Association between physical activity and health - related quality of life in Korean patients with COPD (만성폐쇄성폐질환의 신체활동수준과 삶의 질 간의 관련성)

  • Objective: Patients with COPD have a lower overall quality of life than normal people. If patients with COPD do not regularly perform physical activities, their exercise capacity is reduced. It could lead to muscle loss, and negatively affect their general physical, social, and psycho-social status. The purpose of this study was to examine association with physical activity, sedentary life time and health-related quality of life in patients with COPD. Method: Of the total of 22,948 participants surveyed in Korean National Health and Nutrition Examination Survey (KNHANES 2013-2015), 8,626 participants were used for this study. Of these, under 40 years, over 80 years, missing values and abnormal values were excluded. Study variables included physical activity level, sedentary time and health-related quality of life variables. The physical activity level assessment tool was measured using International Physical Activity Questionnaire (IPAQ). Sedentary time referred to the amount of sitting time without moving the body. EQ-5D (Euro Quality of Life-5 Dimensions) was used as an index of health-related quality of life. Control variables were age, gender, income level, education level, marital status, comorbidity, smoking, BMI, cough, sputum, COPD severity. For this study, descriptive analysis, T-test, ANOVA and multivariate regression analysis were performed. Results: Of the 1,092 patients with COPD, 76.1% (n=831) were male and 23.9% (n=261) were female, while 39.0% (n=2,939) were male and 61.0% (n=4,595) were female in the comparison group without COPD. The COPD group with high level of physical activity showed a high level of EQ-5D scores ($0.9349{\pm}0.11$, p <0.001). Among patients with COPD, after adjusting for control variables, physical activity and sedentary time (physical activity level, Β=0.047, p <.001), (sedentary time, Β=-0.017, p <.05) were associated with health-related quality of life. Conclusion: Patients with COPD have a higher quality of life as their physical activity increases and the quality of life decreases as the time spent sitting increases. This study suggests that public health experts should consider improving COPD patient physical activity.

Effects of Combinational Posture of Shoulder, Elbow and Wrist on Grip Strength and Muscle Activity (어깨, 팔꿈치, 손목의 자세에 따른 최대악력과 근육활동에 관한 연구)

  • This study aimed to analyze postures that were frequently conducted in manufacturing industry. To find grip strength and muscle activities of each posture, Maximum Voluntary Contraction (MVC) and ElectroMyoGraphy (EMG) were measured. Based on the results of this study, the most appropriate posture could be suggested and used as a basic information for preventing musculoskeletal disorders. Most work-related musculoskeletal disorders have been occurred in the fields of manufacturing industry. According to previous studies, it was reported that the rate of musculoskeletal diseases of upper extremity was higher than that of other body parts. Accordingly, there were many studies about discomfort and grip strength of upper extremity. However, these studies dealt with single selection of wrist, elbow and shoulder. So, it was insufficient for comprehensive studies about upper extremity. And in order to improve the work posture, the physiological changes being generated by the combination of wrist, elbow and shoulder postures should be observed and analyzed. In order to conduct this study, thirty university students who had no records of MSDs involved were recruited. Independent variables were postures of wrist(pronation, neutral, supination), postures of elbow(flexion $45^{\circ}$, $90^{\circ}$) and postures of shoulder(flexion $0^{\circ}$, $90^{\circ}$). And dependent variables were MVC values and EMG values. Jamar dynamometer and TeleMyo 2400T G2 was used to measure MVC and EMG. MVC and EMG for 12 postures were measured for three second and for three times. Experiment was performed randomly. A 10 minutes rest period was provided after each t. To measure muscle load, the EMG signals of eight muscles (Biceps, Medial triceps, Lateral triceps, Brachioradialis, Extensor carpi ulnaris, Extensor carpi radialis, Flexor carpi ulnaris and Flexor carpi radialis) were evaluated. MVC values and EMG values were analyzed using Minitab ver. 14. The results showed that MVC value was the highest at shoulder $0^{\circ}$, elbow $45^{\circ}$ and wrist supination. In case of wrist postures, MVC of supination is the highest. In case of elbow and shoulder postures, MVC of flexion $45^{\circ}$ and $0^{\circ}$ was the highest. It was found that there were interaction between wrist and elbow posture under shoulder flexion and between shoulder and wrist under elbow flexion $45^{\circ}$. In case of the angle of shoulder $0^{\circ}$, elbow $45^{\circ}$ and wrist supination, the EMG values of four muscles(Medial Triceps, Extensor carpi ulnaris, Extensor carpi radialis, Flexor carpi ulnaris) were the highest. Based on this study, it is worth to note that the combination postures of upper extremity have a large impact on the MVC and EMG. The optimal condition upper extremity was shoulder flexion $0^{\circ}$, elbow flexion $45^{\circ}$ and wrist supination for preventing work-related musculoskeletal disease.

