Clinical Characteristics and Survival Analysis of Breast Cancer Molecular Subtypes with Hepatic Metastases

  • Background: The liver is one of the most common metastatic sites of breast cancer, hepatic metastases developing in 6%-25% of patients with breast cancer and being associated with a poor prognosis. The aim of this study was to analyze the survival and clinical characteristics of patients with hepatic metastases from breast cancer of different molecular subtypes and to investigate the prognostic and predictive factors that effect clinical outcome. Methods: We retrospectively studied the charts of 104 patients with breast cancer hepatic metastases diagnosed at Sun Yat-sen University Cancer Center from December 1990 to June 2009. Subtypes were defined as luminal A, luminal B, human epidermal growth factor receptor 2 (HER2) enriched, triple-negative (TN). Prognostic factor correlations with clinical features and treatment approaches were assessed at the diagnosis of hepatic metastases. Results: The median survival time was 16.0 months, and the one-, two- three-, four-, five-year survival rates were 63.5%, 31.7%, 15.6%, 10.8%, and 5.4%, respectively. Median survival periods after hepatic metastases were 19.3 months (luminal A), 13.3 months (luminal B), 18.9 months (HER2-enriched), and 16.1 months (TN, P=0.11). In multivariate analysis, a 2 year-interval from initial diagnosis to hepatic metastasis, treatment with endocrine therapy, and surgery were independent prognostic factors. Endocrine therapy could improve the survival of luminal subtypes (P=0.004) and was a favorable prognostic factor (median survival 23.4 months vs. 13.8 months, respectively, P=0.011). Luminal A group of patients treated with endocrine therapy did significantly better than the Luminal A group of patients treated without endocrine therapy (median survival of 48.9 vs. 13.8 months, P=0.003). Conclusions: Breast cancer subtypes were not associated with survival after hepatic metastases. Endocrine therapy was a significantly favorable treatment for patients with luminal subtype.

  • This study was aimed at preparing basic data required for establishment of a cancer screening program by examining the status of cancer screenings performed by women in a city and the factors that had an influence on cervical and breast cancers screening behavior of women. In order to find out the status of cervical and breast cancers screening and the factors having an influence on cervical and breast cancers screening behavior, 1,113 women were selected as study area and subjects by a random cluster sampling method, and the subjects were answered by questionnaires. The major results were as follows: 1. In the cases of the breast and the cervical cancers, 16.7% and 55.7% of the subjects turned out to have had one or more screenings respectively in their life-time. Also the rate of screening group of cervical and breast cancers was 16.7%, the rate of screening group of only cervical cancer was 38.9%, and the rate of non-screening group was 44.4%. 2. As to the screening pattern of cervical and breast cancer, there were significantly for age, income, occupation, married status, spouse, the factors associated with health promotion (doctor visiting, exercising), married age, number of children, and breast feeding(p〈0.05). 3. The factors associated with screening behaviors for cervical and breast cancers were age, income, occupation, married status, doctor visiting, exercising, married age, number of children, and breast feeding(p〈0.05). Based on the above-mentioned results obtained by this study, it was anticipated that this study may be playa vital role as basic data for the development and execution of cervical and breast cancers screening program for women in a community. And the analysis, done on the basis of the status of the cancer screening, of the influence factors on cervical and breast cancers screening behavior showed that for the development of a cancer screening program, the factors like age, social-economic class, married status, health promotion behavior, and reproductive characteristics should be considered.

Applicability of Oncoplastic Breast Conserving Surgery in Asian Breast Cancer Patients

  • Background: There are limited studies on oncoplastic breast conserving surgery in Asian women. We aimed to determine the applicability and safety of oncoplastic surgery, highlighting the specific circumstances when it will be most useful and compare our preferred technique with the worldwide practice of oncoplastic approaches. Materials and Methods: Breast cancer patients who underwent oncoplastic breast conserving surgery at a single institution from 1st May 2014-31st March 2015 were included. Data on patient demographics, tumor characteristics and the type of oncoplastic surgery performed were collected. Results: Nineteen breast cancer patients were identified. 42.1% of patients had grade I ptosis. The indications for surgery included a large tumor to breast size ratio (52.6%), multifocal/multicentric lesions (36.8%) and asymmetric breasts (10.6%), averting a mastectomy in 89.4%. Round block was the commonest technique in 63% of patients, in contrast to the inverted T pattern most frequently used in renowned institutions in the West. Mean and median tumor size and weight of specimen were 29.4/25mm (11 - 75mm) and 77g/64g (10 - 246g) respectively. Re-excision rate was 10.5%. Complete mastectomy was performed for one patient. One patient developed wound dehiscence which was treated conservatively. Cosmetic outcome was rated as excellent/good by 94.7% of patients. The patients remained clinically well after a median 16 months follow up. Conclusions: Oncoplastic breast conserving surgery is useful in a specific subgroup of Asian patients with a relatively small breast volume and minimal ptosis. Round block was the commonest technique in our series, in contrast to the worldwide utility of oncoplastic techniques. It is oncologically safe and has good cosmetic outcomes.

