모유수유아와 분유수유아의 성장 패턴 (Growth Patterns of Breast Fed and Formula Fed Infants)

  • 목 적 : 모유수유아와 분유수유아의 생후 첫 일년간의 성장 패턴을 비교하기 위함이다. 방 법 : 의무 기록지를 통해 출생시, 1, 3, 6, 9 그리고 12개월의 신체 계측치(체중, 신장, 두위)가 수집되었고, 설문 조사를 통해 대상의 특징들이 모아졌다. 358명의 소아들 중 모유수유아는 161명(남아 84명, 여아 77명)이었고, 분유수유아는 90명(남아 42명, 여아 48명)이었다. 두 군 모두에서 4개월 이전에는 이유식을 공급하지 않았다. 각 연령별 체중, 신장, 두위가 계산되었다. 모유수유군은 다시 두 군(4-11개월간 모유수유를 한 군과 12개월 이상 모유수유를 한 군)으로 세분되었다. 결 과 : 엄마의 연령을 제외하면 대상의 특징들은 두 군에서 유사하였다. 모유수유군의 평균 체중은 12개월에만 분유수유군 보다 낮게 측정되었다(male : P=0.004, female : P=0.004). 하지만, 12개월 이상 모유수유만 한 군의 평균 체중은 9개월과 12개월에 분유수유군 보다 낮았다(P<0.05). 평균 신장과 두위는 두 군에서 유사하였다. 결 론 : 모유수유아와 분유수유아의 성장 지표들은 출생시에는 유사하나 생후 첫 일년간 두 군의 체중 곡선이 다르게 나타난다.

1세 유아의 장내균총 조성과 수유방법에 따른 영향 고찰 (Composition of the Intestinal Microflora in Korean 1-Year Infants and the Effect of Feeding Practices)

  • Fecal microflora of 12 breast-fed(BF) and 15 formula-fed(FF) infants were investigated at 1 year after birth and the results were compared to those that had previously been obtained from the same subjects at 1 week after birth, and before and after weaning. At 1 year the two lactation groups showed no significant differences both in frequencies and numbers of each major bacterical species, except eubacteria, which showed higher frequency in FF infants. Bifidobacteria appeared as dominant species in 50% BF infants, heres bacteroides in 73.3% FF infants. Fecal pH was lower, though insignificant, in BF infants(6.5$\pm$0.4) than in FF infants(6.8$\pm$0.5). In BF infants, the number of bifidobacteria, clostridia, and E. coli deceased from birth up to 1 year, whereas the number of bacteroides similar trends to BF infants except that the number of bifidobacteria increased, but insignificantly. This research showed that the floral differences resulted not from the age, but from the types of feeding and that breast feeding and that breast feeding could be better than formula feeding in the BF infants had more stable floral composition and bowels with lower pH, which can play a protection role against pathogen infection.

  • To compare the morbidity between the breast-fed and artificial formula-fed Infants, the frequency of diseases during infancy was studied. The subjects were 37 breast-fed infants and 41 formula-fed infants aged 12-15 months. The data were obtained while they visited the pediatric out-patient clinics. The results were as follows : 1. Most prevalent diseases in the infancy were respiratory and gastrointestinal illnesses. 2. During the first 6 months the morbidity was significantly lower in the breast-fed than in the formula-fed 3. The frequency of respiratory and gastrointestinal diseases was significantly higher in the formula-fed than in the breast-fed infant during the first 6 months.