Sensitivity Analysis and Optimization of Design Variables Related to an Algorithm for Loss of Balance Detection (균형상살 검출 알고리즘에서 검출과 관련된 설계변수의 민감도 해석 몇 최적화)

  • This study suggested an optimized algorithm for detecting the loss of balance(LOB) in the seated position. And the sensitivity analysis was performed in order to identify the role of each design variable in the algorithm. The LOB algorithm consisted of data processing of measured signals, an internal model of the central nervous system and a control error anomaly(CEA) detector. This study optimized design variables of a CEA detector to obtain improved values of the success rate(SR) of detecting the LOB and the margin time(MT) provided for preventing the falling. Nine healthy adult volunteers were involved in the experiments. All the subjects were asked to balance their body in a predescribed seated posture with the rear legs of a four-legged wooden chair. The ground reaction force from the right leg was measured from the force plate while the accelerations of the chair and the head were measured from a couple of piezoelectric accelerometers. The measured data were processed to predict the LOB using a detection algorithm. Variables S2, h2 and hd are related to the detector: S2 represents a data selecting window, h2 a time shift and hd an operating period of the LOB detection algorithm. S2 was varied from 0.1 to 10 sec with an increment of 0.1 sec, and both h2 and hd were varied from 0.01 to 1.0 sec with an increment of 0.01 sec. It was found that the SR and MT were increased by up to 9.7% and 0.497 sec comparing with the previously published case when the values of S2, h2 and hd were set to 4.5, 0.3 and 0.2 sec, respectively. Also the results of sensitivity analysis showed that S2 and h2 had considerable influence on the SR while these variables were not so sensitive to the MT.

Methods for Measurement of Moment of Segmental Inertia Using a Dynamometer (동력계를 이용한 분절관성모멘트 측정 방법)

  • Moments of inertia of limb segments are essential to calculate parameters related to the segmental rotation. To analyze the human motion accurately and specifically, moments of inertia obtained from the individual are required. In this study, a simple method to determine a subject-specific moment of segmental inertia using a dynamometer is introduced. In order to evaluate the method, one male participated to test for his forearm plus hand on a commercial dynamometer. Three passive speeds, i.e. 240, 270, and $300^{\circ}/s$, were chosen to confirm whether the moment of inertia values at each speed approach to a fixed value. The same procedure was repeated on the day after to evaluate whether the method is reproducible. As the results, there were no significant differences among the speeds and between the days. The value of the moment of the forearm inertia was 0.216 $kg{\cdot}m^2$ that is apparently higher compared to values by previous models. Nonetheless, it seems to be acceptable based on our body mass index analysis using reported subject height and mass in each previous study. According to our results, the developed method could be useful to determine the segmental moment of inertia of an individual, showing no significant differences among the speeds and between the days. Thus, we believe that our results are reliable according to two appropriate evaluation procedures. This finding would be helpful to calculate segmental rotation related parameters of an individual.

Image Evaluation and Association Analysis of the Cardiovascular Disease of the Degree of Pancreatic Steatosis in Ultrasonography

  • Increasing fat tissue of obese people, increases the rate of cardiovascular disease, diabetes, metabolic syndromes and dyslipidemia. An increase in the focal tissue of pancreas is a known risk factor of these diseases. Although there exists sufficient research on the diagnosis and treatment of pancreatic cancer, studies have been done on fatty pancreas. In this study, based on ultrasound imaging and using a texture characteristic of GLCM, fatty pancreas was divided into three categories: mild, moderate and severe. We compared and analyzed the three groups was by Pancreatic ultrasonography and body characteristics, serological tests, pressure and the degree of arteriosclerosis, against normal control group. The following parameters of control and test groups were measured: WC (waist circumference),BMI (body mass index), TC (total cholesterol), TG (triglyceride), HDL-C (High-density lipoprotein cholesterol) and LDL-C (Low-density lipoprotein cholesterol), SBP (systolic blood pressure), BST (Blood Sugar Test) and aortic PWV (pulse wave velocity). We observed the values correspondingly increasing fat deposition. However, ABI (Ankle Brachial pressure index) stenosis and HDL-C levels decreased with increasing fat deposit (p <0.05); a drop in these parameters are known to be harmful to the human body. The difference in texture characteristics between normal control group and pancreatic fatty group (mild, moderate, and severe) was statistically confirmed. Ultrasound imaging of pancreatic steatosis categorized the disease as mild, moderate and severe based on the characteristic texture. In conclusion, we observed on increase in metabolic syndrome, dyslipidemia, and arteriosclerosis, proportional to the degree of pancreatic fat deposition. The escalation of these diseases was confirmed and was directly related with predictors of cardiovascular diseases.

