Study on the Bitonghyul (비통혈(脾統血)에 관한 고찰)

  • Although Bi(脾 Spleen) is a major organ which controls digestion and absorption in Korean medicine, there also introduced another function of bi, which is called 'bitonghyul(脾統血)' that means the spleen controls(holdings) the blood. While the spleen is involved in the function of blood generation through the digestion and absorption of food, 'bitonghyul' is another expression regarding function of the spleen. The meaning of 'bitonghyul' is related with the function of keeping the blood flowing within the blood vessels, hemostasis. Comparing the function of the spleen with the mechanisms of hemostais, it is concluded that 'bitonghyul' is a physiological effects which affect hemostatic mechanisms via absorption of vitamin K.

  • Retinopathy of Prematurity (ROP) is a common retinal neovascular disorder of premature infants. It can be characterized by inappropriate and disorganized vessel. This paper present a method for blood vessel detection on infant retinal images. The algorithm is designed to detect the retinal vessels. The proposed method applies a Lapalacian of Gaussian as a step-edge detector based on the second-order directional derivative to identify locations of the edge of vessels with zero crossings. The procedure allows parameters computation in a fixed number of operations independent of kernel size. This method is composed of four steps : grayscale conversion, edge detection based on LOG, noise removal by adaptive Wiener filter & median filter, and Otsu's global thresholding. The algorithm has been tested on twenty infant retinal images. In cooperation with the Digital Imaging Research Centre, Kingston University, London and Department of Opthalmology, Imperial College London who supplied all the images used in this project. The algorithm has done well to detect small thin vessels, which are of interest in clinical practice.

Effects of Saengjihwangeumja-gami on STZ-induced Diabetic Mice (생지황음자가미(生地黃飮子加味)가 Streptozotocin으로 유발된 당뇨생쥐에 미치는 영향(影響))

  • First, mice were dosed with 50mg/kg of streptozotocin(STZ) twice every 24 hours to cause high blood-sugar. Then, after three days, mice were injected with 100mg/kg of STZ again. Two different dosages of Saengjihwangeumja-gami were given to the experiment groups: SA group, 15mg/kg/day, and SB group, 90mg/kg/day, in order to determine the effects of Saengjihwangeumja-gami, which has been known to be good for DM(Diabetes Mellitus). By observing weight and blood-sugar level changes, blood tolerance, the numerical value of BUN(Blood Urea Nitrogen) and creatinine in blood, and through light-electronicmicroscopic and immunohistologic investigations of pancreas and kidneys, the following results were obtained: 1. The experiment groups showed a high suppressive effect of weight-loss. 2. The experiment groups' blood-sugar and blood tolerance showed an effective lowering of blood-sugar levels. 3. The experiment groups did not show any noticeable change in the numerical value of BUN and creatinine in blood compared with that of the control groups. 4. The experiment groups showed a higher Insulin positive reaction of pancreatic islets ${/beta}-cell$ than the control groups. 5. The experiment groups showed a higher immuno-reaction against IGF- II than the control groups. 6. Observation of apoptosis of the pancreatic islets showed that the cells of experiment groups were less injured compared with those of the control groups, and fewer apoptag-positive reaction cells were seen in experiment groups than in the control groups. 7. Uunder electron-microscopy, the insulin-containing granules in pancreatic islets ${/beta}-cells$ had increased more in the experiment groups than in the control groups. 8. Under light microscopy, the injury on the inner & outer membrane of the glomerulus and epithelial cells of capillaries and cells among vessels were fewer in the experiment groups than in the control groups. 9. More apoptag-positive reaction cells in the kidney were seen in the control groups than in the experiment groups. 10. PAS-positive reaction substances had increased more in the substrate among the vessels of a glomerulus belonging to the control group than those of the experiment group. 11. Uunder electron-microscopy, the nucleonic membrane, nucleoplasm and mitochondria of proximal and distal renal tubular were more injured in the control groups than in the experiment groups. In conclusion, strong evidence for the efficacy of Saengjihwangeumja-gami in lowering blood-sugar, and in recovery and generation of pancreatic tissues injured by DM was observed. Results suggest Saengjihwangeumja-gami is an effective treatment for DM. Further study of the principles of blood-sugar dropping effects of Saengjihwangeumja-gami are needed, as well as further study of recovery and regeneration of pancreatic tissues injured by DM.

