Infected Left Atrial Myxoma Presenting Without Bacterial Growth on Blood Cultures: A Case Report

  • Although cardiac myxoma is one of the most common types of benign cardiac tumors, infected cardiac myxoma is very infrequent. The diagnosis of infected cardiac myxoma may be challenging because the presenting symptoms are non-specific and established management guidelines are lacking. This report describes a 39-year-old woman with a 5-month history of uncontrolled fever, chills, and myalgia who was diagnosed with myxoma and underwent mass excision. Although blood and urine cultures were negative for growing bacteria, a pathologic examination showed that the excised mass was a left atrial myxoma, with pan-bacterial polymerase chain reaction (PCR) of the surgical specimen revealing Haemophilus parainfluenzae at 99.87%, resulting in a diagnosis of infected cardiac myxoma. Laboratory tests, such as PCR, may supplement culture results in the diagnosis of infected cardiac myxoma.

Nationwide Cross-sectional Analysis of Determinant Agents for Sarcopenia in Men in Their Forties: Based on the E WGSOP Criteria

  • PURPOSE: This study examined sarcopenia in middle-aged men aged 40 to 49 years. METHODS: The study design was a nationwide cross-sectional study involving 1,564 participants. The participants were divided into two groups based on their skeletal muscle mass index scores, with 36 and 1,528 individuals classified into the sarcopenia and normal groups, respectively. The variables examined included age, height, weight, body mass index (BMI), waist circumference, skeletal muscle mass index, and systolic and diastolic blood pressure. The results from blood tests included fasting glucose, triglycerides, and total cholesterol levels. The smoking and drinking status were also recorded. RESULTS: The anthropometric variables, such as weight, BMI, and waist circumference, revealed significant differences between the two groups (p < .05), except for height (p > .05). In addition, total cholesterol in the laboratory test and drink status were significant between the two groups (p < .05). CONCLUSION: This study identified significant determinants for sarcopenia among community-dwelling middle-aged men.

The Second Animal Tests of Artificial Heart Valves (인공심장판막의 개발과 동물실험 -인공심장판막의 2차 동물실험-)

  • A heart supplies blood of about 15, 000 liters to each human organ in a day. A normal function of heart valves is necessary to accomplish these enormous work of heart. The disease of heart valve develops to a narrowness of a closure, resulting in an abnormal circulation of blood. In an attempt to eliminate the affliction of heart valves, the operative method to replace with artificial heart valves has developed and saved numerous patients over past 30 years. This replacement operation has been performed since early 1960`s in Korea, but all the artificial heart valves used are imported from abroad with very high costs until recent years. New artificial heart valves have been developed in Korea Advanced Institute of Science and Technology since early 1980`s. The first developed valve was designed with a free-floating pyrolytic carbon disk that is suspended in a titanium cage. The design of the valve was tested in vitro, and in animals in 1987. The results from this study was that the eccentrically placed struts creates a major and minor orifice when the disc opens and stagnation of flow in the area of the minor orifice has led to valve thrombosis. In this work, the design of the valve was changed from a single - leaflet valve to double - leaflet one in order to resolve the problems observed in the first - year tests. Morphological and hemodynamic studies were made for the newly designed valves through the in vitro and in vivo tests. The design and partial materials of the artificial heart valve was improved comparing with first - year`s model. The disc in the valve was modified from single - leaflet to bi - leaflet, and the material of the cage was changed from titanium metal to silicon - alloyed pyrolytic carbon. A test was made for the valve in order to examine its mechanical performance and stability. Morphological and hemodynamic studies were made for the valve that had been implanted in tricuspid position of mongrel dogs. All the test animals were observed just before the deaths. A new artificial heart valve was designed and fabricated in order to resolve the problems observed in the old model. The new valve was verified to have good stability and high resistance to wear through the performance tests. The hemodynamic properties of the valve after implantation were also estimated to be good in animal tests. Therefore, the results suggest that the newly designed valve in this work has a good quality in view of the biocompatibility. However, valve thrombosis on valve leaflets and annulus were found. This morphological findings were in accordance with results of surface polishing status studies, indicating that a technique of fine polishing of the surface is necessary to develop a valve with higher quality and performance.

