A Literature Study on the Diagnostic Factors and Value as a Syndrome of Damjeok (담적의 진단요소 및 증후군으로서의 가치에 대한 문헌고찰)

  • Objectives: The purpose of this study is to investigate the diagnostic factors of Damjeok in the literature. Methods: This study used the databases of five (Mediclassics, RISS, KMbase, OASIS, CNKI) to analyze diagnostic factors of Damjeok. Literatures published by May 2023 were assessed and we classified 91 literatures dealing with diagnostic factors by 5 categories, 'characteristics', 'location', 'symptom', 'pulse wave & tongue feature', and 'eating habits & lifestyle'. Results: Damjeok is mainly formed at the stomach in the form of lump or hard seed, and it can be observed in other regions or even the whole body by its trait of floating. Damjeok mainly shows symptoms of the digestive system and also causes various symptoms associated with the respiratory system and infection, musculoskeletal system, urogenital system, neuropsychiatric system and cardiovascular system. Regarding pulse wave and tongue feature, a slippery wave and greasy coated tongue are typical features of Damjeok. Additionally, bad eating habits and a lack of exercise have been described as a factor that cause and aggravate Damjeok. Conclusions: The two most important factors in the diagnosis of Damjeok are abdominal stiffness by physical examination and digestive symptoms, and these correspond to the definition of a syndrome. Diagnostic factors based on literature evidence will provide clues to the clinical diagnosis of Damjeok syndrome.

Effect of X-irradiation on Fetal Development During Pregnancy in the Rats (X-線 照射가 래트 태아의 발육에 미치는 영향)

  • This study was carried out to find if the X-irradiation being used for clinical diagnosis during pregnancy would affect fetal development and cause fetal malformation in rats or not. To determine the dose and irradiation frequency of X-irradiation and gestation period by which fetal development would be affected when irradiated during pregnancy, seventy-two Sprague Dawley female rats (8 weeks old) were used for the experiment and grouped into three according to different gestation period of 5-8 days, and 6-12 days of gestation. Experimental rats were irradiated on the daily irradiation conditions of 40, 60, 80 kvp(kilo volt peak), 150 mA(milliampere), 0.25 sec and 4 times/day for both 5-8 days and 10-13 days of gestation, and 100 kvp, 100 mA, 2 min. and 4 times/day for 6-12 days of gestation. Rats were put in a small dark box when irradiated, which animals were sacrificed on the 20th day of gestation and mean litter size, fetal body weight, fetal crown-rump length(CRL) were investigated along with pathological findings. 1. Litter size were significantly decreased in the rats which were irradiated by both 60 and 80 kvp during 5 to 8 days of gestation and by 100 kvp during 6-12 days of gestation compared to those from the control rats(p<0.05) 2. Fetal body weight was significantly decreased in the fetus from the rats which were irradiated by both 60-80 kvp during 5-8 days of gestation and by 100 kvp during 6-12 days of gestation compared to those from the control rats(p<0.05). 3. There was no significant difference of fetal crown-rump length between all the experimental rats and the controls. 4. Fetal absorption, fetal death, and fetal malformation were not observed in the fetus form the rats irradiated by 40-80 kvp during 5-8 and 10-13 days of gestation, however, the pathological findings were found in those from the rats irradiated by 100 kvp during 6-12 days of gestation. 5. The harmful effect of x-irradiation on fetal development was estimated to occur when irradiated during 5-8 days of gestation. These results indicated that even X-irradiation for clinical diagnosis could affect fetal development in the early embryonic stage and when the fetus were exposed to frequent and prolonged x-irradiation with over dose.

A Study on Fluid Intake and Output Measurements (수분 섭취 및 배설량의 측정방법에 관한 연구)

