  • Primary malignant lymphomas of the lung are rare neoplasms representing 3.6% of all extranodal lymphomas and less than 1% of pulmonary malignancies, with its distinct clinicopathological features. A 60-year-old symptomatic man, who was a former non-smoker, had a cough and sputum. Chest roentgenogram and computed tomographic scan showed the presence of a posterior mass in the basal segment of the right lung. Transthoracic needle aspiration cytology was inadequate for the diagnosis of cancer. A subsequent right thoracotomy was carried out, which showed a primary pulmonary lymphoma of the bronchus-associated lymphoid tissue.

  • Mucoepidermoid carcinoma of the lung is a very rare and potentially malignant tumor that arises from submucosal gland of the trachea and bronchi. We experienced two cases of mucoepidermoid carcinoma which was located in the left main bronchus and right intermediate bronchus. They were treated with left sleeve resection and right bilobectomy. Postoperative course was uneventful, and the followed up for each of them was 3 year and 18 months without any evidence of recurrence.

  • We report very rare case of concurrent bronchial atresia and bronchogenic cyst. Morphologic apical segment of right upper lobe directly stemmed from right main bronchus. Bronchogenic cyst was communicating with atretic segmental bronchus and both were filled with mucus. The etiology is not well known, however it is likely that a single insult arround the 5th∼6th week causes both malformations.

  • We present an interactive virtual bronchoscopy method, which uses a tree structure of the objects and physically based camera control model. The proposed method archieves faster response by rendering only visible branches using the tree structure of the bronchus. A collision detection algorithm supplies a convenient and intuitive mechanism for examining the bronchus inner surface while a voiding collisions. We have improved the performances of navigation speed in virtual bronchoscopy.

  • Extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue type (extranodal MZL) is a distinct subgroup of non-Hodgkin's lymphoma. Pulmonary extranodal MZL is a rare entity and accounts for less than 0.5% of primary pulmonary malignancies. Only a few cases of simultaneous occurrence of lung cancer and pulmonary extranodal MZL have been reported. A 60-year-old woman was referred to our hospital with a pulmonary nodule. She was diagnosed with lung adenocarcinoma by percutaneous needle biopsy. The protrusions into the left main bronchus were found by accident while performing bronchoscopy during lung cancer evaluation. The bronchial lesions were diagnosed as extranodal MZL. Although the patient underwent surgical resection for the lung adenocarcinoma, the pulmonary extranodal MZL was left untreated; it was monitored during follow-up visits. To our knowledge, this is the first report of synchronous lung adenocarcinoma and primary extranodal MZL of the main bronchus.

  • Pulmonary hamartoma is relatively common among the benign tumors of lung, and is credited to Albrecht who In 1904 described a disorganized arrangement of tissue normally present In an organ. But hamartoma has rare incidence of total pulmonary tumors, and especially endobronchial origin is extremely rare. We have experienced three cases of pulmonary hamartoma Including one bronchial origin. First case was a 27 years old woman who had multiple hamartomas, located all in right middle lobe and middle lobectomy was performed. Second case, a 56 years old woman, had endobronchial hamartoma, l x l .5x l .8cm in size and located at right intermediate bronchus. Right pneumonectomy was Inevitable because of It`s proximal location near the hilum. Third case, a 55 years old man, revealed sclerosing hemangioma microscopically, 4x4x5 cm In size, in right lower lobe and right lower lobectomy was performed. Postoperative course of all of them were uneventful and discharged with good general condition on the ~ 4th to 26th day postoperatively.

  • Tracheobronchopathia osteochondroplastica (TO) is a rare idiopathic tracheobronchial abnormality characterized by diffuse cartilaginous and osseous nodules protruding into the airway lumen of the trachea and bronchus. TO is easy to misdiagnose because of nonspecific symptoms and chest CT scan with pathologic biopsy is necessary for definitive diagnosis. We report two cases of patient with TO who underwent laryngomicroscopic biopsy and tracheostomy with literature review.

Clinical analysis of expandble metallic stent in benign tracheal & bronchial disease (양성 기관, 기관지 질환에서 확장성 금속 스텐트 사용에 관한 임상적 고찰)

  • Background Insertion of tracheal stent in the treatment of benign tracheal & bronchial disease has increased since the introduction of expandable metallic stent. Material & Methods : Between Jan, 1995 and Feb. 2004, eight patients who had benign tracheo-bronchial disease underwent insertion of expandable metallic tracheal stent. We retrospectively analyzed stent insertion indications, complications, and following the result. Results : Surgical indications were post-intubation tracheal stenosis (1 case), tracheal stenosis following tracheal surgery (2 cases), tracheo-esophageal fistula (2 cases), broncho-pleural fistula(1 case), left main bronchus stenosis following bronchoplasty (1 case), and left main bronchus stenosis due to mediastinal repositioning (1 case). Expandable metallic tracheal stent was inserted in five patients to resolve dyspnea caused by airway obstruction, and to prevent recurrent pneumonia in three patients. The complication developed in 6 patients $75\%$; 3 cases of distal stenosis due to growth of granulation tissue, and one case each of tearing of posterior membrane, aggravation of tracheo-esophageal fistula, and airway partial obstruction due to stent migration. The stent was removed in 5 patients and tracheal surgery (tracheal resection and end to end anastomosis with primary repair of esophagus, pericardial patch tracheo-bronchoplasty, tracheal repair and omental wrapping) was performed in 3 patients. Conclusion Insertion of self expandable metallic stent in benign tracheo-bronchial disease is an effective means of relieving dyspnea for only a short period, and it did not increase the long term survival. Better means of treatment of benign tracheo-bronchial stenosis in necessary.
