The Precision Test Based on States of Bone Mineral Density (골밀도 상태에 따른 검사자의 재현성 평가)

  • Purpose: ISCD (International Society for Clinical Densitometry) requests that users perform mandatory Precision test to raise their quality even though there is no recommendation about patient selection for the test. Thus, we investigated the effect on precision test by measuring reproducibility of 3 bone density groups (normal, osteopenia, osteoporosis). Materials and Methods: 4 users performed precision test with 420 patients (age: $57.8{\pm}9.02$) for BMD in Asan Medical Center (JAN-2008 ~ JUN-2008). In first group (A), 4 users selected 30 patient respectively regardless of bone density condition and measured 2 part (L-spine, femur) in twice. In second group (B), 4 users measured bone density of 10 patients respectively in the same manner of first group (A) users but dividing patient into 3 stages (normal, osteopenia, osteoporosis). In third group (C), 2 users measured 30 patients respectively in the same manner of first group (A) users considering bone density condition. We used GE Lunar Prodigy Advance (Encore. V11.4) and analyzed the result by comparing %CV to LSC using precision tool from ISCD. Check back was done using SPSS. Results: In group A, the %CV calculated by 4 users (a, b, c, d) were 1.16, 1.01, 1.19, 0.65 g/$cm^2$ in L-spine and 0.69, 0.58, 0.97, 0.47 g/$cm^2$ in femur. In group B, the %CV calculated by 4 users (a, b, c, d) were 1.01, 1.19, 0.83, 1.37 g/$cm^2$ in L-spine and 1.03, 0.54, 0.69, 0.58 g/$cm^2$ in femur. When comparing results (group A, B), we found no considerable differences. In group C, the user_1's %CV of normal, osteopenia and osteoporosis were 1.26, 0.94, 0.94 g/$cm^2$ in L-spine and 0.94, 0.79, 1.01 g/$cm^2$ in femur. And the user_2's %CV were 0.97, 0.83, 0.72 g/$cm^2$ L-spine and 0.65, 0.65, 1.05 g/$cm^2$ in femur. When analyzing the result, we figured out that the difference of reproducibility was almost not found but the differences of two users' several result values have effect on total reproducibility. Conclusions: Precision test is a important factor of bone density follow up. When Machine and user's reproducibility is getting better, it’s useful in clinics because of low range of deviation. Users have to check machine's reproducibility before the test and keep the same mind doing BMD test for patient. In precision test, the difference of measured value is usually found for ROI change caused by patient position. In case of osteoporosis patient, there is difficult to make initial ROI accurately more than normal and osteopenia patient due to lack of bone recognition even though ROI is made automatically by computer software. However, initial ROI is very important and users have to make coherent ROI because we use ROI Copy function in a follow up. In this study, we performed precision test considering bone density condition and found LSC value was stayed within 3%. There was no considerable difference. Thus, patient selection could be done regardless of bone density condition.

  • Purpose : This study is to identify the aftermath of hysterectomy in the treatment of a uterine disease. Methods : We collected treatises on the aftermath of hysterectomy and analyzed those. Those treatise had relation to change in ano-rectal function, ovarian function, change of serum sex hormone levels, bone mineral density, quality of life and so on. Results : After the treatises on the aftermath of hysterectomy, common symptoms after hysterectomy were general weakness, loss of taste, sweating, abdominal pain, dysuria, vaginal bleeding, weight loss, emptyness on lower abdomen and pains on operation. Conclusion : The result of this study suggest the aftermath of hysterectomy in the treatment of a uterine disease. In conclusion, our result support the importance of earlier prediction and a proper management plan to improve the quality of life in women.

Effects of a safflower tea supplement on antioxidative status and bone markers in postmenopausal women

  • We conducted this study to examine the effects of safflower seed granular tea containing physiologically active polyphenols on antioxidative activities and bone metabolism. Forty postmenopausal women ages 49 to 64-years were recruited from Daegu and Gyeongbuk and were randomly assigned to either a safflower tea supplement (Saf-tea) group (n=27) or a placebo group (n=13). The Saf-tea group received 20 g of safflower seed granule tea per day containing a 13% ethanol extract of defatted safflower seeds, whereas the placebo group received a similar type of tea that lacked the ethanol extract. No significant changes in nutrient intake for either the placebo or Saf-tea groups were observed before or after the study period, except vitamin A intake increased after 6 months in the Saf-tea group. Dietary phytoestrogen intakes were similar in the Saf-tea group (60.3 mg) and placebo group (52.5 mg). Significant increases in plasma genistein and enterolactone were observed in the Saf-tea group. After 6 months of supplementation, serum levels of antioxidant vitamins such as a-tocopherol and ascorbic acid increased significantly, and TBARS levels decreased in the Saf-tea group compared to the placebo group. Serum osteocalcin levels were reduced (P<0.05) in the Saf-tea group after 6 months, whereas serum osteocalcin did not change in the placebo group. Urinary deoxypyridinoline/creatinine excretion was not different between the two groups at baseline, and did not change in either group after 6 months. Bone mineral density decreased significantly in the placebo group (P<0.01) but not in the supplemented group. It was concluded that polyphenols (72 mg/day), including serotonin derivatives, in the Saf-tea had both antioxidant and potential bone protecting effects in postmenopausal women without liver toxicity.