Case-control Study : Cerebral Blood Flow as Measured by TCD in Patients with Risk Factors for Stroke (TCD를 이용한 정상군과 중풍원인질환군의 혈류측정에 관한 비교연구)

  • Objectives : The purpose of this study was to evaluate the blood flow using doppler ultrasound of the MCA, ACA, PCA, BA, ICA in the patients with risk factor for stroke. Methods : 110 patients with risk factor for stroke were selected who had hypertension, diabetes mellitus, hyperlipidemia, or heart disease, as well as 89 healthy adults who did not have any symptoms of those diseases. To evaluate the blood flow, the Vs and Vm of the MCA, ACA, PCA, BA, and ICA in the two groups were measured. Result : In normal healthy adults, subjects showed a decrease in the Vs and Vm: with advancing in age, there was a significant difference in the Vs of MCA, PCA, BA, and ICA. There was a significant difference in the Vm of MCA, PCA, BA, and ICA. In normal healthy adults, females showed high velocities in all examined vessels. There was a significant difference in the Vs of BA. There was a significant difference in the Vm of BA, and ICA. There was a decrease in the Vs, and Vm of all examined vessels of patients with risk factors for stroke in comparison with normal healthy adults. There was no significant difference in the Vs. There was a significant difference in the Vm of MCA, ACA, and PCA. Results were the same between patients aged under 50 and total patients in the Vs and Vm of examined vessels. There were differences between patients aged over 50 and total patients in the Vs of MCA, PCA, BA and for patients aged over 50; patients with risk factor for stroke who had hypertension, diabetes mellitus, hyperlipidemia, or heat disease has higher Vs than healthy adults. Conclusions : There was a significant difference in the blood flow velocity between patients with risk factors for stroke and healthy adults. However, this result was different from results of comparison of TCD findings between patients and healthy individuals by age. Therefore, more detailed studies about aged patients are needed.

Study on the Anatomical Pericardium Meridian Muscle in Human (수궐음 심포경근의 해부학적 고찰)

  • Objectives : This study was carried to identify the component of the Pericardium Meridian Muscle in human. Methods : The regional muscle group was divided into outer, middle, and inner layer. The inner part of body surface were opened widely to demonstrate muscles, nerve, blood vessels and to expose the inner structure of the Pericardium Meridian Muscle in the order of layers. Results We obtained the results as follows; He Perfcardium Meridian Muscle composed of the muscles, nerves and blood vessels. In human anatomy, it is present the difference between terms (that is, nerves or blood vessels which control the muscle of the Pericardium Meridian Muscle and those which pass near by the Pericardium Meridian Muscle). The inner composition of the Pericardium Meridian Muscle in human is as follows ; 1) Muscle P-1 : pectoralis major and minor muscles, intercostalis muscle(m.) P-2 : space between biceps brachialis m. heads. P-3 : tendon of biceps brachialis and brachialis m. Supercharged Technique in TRAM flap Breast Reconstruction (과급 횡복직근피판술(Supercharged TRAM)을 이용한 유방재건술)

  • Purpose: When reconstruction for patients who have the large contralateral breast or a following large defect after mastectomy is required, conventional pedicled TRAM flap shows the unpredictable occurrence of fat necrosis and skin flap loss in a relatively high percentage due to insufficient blood supply. In an effort to obtain more stable TRAM flap blood circulation, we have performed a supercharged technique using deep inferior epigastric perforators (DIEP) with conventional pedicled TRAM flap. Methods: From September of 2006 to December of 2008, Fourteen supercharged TRAM flap were performed for breast reconstruction after modified radical mastectomy. The contralateral DIEP was anastomosed to the internal mammary vessels in contralateral pedicled TRAM flap or thoracodorsal vessels in ipsilateral pedicled TRAM flap. Nutrient vessels were selected by Multi-Detector Computed tomography (MD-CT) modalities. For the nutrient vessel, we used deep inferior epigastric vessels (DIEV) of the ipsilateral side in 8 patients, DIEV of the contralateral side in 6 patients. In addition, for the recipient vessel, we used thoracodorsal vessels in 8 patients, internal mammary vessels in 5 patients, intercostals artery perforators in 1 patient. Results: The mean age was 46.8 years and the average follow-up interval was 14 months. There were 11 immediate and 3 delayed breast reconstructions. Fat necrosis incidence rate in supercharged TRAM group was lower than in conventional TRAM flap group. There were no differences of the incidences of abdominal hernia in both groups. Conclusion: The supercharged TRAM flap produces an improvement in vascularity that permits use of all four zones of the flap. The breast reconstruction with supercharged technique is reliable and valuable methods which provide sufficient soft tissue from abdomen without significant complications.

