익수사고자에 대한 효과적인 응급처치 방법 (An effective emergency care of a person from water submersion)

  • This study was to exhibit the effective emergency care method for the drowning and non-drowning who are reached two-thousand peoples every year in our country. For investigate the effective emergency care, this study was discussed as follows ; Pathophysiology of the water submersion, Fresh-water & sea-water drowning, Factors affecting survival, and Prehospital management. The conclusions from this study were summarized as follows; 1. Remove the patient from the water. If you suspect neck or spinal injuries, Always support the head and neck level with the back and, begin rescue breathing. 2. Maintain the airway and support ventilation in the water use the jaw-thrust technique to avoid farther injury to the neck or spine. We might encounter more resistance to ventilations than you expect because of water in the airway. Once you have determined that there are no foreign objects in the airway, apply ventilations with more force; adjust ventilations until you see the patient's chest rise and fall but not until you see gastric distention. Do not attempt to remove water from the patient's lungs or stomach. 3. If there is no pulse, begin CPR. 4. Administer high-flow supplemental oxygen; suction as needed. 5. Once the patient is breathing and has a pulse, assess for hemorrhage; control any serious bleeding that you find. 6. Cover the patient to conserve body heat, Handle the patient very gently, and, Transport the patient as quickly as possible to Emergency Department, Continuing resuscitative measures during transport. If the patient have the hypothermia, follow hypothermia management.

한국항공우주연구원 스크램제트 엔진 시험설비의 개발 (Development of the Scramjet engine Test Facility(SeTF) in Korea Aerospace Research Institute)

  • 한국항공우주연구원은 2000년부터 극초음속 공기흡입식 추진기관 시험설비의 설계와 개발에 착수하여 2009년 7월 시험설비의 구축을 완료하였다. 스크램제트 엔진 시험설비(SeTF)로 명명된 본 시험설비는 자유제트 형식 시험부를 갖춘 불어내기식, 고 엔탈피 풍동으로 고압공기 공급원, 고온 공기 공급시스템, 엔진 시험부, 연료 공급시스템, 설비 제어 및 데이터 처리 시스템 그리고 배기 시스템으로 구성되어 있다. 본 논문에는 SeTF의 설계, 사양을 소개하였으며 현재 수행 중인 SeTF의 특성 파악 시험에 대한 소개 및 일부 시험 결과를 정리하였다.

한국무용 걸음체 동작 시 호흡의 사용유무가 하지의 운동역학적 변인에 미치는 영향 (The Effects of Breathing Control on Kinetic Parameters of Lower Limbs during Walking Motion in Korean Dance)

  • 본 연구는 한국무용 시 내면의 아름다움뿐만 아니라, 외면으로 표현되고 기술적으로 한층 세분화된 한국무용의 호흡에 대한 중요성을 인식하는 연구의 필요성을 갖고, 호흡의 사용이 한국무용 걸음체 동작에 어떠한 영향을 미치는 가를 분석하여, 무용의 추상적인 아름다움을 과학적인 근거로 명확하게 제공하는 데 연구의 목적을 갖는다. 연구 결과, 호흡을 사용한 걸음체가 호흡을 사용하지 않는 걸음체 보다 신체중심과 골반이 이동변위가 낮게 나타났다. 슬관절과 족관절의 각변위에서 호흡을 사용하는 걸음체가 호흡을 사용하지 않는 걸음체보다 굴곡되어 굽힘이 많이 사용되었고, 이것은 굴신이 일어나고 있는 시전에서 나타났다. 호흡을 사용한 걸음체 동작은 수직지면반력에서 호흡을 사용하지 않은 걸음체에서 볼 수 없는 pre정점이 나타났고 이것은 한국무용 걸음체 시 호흡이 시작되는 들숨의 시점을 의미하며, 뒤꿈치 닿는 구간(1정점)에서 호흡을 사용한 걸음체의 부하 값이 날숨에 의해 호흡을 사용하지 않는 걸음체 보다 부하 값이 더 크게 나타났다. 전후지면반력 결과 호흡을 사용한 걸음체가 제동력이 더욱 크게 나타났다.

Geant 4 시뮬레이션 코드를 이용한 호흡 동조 방사선치료의 유용성 비교 (Comparison of Practical Usefulness of Respirational Radiation Treatment Using Geant 4 Simulation Code)

  • 인체 내부의 움직임을 확인하기 위해서 CT에서 얻은 DICOM 파일과 Geant4 코드를 이용하여 폐암환자를 모사하였다. 자체 제작한 Moving Phantom에 가상의 종양을 만들어 종양의 움직임이 호기와 흡기에 위치했을 때 종양의 움직임을 측정하는 방법과 이것을 적용하여 Moving Phantom을 통한 치료계획시의 선량분포와 호흡동조 폐암환자 PTV내의 종양의 정확도를 확인하고자 한다. 움직이는 영역이 3cm 이상이라도 40~70 % 구간에서 호흡 동조 방사선치료를 시행 할 경우 97% 이상 효과적임을 확인하였다. 움직이는 표적에 호흡 연동 방사선치료를 적용한 선량 분포인데 이는 구동 팬텀 내 필름으로 측정한 것으로 90%가 포함되는 선량분포의 오차범위 3mm이내에 분포됨을 알 수 있었다. 실제 환자 호흡곡선의 경우 호기 상태의 시간이 사인곡선에 비하여 길기 때문에 치료시간이 이보다 짧을 수 있음을 확인하였다.