  • The Fluid and electrolytes balance in the body is of critical importance in maintaining good health. When the fluid and electrolyte imbalance is present, patients are in great danger. They must be assessed immediately by a nurse so that appropriate treatment can be started as soon as possible. Patients' fluid intake and output records contain highly important information for the diagnosis and treatment of fluid imbalance, but, these records are often inaccurate and the method of recording the fluid intake is not universal for every hospital. Be-cause they are few quantitative measurements of a patient's hydration, the need to improve the accuracy of fluid intake records is very important. However, very few studies have been done to investigate the accuracy of measurements of patients' fluid intake and output. The purpose of this study was to investigate the methods used for calculation of fluid intake which is most similar to fluid output in normal adults and hospitalized patients. This study focused on three different calculation methods for fluid intake and compared these to fluid output and developed suggestions as to the ideal way to record fluid in-take. Data for 43 hospitalized patients and 37 normal adults were analyzed. The findings of this study are as follows ; 1) In normal adults, the daily intake of water which enteres by the oral route was 2415m1 (the first method of calculation). The daily intake of water in the form of pure water or some other beverage was 1365m1 (the third method of calculation) The daily intake of water including fresh fruits and vegetables, rice, porridges, and Me m which have water content more than 80% were 2186m1 (the second method of calculation). 2) The urine output of the normal adults was 1350m1. This apprroximates the amount of fluid an adult takes in the form of pure water. 3) In patient group, the total intake of water was 2550m1 (the first method of calculation). The in-take of water in the form of pure water or as some other beverage and IV fluid was 1661m1 (the third method of calculation). The daily in-take of water including foods which have high water content was 2356m1 (the second method of calculation). 4) The urine output of the patient's group was 1728m1. This approximates the amount of fluid an adult takes in the form of pure water. 5) Investigation of the method of calculation of the patient fluid intake showed that among the 31 hospitals studied, only eight use the third method of calculation which reflects the most close value to urine output. From the results obtained in this study, it was indicated that the amount of fluid taken in the form of pure water reflects the most close value to urine output. Therefore, it can be suggested that the third method of calculation which includes water in-take only in the form of pure water or beverage should be used as patients' fluid intake record.

  • Panoramic radiographs are a relatively simple technique that is commonly used in all dental specialties. In panoramic radiographs, in addition to the formation of real images of metal objects, ghost images may also form, and these ghost images can hinder an accurate diagnosis and interfere with the accuracy of radiology reports. Dentists must understand the formation of these images in order to avoid making incorrect radiographic diagnoses. Therefore, the present study sought to present a study of the formation of panoramic radiograph ghost images caused by metal objects in the head and neck region of a dry skull, as well as to report a clinical case in order to warn dentists about ghost images and to raise awareness thereof. An understanding of the principles of the formation of ghost images in panoramic radiographs helps prevent incorrect diagnoses.

Gastric Follicular Lymphomas Presenting as Subepithelial Tumors: Two Cases (위 상피하 종양으로 발견된 소포림프종 2예)

  • Follicular lymphoma is the most common form of low-grade B cell lymphoma. Follicular lymphoma occurs predominantly at lymph node sites and rarely in the gastrointestinal tract. Rare gastrointestinal follicular lymphoma is most commonly found in the small intestine, especially in the duodenum, and appears as multiple granules. However, gastric follicular lymphoma mostly appears as a subepithelial tumor. We observed two primary gastric follicular lymphomas that resembled subepithelial tumors located in the body of the stomach. Endoscopic ultrasound revealed hypoechoic lesions located in the submucosa layer. Since endoscopic forceps biopsies were inconclusive, we performed endoscopic submucosal dissection, which resulted in a final pathologic diagnosis of follicular lymphoma. Because of the indolent nature of gastrointestinal follicular lymphoma, the "watch and wait" strategy can be applied in the early phase. The identification of endoscopic characteristics of gastric follicular lymphoma can be helpful for differential diagnosis and decision of treatment strategy. Therefore, we report two cases of primary gastrointestinal follicular lymphoma diagnosed following endoscopic submucosal dissection.

  • The time therapy in oriental medicine is to reserch physiological activity and pathological variation with time, or time with disease. In that, this therapy is to research the development of disease, progress, diagnosis, treatment, and the meaning of preventive medicine. The theory of time therapy has been developed form heaven-human corresponding principle, and fourtimes of seasons is related with human life. After that, this theory affects in yin-yang five circle principle and organ-meridian match system. The independent development was formed later, but the origin has progressed for a logn times. in relation with time therapy, traditional acupuncture therory suggested five semen. In twenty century, the doctor rheinhold voll suggested energy exchange theory, this theory is based on time therapy, but has partial differance also too. His oppinion is this´ the energy present in the body has a circulation following the acupuncture teaching, i.e., the organs and their meridians are flooded in a certain sequence by this flowing energy. This maximum times of the organs are of physiologic and pathologic significance to man. In next time, we will discuss the indivisual theory in detail, and wil have comparison, and considering too.

