  • Purpose: The objective of this study was to estimate the radiation absorbed doses in certain critical organs in the head and neck region with 16 imaging programs available on the Orthopos/sup (R)/ panoramic machine. Materials and Methods: A Rando phantom and LiF TLD chips were used for dosimetry. The absorbed doses were measured at the thyroid gland, the submandibular gland, the parotid gland, the mouth floor. the maxillary sinus, the brain, the mandibular body, the mandibular ramus. the 2nd cervical spine and the skin over TMJ area. Results : The overall absorbed doses with imaging programs available on the Orthopos/sup (R)/ panoramic machine were much less than that of standard program (program 1) except program 8, 11, and 16. Generally, the absorbed doses to the bone marrow of the mandibular ramus and the parotid gland were high, but the absorbed doses to the bone marrow in the mandibular body, brain, maxillary sinus, and, especially, the thyroid gland were very low. Conclusion : The modified imaging programs available on the Orthopos/sup (R)/ panoramic machine can be effectively used in aspect of radiation protection.

Effect of Achyrantis Radixs Administration and Cervi Cornu Parvum Acupuncture in Experimental Osteoarthritis Rats (우슬 투여와 녹용약침이 실험적 퇴행성 관절염 유발 모델에 미치는 영향)

  • Osteoarthritis(OA) is a degenerative joint disease characterized by fibrillation and erosion in cartilage tissue, chondrocyte proliferation and osteophyte formation at the joint margins, and sclerotis of subchondral bone. We investigated the effects of Acyranthes Radix administration and Cervi Cornu Parvum aqua-acupuncture in monosodium iodoacetate(MIA) induced experimental osteoarthritis model. Sprague-Dawley 60 rats of 7-8 weeks, weight $240{\pm}10\;g$ were divided into two groups including the sham operation group(15 rats) and ostoarthritis group(45 rats). Histopathological examination, Mankin's score, and the measurement of inflammation factor were performed. Histological findings that are similar to those observed in human osteoarthritis, such as disorganization of chondrocytes, erosion and fibrillation of cartilage surface, and subchondral bone exposure were observed in a MIA-induced osteoarthritis model. Saflanin-O fast green staining revealed that marked diffuse reduction of proteoglycans treated with MIA. The Mankin's score were closely correlated to the grade of histological findings. The level of prostaglandin E2 and C-reactive protein were decreased experimental groups. We conclude that Acyranthes Radix administration and Cervi Cornu Parvum aqua-acupuncture, and combination treatment exerts a beneficial influence on the cartilage lesion in osteoarthritis rat.

  • Vitamin D is an essential component of bone and mineral metabolism; its deficiency causes growth retardation and skeletal deformities in children and osteomalacia and osteoporosis in adults. Hypovitaminosis D (vitamin D insufficiency or deficiency) is observed not only in adults but also in infants, children, and adolescents. Previous studies suggest that sufficient serum vitamin D levels should be maintained in order to enhance normal calcification of the growth plate and bone mineralization. Moreover, emerging evidence supports an association between 25-hydroxyvitamin D (25[OH]D) levels and immune function, respiratory diseases, obesity, metabolic syndrome, insulin resistance, infection, allergy, cancers, and cardiovascular diseases in pediatric and adolescent populations. The risk factors for vitamin D insufficiency or deficiency in the pediatric population are season (winter), insufficient time spent outdoors, ethnicity (non-white), older age, more advanced stage of puberty, obesity, low milk consumption, low socioeconomic status, and female gender. It is recommended that all infants, children, and adolescents have a minimum daily intake of 400 IU ($10{\mu}g$) of vitamin D. Since the vitamin D status of the newborn is highly related to maternal vitamin D levels, optimal vitamin D levels in the mother during pregnancy should be maintained. In conclusion, given the important role of vitamin D in childhood health, more time spent in outdoor activity (for sunlight exposure) and vitamin D supplementation may be necessary for optimal health in infants, children, and adolescents.