모유양양아와 인공영양아의 성장발육 상태에 관한 횡단적 연구 (Cross-sectional Study of Growth of Korean Breast Fed Formula Fed Infants from 1 to 3 Postpartum Months)

  • One-to three -month-old infants(n=252) were compared their growth performance(weight, length, head circumference, chest circumference) by feeding methods in cross-sectional study. There were two groups : breast-fed(BF) and formula-fed groups(FF). The weights of infants at 1, 2, and 3 postpartum month were 4.50$\pm$0.48kg, 5.76$\pm$0.62kg and 6.67$\pm$0.68kg, respectively. The weigh of male infants was higher than females. The length were 53.7$\pm$2.6cm, 57.4$\pm$3.3cm, 61.1$\pm$3.1cm at one to three postpartum months, respectively. The length of male infants was higher than females, too. Overall there were no significant differences between feeding methods in weight, length, head and chest circumferences, and monthly weight and length velocity. But 3-month-old formula-fed male infants showed bigger significantly in weight, monthly weight velocity and chest circumference than the breast-feds. By Korean standards, weight-for-height Z-score were distributed from 0 to +2. Not only was there a few malnourished infants but also overnourished were noticed, especially in formula-fed group. From this survey, long-time survey on growth and development of large scales might be necessary to determine the effect of feeding methods of infants after 3 months and to set proper grwoth standard for infant. And it was suggested the that the careful attention of polarized nutritional problems of early infants was needed, especially in formula-fed group. (Korean J Community Nutrition 2(1) : 3~12, 1997)

수유방법에 따른 한국 신생아의 장내균총 조성에 관한 연구 (Composition of the Gastrointestinal Microflora in Korean Breast-fed and Formula -fed Newborn Infants)

  • Intestinal microflora in 17 breast-fed(BF) and 14 formula-fed (FF) newborn infants at about 1 week after birth were investigated to determine the floral differences of the two lactation groups. The fecal conditions were significantly different between the two groups in that the number of subjects who showed watery feces, and the times of defecation per day, were higher in the BF group. In addition, the mean fecal PH of the BF group was lower than that of the FF group. The dominant species in the BF groups which showed the highest count among the species of microflora was bifidobacteria , while the dominant species in the FF group was streptococci. The frequency of staphylococci and the count of bifidobacteria were significantly higher in the BF group. However, the frquencies of streptococci and Peptococcaceae were higher in the FF group. Through the differences were not significant, the frequency of bvifidobacteria and the count of staphylococci were higher in BF group, whereas the frequencies of bacteriodes, clostridia, enterobacter and the count of clostridia were higher in FF group.

모유 영양아와 인공 영양아의 에너지, 단백질, 지방 및 유당 섭취 (Intakes of Energy, Protein, Lipid and Lactose in Korean Breast-Fed and Formula-Fed Infants)

  • Korean breast-fed(BF) and formula-fed (FF) infants(n=31) were studied at their age of 1, 2 and 3 months to compare their intakes of energy, protein, lipid and lactose. Formulas had more energy, protein and lipid than breast milk, but the former had less lactose than the latter. The milk consumption of the FF infants was, however, greater than that of the BF infants with wide individual variation. As a result, the FF infants took in more energy, protein and lipid than the BF infants except lactose. The total daily caloric intakes(kcal/day) of the BF infants were 521.418 and 425 at their age of 1, 2 and 3 months respectively, and those of the FF infants were 676.752 and 723(at their age of 1, 2 and 3 months respectively). Daily protein intakes(g/days) of the BF infants were 9.5, 8.0 and 7.6 at their age of 1, 2 and 3 months respectively, and those of the FF infants were 13.9, 15.5 and 14.8(at their age of 1, 2 and 3 months respectively). The energy and protein of both the BF and the FF infants were deficient compared to the present Recommended Nutrient Allowances, and the deficiency of the BF infants was greater than that of the FF infants. But the energy intakes per kg of body weight of the FF infants at their age of 1 and 2 months were more than the present recommended energy allowances. And the protein intake per kg of body weight of the FF infant at the age of 1 month was the same as the present recommended protein allowances.

