외국인 이주노동자의 칫솔질행태와 구강건강상태 (Toothbrushing Behavior and Oral Health State of Immigrant Workers)

  • 본 연구는 다양한 나라에서 우리나라에 이주해 온 외국인 노동자 426명을 대상으로 일반적인 특성에 따른 칫솔질 행태와 구강건강상태, 칫솔질과 구강건강상태와의 관계를 분석한 결과 다음과 같은 결과를 얻었으며 통계적으로 유의한 결과이다. 일반적인 특성에 따른 하루 3회 이상 칫솔질 빈도는 여성이 63.0%로 남성보다 높았으며, 교육수준은 학력 수준이 높을수록 칫솔질 빈도가 높았다. 체류기간은 국내에 체류기간이 어느 정도 지난 경우 높게 나타났다. 건강보험은 전반적으로 건강보험이 가입된 경우 빈도가 높았다. 의료비 부담은 무료인 경우 50.0%로 가장 높았고, 친구 또는 정기적인 모임이 있는 경우 칫솔질 빈도가 높게 측정되었다. 칫솔질 부위 중 치아, 잇몸, 혀를 전체적으로 잘 닦는 경우는 연령이 적을수록, 미혼인 경우가 전체적으로 잘 닦고 있었으며 수입은 보통인 경우가 잘 닦고 있었고, 건강보험에 가입한 경우가 잘 닦고 있었다(p<0.05). 일반적인 특성에 따른 칫솔질 소요시간 3분 이상인 집단은 미혼인 경우와 건강보험에 가입된 경우가 칫솔질 소요시간이 길었으며, 칫솔질 교육유무에서는 국내 체류기간이 5년 초과인 경우와 정기적인 모임이 있는 경우가 칫솔질 교육을 많이 받았다. 구강위생용품은 성별은 여성, 학력은 학력수준이 높을수록, 체류기간은 3~5년 사이가 많이 사용하고 있었으며 의료비 부담은 무료인 경우와 정기적인 모임이 있는 경우 구강위생용품을 많이 사용한다고 나타났다(p<0.05). 일반적인 특성에 따른 DMFT index의 평균은 6.98, CPITN의 평균은 3.03으로 우리나라 2010년 국민구강건강 실태조사에서 성인 18세에서 44세의 DMFT index 5.9, CPITN은 1.6에 비하여 높은 것으로 나타났다. 성별에서는 DMFT index가 여성이 높았고, CPITN은 남성이 높았다. 연령, 결혼여부, 학력의 CPITN은 연령이 높을수록 높았으며 결혼여부는 기혼이 낮았다. 학력은 고졸 이하가 낮았다. 동거인이 없는 경우는 DMFT index가 높았고, CPITN은 동거인이 있는 경우가 2.97로 낮았다. 수입은 100만원 미만이 8.17로 DMFT index가 가장 높았고, CPITN은 100만원 미만이 2.75로 가장 낮았다. 건강보험이 있는 경우 DMFT index는 높았고, CPITN은 낮았다(p<0.05). 칫솔질 행태에 따른 DMFT index와 CPITN의 결과는 칫솔질 빈도 3회 이상에서 DMFT index가 8.34로 가장 높았으며, CPITN은 2.96으로 가장 낮았다. DMFT index는 칫솔질 행태가 좋을수록 높았으며 CPITN은 칫솔질 행태가 좋지 않을수록 낮게 나타났다. 상관분석 결과는 DMFT index에서 칫솔질 빈도와 칫솔질 부위, CPITN에서는 칫솔질 부위와 칫솔질 소요시간이 유의성을 나타냈다(p<0.05). 이상과 같은 결과를 바탕으로 외국인 이주 노동자들의 구강건강증진을 위한 질 높은 의료서비스를 제공하기 위해서는 치아우식증과 치주질환의 예방법으로 가장 기본인 칫솔질의 중요성을 인식시키고 올바른 칫솔질 관리를 위한 지속적인 교육이 이루어져야 할 것이다.

Preventive Cares for Orthodontic Dental Patients

  • In recent years, the numbers of the orthodontic dental patients have been rapidly increased in adolescent or young adult aged generation. It has been well known that it would be very hard to control the oral hygiene cares for orthodontic applied dental patient because of the complexity of the appliance. So the caries prevalence of the orthodontic dental patients would be higher than non-appliance persons, and it might be easy to cause the dental caries especially on the labial or buccal surface of the tooth through equipping the fixed type appliance with a long period, even though the alignment of the teeth would be arranged well. So, the massive preventive program for preventive dentistry should be needed for the dental patients for orthodontic treatment, in order to protect the dental caries and the periodontal disease for them. But, lots of the dentists or dental hygienists sometimes neglect of this point for preventive dental cares orthodontic dental patients, or do not know the importance and how to manage the skill for the preventive dental works in clinical. In this article, it will be introduced the basic theories and skills for preventive cares as tooth-brushing instruction, fluoride topical application and pit and fissure sealant, scaling and professional mechanical tooth cleansing and the diet control, for the dental patients with the fixed type of the orthodontic appliance, in case by case.

