  • This study was conducted to observe the condition of the ramification of the bronchus and pulmonary blood vascular system by injection of vinylite into the bronchial tree and pulmonary blood vessels in normal adult swines. The results obtained were summarized as follows. 1. Lungs of swine were composed of the same pulmonary territories as in lungs of human and dog. 2. Bronchial tree of swine also were axial divergency in the patterns. 3. Ramification of the left and right apical lobes are especially complex patterns but cardiac and diaphragmatic lobes are a little monotonous. 4. Intermediate lobe corresponding to mediobasalis branch of human lungs formed only one lobe in swine lung. 5. Pulmonary artery of right apical lobe was mono branch form in all case by authors observation. 6. $B._2$ streched into the seg. dorsalis of the right apical lobe was especially developed compare to $B._1$, $B._3$ of the seg, apicalis and seg, ventralis.

Aortobronchial Fistula in a Chronic Traumatic Aortic Aneurysm - One case - (대동맥 기관지루)

  • An aortobronchial fistula is a rare complication of aneurysm of the aorta. The fistula starting from a chronic traumatic aortic aneurysm is exceptionally rare. Our observation concerns a patient of 26 with previous chest trauma who had atelectasis of left lung following dyspnea and hemoptysis. Aortography and surgical intervention revealed that this was a chronic traumatic aortic aneurysm of descending thoracic aorta, which developed a fistula in the bronchus. She underwent left posterolateral thoracotomy and the surgical repair of the aneurysm was performed with a woven Dacron patch graft using a temporary external bypass between the ascending and the descending aorta. The fistula in the bronchus was closed with simple interrupted sutures. In the immediate postoperative period, double vision, headache, and hoarseness developed but returned normal.

  • Benign papillary tumors commonly arise in the upper respiratory tract but are rare in the lower respiratory tract, especially in the bronchi. Solitary benign papillary tumors arising at the bronchi,covered with stratified squamous epithelium and associated with little or no inflammatory cell response,seem to be rarest of all types of benign bronchial tumors. Only six cases of such tumors had been reported in the medical literature over the past 74 years until 1968. The following is a case report of solitary benign papilloma arising at the opening of the left main bronchus which was treated with bronchoscopic removal at Chest Surgery Depariment, N.M.C. on Sept.

  • Rupture of the main bronchus due to blunt chest trauma is very rare, especially In childhood although the incidence is increasing. Early diagnosis and primary repair not. only restore normal lung function but also avoid the difficulties and complications associated with delayed diagnosis and repair. We experienced 2 cases of right main bronchial rupture caused by traffic accidents. Patients suffered from progressively developing dyspnea and subcutaneous emphysema on the neck, anteriorchest,andanteriorabdominalwall. Emergency operations were performed through right posterolateral thoracotomy incision at the 4th intercostal space. Intraoperatively, the right main bronchus completely transsected and separated. Corrective bronchoplasty was performed with end-to-end anastomosis using interrupted suture with 3-0 Vicryle and the suture line was reinforced with azygos vein and parietal pleural flap. Postoperative courses were uneventful and patients discharged without any specific pro lems.

Congenital Bronchoesophageal Fistula: A report of one case (선천성 기관지 식도루 -H-자형 수술 1례보고-)

  • A case of congenital bronchoesophageal fistula was treated surgically. A 45 year-old woman has been suffered from coughing with liquid diet and recurrent pneumonia for 20 years. Esophagogram and bronchoscopy revealed a fistula tract between midesophagus and right lower lobe bronchus just opposite site of the orifice of the superior segmental bronchus. Preoperative laboratory results were normal and Mantoux test was also negative. The fistula was dissected without difficulty and the lumen was covered with intact mucosa and there were no calcified lymph nodes around the lesion. The fistula was divided and closed with interrupted silk sutures on both sides. The postoperative course was uneventful.

  • Most of the patient with endobronchial tuberculosis have some degree of bronchial stenosis. however, a part of bronchial stenosis need aggressive treatment for the patency because of severe symptoms. The self-expendable metallic stents provide palliative treatment for narrowed airways where surgical resection is inadvisable. We experienced a successful left wedge pneumonectomy on a 29-year-old woman with obstruction of left main bronchus due to complication of the bronchial stent. She had inserted self-expendable metallic stents on left main bronchus of the tuberculous bronchial stenosis two times. There was no specific postoperative complication.

  • As a safety measure, dentures are routinely removed before surgery. Aspiration of a denture could be catastrophic, with medicolegal implications. Foreign body aspiration is uncommon in adults; however, aspirations may remain asymptomatic and undiagnosed for long periods of time. We report an adult male who presented with a cough for more than 6 months. On radiography, a foreign body was found migrating within the tracheobronchial tree from one mainstem bronchus to the other, at different time points. The foreign body was later found to be a portion of his denture. The aspiration may have occurred at the time of a surgical procedure.

  • Choroidal melanoma is the most common primary intraocular cancer in adults. The predominant sites of metastasis that are associated with a poor prognosis are liver, lung and bone. The authors report here on a case of metastatic choroidal melanoma in the rib and bronchus, and this was all treated by surgical resection.

  • A 2-year-old, female Pomeranian dog was referred for dyspnea. Thoracic radiographs revealed left-sided mediastinal shift, increased soft tissue opacity in the caudal aspect of left thorax with loss of the left diaphragmatic silhouette, and dorsal elevation of mediastinal structures and heart from the sternum by lung tissue. The left main bronchus was visualized as an air-bronchogram and observed to abruptly discontinue at the level of the 10th rib. Thoracic computed tomography (CT) revealed absence of the left lung parenchyma and left pulmonary vessels with a rudimentary left main bronchus. The case was congenital pulmonary aplasia diagnosed via radiography and CT.

  • Broncholithiasis is defined as a condition in which a concretion is present within a bronchus or a cavity in the lung communicating with a bronchus. Broncholithiasis, although recognized in antiquity by Aristotle and well known to accompany the mediastinal calcifications of tuberculosis and histoplasmosis frequently seen in our society, is rarely mentioned in recent medical literature. Esophagobronchial fistula is an uncommon finding in broncholithiasis. This report is a case of esophagobronchial fistula with broncholithiasis in 58-year-old male patient who complained paroxysmal coughing after ingestion of fluids. The fistulous tract was successfully resected and reinforced by mediastinal pleura. Broncholithiasis was confirmed by lithoptysis as the cause of previous esophagobronchial fistula.