Investigation of Domestic and Foreign Unexpected Antibodies for Emergency Blood Transfusion (응급수혈을 위한 비예기 항체의 국내·외 실태조사)

  • Certain pre-transfusion tests are not commonly performed during emergency blood transfusion. In this study, we reviewed and analyzed the data of post-blood transfusion antibody screening tests to establish the effects of unexpected antibodies causing hemolytic transfusion reactions. We reviewed information published domestically and internationally, and selected the data of 68,602 antibody screening tests and 528 antibody identification tests conducted at P hospital. We found that unexpected antibody positive (1198,1.74%), Rh type (161, 30.49%), Lewis type (67, 12.69%), others (Di (a), 28, 5.30%). The anti-E type positive was 93 (17.61%), and that of the cases with anti-C (13, 2.46%). Only data of domestic cases were included for analysis that were published before 2007, which established the presence of antibodies of the following types and numbers of cases: anti-E (196, 22.45%), anti-Le a (82, 9.39%), and anti-E+C (60, 6.87%). In 2018, anti-E (107, 17.12%), anti-E+Canti-E+C (56, 8.96%), and anti-Di a (28, 4.48%) were detected. In other domestic cases, S hospital was detect to anti-E, anti-Le a, anti-E+C. The Anti-E, anti-D, anti-E+C, and anti-C+E were detected in D hospital. In Saudi Arabia, Anti-D, anti-E, and anti-Jka was detected. The Anti-M, Anti-N, Anti-Le (a), and Anti-D were detected in India. Requests for emergency blood transfusion increased 1.8 times after the opening of the trauma center. This study has the disadvantage of being a cross-sectional study. additional studies are needed to provide basic information on alternative treatments that can increase the safety and reduce the side effects of hemolytic transfusion in emergency transfusion situations.

Effects of Mobile based-Healthcare Service for Hypertension Patients (고혈압 환자 대상의 모바일 헬스케어서비스 운영 효과)

  • This study aims to evaluate the effect of mobile healthcare service which was provided to employees diagnosed with hypertension in employee's health checkup. The study subjects were 146 employees and nurse measured their blood pressure, body mass index, and blood tests at the clinic in a workplace. The general characteristics of the participants were analyzed using descriptive statistics. After providing health care service using mobile text message for 8 weeks, blood test and blood pressure was measured. As a result, it was confirmed that blood pressure(SBP: 139.32±10.38 → 133.96±11.31, DBP: 98.13±6.21 → 94.28±8.56) and blood test(HDL: 47.90±9.79 → 51.40±9.79, HbA1C: 5.96±.66 → 5.65±.71) were more significantly reduced in 56 employees with a systolic blood pressure of 140 mmHg or diastolic blood pressure of 90 mmHg or more. It was confirmed that the program for self-management of blood pressure by receiving text messages on a mobile basis was effective. It is expected that the results of this study will be used as basic data for healthcare services that provide text information using mobile to improve drug usage, physical exercise, and eating habits.

An Exploratory Study on the Effects of Barley Sprout Powder Intake on Blood Lipids in Women in Their 50s: One Case Report (보리 새싹 분말 섭취가 50대 여성의 혈중지질에 미치는 효과 탐색 연구: one case 보고)

  • Purpose: This study aimed to study barley sprout powder's effect on blood lipids in women in their 50s. Methods: For 18 months of ingesting 3 grams of barley sprout powder daily, and the changes were investigated before and after the blood tests every six months. Results: The systolic blood pressure decreased from 107 mmHg for the first test to 91 for the fourth test. Diastolic blood pressure dropped from 69 mmHg to 49 mmHg at the first test. Abdominal circumference decreased from 84 cm at the first measurement to 78 cm at the fourth round. Fasting glucose and HDL were not affected. Total cholesterol was significantly refused from 264 mg/dl in the first measurement to 172 mg/dl in the fourth measurement. The first measurement of triglycerides in 2018 was 179 mg/dl, and it was highly reduced at the fourth measurements to 80 mg/dl. The first measurement of LDL was 182 mg/dl, and the fourth was a high decrease to 97 mg/ dl. Conclusions: After the women in the 50s ingesting sprout barley for 18 months, the overall improvements were shown by reducing significantly with fasting blood sugar, blood pressure, blood triglyceride, and LDL concentrations.

