Nature and Issues of Hospital Management (병원관리학의 본질과 과제)

  • Hospital Management, as a branch of science, is now in a formative period in Korea. Proper understanding on the scienctific nature and issues of the subject, therefore, is important for both its developement and education of the students. As an academic subject, Hospital Management can be pointed out to have three characteristics, Firstly, when looking it in teleological viewpoint, it is a branch of health science. Ultimate goal of the hospital lies in promoting and keeping health of the people residing in the catchment area in which it places, Secondly, in terms of its contents, Hospital Management can be divided into tow subsubjects ; hospital administrative management and hospital business management. The former stands at the point of looking the hospital as the object of goverment administration, while the latter as the subject of organizational operation. Flanlly, Hospital Management, in its academic appoach, is a science that adopts management theories, what is called, traditional and/or modern. For the settlement and development of the Hospital Management, four major issues were suggested for discussion now and in the future. They include ; (1) boundary between Hospital Management and its related academic fields (2) subdivision of the subject and its contents, (3) activation of study, theortical and empirical, and (4) distinction of the level of lecture among lecfnical, four-year and gradudate courses.

  • Many methods to examine the vulnerable areal for the contingencies in the power system. The most widely used index for the vulnerable area investigation has been the reactive power margin or sensitivity analysis. But we can get the results of these analyses if only the results of load flow are convergent in severe contingencies, otherwise these methods are not adoptable. We can present a good index for overcoming severe contingencies, if we can get the vulnerable areas by bus sensitivity in severe contingencies, though the power flow equation is unsolvable. This paper simulates unsolvable severe contingencies by using branch parameter continuation power flow. We can compute the vulnerable areas in unsolvable severe contingencies by normal vector at a nose point of a $\nu-V$ curve. Presented method is checked the input reactive power of the vulnerable areas in KEPCO system.

An Optimal Algorithm for the Sensor Location Problem to Cover Sensor Networks

  • We consider the sensor location problem (SLP) on a given sensor field. We present the sensor field as grid of points. There are several types of sensors which have different detection ranges and costs. If a sensor is placed in some point, the points inside of its detection range can be covered. The coverage ratio decreases with distance. The problem we consider in this thesis is called multiple-type differential coverage sensor location problem (MDSLP). MDSLP is more realistic than SLP. The coverage quantities of points are different with their distance form sensor location in MDSLP. The objective of MDSLP is to minimize total sensor costs while covering every sensor field. This problem is known as NP-hard. We propose a new integer programming formulation of the problem. In comparison with the previous models, the new model has a smaller number of constraints and variables. This problem has symmetric structure in its solutions. This group is used for pruning in the branch-and-bound tree. We solved this problem by branch-and-cut(B&C) approach. We tested our algorithm on about 60 instances with varying sizes.

  • In this case report regarding one patient with complete left bundle branch block and mild hypokinesia of left ventricle, who had cebebellar infarction and therefore showed the symptoms of cerabellar dyskinesia followed by dyspnea, chest discomfortness, insomnia and dry cough. From the point of oriental diagnostic criteria, the patient s clinical conditions were all classified as $^{\circ}AEdeprivation$ of kidney essence' and treated accordingly. He showed no change in EKG monitoring but above symptoms were removed markedly after following treatment. Further elaboration of oriental diagnostic classification could possibly lead to the fundamental treatment.

  • An electro-optic polymer digital optical switch with a coupling region modified for optimum coupling is designed and demonstrated. Its branch waveguide is fabricated by reactive ion etching. Then, the modified coupling region is adiabatically introduced along the propagation direction from the branching point of the two waveguides, and it is implemented by photobleaching after the device fabrication. The structure of the modified coupling region and its refractive index profiles are designed to optimize and mode coupling in the Y-branch waveguide. Therefore, the switching performance of the device was shown to be enhanced with a fixed device length. The measured drive voltage is reduced by more than 30 percents, and the crosstalk is also improved by about 4~6 dB.

Setting the Stomach Transection Line Based on Anatomical Landmarks in Laparoscopic Distal Gastrectomy

  • Laparoscopic distal gastrectomy has become widespread as a treatment for early gastric cancer in eastern Asia, but a standard method for setting the stomach transection line has not been established. Here we report a novel method of setting this line based on anatomical landmarks. At the start of the operation, two anatomical landmarks along the greater curvature of the stomach were marked with ink: the proximal landmark at the avascular area between the last branch of the short gastric artery and the first branch of the left gastroepiploic artery, and the distal landmark at the point of communication between the right and left gastroepiploic arteries. Just before specimen retrieval, the stomach was transected from the center of these two landmarks toward the lesser curvature. Then, about two-third of the stomach was reproducibly resected, and gastroduodenostomy was successfully performed in 26 consecutive cases. This novel method could be used as a standard technique for setting the transection line in laparoscopic distal gastrectomy.