Antimicrobial effect of different brushing frequencies with fluoride toothpaste on Streptococcus mutans and Lactobacillus species in children with fixed orthodontic appliances

  • Objective: To evaluate the antimicrobial effect of different frequencies of brushing with fluoride toothpaste on the levels of salivary mutans streptococci and lactobacilli in children undergoing fixed orthodontic treatment. Methods: The study included 22 patients scheduled for fixed orthodontic therapy distributed between 2 groups with different hygiene regimes. All the subjects received identical braces, bands, and brackets bonded with the same material. Stimulated saliva samples were obtained before placement of the appliance and at 6, 12, and 18 weeks during the therapy. Saliva samples were cultured on selective microbial agar for the detection of microorganisms. Results: Salivary mutans streptococci were significantly suppressed throughout the experimental period in the group that brushed 4 times a day as compared to the group that brushed twice a day. Salivary lactobacilli were not significantly affected by the frequency of brushing with 0.32% sodium fluoride (NaF) toothpaste. Conclusions: The use of 0.32% NaF-containing toothpaste more than 3 times a day has effective antimicrobial activity on mutans streptococci but not lactobacilli in the saliva of children with fixed orthodontic appliances.

수종의 구취처치법의 구치감소효과에 관한 연구 (Effect of Tongue Scraping, ZnCl2 Mouth Rinse, and Periodontal Treatment on the Reduction of Oral Malodor)

  • This study was performed to investigate the effect of three modalities commonly used for reduction of oral malodor and to review the contributing factors for halitosis. For this study, 45 subjects of good general health in their third decades were selected, and they were divided into three groups by each modality, that is, tongue scraping, $ZnCl_2$ mouth rinse, and periodontal treatment. Subjective sense of halitosis, diets, Oral hygiene index based on tooth brushing and tongue brushing habit, resting salivary flow rate, gingival index, degree of tongue coating were recored in the Halito-Chart designed by the author. Oral malodor converted from volatile sulfur compound was measured by the Halimeter$^{(R)}$(Interscan Co., USA), and the correlation between the oral factors and pre-treatment halimeter score were analysed. Data obtained were analysed by the SPSS windows program and the results of this study were as follows : 1. There were no significant correlation between oral factors such as oral hygiene index, salivary flow rate, gingival index, degree of tongue coating and pre-treatment halimeter score. 2. Difference between pre- and post-treatment halimeter score by the three modalities were significant, but correlation between pre- and post-treatment score within group was most significant in the periodontal treatment group followed by the tongue scraping group and lowest in the $ZnCl_2$ mouth rinse group. 3. Reduction of halimeter score was significant only in subjects with relatively more tongue coating in the tongue scraping group, but in the $ZnCl_2$ mouth rinse group and in the periodontal treatment group, the modality applied to each group had same significant effects without regard to the degree of oral hygiene index or gingival index. 4. Men had more tongue coating than women, and reduction of halimeter score was significant in men, but not in women.

군복무중인 성인남자의 치주상태에 관한 연구 (EPIDEMIOLOGICAL ANALYSIS ON PERIODONTAL STATUS ON MILITARY POPULATION IN PERIODONTAL DISEASE BY MEANS OF THE BANA ASSAY)

  • The purpose of this epidermiological analysis was to evaluate the periodontal status of Korean young adults(twenties) in order to provide detail & baseline data for frequence of periodontal disease. Two hundred and fifty young adults, aged 20-29 years, were selected by random sampling. Dental visity, scaling treatment, education, income, toothbrushing frequence & method were checked, and plaque index(Loe and silness), calculus index(Ramfjord), gingival index(Loe and silness), attached gingival width, perio probing depth, gingival recession were measured. The obtained results were as follows. 1. Average plaque index(1.96), calculus index(1.43), gingival index(1.7) were higher in mandible than maxillar. It was most prevalent in lst molar. 2. Average attached gingival width(4.0mm) was wider in maxillar than mandible. It was most prominent in lateral incisor. 3. Pocket depth(>4mm) was distributed in 42% subject, it was higher in mandible than maxilla and most prevalent in 1st molar. 4. Gingival recession(>1mm) was distributed in 94% subject, it was higher in mandible than maxilla, and most prevalant in canine. 5. According to unpaired t-test, palque index, calculus index, gingival index were not statistically significant in history of scaling treatment, level of eduction and account of income, but were showed statistically significant in histrory of dental clinic.(PB0.05) 6. According to ANOVA test, correlation between tooth-brushing(frequence, method) and gingival index was showed statistically significant.(P<0.05) 7. There was gingival recessionof 87% subject in only one time brushing, 80% subject in two time, and 68% subject in three times. There was gingival recessionof 68% subject in leftright direction tooth brushing, 73% subject in upper-low method and 77% subject in combination method.