Effects of Artemisia Iwayomogi Oligosaccharide on the Blood Lipids, Abdominal Adipose Tissues and Leptin Levels in the Obese Rats (인진쑥 올리고당의 섭취가 비만쥐의 혈중 지질, 복강내 지방조직과 렙틴 수치에 미치는 영향)

  • As obesity is known to be related to hyperlipidemia, insulin and leptin resistance, and other chronic diseases, much recent research has focused on functional food materials and their anti-obesity activity. This study was performed to study the effects of Artemisia Iwayomogi oligosaccharide AIPI on the anti-obesity function in normal rats and diet-induced obese (DIO) rats. F344 male rats were divided into four groups: Normal-control (CONT), normal-AIPI, DIO-CONT and DIO-AIPI. The groups were provided with water (in the CONT groups) or another drink for 4 weeks. The final body weights of rats in the DIO-AIPI group were lower than those in the CONT group. Abdominal adipose tissue weight per kg of body weight in the DIO-AIPI group was significantly lower than that in the DIO-CONT group. Also, the final levels of serum-triglyceride, serum-total cholesterol and serum-low density lipoprotein cholesterol in the DIO-AIPI group were lower than those in the DIO-CONT group. Moreover, the serum-high density lipoprotein cholesterol level in the normal-AIPI group was significantly higher than that in the normal-CONT group. Finally, the serum-leptin concentration was significantly lower in the DIO-AIPI group. Total lipid, triglyceride, and total cholesterol contents in the feces of the DIO-AIPI group were as high as 142%, 199%, and 165% of the respective values of the DIO-CONT GROUP. These results indicate that orally administered Artemisia Iwayomogi oligosaccharide not only has hypotriglycemic and hypocholesterolemic effects, but also has the effect of reducing the body weight and the abdominal adipose tissue weights obese rats. Therefore, we expect that Artemisia Iwayomogi oligosaccharide AIPI may have an anti-obesity function in F344 diet-induced obese rats. (Korean J Nutrition 36(5): 437∼445, 2003)

A Study on the Modern Sport-Fashion (현대 스포츠패션에 관한 연구)

  • The purpose of this is to analyze the aesthetics characteristics of our modern sports fashion, and thereby, discuss them in the light of the overall mentality or the 20th century, and thus, present the conditions or sports fashion design meeting modem people's divers aesthetics values and desired. Modern sports fashion sues can be categorized into futurist sports style, erotic sports style and American sheet sports style. And the 20th century mentality characterized by changes of lifestyle, identity and aspiration has influenced the aesthetic features of such sports fashion sues, which can be summed up as follows; first, the futurist sports style applies the functional items and details of active sportswear to design, while heralding a positive and hopeful message of technology and future by using the material of hi-tech functions and senses. This sports style was affected much by shift from social status, attraction and wealth to demonstration of state-of-the-art science, pursuit of functionality in terms of shapes and materials, convenience through See combinations of sportswear items or design elements. Second, the erotic sports style based on minimalism attempted to express the erotic body beauty indirectly by exposing some parts of body or using the material pressed against the body. This sports style was closely related with the changes of sexual identity such as neutral sexualism, bi-sexualism and homo-sexualism. Lastly, the American street sports style was born from black Americarns' sports and dances. This spors style pursues “youth” beyond TPO concept As mass media and commercial sports developed, the young generation copied sports stars' uniforms or fashions to share honor, wealth and youth with them. In sort, the American sheet sports style was affected much by the so-called “heroism”. Such a changed object of aspiration influenced the aesthetic characteristics of American shot sports style directly.