The Effect of the Aerobic Exercise on Blood Lipids in the Obesity High School Student (유산소성 운동이 비만 고등학생의 혈중지질 농도에 미치는 영향)

  • The purpose of this paper was to evaluated a effect of aerobic exercise for obesity high school student. The subjects were 20 obesity boys and 20 obesity girls as above 25% body fat. The exercise program of this study composed to intensity of $60{\pm}10%$ HRmax, 4 day/weeks and $40{\sim}50$ min/day during twelve weeks. The results were summerized as follows : 1. There were significantly decreased on T-cholesterol, LDL, Triglyceride between pre and post tests of exercise group and HDL-cholesterol was significantly increased but no significant difference of the non exercise group in men. 2. There were significantly decreased on T-cholesterol, LDL, Triglyceride between pre and post tests but no significant difference in HDL of the exercise group in women. 3. There were no significant difference between men and women.

General Pharmacology of Recombinant Human Granulocyte-Macrophage Colony Stimulating Factor Expressed in Saccharomyces cerevisiae (효모에서 발현된 유전자 재조합 인간 GM-CSF의 일반 약리작용)

  • The general pharmacological tests with rhGM-CSF indicated that it had no influences on rotarod and locomotor activity tests, but shortened hexobarbital-sleeping time at the large dose of 3 mg/kg s.c. in mice. It elicited no hypothermic, analgesic and antiepileptic action. No influences on blood pressure and respiration in rabbits were observed at the dose of 1 mg/kg, i.v. and it did neither affect the receptors of adrenaline, acetylcholine, serotonin, histamine, kinin and oxytocin, nor antagonize the actions of histamine, serotonin and oxytocin at its concentrations of 1$\times$$10^{-6}$g/ml. However, this substance was demonstrated to stimulate the formation of leucocytes in rats.

  • All newborn infants with clinically significant bleeding should be evaluated for a hemostatic deficit. Medical history should include the following data: familial bleeding disorders, maternal illness and medication, age of bleeding onset, and prophylactic administration of vitamin K. The first essential step for evaluating bleeding neonates is determining whether the baby is sick or well. The physician should also evaluate the extent of the bleeding, features of bleeding lesions, and other abnormal findings from the physical examination. Skeletal anomalies may provide diagnostic clues. Depending on the clinical features and results of screening tests, other tests including coagulation factors may be useful for determining the diagnosis. All laboratory results must be considered in the context of age-related reference values. The platelet function analyzer provides a promising alternative to bleeding time. Fibrin degradation products and D-dimers are used for screening and specially testing fibrinolytic activity, respectively. The Apt test may help to rule out factors derived from maternal blood. Radiologic imaging studies are important because asymptomatic intracranial hemorrhages are common in neonates.

A Case Report on Papillary Thyroid Cancer Patients after Thyroidectomy Treated with Korean Medicine and Immunotherapy (갑상선 유두암의 외과적 절제술 이후 발생한 후유증에 대한 면역요법을 병행한 한방 치험 3례)

  • Objectives: This study assessed the effect of a combination of Korean medicine and immunotherapy on three papillary thyroid cancer patients following thyroidectomy. Methods: The three patients who underwent thyroidectomy received Korean medical treatments, including acupuncture, moxibustion, pharmacopuncture, and immunotherapy. To evaluate the patients, symptoms were measured using the Numerical Rating Scale (NRS) and Karnofsky Performance Status Scale (KPS). Blood tests, including thyroid function tests, were conducted during treatment. Results: After treatment, postoperative pain and general weakness were gradually alleviated. Conclusions: These cases provide evidence that treatment with a combination of Korean medicine and immunotherapy can have substantial benefits for postoperative complications following thyroidectomy.
