosamines and Herpes Simplex Virus on Oral Carcinogenesis in Hamsters)

  • We have previously reported that simulated snuff dipping in conjunction with type I herpes simplex virus (HSV-1) induced oral malignant changes in hamsters. Present study was designed to investigate the carcinogenic effect of tobacco specific-N-nitrosamines (TSNAs) and HSV-1, alone or in combination, in hamsters. Hamsters were divided into 6 groups and the right buccal pouch mucosa were treated as follows: Grp 1, Control (Mock inoculation) [MI]+Topical Application [TA] of mineral oil[MO] : Grp 2, TA of 1% n'- nitrosonornicotine [NNN] + IM: Grp3, TA of 1% 4-N-nitrosomethylamino-1- (3-pyridyl)-1-butanone [NNK] + MI: Grp 4, HSV-1 inoculation [HI]+TA of MO : Grp 5, TA of 1% N-nitrosonornicotine [NNN] + HI: Grp 6, TA of 1% NNK + HI. TA of MO or TSNAs was initiated 1 day after the MI or HI and given 3 times per week for 20 consecutive weeks. At the buccal pouches were fixed for light microscope examination. No animal s developed tumors or malignant histopathologic changes in the mucosa of the buccal pouches. These data indicate that individual TSNAs, alone or in conjunction with HSV-1 infection, do not develop malignant changes in hamster buccal pouches.

Successful Epithelialization Using the Buccal Fat Pad Pedicle in Stage 3 Bisphosphonate-Related Osteonecrosis of the Jaw

  • Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is defined as exposed necrotic bone without evidence of healing for at least 8 weeks in the maxillofacial area in a patient with history of bisphosphonate use. Obtaining complete coverage of the hard tissue by soft tissue in BRONJ patients is especially important. Therefore, managing the mucosa is one of the key factors in a successful outcome, but this is especially hard to achieve in BRONJ patients. Various applications of buccal fat pad in oral reconstruction-including the closure of surgical defects following tumor excision, repair of surgical defects following the excision of leukoplakia and submucous fibrosis, closure of primary and secondary palatal clefts, coverage of maxillary and mandibular bone grafts, and lining of sinus surface of maxillary sinus bone graft in sinus lift procedures for maxillary augmentation-have been studied. Eliminating all potential sites of infection and post-operative infection control is crucial in BRONJ. We present a case using the buccal fat pad pedicle for a stage 3 BRONJ defect. Uneventful total epithelialization of the buccal fat pad regardless of size was noted. In summary, the buccal fat pad has versatile application and various recipient sites for surgical utilization. It is an easy technique, with promising overall success rates. With careful selection and handling, buccal fat graft can resolve problems with soft tissue coverage in stage 2 or 3 BRONJ patients.