Oral Care Behavior according to Orthodontic Treatment Experience: Use of the 9th National Health and Nutrition Survey

  • Among all subjects who participated in the survey in the 9th 1st year (2022), 2,448 men (44.0%) and 3,166 women (56.0%) aged 12 or older were identified to determine oral care behavior according to orthodontic treatment experience. The purpose was to provide basic data on the oral care of patients wearing orthodontic appliances. 1. When it comes to brushing teeth, brushing before going to bed was the highest. Regarding the use of oral care products, 3,965 people (69.1%) said they 'do not use dental floss', 4,064 people (73.2%) said they 'do not use interdental brushes', and 4,064 people (73.2%) said they used oral rinse. 'I don't do it' was the highest at 4,417 (77.7%), and electric toothbrushes were the highest at 'I don't do it' at 5,241 (93.6%). 2. In terms of the effect of orthodontic treatment experience on toothbrushing time, with orthodontic treatment experience, toothbrushing after breakfast was 0.598 times more likely (P<0.001), and toothbrushing after lunch was 1.482 times more likely (P<0.001). Toothbrushing after dinner was 0.805 times higher (P=0.049), and toothbrushing before going to bed was 1.794 times higher (P<0.001), which was statistically significant. 3. When having orthodontic treatment experience, dental floss was found to be used 1.434 times more often (P=0.002), interdental brushes were used 1.464 times more times (P<0.001), and oral rinses were found to be used 1.457 times more times (P=0.002), which was statistically significant.

기모직물의 가공공정 및 기모횟수가 역학적 특성치에 미치는 영향(II) -드레이프성 및 방추성을 중심으로- (Effects of Treatment Process and Brushing Condition on Mechanical Properties of Brushed Fabrics (II) -Drapability and Wrinkle Recovery of Brushed Fabrics-)

  • This study was accomplished to investigate drape coefficient and wrinkle recovery of brushed fabrics according to treatment processes and brushing conditions. For this study, grey fabrics of blended ratio of polyester 65% : cotton 35% were bleached and dyed. These fabrics were then brushed 1, 3, 5 and 7 times. Drape coefficient and wrinkle recovery of brushed fabrics were measured by drape tester and shirley crease recovery tester respectively. The experimental results were analysed statistically to relate mechanical properties, handle, drapability, and wrinkle recovery 1) Drape coefficients of dyed fabric showed lower values and wrinkle recovery showed higher ones than those of bleached and dyed fabrics. Especially dyed fabric brushed 3 times showed good appearance and form stability. 2) Considering the blocked properties that contribute to the drape coefficient and wrinkle recovery of brushed fabrics, bending and shearing properties had a high degree of correlation with brushed fabrics, and 2HB/W showed a high positive correlation with drape coefficient and a high negative correlation with wrinkle recovery.

일개대학 National Competency Standard (NCS) 교과목 운영에 관한 연구: 예방치학 및 실습 교과목 중심으로 (Development and implementation of national competency standard: based on preventive dentistry and practice)

  • Objective: The purpose of the study is to investigate the curriculum development and operation based on national competency standard (NCS). Methods: The duty of the dental hygienist was analyzed based on DACUM by ten experts in January, 2011. The duty model of the dental hygienist was inspected after duty analysis. The subjects of choice were preventive dentistry and practice. The satisfaction with the subjects were carried out from March to June, 2015. Results: The duty analysis of dental hygienist by DACUM produced preventive dental treatment(11 tasks), oral health education(3 tasks), comprehensive dental hygiene treatment(6 tasks) and 12 categories(156 tasks). Preventive dental treatment was divided into preventive dentistry and practice, oral health education was changed into oral health education and practice, and comprehensive dental hygiene treatment was replace by comprehensive dental hygiene and practice. The contents of preventive dentistry and practice included outline, learning objective, related knowledge and self evaluation. Professional evaluation required mutual experience and evaluation of the students. The mutual evaluation of the students was $4.61{\pm}0.506$(dental plaque control) and $1.80{\pm}0.316$(tooth brushing). The professional evaluation was $1.73{\pm}0.274$(dental plaque control) and $1.60{\pm}0.322$(tooth brushing)(p<0.01). The satisfaction with preventive dentistry and practice was $4.61{\pm}0.506$(improvement in practical work ability), $4.58{\pm}0.511$(knowledge improvement) and $4.55{\pm}0.572$(NCS educational environment) in order. Conclusions: The operation of NCS curriculum is considered to improve practical work ability and to solve skill mismatch between dental industries and educational training institutions.