Modeling of Hemodynamics in Stenosed Artery (협착 동맥혈관의 혈류유동 모델링)

  • This study is about atherosclerosis which occupies the highest rate in many diseases people have and we have studied about atherosclerosis for abdominal aorta. Atherosclerosis is the phenomenon which blood vessel gets narrower, harder and thicker due to the stenosis of colesterol in blood vessel. If it becomes worse, arteries will be hard and blood can't flow smoothly, and even it can reach to death. In this study, the geometric models of the considered stenotic blood flow are two different types of constriction of cross-sectional area of blood vessel; 20 and 45% of constriction in each elastic wall and rigid wall. We have modeled by using finite element method to observe the changes of velocity and pressure. In case of the diameter of blood vessel decreased 45% in elastic wall model, the values of velocity and pressure were higher than the case of 20% and in case of the diameter of blood vessel decreased 45% in rigid wall model, the values of velocity and pressure were higher than the case of 20%. In cases of elastic wall models of the diameters of blood vessels decreased each of 20% and 45%, recirculation zones appeared. This results show understanding of hemodynamic properties depending on stenosed blood vessels.

Superoxide Dismutase Activity in Small Mesenteric Arteries Is Downregulated by Angiotensin II but Not by Hypertension

  • Many studies reported reduced antioxidant capacity in the vasculature under hypertensive conditions. However, little is known about the effects of antihypertensive treatments on the regulation of vascular antioxidant enzymes. Thus, we hypothesized that antihypertensive treatments prevent the reduction of antioxidant enzyme activity and expression in the small vessels of angiotensin II-induced hypertensive rats (ANG). We observed the small mesenteric arteries and small renal vessels of normotensive rats (NORM), ANG, and ANG treated with a triple antihypertensive therapy of reserpine, hydrochlorothiazide, and hydralazine (ANG + TTx). Systolic blood pressure was increased in ANG, which was attenuated by 2 weeks of triple therapy (127, 191, and 143 mmHg for NORM, ANG, and ANG + TTx, respectively; p < 0.05). Total superoxide dismutase (SOD) activity in the small mesenteric arteries of ANG was lower than that of NORM. The protein expression of SOD1 was lower in ANG than in NORM, whereas SOD2 and SOD3 expression was not different between the groups. Reduced SOD activity and SOD1 expression in ANG was not restored in ANG + TTx. Both SOD activity and SOD isoform expression in the small renal vessels of ANG were not different from those of NORM. Interestingly, SOD activity in the small renal vessels was reduced by TTx. Between groups, there was no difference in catalase activity or expression in both the small mesenteric arteries and small renal vessels. In conclusion, SOD activity in the small mesenteric arteries decreased by angiotensin II administration, but not by hypertension, which is caused by decreased SOD1 expression.

The Effect of Sohaphyangwon water extract on Endothelial cells by Free choleserol in blood (소합향원(蘇合香元)이 혈액내 Free Cholesterol에 의한 혈관내피세포 손상에 미치는 영향)

  • FC(free choresterol) plays an important role in normal and pathophysiological cells including that of messenger molecule or dilator of blood vessels in such illnesses as artheriosclerosis, hypertension and myocardial infarction. Smooth muscle and endothelial cell functions in the arteria wall are unified by complex intercellular signalling processes. In arteria comprised of one layer of smooth muscle cells surrounding the endothelium, the close apposition of the two cell types enables a signal derived from one cell to rapidly diffuse to neighboring cells. Experimentation was conducted to investigate the potential contribution of Sohaphyangwon(SHHW) on levels of FC generated by goaded microphages, and mechanisms of protection against ACAT inhibitor. It was found that J774 macrophages, which normally do not express FC were expressed by oxLDL and ACAT inhibitor. SHHW protected cells were found to be resistant to oxLDL and delayed death following the FC. Inhibition of FC formation abolished the protective effect against ACAT inhibitor exposure. Cadiovascular diseases include abnormalities of blood vessels dysfunction of the renin-angiotension system. What relation herbal medicine may have with vessel endothelium necrosis was here studied. In Oriental Medicine, SHHW water extract used for diseases in relation to cardiovascular systems. The resistence to cardiovascular disease of ACAT inhibitor induced J774 macrophage cells were studied through analysis of cell morphological patterns and immunochemistry of SHHW. The results of this study suggest that SHHW has protective effects on the cardiovascular system, and that it is effective in both prevention and treatment of diseases of the cardiovascular system, particularity against necrosis of blood.

  • The distributions of blood pressure, blood flow, and blow volume in the left common artery (LCCA) were determined using the lumping parameter method. In order to develop a mathematical model for microcirculation in LCCA, the present study adopted preexisted set of measured morphological data on anatomy, mechanical properties of the coronary vessels, viscosity of blood, the basic laws of physics, and the appropriate boundary condition. Pressures and volumes of blood and flow resistance were expressed in terms of electrical voltages, current, and resistances, respectively, in the electrical analog model. The results of two mathematical models, symmetrical and asymmetrical models, were compared with other investigator's data. The present results were in good agreement with previous studies. It was found that the mean pressure profiles were similar in both models.