A Study on the Effect of Caffeine Intake on calcium and Phosphorus Metabolism in Ovariectomized Rats (Caffeine 섭취수준에 따른 난소절제 흰쥐의 칼슘과 인 대사 연구)

  • This study was designed to investigate the effects of caffeine intake on Ca and P metabolism in ovariectomized rats. For this purpose, the ovariectomized female rats weighing 230.8$\pm$3.5g were divided into four groups, eight rats each ; control, low caffeine(LC), medium caffeine(MC)and high caffeine(HC) groups. They were supplied for six weeks with the caffeine of 0mg, 66.8mg, 167mg and 334mg per kilogram of diet, respectively, and the results are summarized as follows. 1) There were no significant differences in feed intake, feed efficiency ratio and body weight change among all of experimental groups. But liver weight(both total weight and weight/body weight) was significantly decreased by caffeine in MC group. 2) The weight of tibia was decreased by caffeine intake in MC and HC groups. But the length, Ca and P content of tibia and femur was not changed in all of caffeine groups. 3) Ca, PTH and calcitonin levels in serum were not affected by caffeine. While, serum P level in HC group was significantly increased as compared with the control. 4) The fecal excretion of Ca and P tended to be higher in the caffeine groups, and as the result, the absorption rate, retention and retention rate of Ca and P tended to be decreased. It was noteworthy that P retention was significantly lowered in HC group as compared with LC group. The results showed that, when caffeine was taken by ovarietomized rats, the weight of tibia was decreased and the retention rate of Ca and P tended to be lowered. Therefore, too much intake of caffeine for women whose bone mineral density of tibia is decreased after postmenopause seems to accelerate the decrease of bone mineral density due to the negative effect on metabolism of Ca and P.

The Effects of Meridian Massage on Climacteric Symptoms, BMD and Serum Hormone in Perimenopausal Women (경락마사지가 폐경기 여성의 폐경 증상, 골밀도 및 혈청호르몬에 미치는 효과)

  • Purpose: We analyzed climacteric symptoms, bone mineral density (BMD), serum estradiol ($E_2$) and follicle stimulating hormone (FSH) to identify the health benefits of meridian massage in perimenopausal women. Methods: There were 16 women in the experimental group and 17 people in the control group. Meridian massage was performed for 4 weeks, 3 times a week for 20 minutes each session. The data were collected pre-treatment, posttreatment and 4 weeks after treatment. SPSS/WIN 11.5 was used for data analysis. Results: After meridian massage, there were significant differences in climacteric symptoms (U = 65.50, p = .011) and BMD (U = 65.50, p = .011) between the two groups. The E2 level showed a significant difference between the two groups pre- and posttreatment (U = 75.00, p = .028). FSH showed a significant increase when measured at 4 weeks after the treatment as compared with the amount when measured post-treatment within the control group (z = -2.249, p = .025), experimental group showed a stable change in FSH. but there was no significant difference between the groups. Conclusion: In this study, we confirmed the effects of Meridian massage in decreasing climacteric symptoms, inhibiting the decrease of BMD and stabilizing serum hormone in perimenopausal women. Therefore, it can be considered for use as a nursing intervention for health management in perimenopausal women.

  • Objective : The purpose of this study is to investigate the role of the osteoporosis in low back pain(LBP). Methods : We use method of projection with Quantitative computed tomography(QCT) to estimate Bone mineral density(BMD) of lumbar spine more precisely. From January 1999 to August 2000 we studied lumbar BMD of 46 women with LBP and 53 women with non-LBP, musculoskeletal symptoms who visited Dong-Guk University Bundang Hospital by QCT. Results : 1. Lumbar BMDs in women with LBP and women with non-LBP, musculoskeletal symptom were lower than in normal women and BMDs in women with LBP was higher than in women with non-LBP, musculoskeletal symptom but the significant difference was not revealed between two group in statistics(p<0.05).. 2. Radiological study shows that the number of degenerative spondylosis was 19, compression fracture was 11, non-radiological study was 10, normal was 5 in women with LBP. 3. Radiological study shows that the number of degenerative change was 7, compression fracture was zero, non-radiological study was 33, normal was 5 in women with non-LBP, musculoskeletal symptom. Conclusion : BMDs in women with LBP was higher than in women with non-LBP, musculoskeletat symptom but the significant difference was not revealed between two group in statistics(p<0.05).

Development of a Femur Neck Bone Mineral Density Measuring Device for Accurate Examination

  • In bone density examinations, a change in the measured BMD occurs owing to the differences between the measured areas. To address this problem, we aimed to develop a new auxiliary device that could be rotated by $15^{\circ}$ by fixing the ankle to the distal femur neck. Dual energy X-ray absorptiometry (DXA) of BMD examinations were performed once a year, but 10 patients were examined over three sessions to analyze the area for measuring the femur neck BMD. The goal of this test was to determine the device's reliability, and the results were expressed in terms of the standard deviation of measurements. After performing bone density measurements using the new auxiliary device on 10 normal patients, with three measurements for each patient, the obtained standard deviation was 0.03. The standard deviation of the measured BMD was 0.19 when using the currently existing auxiliary device, while the standard deviation of the measured BMD was 0.03 when using the new auxiliary device. By using the new auxiliary device, the standard deviation could be reduced by ~80%. Accurate rotation of the femur neck was possible in all examinations, and the standard deviation of BMD measurements could be reduced by up to 80% compared with the measurements performed using the currently existing auxiliary device. We hope that this advantageous new design can be used as a standard auxiliary device for measuring the femur neck BMD.