Lack of connexin 32 does not enhance the benzene-induced hematotoxicity and hemopoietic tumor incidence in mice

  • This study was performed to evaluate using wild-type (WT) and $C{\times}32$ knockout (KO) mice if lack of cell to cell communication by connexin 32 gap junction enhances the benzene-induced hematotoxicity and hemopoietic tumor development. The WT and $C{\times}32$ KO mice were exposed to 300 ppm of benzene for 6 hours/day, 5 days/week for a total of 26 weeks by inhalation, and then sacrificed to evaluate the toxicities of hemopoietic organs or allowed to live out their life span to evaluate the hemopoietic tumor incidence. The significant increase and decrease of organ weight were respectively noted in spleen and thymus of both WT and $C{\times}32$ KO mice without significant difference between the genotypes. Histopathologically, benzene exposure for 26 weeks induced the morphological changes in hemopoietic organs, characterized by fat cell accumulation in the bone marrow and extramedullary hemopoiesis in the spleen. The fat cell accumulation was, compared with that of WT mice, considerably exacerbated in the $C{\times}32$ KO mice. However, no significant difference was observed in the changes of hematological values and bone marrow cellularity as well as in the onset and incidence of hemopoietic tumors between WT and $C{\times}32$ KO mice. In conclusion, this study indicated little significant role of the cellular communication by $C{\times}32$ gap junction in the action mechanism of benzene hematotoxicity and leukemogenicity.

Reverse Dorsalis Pedis Flap Based on the Distal Communicating Artery of the Dorsalis Pedis Artery for the Reconstruction of the Forefoot Defect (원위교통동맥 기저 역행성 족배피판을 이용한 전족부 결손의 재건)

  • A 31-year-old female patient presented with a skin and soft tissue defect measuring $8{\times}6cm$ in size with exposure of the extensor hallucis longus tendon and the first metatarsal bone after metatarsal lengthening for brachymetatarsia. The defect was covered with a distally based dorsalis pedis flap based on the distal communicating branch of the dorsalis pedis artery. Secondary defect was covered by a split thickness skin graft. There was congestion of the flap tip after the operation; however, it was resolved using medical leeches and anti-coagulants. No necrosis or infection was encountered and the contour of the flap was satisfactory. There was no donor site morbidity. Reverse dorsalis pedis flap has not been commonly used due to the anatomical variation and uncertainty, which is different from the reverse radial forearm flap. However, when faced with the challenge of a moderate soft tissue defect of the distal forefoot, we believe that the reverse dorsalis pedis flap offers a good option with various advantages.

Computed Tomographic Characteristics Of Nasal Squamous Cell Carcinoma In a Dog (개의 비강 편평상피세포종의 컴퓨터단층촬영상의 특징)

  • A female unknown aged Yorkshire terrier, weighing 3.5 kg with nasal discharge, left facial swelling and severe inflammation was referred to Veterinary Medical Teaching Hospital, Seoul National University. Radiographic findings were lytic change and soft tissue swelling of left zygomatic process and increased radiopacity of nasal cavity. In computed tomography, turbinate bone destruction was observed as well as lytic change of left zygomatic process and left maxilla indicating aggressive lesion. Then 3-dimensional reconstruction was also performed. Biopsy of tissue mass revealed it as squamous cell carcinoma. For this patient, euthanasia was performed guarded prognosis. Generally, tumors of the nasal cavity in dogs account for approximately 1 % to 2% of all neoplasms. The prevalence of these tumors increased with age and exposure to the ultraviolet. About two thirds of all nasal tumors are malignant tumors such as squamous cell carcinoma and adenocarcinoma. As these tumors of nasal cavity are locally invasive, and external beam radiotherapy is the current treatment of choice. Unfortunately, diagnosis of these tumors often occurs late in the course of disease, resulting in a poor prognosis for outcome in many patients. It is considered that computed tomography could be used as an aid modality for the exact evaluation of extend and degree of bone lysis of nasal cavity and metastasis.