모유수유아의 비타민 D 섭취에 대한 한국형 가이드라인 제안 (Korean Guidelines for Breast-fed Infants for Vitamin D Supplements)

  • Objectives Recently, vitamin D supplements to breast-fed infants are being encouraged in Korea, and other countries. However, the reliability and validity assessment of supplements is insufficient. Therefore, this study suggests new Korean guidelines for vitamin D supplements of breast-fed infant. Methods The roles of Vitamin D for bone metabolism in children and the correlation between breast milk and Vitamin D were examined throughout domestic and international literature review. In addition, the efficacy and safety of vitamin D supplements were reviewed. Results Preventive effects of rickets by vitamin D supplementation remain unclear. Furthermore, concerns about the safety of vitamin D supplements intake have been raised. Korean guideline suggests breast-fed infants can get vitamin D through the skin safely, but maternal intake of vitamins through sunbath and diet is more effective and safe. Conclusions Limiting sunlight excessively and applying foreign countries' medical guideline for vitamin D supplements are not valid to apply as a domestic guideline for Korean breast-fed infants without considering ethnic and cultural characteristics.

Association of infant feeding practices in the general population with infant growth and stool characteristics

  • This was a prospective cohort study of 976 infants from birth to 12 months of age. Infants were fed breast milk, goat infant formula, cow infant formula, or a combination of formula and breast milk during the first 4 months of age. Data on type of milk feeding and infant growth (weight and height) were collected at birth and at 4, 8, and 12 months during routine clinical assessment. The number and consistency of bowel motions per day were recorded based on observational data supplied by the mothers. Infants fed breast milk or goat or cow infant formula during the first 4 months displayed similar growth outcomes. More of the infants fed cow infant formula had fewer and more well-formed bowel motions compared with breast-fed infants. The stool characteristics of infants fed goat formula resembled those of infants fed breast milk.

모유 영양아의 아연과구리 섭취량에 관한 연구 (Study on Zinc and Copper intakes of Breast-fed Infants)

  • The purpose of this study is to investigate zinc and copper intakes of exclusively breast-fed infants. We measured consumed volume and the minerals concentrations of human milk from thirty three lactating women and their infants during the first 5 months of lactation , Zinc concentrations of human milk showed 0.394, 0.290, 0.191, 0.147 and 0.139mg/100g at 0.5, 1, 2, 3, 4 and 5 months of lactation respectively. Copper concentrations in the milk showed 61.04, 45.97, 37.15, 3.94, 26.22 and 26.21$\mu\textrm{g}$/100g respectively. Consumed volume of the milk of infants were 515, 650, 718, 731, 746 and 769g/day. There is significant difference between boys and girls in consumed volume of the milk at 2, 3 and 4 months of lactation. Zinc intakes of breast-fed infants declined(p<0.05) from 2.13mg/day at 0.5 months to 1.21mg/day at 3 months with no change thereafter significantly. Copper intakes of the infants declined(p<0.05) from 323.2$\mu\textrm{g}$/day at 0.5 months to 238.9$\mu\textrm{g}$/day at 3 months with no change thereafter. Form this survey, the reevaluation of zinc and copper intakes and recommended dietary allowance of those minerals during infancy are merited. (Korean J Nutrition 33(8) : 857-863, 2000)

모유 영양아의 모유 섭취량과 체중 변화 (Changes on Breast Milk Intake and Weight of Breast-fed Infants during the Lactation)

  • In order to investigate the body weight change and human milk intake of breast-fed infants, we examined thirty-three infants with test-weing method in Sokcho area during the first 5 months of lactation. The average birth weight of infants was 3300g. They grew 3300, 3805, 4676, 5878, 6786, 7403, and 8111g when they became 0.5, 1, 2, 3, 4, and 5 months old. The is human milk intake was 515, 650, 718, 731, 746 and 796g/day during the lactatin. Human milk intake of boys was significantly higher than that of girls at 1(p<0.05), 2(p<0.01), and 3(p<0.01) months of lactation. During lactatin, the number of feedings per day decrerased. The human milk intakes per feeding had increased from 54.4g at 0.5 months to 98.9g at 5 months of lactation The human milk intake on infants was not affected by gestational period and birth weight. From this survey, we found a difference between boys and girls in human milk intake.