지각과민 처치제 후 접착레진 처리가 상아질 투과도에 미치는 영향 (The effects of desensitizing agents, bonding resin and tooth brushing on dentin permeability, in vitro)

  • 목적: 상아질 지각과민증의 치료를 위해 사용되는 지각과민 처치제의 효과는 일시적인 것으로 보이며 이는 상당부분 칫솔질에 의한 마모소실로 판단된다. 지각과민 처치제의 칫솔질에 의한 마모를 감소시키기 위해 bonding resin을 지각과민 처치제에 추가 도포하여 칫솔질 시행 후 상아질 투과도의 변화에 대해 연구하였다. 재료 및 방법: 우식이 없는 치아를 이용하여 1 mm 두께의 치관부 상아질 디스크를 제작한 다음, All-Bond 2$^{(R)}$, Seal & Protect$^{(R)}$, Gluma$^{(R)}$, MS Coat$^{(R)}$의 4 종류의 지각과민 처치제만 처리한 시편과 Dentine/Enamel Bonding Resin$^{(R)}$ (Bisco Inc.)을 추가로 도포한 시편에 대해 처리하기 전과 처리한 후, 1주(왕복 140회), 2주(왕복 280회), 6주(왕복 840회)에 해당하는 칫솔질을 적용한 후에 측정한 hydraulic conductance (Lp)와 주사 전자 현미경 사진을 비교 분석하였다. 결과: 지각과민 처치제 만을 처리한 군과 bonding resin (Dentine/Enamel Bonding Resin$^{(R)}$)을 추가 도포한 군 모두에서 처리 직후 hydraulic conductance가 급격히 감소하였다. 지각과민 처치제만 처리한 군과 bonding resin을 추가 도포한 군 간의 hydraulic conductance를 비교한 결과 All-Bond 2$^{(R)}$, Gluma$^{(R)}$, MS Coat$^{(R)}$와 bonding resin을 도포한 군에서는 1주(왕복 140회), 2주(왕복 280회), 6주(왕복 840회)의 칫솔질을 시행한 후 hydraulic conductance가 지각과민 처치제만 도포한 군에 비해 현저히 낮았고, Seal & Protect$^{(R)}$ 에서는 6주(왕복840회)의 칫솔질을 시행한 후에만 같은 결과를 보였다. 주사 전자 현미경 관찰에서 지각과민 처치제만 처리한 군과 bonding resin을 추가로 도포한 군 모두에서 처리 직후에 상아세관이 완전히 폐쇄되거나 일부만 폐쇄되어 상아 세관의 내경이 감소된 양상을 볼 수 있었으나, 6주(왕복 840회) 칫솔질 시행 후에는 bonding resin을 추가 도포한 군에서만 현저하게 폐쇄 또는 내경이 감소된 상아세관이 관찰되었다. 결론: 지각과민 처치제 처리 후 bonding resin (Dentine/Enamel Bonding Resin$^{(R)}$, Bisco Inc.)을 추가 도포하는 것이 지속적인 상아세관 폐쇄에 효과적임을 알 수 있었다.

구취의 심도에 따른 치료 효과에 대한 비교 연구 (Treatment Efficacy on Oral Malodor according to Pre-treatment Volatile Sulfur Compound Level)

  • Considering various factors contributing oral malodor, the accurate prediction of prognosis is very important to both clinician and patients. The present study has been performed to invetigate the relationship between treatment effeicacy and pre-treatment volatile sulfur compounds (VSC) level. Ninety patients were divided into three groups, A(<150ppb), B(150< <200ppb), and C(>200ppb) groups, according to pre-treatment VSC level detected by Halimeter, and each group included 30 patients. Routine therapeutic measures for oral were provided to each patient which consisted of oral prophylaxis, tooth brushing and flossing instruction, tongue scraping by proper device, and gargling of 0.25% ZnCl2 Solution. The group with high pre-treatment VSC level (>150ppb) showed significant reduction of VSC level at 1 and 3 weeks after. However, the group with low pre-treatment VSC level (<150ppb) did not show any significant reduction during the experimental periods. Collectively, the results suggested that patients with high pre-treatment VSC level show better prognosis.