자조집단 참여여부에 따른 유방암 환자의 성생활 만족 영향요인 (Factors Influencing Sexual Satisfaction in Patients with Breast Cancer Participating in a Support Group and Non Support Group)

  • Purpose: This study was to identify the influence of sexual behavior, body image, social support, and other characteristics on sexual satisfaction in patients with breast cancer according to their participation in a support group. Method: Data was collected by self-report questionnaires. Participants included 63 patients attending a support group and 76 patients who did not participate in the support group. The questionnaire sections consisted of sexual satisfaction, sexual behavior, body image, social support and information on general characteristics, disease-related characteristics, and sexual life-related characteristics. Result: There was no statistically significant difference in sexual behavior, body image and sexual satisfaction between the two groups. Social support scores were significantly higher in the support group. Sexual satisfaction was positively related with sexual behavior, post-op change of sexual intercourse frequency, body image, and patient's education level, and negatively related to age in the support group. Sexual satisfaction was positively related with sexual behavior, social support and body image in the non support group. Sexual behavior is predictable 37.0% of sexual satisfaction in the support group. Sexual behavior, body image, and social support is predictable for 38.0% of the sexual satisfaction in non support group participants. Conclusion: Implications point to the need for the development and implementation of programs that focus specifically on sexual life issues for breast cancer patients, as well as further research measuring the effects of such intervention programs. Continuous education and counseling through participation in support groups can contribute to promote and affirm a healthy sexual life for patients with breast cancer.

Characteristics of Invasive Breast Ductal Carcinoma, NOS, Diagnosed in a Tertiary Institution in the East Coast of Malaysia with a Focus on Tumor Angiogenesis

  • Background: Prognosis of breast cancer depends on classic pathological factors and also tumor angiogenesis. This study aimed to evaluate the clinicopathological factors of breast cancer in a tertiary centre with a focus on the relationship between tumor angiogenesis and clinicopathological factors. Methods: Clinicopathological data were retrieved from the archived formal pathology reports for surgical specimens diagnosed as invasive ductal carcinoma, NOS. Microvessels were immunohistochemically stained with anti-CD34 antibody and quantified as microvessel density. Results: At least 50% of 94 cases of invasive breast ductal carcinoma in the study were advanced stage. The majority had poor prognosis factors such as tumor size larger than 50mm (48.9%), positive lymph node metastasis (60.6%), and tumor grade III (52.1%). Higher percentages of estrogen and progesterone receptor negative cases were recorded (46.8% and 46.8% respectively). Her-2 overexpression cases and triple negative breast cancers constituted 24.5% and 22.3% respectively. Significantly higher microvessel density was observed in the younger patient age group (p=0.012). There were no significant associations between microvessel density and other clinicopathological factors (p>0.05). Conclusions: Majority of the breast cancer patients of this institution had advanced stage disease with poorer prognostic factors as compared to other local and western studies. Breast cancer in younger patients might be more proangiogenic.

High Expression of Stem Cell Marker ALDH1 is Associated with Reduced BRCA1 in Invasive Breast Carcinomas

  • Background: Cancer stem cells (CSC) have been described in a variety of malignancies, including breast carcinomas. Among several markers, aldehyde dehydrogenase 1 (ALDH1) has been identified as reliable for breast cancer stem cells. Knockdown of BRCA1 in primary breast epithelial cells leads to an increase in cells expressing ALDH1. Methods: We examined 127 breast carcinomas for expression of ALDH1, using immunohistochemistry and correlated with clinicopathological parameters as well as the BRAC1 status. Results: Comparing the results for both ALDH1 and BRCA1 expression showed a significant inverse association between the two, indicating that reduced BRCA1 was more often seen in breast cancer cells expressing ALDH1 (p-value = 0.044). A total of 24/110 (22%) of tumours displayed the ALDH1 + / BRCA1 -/low phenotype, which showed a trend for a relation with a high grade (p-value= 0.056). Cytoplasmic expression of ALDH1 was not correlated with tumour characteristics. Conclusion: Taken together, our findings suggest that increased ALDH1 is inversely correlated with decreased BRCA1 in a series of unselected breast carcinomas. Therefore, ALDH1 positive (cancer stem) cells with reduced BRCA1 phenotype may indicate a subset of patients for whom specific targeting of the CSC marker ALDH1 and more aggressive adjuvant treatment is appropriate.

Hormone Replacement Therapy and Risk of Breast Cancer in Korean Women: A Quantitative Systematic Review

  • Objectives: The epidemiological characteristics of breast cancer incidence by age group in Korean women are unique. This systematic review aimed to investigate the association between hormone replacement therapy (HRT) and breast cancer risk in Korean women. Methods: We searched electronic databases such as KoreaMed, KMbase, KISS, and RISS4U as well as PubMed for publications on Korean breast cancer patients. We also conducted manual searching based on references and citations in potential papers. All of the analytically epidemiologic studies that obtained individual data on HRT exposure and breast cancer occurrence in Korean women were selected. We restricted the inclusion of case-control studies to those that included age-matched controls. Estimates of summary odds ratio (SOR) with 95% confidence intervals (CIs) were calculated using random effect models. Results: One cohort and five case-control studies were finally selected. Based on the heterogeneity that existed among the six studies (I-squared=70.2%), a random effect model was applied. The summary effect size of HRT history from the six articles indicated no statistical significance in breast cancer risk (SOR, 0.983; 95% CI, 0.620 to 1.556). Conclusions: These facts support no significant effect of HRT history in the risk of breast cancer in Korean women. It is necessary to conduct a pooled analysis.

