The First Korean Case of Nontuberculous Mycobacterial Lung Disease Caused by Mycobacterium abscessus Subspecies bolletii in a Patient with Bronchiectasis

  • We report the first Korean case of lung diseases caused by Mycobacterium abscessus subsp. bolletii in a previously healthy male, except for a previous history of pulmonary tuberculosis and bronchiectasis. All serial isolates are identified as M. abscessus subsp. bolletii by multi-locus sequence analysis based on the hsp65, rpoB, and 16S rRNA fragments. At the genetic level, the isolate has the erm(41) gene with a T28 sequevar, associated with clarithromycin resistance, and no rrl mutation. The isolate is resistant to clarithromycin. Although the symptoms and radiographic findings have improved after combination of antibiotics, the follow-up sputum cultures are persistently positive.

기관지확장증 환자에서 환기 폐주사의 임상적 의의 (Clinical Significance of $^{99m}Tc-DPTA$ Ventilation Scan in Patient with Bronchiectasis)

  • To evaluate the clinical significance of lung ventilation scan using $^{99m}Tc-DTPA$ in patient with bronchiectasis, we compared the involovement area of bronchogram and lung ventilation scan according to lobar and segmental distribution. There were no correlation between impairment of pulmonary function test and the number of bronchiectatic lobe and segment(p>0.5). Lung ventilation scan showed 66.7% of sensivity, 100% of specificity, ana 91.7% of accuracy according to lobar distribution, and 51.9% of sensivity, 96.9% of specificity, and 88.9% of accuracy according to segmental distribution. These results suggest that lung ventilation scan can be used as diagnostic tool in patient with bronchiectasis in whom bronchogram is not tolerable.

개에서 발생한 낭상 기관지 확장증에 대한 컴퓨터단층촬영 소견 1례 (Computed Tomographic Features of Saccular Type of Bronchiectasis in a Dog)

  • 9년령의 중성화한 말라뮤트가 만성 기침 증상으로 내원하였다. 흉부 컴퓨터단층촬영상에서 말초 기관지가 확장된 모습으로 관찰되었다. 기관내강과 폐동맥 직경 비율은 약 2.2로 정상보다 큰 비율을 나타내었다. 확장된 기관지들은 특히 좌측 전엽과 후엽, 우측 중엽에서 포도송이 모양과 같은 형태를 보였다. 이러한 컴퓨터단층촬영 소견들을 바탕으로 낭상 기관지 확장증을 진단하였다.

  • CA19-9의 상승은 췌장 및 담도계 악성 종양을 시사하는 소견이다. 하지만, 최근에 양성 소화기계 질환 뿐 아니라 특발성 간질성 폐렴, 교원성 폐 질환, 미만성 범세기관지염, 기관지확장증 등 다양한 양성 폐 질환에서도 혈중 CA19-9 수치가 증가되어 있다고 보고된다. 저자들은 최근 지속적으로 혈중 CA19-9 수치는 증가되어 있지만, 내시경적 역류성 담도 췌장 조영술, 복부 전산화 단층 촬영, PET-CT에서 악성 종양이 발견되지 않은 기관지확장증 1예를 경험 하였다. 3년 10개월의 추적 관찰 기간 중에도 췌장암과 같은 악성 종양은 발견되지 않아서, 혈중 CA19-9 수치의 상승은 기관지확장증에 의한 것으로 추정 되었다. 혈중 CA19-9 수치의 상승은 환자의 임상 양상과 연관하여 조심스럽게 판단하여야 될 것으로 생각된다.

  • 기관지확장증은 일반적으로 원인을 찾기 위한 광범위한 조사없이 증상적 치료를 하는 경우가 많으나 기관지확장증을 유발할 수 있는 원인을 아는 경우 특별한 치료 방침을 정할 수도 있다. X연관 무감마글로불린혈증은 세포질 내 X 염색체에 존재하는 Bruton's tyrosine kinase (Btk) 유전자의 돌연변이에 의해 성숙 B 림프구가 분화하지 못하여 발생하는 체액성 면역결핍질환으로 반복적인 호흡기감염을 통해 기관지확장증을 유발한다. 이는 드문 유전적 질환으로 조기 발견과 주기적인 면역글로불린 치료가 중요하다. 저자들은 반복적인 감염의 과거력과 함께 미만성 기관지확장증을 가진 성인 남성에서 BTK 유전자의 돌연변이를 확인하여 X연관 무감마글로불린혈증을 진단하고 치료한 사례를 경험하여 이를 보고하는 바이다.