Effects of Dietary from Safflower Bud on the Osteoporosis in Ovariectomized Rats

  • It has been reported that safflower seeds have preventive effects against osteoporosis. Recently, safflower buds (SB) were found to have more useful functional ingredients than safflower seeds. In the current study, we evaluated the anti-osteoporosis effects of SB diet in ovariectomized (OVX) rats. The rats were divided into five groups; sham operated group, OVX alone group, OVX plus $17{\beta}$-estradiol ($E^2$ $10{\mu}g/kg$, i.p.) and OVX plus SB diet feeding group (0.3% or 1%). Feeding of SB diet (0.3% or 1%) to OVX rats markedly increased bone mineral density (BMD) of femurs, compared to the OVX group. The OVX rats exhibited a marked increase in trabecular separation (Tb.Sp) and this change was inhibited by the feeding of SB diet, similar to that seen with OVX+E2 group. Moreover, feeding of SB diet to OVX rats decreased the markers of bone turnover, including osteocalcin and alkaline phosphatase (ALP). These results suggest that SB extract has a bone sparing effect in OVX-induced trabecular bone loss and prevents deterioration of bone microarchitecture by suppressing the rate of bone turnover. Therefore, SB may be useful for preserving bone mass and structure in estrogen deficient women with a potential role in reducing postmenopausal osteoporosis.

A Cost-effectiveness Analysis of the Medication for Osteoporosis (골다공증 치료약제의 비용-효과 분석)

  • The purpose of this study is to analyze the cost-effectiveness of four medications for treating and preventing osteoporosis -HRT therapy(conjugated equine estrogen 0.625mg for 25 days and medroxyprogesterone acetate 5mg for 01112 days), Alendronate(10mg and 5mg), Active Vitamin D(Calcitriol), and Calcium. Total costs include the direct medical cost -examination fee, consultation fee, prescription fee, fee for preparing medications, and the price of pharmaceuticals- and the indirect cost of patients such as traffic expenses and time cost. In addition, the costs of monitoring in adverse reactions are added. The effects of four medications are expressed as BMD(Bone Mineral Density) percent change measured by DEXA(Dual Energy X-ray Absorptiometry) in lumbar spine(L2-L4) and femoral neck site. A mixed model based on meta analysis provides the estimates of effectiveness, which are then appled to the hypothetical cohort consisting of postmenopausal women at the age of 50-59. HRT therapy is the most cost-effective medication at 172,433.64 won (lumbar spine site) and 546,328.28 won (femoral neck site) per BMD percent change for osteoporosis. Alendronate 10mg is more cost-effective than Alendronate 5mg as 345,971.23 won and 378,441.63 won per lumbar BMD percent change at 0.991g/$cm^2$, respectively. Alendronate 10mg is more cost-effective than Alendronate 5mg as 1,329,257.89 won and 1,467,291.23 won per femoral neck BMD percent change at 0.834g/$cm^2$, respectively.

  • Various biological approaches to the promotion of periodontal regeneration have been used. These can be divided into the use of growth and differentiation factors, application of extracellular matrix proteins and attachment factors and use of mediators of bone metabolism. The purpose of this study was to evaluate the effect of enamel matrix protein and platelet-rich plasma on the treatment of intrabony defect, with bovine-derived bone powder in humans by digital subtraction radiography. 12 teeth(experimental I group) were treated with enamel matrix protein combined with bovine-derived bone powder and 12 teeth(experimental II group) were treated with platelet-rich plasma combined with bovine-derived bone powder. The change of bone density was assessed by digital subtraction radiography in this study. The change of mineral content was assessed in the method that two radiography were put into computer program to be overlapped and the previous image was subtracted by the later one. Both groups were statistically analyzed by Wilcoxon signed Ranks Test and Mann-whitney Test using SPSS program for windows(5% significance level). The results were as follows: 1. The radiolucency in 3 months after surgery was significantly increased than 1 month after surgery in both groups(experimental I and II groups)(p<0.05). 2. The radiopacity in 6 months after surgery was significantly increased than 3 months after surgery in both groups(experimental I and II groups) (p<0.05). 3. In experimental I group, there was no significant difference between 1 month and 6 months after surgery. 4. In experimental II group. the radiopacity in 6 months after surgery was significantly increased than 1 month after surgery(p<0.05). 5. There was no significant difference between experimental I and II group at 1 month and 3 months after surgery, but the radiopacity in experimental II group was significantly increased at 6 months after surgery(p<0.05). In conclusion, platelet-rich plasma can enhance bone density than enamel matrix protein until 6 months after surgery.