유방 확대수술 환자용 수술 직후 보정용 브래지어 개발 (Development of the Post-Operative Bra for Breast Augmentation Patients)

  • This study develops a post-operative brassiere suitable for changed breast shapes and the characteristics of breast augmentation patients. Six subjects who experienced breast augmentation surgery with a cup size of C or D cup participated in the wearing test. We conducted a wearing test to evaluate the superiority of the developed post-operative bra. As a result, the evaluation of the developed bra was excellent in the outer cup, outer circle of breast front gore, shoulder straps and the wing pressures. Second the satisfaction of the developed brassiere was high in the front center part, and in the evaluation of wearing satisfaction of the subjects; in addition, the degree of close contact with the cup, flexibility, touch, and overall evaluation. The satisfaction of the developed bra was also higher than a commercial bra. Third, the developed bra proved superior in the adaptability test, such as prevention of breast shaking, prevention of bra and breast separation during movements, and ease of body movement.

유방암 환자의 Digital Mammography에 관한 연구 (A Study on the Digital Mammography for Breast Cancer Patients)

  • 본 연구는 유방영상검사의 최고의 관심영역인 DR 방식의 유방영상에서 유방암 진단의 정확도를 평가하고, 유방암 환자들의 특성을 알아보고자 DR 방식으로 유방영상검사를 실시한 유방암 환자 57명을 대상으로 2010년 5월부터 2011년 6월까지 조사되었다. 유방암 환자들의 평균 나이는 50.8세이었고, 유방암의 발생 부위는 상외측(UOQ)이 전체의 33.3%로 가장 많은 것으로 조사되었다. 연령에 따른 유방암 발생은 40-49세에서 42.1%로 모든 연령대에서 가장 높게 나타났다. 유방암 환자들의 유방조직 밀도는 지방형 유방이 31.6%(18/57), 치밀유방이 68.4%(39/57)로 나타나, 70%에 가까운 유방암 환자들이 치밀유방의 형태를 띠고 있었다. 유방영상에서 방사선학적 병변 중 종괴와 미세석회화가 함께 동반된 경우 (45.3%)가 유방암 발견율이 가장 높은 것으로 조사되었고, 치밀유방에서 미세석회화를 동반하지 않는 종괴의 경우 지방형 유방에서보다 유방암의 발견율이 더 낮은 것으로 조사되었으며, 이러한 종괴는 치밀유방에서 위음성율을 높이는 원인이 되고 있다. Digital mammography의 위음성률은 7.0%이었고, 민감도는 93.0%인 것으로 조사되었다. 치밀유방의 위음성율은 12.8%, 민감도는 87.2%로 조사되었는데 이는 이전에 보고된 screen film mammography의 치밀유방에서 보다 유방암에 대한 민감도가 더 높은 것으로 조사되었다.

Young Women with Breast Cancer in the United States and South Korea: Comparison of Demographics, Pathology and Management

  • Background: Breast cancer diagnosed in young women may be more aggressive, with higher rates of local and distant recurrence compared to the disease in older women. Epidemiologic evidence suggests that Korean women have a lower incidence of breast cancer than women in the United States, but that they present at a younger age than their American counterparts. We sought to compare risk factors and management of young women with breast cancer in Boston, Massachusetts (US) with those in Seoul, South Korea (KR). Materials and Methods: A retrospective review was performed of consecutive patients less than 35 years old with a diagnosis of breast cancer at academic cancer centers in the US and KR from 2000-2005. Patient data were obtained by chart review. Demographic, tumor and treatment characteristics were compared utilizing Pearson's chisquare or Wilcoxon rank-sum tests where appropriate. All differences were assessed as significant at the 0.05 level. Results: 205 patients from the US and 309 from KR were analyzed. Patients in US were more likely to have hormone receptor positive breast cancer, while patients in KR had a higher rate of triple negative lesions. Patients in US had a higher mean body mass index and more often reported use of birth control pills, while those in the KR were less likely to have a sentinel node procedure performed or to receive post mastectomy radiation. Conclusions: Patients under 35 diagnosed with breast cancer in the US and KR differ with respect to demographics, tumor characteristics and management. Although rates of breast conservation and mastectomy were similar, US patients were more likely to receive post mastectomy radiation. The lower use of sentinel node biopsy is explained by the later adoption of the technique in KR. Further evaluation is necessary to evaluate recurrence rates and survival in the setting of differing disease subtypes in these patients.