  • Three planks of the Triangular position theory was first introduced by Sanghallon(傷寒論) authored by Jang Gi. It is the regulation for the matter of diagnosis of three different clinical symptoms in the causes of exogenous disease. It brought two questions to the scientists. First question is what are the Wind stroke(中風), Affection by cold(傷寒) and the complex form of Wind stroke(中風) and Affection by cold(傷寒) mentioned in Sanghallon. It is related to the theoretical analysis of the causes for the outbreaks of exogenous disease. Second question is what are the characteristics of symptoms to use Gyejitang, Mahwangtang, Daecheongnyongtang according to Sanghallon? It is the matter of clinical skills for dispensing a prescription. Through the theoretically and empirically deepening processes for last two millenniums, those two questions brought us two more problems further. That is, how and by what processes the Six vital substances for humal life outbreaks disease as a energy transformation? And, how the individual peculiarities of human body as a disease container should be treated? In conclusion, it is also a matter of the existence of dialectic medical science.

A Case of Granular Cell Tumor in the Area of Arytenoid Cartilage (피열연골 부위에 발생한 과립세포종 1례)

  • Granular cell tumors are relatively uncommon benign laryngeal lesions thought to originate form Schwann cells. The granular cell tumor occurs everywhere in the body, especially in the head and neck. The larynx is relatively an uncommon location, accounting for approximately 3 to $10\%$ of all reported cases. Typically the most common presenting symptom is hoarseness, with some patients also presenting stridor, hemoptysis, dysphagia, and otlagia. But the tumor may be asymptomatic and discovered only incidentally during a routine examination. The diagnosis of granular cell honor can be confirmed by histopathologically and immunocytochemical staining fer S-100 antigen. Treatment of a granular cell tumor consists of a wide local excision by the endoscopic, transoral or laryngofissure methods. Recently, CO2 laser has been used to remove granular cell tumor with clear resection margin. This article describes one such case in a 62-year-old man, followed by a brief review of the literature on this subject.

Gastrointestinal Stromal Tumor of the Stomach Presenting as Multilobular with Diffuse Calcifications

  • Gastrointestinal stromal tumors (GISTs) are the most common primary mesenchymal neoplasms of the gastrointestinal tract and usually appear as a well-circumscribed mass. However, it may be difficult to confirm the extent of the disease for some GISTs. A 70-year-old asymptomatic female presented for a regular physical exam. An esophagogastroduodenoscopy showed a 2.0 cm protruding mass on the gastric fundus. Endoscopic ultrasound revealed an ill-defined heterogenous hypoechoic lesion ($3.0{\times}1.5cm$). A computed tomography (CT) scan demonstrated a 4.5 cm multifocal calcified mass at the gastric body as well as at the gastric fundus. Laparoscopic gastric wedge resection was performed according to the extent of multifocal calcifications that are shown on the CT. Intraoperative specimen mammography and intraoperative biopsy might be helpful to obtain a tumor-free margin. Final pathologic diagnosis was an intermediate risk GIST in multilobular form. In patients with diffuse multifocal calcifications in the stomach, the possibility of GIST should be considered.

Cardiac Tumors (심장종양 6례 보고)

  • Primary cardiac tumors are uncommon in all age group. In contrast, tumors metastatic to the heart are significantly more common. On rare occasions, tumor may extend into the heart chamber via inferior vena cava from other parts of the body, such as liver, kidney, and uterus cava. With recent advancement in diagnostic imaging modalities and surgical techniques, cardiac tumors are now potentially curably form of heart disease. The most important factor in diagnosing the tumor is a high index of clinical suspicion. Six patients underwent surgical removal of intracardiac tumor during a 5-year period. The mean age of the 4 women and two men was 40 years [range 23 to 60]. All patients were operated on in the last five years of the studied period. All patients had symptoms varying in duration from 1 month to 4 years [average 13 months]. 2-Dimensional echocardiography contributed most to preoperative diagnosis, confirming presence of an intracardiac tumor in all examined patients. Of the six intracardiac tumor, 5 were myxomas [4 left atrial and 1 right ventricular] and one right atrial metastasis from hepatocellular carcinoma of the liver. In all cases, tumor masses were successfully excised. One patient expired after the operation on account of low cardiac out-put syndrome. Remained one patient among six, tumor mass extended into RA and RV with a stalk via IVC. On later follow-up study showed cold area on liver scan [hepatocellular ca.], so she was transferred to internal medicine, department for chemotherapy. Follow up results showed no signs of tumor recurrence in 4 myxoma cases.