Hypocholestrolemic Effect of CJ90002 in Hamsters: A Potent Inhibitor for Squalene Synthase from Paeonia moutan

  • Squalene synthase catalyzes the reductive dimerization of two molecules of farnesyl diphosphate to form squalene at the final branch point of the cholesterol biosynthetic pathway. Due to the unique position of this enzyme in the pathway, its inhibitors may have advantages as antihypercholesterolemic agents. Therefore, selective inhibitors of squalene synthase do not prevent the formation of the essential branch products of the isoprene pathway, such as dolichol, coenzyme-Q, and prenylated proteins, as might be expected for inhibitors of enzymes earlier in the pathway; for example, lovastatin and mevalotin. The current study reports that CJ90002, a pentagalloylglucose isolated from Paeonia moutan SIM (Paeoniaceae), which is an important Chinese crude drug used in many traditional prescriptions, was a potent inhibitor of rat microsomal squalene synthase, and also a potent inhibitor of cholesterol biosynthesis in vitro. In addition, the intraperitoneal and oral administration of CJ90002 had a significant lowering effect on plasma cholesterol levels in hamsters.

Back Pain of Muscular Origin (근긴장성 요통의 치료에 대한 새로운 소견)

  • In out-patient clinic, it seems to be common that most back pain arise from muscular origins rather than from skeletal origins. Most physicians have wished to diagnose lower back pain from the radiologic findings only. From clinical experiences and anatomical studies, I have gotten a different opinion from common sense about backaches. If I met a patient who had lower back pain around the posterior superior iliac crest(P.S.I.C.) area, I would had to search a trigger point in the erector spinae muscles at the level of thoraco-lumber junction rather than at the level of the painful site. It is why that sensory innervation over the posterior superior iliac crest area is the posterior primary branch of T12 spinal nerve running down through the erector spinae muscles. Pain on the iliac crest area is supposedly due to hyperirritability of the sensory nerve distributing to this area. Hyperirritability of the posterior primary branch of $T_{12}$ spinal nerve may be due to the spasm of the longissimus thoracis muscle in the erector spinae muscles at the level of the thoraco-lumbar junction. So finally, I would like to insist that spasmolytic treatment on the muscle at the level of the thoraco-lumbar junction would be better for pain relief around P.S.I.C. than treatment at the painful site only.

Performance of concrete structures with a combination of normal SCC and fiber SCC

  • Fiber reinforced concretes exhibit higher tensile strength depending on the percent and type of the fiber used. These concretes are used to reduce cracks and improve concrete behavior. The use of these fibers increases the production costs and reduces the compressive strength to a certain extent. Therefore, the use of fiber reinforced concrete in regions where higher tensile strength is required can cut costs and improve the overall structural strength. The behavior of fiber reinforced concrete and normal concrete adjacent to each other was investigated in the present study. The concrete used was self-compacting and did not require vibration. The samples had 0, 1, 2 and 4 wt% polypropylene fibers. 15 cm sample cubes were subjected to uniaxial loads to investigate their compressive strength. Fiber Self-Compacting Concrete was poured in the mold up to 0, 30, 50, 70 and 100 percent of the mold height, and then Self-Compacting Concrete without fiber was added to the empty section of that mold. In order to investigate concrete behavior under bending moment, concrete beam samples with similar conditions were prepared and subjected to the three-point bending flexural test. The results revealed that normal Self-Compacting Concrete and Fiber Self-Compacting Concrete may be used in adjacent to each other in structures and structural members. Moreover, no separation was observed at the interface of Fiber Self-Compacting Concrete and Self-Compacting Concrete, either in the cubic samples under compression or in the concrete beams under bending moment.

Absolute calibration of near-infrared Period-Luminosity-Metallicity relations for RR Lyrae variables using Gaia EDR3

  • RR Lyrae stars are sensitive probe for the precision stellar astrophysics and also for the cosmic distance scale thanks to their well-defined near-infrared Period-Luminosity relations (PLRs). These horizontal branch variables can be used for primary calibration of the first-rung of population II distance ladder providing an evaluation of the ongoing tension between Cepheid-Supernovae based Hubble constant and the Planck results. Therefore, absolute calibration of RR Lyrae PLRs is now crucial to complement or test the tip of the red giant branch based distances, and in turn, population II star based Hubble constant measurements. While the pulsation models of RR Lyrae can reproduce most observables, they predict a significant metallicity effect on their JHKs-band PLRs that is inconsistent with so-far limited observational studies. We remedy this inconsistency of metallicity dependence in RR Lyrae PLRs by combining their near-infrared observations in the globular clusters of different mean-metallicities with the new parallaxes from the Gaia early data release 3 (EDR3). Our empirical results on Period-Luminosity-Metallicity (PLZ)relations are consistent with theoretical predictions but the precision of absolute calibrations is still affected by the parallax uncertainties and the systematic zero-point offset present in the Gaia EDR3.
