상완 신경총 손상에서 자연 회복과 신경 재건술간의 비교 (Comparison of Spontaneous Recovery and Nerve Surgery in Brachial Plexus Injury)

  • 서울대학교 의과대학 정형외과학 교실에서는 1985년 1월부터 1994년 12월까지 치험하였던 103명의 환자를 대상으로 최소한 8개월간 보존적 치료를 시행하며 자연 회복을 기다렸고, 수상 후 8개월에서 10개월까지 3개월마다 반복된 근전도 검사상 회복이 없거나 경미한 31명에 대하여 신경 복원술을 시행하여 다음과 같은 결과를 얻었다. 1) 자연 회복은 상완 신경총 손상 환자의 47명(46%)에서 일어났으며, 자연 회복된 환자의 3분의 2(31명)에서 근전도 검사상의 변화가 3개월에서 9개월 사이에 처음 발견되었고, 나머지 3분의 1(16명)의 환자에서 9개월에서 16개월 사이에 발견되어, 평균 7.8개월에 시작됨을 보여주었다. 수정된 AMA score상 내원 당시 14.8점에서 최종 추시 관찰시 39.8점으로 개선되었다. 2) 신경 복원술을 실시한 31명 중 52%가 기능적 호전을 보여주었고 수정된 AMA score상 술전 21.5점에서 술후 36.3점으로 14.8점이 개선되었다. 3) 양군에서 기능적 호전을 보인 비율은 유의한 수준이 아니지만, 기능적 호전의 정도는 25점과 14.8점으로 자연 회복군이 신경 복원술을 실시한 군보다 통계적으로 더 우수한 기능적 호전을 보임을 알 수 있었다(p<0.05). 4) 결국 저자들은 현재까지 손상의 부위와 정도를 정확하게 진단할 수 있는 방법이 부족한 상태에서 이론적으로 많은 문제점을 가지고, 기껏해야 근력 3 내지 4등급의 회복을 위해 환자에게 큰 부담을 주는, 결과가 확실하지도 않은 수술을 하는 것보다 복잡한 해부학적 구조 및 이에 따른 많은 변종을 가진 상완신경총 손상 환자에게 일단 회복이 되면 더 많은 기능 회복을 줄 수 있는 보존적 요법을 시행하며 자연 회복을 기다리는 것이 났다고 생각한다. 그러므로 저자들은 자연 회복이 수상 후 평균 7.8개월에 시작됨으로 자연 회복을 기대하며 1년간 기다려 본 후 1년이 경과하여도 자연회복이 되지 않는 경우에서 수술적 치료를 시행할 것을 제안한다.

Droopy Shoulder Syndrome 환자의 치료경험 -증례보고- (The Treatment of a Droopy Shoulder Syndrome Patient -A case report-)

  • Droopy shoulder syndrome (DSS) is a rare disease, characterized by drooping shoulders, which stretches the brachial plexus, and causes pain, but without any signs of neurological impairment. These patients suffer from pain in the neck, shoulders, arms and hands, which result in long, graceful, swan necks, low-set shoulders, and horizontal or down sloping clavicles. No abnormalities in the vascular, neurological or electrical findings have also been known. The T1 and/or T2 bodies can be seen in the lateral view in a radiological study of the cervical spine. In the majority of cases, conservative treatments, such as postural correction and shoulder girdle strengthening exercise, are commonly recommended. However, DSS may be misdiagnosed as severe thoracic outlet syndrome or herniated cervical disc disease, leading to unnecessary and hazardous invasive treatments. The presented case was consistent with DSS, and was treated with stellate ganglion block, trigger point injection, and shoulder girdle strengthening exercise.

Neuromuscular Skeletal Plasticity Moving on from Traditional Physiotherapy Concepts

  • Purpose : N.A.P.(Neuromuscular Skeletal Plasticity) an integrative neuro-orthopedic concept to facilitate motor strategies in daily life. The primary thesis is, that treatment of body functions and structural impairments should be integrated within goal-oriented activities. The purpose of this article is to demonstrate that the functional activity itself, determines the structure. Material and Methods : A case report of a dentist with brachial plexus lesion after a motor vehicle accident is presented. The necessity for training body functions within relevant tasks is undermined by references which emphasize the importance of training realistic activities to enhance long-term changes in neural representation. Results : The treatment methods presented in this case show significant effects for the patient's ability to participate in his profession within less than a year's time after his motor vehicle accident. Conclusions : Current evidence supports the treatment methods of this concept. The inability to flex his elbow and supinate his forearm placed a considerable doubt to his ability to ever be able to participate in his profession again. Structural reorganization is possible and depends on functional demands, which need to be trained task-specifically. Single case reports may serve as the basis for further randomized controlled studies to support the efficacy of the treatment methods within the N.A.P. concept.

Hypersensitivity Reaction to Perioperative Drug Mistaken for Local Anesthetic Systemic Toxicity in a Patient under Brachial Plexus Block

  • Perioperative anaphylaxis, although rare, is a severe, life-threatening unexpected systemic hypersensitivity reaction. Simultaneous administration of various drugs during anesthesia, the difficulty of communicate with patients in sedation and anesthesia, and coverage of the patient with surgical drapes are considered to be factors that impede early recognition of anaphylactic reactions. It is very important to perform an intradermal skin test because antibiotics are the most common cause of perioperative anaphylaxis. We report a case of negative-intradermal skin test antibiotic anaphylaxis mistaken for local aesthetic systemic toxicity without increase of serum tryptase for confirmative diagnostic biomaker during surgery under brachial plexus block. It is not possible to exclude the danger of anaphylaxis completely, even if it is negative-intradermal skin test and normal tryptase level. Therefore, anesthesiologists should be closely monitored and treated early for antibiotics related hypersensitive reaction, like other medicines during anesthesia.

The anatomical variations of median nerve in Shiraz, Iran

  • The origin and distribution of median nerve varies among the different individuals. The median nerve variations in axillary region were reported by many authors previously. Understanding of these variations is especially necessary for clinicians to prevent iatrogenic nerve damage. The current work aimed to evaluate the possible anatomical variations of median nerve in the axillary region in a sample of the Iranian cadavers (Shiraz, Fars). We dissected 26 upper limbs from 13 male cadavers to investigate the different variations of median and musculocutaneous nerves according to Venieratos and Anagnostopoulou classification. In 23.07% of specimens (n=6), the medial root united with 2 lateral roots and formed the median nerve proximal to the coracobrachialis muscle. In one case, a communicating branch separated from the musculocutaneous nerve distal to the coracobrachialis and connected to the median nerve in upper arm. Our results suggest that there are anatomical variations of the median nerve in terms of its origin and its communication with the musculocutaneous nerve in the population of southern Iran. The anatomical knowledge of the median nerve variations is important for clinicians to improve patient health outcome. Theses variations of the median nerve should be considered during surgical procedures of the axillary region and nerve block of the infra clavicular part of the brachial plexus.