Curing Behaviors and Viscoelastic of UPE Resins with Different Glycol Molar Ratios (글리콜 몰비가 다른 불포화 폴리에스테르 수지의 경화거동 및 점탄성)

  • In this study, the effects of different glycol molar ratios of unsaturated polyester(UPE) resins on the curing behaviors were investigated. The cross linking process was checked or monitored by differential scanning calorimetry(DSC) and by viscoelastic properties of rigid-body pendulum model. The knife-edge from which the pendulum is suspended, is immersed in a reaction mixture, and the change of the viscoelastic behavior brings on those of the period(T) and logarithmic decrement(${\Delta}$) of the damped free oscillations of the pendulum. The values of T and ${\Delta}$ obtained are related to the dynamic modulus(E') and modulus loss(E'). The information on the viscoelastic behavior of unsaturated polyester(UPE) resins during the curing process are shown to illustrate the usefulness of the techniques. As the content of NPG in a propylene glycol(PG)/NPG glycol mixture increased, both the cycle time during cure and the change of damping during cure of UPE resin decreased.

Nutrient Intake and Risk of Metabolic Syndrome in Male Smokers (일부 흡연 남성의 식사섭취와 대사증후군 위험도)

  • This study was intended to investigate the risk of metabolic syndrome (MS) in healthy 92 male smokers (mean age: $44.4\;{\pm}\;7.8\;yrs$). We investigated the anthropometric assessment and dietary intake survey for 2 days by 24-recall method, also blood pressure and serum lipids were measured. The average numbers of cigarettes smoking a day were 21.3/d, smoking duration were 21.5 years. The proportion of fat energy was 24.9% and intakes of vitamin B2, folate, calcium, potassum and fiber were lower than KDRI. The body mass index (BMI) and body fat % were 24.8, 23.9% respectively. The systolic ($134.1\;{\pm}\;1.4\;mmHg$) and the diastolic blood ($87.9\;{\pm}\;1.1\;mmHg$) pressure were in borderline hypertension. Among biochemical parameters, TG ($173.6\;{\pm}\;9.4\;mg/dL$) and fasting blood glucose ($109.0\;{\pm}\;2.4\;mg/dL$) levels were out of normal values. The most occurred problem among the risk factors related to MS was the borderline hypertension (63%) in subjects. Regarding the correlations of anthropometric data with biochemical factors, TG was significantly correlated with the BMI, body fat % and waist circumference. Smoking years showed positive correlation with AI. These results suggest that the smoking habit has significant relations with the risk factors of MS. Therefore, quitting is necessary to prevent MS, and nutrition education and dietary management program are required to prevent the degenerative disease.

Association between body mass index and hepatitis B antibody seropositivity in children

  • Background: The seropositivity rate of hepatitis B surface antigen (anti-HBs) antibodies is known to be ≥95% after hepatitis B virus vaccination during infancy. However, a low level or absence of anti-HBs in healthy children is discovered in many cases. Recent studies in adults reported that a reduced anti-HBs production rate is related to obesity. Purpose: To investigate whether body mass index (BMI) affects anti-HBs levels in healthy children following 3 serial dose vaccinations in infancy. Methods: We recruited 1,200 healthy volunteers aged 3, 5, 7, or 10 years from 4-day care centers and 4 elementary schools. All subjects completed a questionnaire including body weight, height, and vaccine type received. Levels of serum hepatitis B surface antigen (HBsAg) and anti-HBs in all subjects were analyzed using electrochemiluminescence immunoassay. The standardized scores (z score) for each sex and age were obtained using the lambda-mu-sigma method in the 2017 Korean National Growth Charts for children and adolescents. Results: Our subjects (n=1,200) comprised 750 males (62.5%) and 450 females (37.5%). The overall anti-HBs seropositivity rate was 57.9% (695 of 1,200). We identified significant differences in mean BMI values between seronegative and seropositive groups (17.45 vs. 16.62, respectively; P<0.001). The anti-HBs titer was significantly decreased as the BMI z score increased adjusting for age and sex (B=-15.725; standard error=5.494; P=0.004). The probability of anti-HBs seropositivity based on BMI z score was decreased to an OR of 0.820 after the control for confounding variables (95% confidence interval, 0.728-0.923; P=0.001). Conclusion: There was a significant association between anti-HBs titer and BMI z score after adjustment for age and sex. Our results indicate that BMI is a potential factor affecting anti-HBs titer in healthy children.