마스크 필터가 만성 뇌졸중 환자의 호흡기능에 미치는 영향 (Effect of Mask Filter on Respiratory Function in Chronic Stroke Patients)

  • Purpose : This study investigated the effects of wearing a mask and different mask filters on the respiratory function of stroke patients. Methods : A total of 15 stroke patients were selected according to the inclusion and exclusion criteria. The respiratory functions were compared between participants with and without masks and among respiratory functions with three different mask filters. The order of using masks was non-wearing masks, Dental masks, KF80 masks, and KF94 masks; the difference in respiratory volumes among these conditions were measured. For accuracy of the measurement, sufficient education on the respiratory measurement method was provided to the researcher, and the heart rate of the participants was estimated to confirm their stability before the measurements. To ensure accuracy, the subjects were educated on the researchers' respiratory measurement methods. Each measurement was followed by 10 min breathing stability before replacing the next mask. Results : The results of this study showed that the difference in respiratory functions, including forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), and maximal voluntary ventilation (MVV), in stroke patients was statistically significant among different masks (p<.05). Afterwards, the values of FVC, FEV1, and MVV in stroke patients wearing masks were significantly lower than those of the non-masked control group (p<.05). The difference in respiratory functions with different mask filters showed no statistical significance (p<.05). Conclusion : This study showed that participants wearing any of the masks presented a lower respiratory function than that of those without using masks; additionally, no difference in respiratory functions was observed with differences in mask filters. Therefore, wearing a mask for a prolonged period is confirmed to affect breathing in stroke patients with weak respiratory function.

Integration of the Engine Control into the Optimal Trajectory Determination for a Spaceplane

  • In this paper are presented TSTO system analysis including some controlled variables on the engine operation such as a fuel flow rate and a pressure ratio of compressor, as well as variables on the trajectory. TSTO studied here is accelerated up to Mach 6 by a fly-back booster powered by air breathing engines. Three different types of engine cycle were treated for propulsion system of the booster, such as a turbo ramjet, a precooled turbojet and an EXpander cycle Air Turbo Ramjet (ATREX). The history of the controlled variables on the engine operation was optimized by Sequential Quadratic Programming (SQP) to accomplish the minimum fuel consumption. The trajectory was also optimized simultaneously. The results showed that the turbo ramjet gave the best fuel consumption. The optimal trajectory was almost the same except in the transonic range and just before reaching to Mach 6. The history of the pressure ratio of compressor considerably depended on the engine type. It is concluded that simultaneous optimization for engine control and trajectory is effective especially for a high-speed airplane propelled by turbojets like the TSTO booster.

  • The goal of this paper is to describe an advanced method of the fault diagnois using Control Theory with reference to a crack detection, a new way to localize the crack position under infulence of the plant disturbance and white measurement noise on a rotating shaft. As a first step, the shaft is physically modelled with a finite element method as usual and the dynamic mathematical model is derived from it using the Hamilton - principle and in this way the system is modelled by various subsystems. The equations of motion with crack is established by adaption of the local stiffness change through breathing and gaping from the crack to the equation of motion with un-damaged shaft. This is supposed to be regarded as reference for the given system. Based on the fictitious model of the time behaviour induced from vibration phenomena measured at the bearings, a nonlinear State Observer is designed in order to detect the crack on the shaft. This is elementary NL- observer(EOB). Using the elementary observer, an Estimator(Observer) Bank is established and arranged at the certain position on the shaft. In case a crack is found and its position is known, the procedure for the estimation of the depth is going to begin.

기관절개술후 종격동기관 협착증에 대한 기관절제 단단 문합술 (Circunferential resection and direct end to end anastomosis of mediastinal trachea on a post tracheostomy stenosis)

  • A 37 year old male patient was suffered from severe labored breathing caused by post tracheostomy stenosis, which was localized at the mediastinal trachea [cuffed tracheal stenosis] and ranged 1.5 cm in length and approximately 3 ram. in diameter on tracheogram. After dilation of tracheal stenosis with dilator, endotracheal intubation was tried for induction of anesthesia and control of respiration during operation. A tube was placed just beyond the tracheal stenosis without respiratory difficulty. Under the endotracheal anesthesia, circumferential resection of the mediastinal trachea containing the stenosis, approximately 2 cm in length [4 tracheal rings}, was carried out and primary direct end to end anastomosis was performed with interrupted submucosal sutures [3-0 Dexon] and mobilization of trachea Postoperative tracheostomy was not performed. The patient was completely relieved from dyspnea immediately after operation. Post-operative convalescence was entirely uneventful and at present, about 3 months after operation, he is now conducting a usual life. From the literature and our experience, the etiology and treatment of post-tracheostomy stenosis were discussed.

  • The present study conducts a series of experiments to develop a novel air supplying module for a micro fuel cell using piezoelectric linear actuator. An intermittently and operating air breathing module with reciprocating motion of the linear actuator has been suggested in the present study. A test bench for a micro fuel cell system has been constructed to estimate performance of the active fuel cell system using the air supplying module. With the stroke and operating duty as main control parameters, the optimal operating method of the air supplying module has been discussed.