  • It has been known which the pulmonary mycosis usually results from secondary invader of preexisting bronchopulmonary diseases, e.g. pulmonary tuberculosis, bronchiectasis, abscess, cysts, or pulmonary malignancy and as the predisposing factor appears to be concerned with long-term therapy of several antibiotics, steroids, or chemotherapeutic agents, etc. Recently, it has been interested rather the diagnosis and pathogenesis of then pulmonary mycosis than the treatment, especially because of some difficulty of the diagnosis. The authors experienced too cases of the pulmonary mycosis which were treated onc case medically, as moniliasis and the other surgically, as aspergillosis. There was noticed that our pulmonary moniliasis developed probably due to long-term therapy of antibiotics and aspergillosis resulted from secondary invader in the previous cyst of bronchiectasis.

남매에서 발생한 Kartagener씨 증후군 - 2례 보고 - (Kartagener's Syndrome: A Report of 2 Cases (Review of the literature and report of two cases in same family))

  • Kartagener`s syndrome is a clinical entity composed of situs inversus, bronchiectasis and chronic sinusitis. This syndrome is rare and is usually detected in childhood. Whether the bronchiectasis in Kartagener`s syndrome is congenital or acquired is still controversial. Some familial cases of this syndrome have been reported, and in these cases the genetic pattern is compatible with an autosomal recessive inheritance with some degree of pleotropism. The authors experienced having 2 cases of Kartagener`s syndrome among 6 siblings in a family. The case No. 1 have had bilateral Caldwell Luc operation, posterior ethmoidectomy, left upper Iobec-tomy with excellent result. The case No. 2 denied to have surgical management.

기관지확장증의 외과적 요법 (Surgical Treatment of Bronchiectasis)

  • During the past ten years from 1972 to 1981, a total of 100 cases of bronchiectasis were treated by pulmonary resection at C.A.F.G.H. Pulmonary tuberculosis and frequent U.R.I. were the most frequent associated disease and encountered in 54% in this series. Various types of pulmonary resection were performed on 100 patients; left lower lobectomy in 40 cases, left lower lobectomy and lingular segmentectomy In 29 cases, right lower lobectomy in 12 cases, right middle and lower lobectomy in 12 cases, lingular segmentectomy in 3 cases, left pneumonectomy in 3 cases and both lower lobectomy in 1 case. Complications developed in 9 cases and 1 case among them died of sepsis following secondary opera-tion. Among complications of 9 cases, postoperative atelectasis showed in 4 cases, hemorrhage in 2 cases, bronchopleural fistula in 2 cases, pulmonary edema in 1 case. Complications were treated by conservative and secondary operative management with satisfactory result except one death. Remainders without complication showed good result without symptom in postoperative and follow-up periods.

폐 국균증에 대한 외과적 치료 (Surgical Treatment of Pulmonary Aspergillosis)

  • We experienced eleven cases of pulmonary aspergllosis treated surgically in the period from 1981 to 1992. There were 5 men and 6 women, ranging in age from 28 to 64 years [mean age 40.4 years]. The most common chief complaint of the patients was hemoptysis and blood tinged sputum[7 cases, 63.6%], On preoperativechest film, the case of cavity with fungus ball[7 cases] and only cavity[4 cases] were seen. The location of the lesion were both upper lobe[6 cases] and lower lobe[5 cases]. The underlying disease were tuberculosis[5 cases], bronchiectasis[2 cases], tuberculosis and bronchiectasis[1 case], pneumonia[1 case] and none[2 cases]. The operative procedures of pulmonary aspergillosis were lobectomy[8 cases], cavernostomy[1 case] and thoracoplasty[2 cases]. The postoperative complications were postoperative massive bleeding[reoperation, 2 cases], wound infection[2 cases] and no operative mortality.