  • Reduction cranioplasty is one of the treatment modality among many treatment options for macrocephaly with hydrocephalus. The most previous techniques of reduction cranioplasty have some disadvantages such as difficult airway maintenance, pressure sore due to modified prone position, severe venous infarct due to obstruction of venous blood flow and large amount of bleeding from the dissection of superior sagittal sinus to obtain bone fragment needed. A 28-month-old girl had extreme macrocephaly. She couldn't control head rotation and keep sitting position. The operation was performed at supine position with adequate exposure of entire calvarium and the hinge was made in occipital bone fragment that covered posterior part of superior sagittal sinus. Bleeding volume and the other complications were decreased and acceptable reduction was achieved with this method.

Medication-Related Osteonecrosis of the Jaws: A Literature Review

  • Medication-related osteonecrosis of the jaw (MRONJ) is a serious side effect of antiresorptive agents and bone-modifying agents. It is of the utmost importance to know the management of the MRONJ to improve the patient's quality of life. This study comprehensively reviews the current definitions of MRONJs, and antiresorptive medications, clinical manifestation and staging, risk factors, treatment strategies, and prevention methods of MRONJ. The disease is defined as an exposure of bone and osteonecrosis of the jaw in the oral cavity for at least 8 weeks in patients taking antiresorptive drugs or antiangiogenic agents and with no history of radiotherapy treatment of the jaws. Many articles have reported risk factors associated with MRONJ such as systemic diseases, antiresorptive medication, oral infection, and poor oral hygiene. Osteonecrosis and antiresorptive medications including bisphosphonate and denosumab have been strongly associated, but the pathology of MRONJ is only limited. Hence, an effective and appropriate management and treatment for MRONJ is still to be defined. The objectives of MRONJ treatment are to minimize osteonecrosis and relieve symptoms, and many treatments are suggested from conservative treatment to marginal resection, but this remains controversial. Appropriate treatment of MRONJ remained difficult, although many studies are being covered.

A RETROSPECTIVE EVALUATION OF IMPLANT INSTALLATION WITH MAXILLARY SINUS AUGMENTATION BY LATERAL WINDOW TECHNIQUE (측방접근법을 이용한 상악동거상술 후 임프란트 식립에 대한 후향적 평가)

  • Purpose: The aim of this study was to evaluate the clinical results of implants which were installed with maxillary sinus elevation by using lateral window technique. Materials and methods: We performed the maxillary sinus elevation by lateral window technique to 87 patients who visited Dept. of Oral & Maxillofacial Surgery, Chonnam National University Hospital from January, 2003 to January, 2007. When the residual bone height was from 3 mm to 7 mm, the sinus elevation and simultaneous implant installation was mostly performed. When the residual bone height was less than 3 mm, the sinus elevation was performed and the delayed implant installation was done after 5 or 6 months. No artificial membranes were used for coverage of the lateral bony window site and freeze dried fibrin sealant was applied to the grafted bone. The mean follow-up period was 28.5 months (ranged from 10 months to 48 months) Results: 1. Unilateral sinus elevations were performed in 51 patients and bilateral sinus elevations were performed in 36 patients. And the total number of sinus elevation procedure was 123 cases. 2. The sinus elevation and simultaneous implant installation was performed in 89 sinuses and 249 implants were installed. The sinus elevation and delayed implant installation was performed in 44 sinuses and 141 implants were installed. The total number of implants were 390 in 133 sinuses. The average healing period after sinus elevations was 6.1 months in delayed implant installation. 3. Only autogenous bone, autogenous bone mixing with allografts or autogenous bone mixing with xenografts were used as graft materials. 4. The average period from first surgery to second surgery was about 7.2 months. 5. Some patients complications, such as perforation of sinus membrane, swelling, infection and exposure of cover screw. Two implants were removed in the infected sinus. 6. The survival rate of implants with maxillary sinus elevation by lateral window technique was 99.5% and the success rate of implants was 95.1%. Conclusions: These results indicated that the implants which were installed with maxillary sinus elevation by lateral window technique showed high survival and success rates.
