상완혈압과 두부혈압의 상관성 분석에 의한 진단요소 추출과 다기능 전자혈압계의 개발 (Diagnosis parameters extraction by correlativity analysis of blood pressure(BP) and head blood pressure(HBP) and Development of multi-function automatic blood pressure monitor)

  • 각종 성인병과 난치성질환인 뇌졸중과 치매, 중풍의 원인으로는 혈액순환장해에 의한 고혈압과 두부혈압의 상승이 주된 요인으로 작용하고 있다. MRI , X-ray 같은 의료장비는 뇌졸중이나 중풍 등의 발병전 예방보다는 발병후의 치료를 위한 목적으로 이용되고 있다. 최근에는 이러한 난치성 질환이 정상인에서도 발생하고 있어서, 상완혈압에 의한 진단보다는 뇌혈류 상태를 반영하는 두부혈압을 진단하는 것이 더 정확한 임상진단을 할 수 있다 그러나 기존 혈압계나 전자혈압계로는 두부혈압을 측정하거나 개선할 수 있는 기능을 수행할 수 없다. 따라서, 본 논문에서는 혈압과 두부혈압을 쉽고 간편하게 측정하고, 상완혈압에 대한 두부 혈압의 상관성을 추출하여 뇌혈류 상태 및 건강상태를 조기 진단/예방할 수 있는 시스템 및 알고리즘을 개발하였다. 임상실험 결과, 기존의 혈압계보다 측정오차가 작고 재현성이 우수하여 신뢰성을 확인하였고 ,두부혈압의 최고압은 상완 최고압의 62%, 최저혈압은 상완 최저혈압의 46%에 해당하는 상관성을 확인하였다. 정상인과 환자간의 상관성에 큰 차이가 있고, 이는 상완혈압 측정에 의존한 진단보다 두부혈압 측정에 의해 더 중요한 진단요소론 추출할 수 있어서 더 정차한 진단을 할 수 있음을 의미한다. 따라서, 뇌혈류에 이상이 있을 때 달라지는 두부혈압을 측정하여 상완혈압과의 상관성을 추출하고, 환자를 대상으로 분석하여 뇌혈류 상태를 진단/개선함으로서 새로운 진단체계를 구축할 수 있는 시스템을 개발하였다.

중풍환자의 혈압 상승에 대한 대추혈 사혈의 혈압 강하 효과 (Decreasing Effect of Wet-Cupping at Daechu (대추, Dazhui:GVl4)-point on the Increase of Blood Pressure in Patients with Stroke)

  • Objectives : Daechu (Dazhui:GV14 (Governor Vessel 14))-point is located between the spinous process of the 7th cervical vertebra and that of the 1st thoracic vertebra. GV14 has been used to treat hypertension, high fever, neck pain, common cold, headache and so on. Hypertension often negatively affects the improvement of stroke patients. We investigated whether wet-cupping at GV14 had any effect of decreasing blood pressure (BP). Methods : In this study, 81 stroke patients were studied, from Oct. 2000 to Sept. 2001. They were composed of a Sample group (n=36) and Control group (n=45). The Sample group (n=36) was divided into Sample-Normal (n=9), Sample-Mild (n=7), Sample-Moderate (n=l1), and Sample-Severe group (n=9). The Control group (n=45) was divided into Control-Moderate (n=34) and Control-Severe group (n=ll). We checked blood pressure 5 times (just before treatment, then after 30,60, 90, and 120min. (2hrs.)) in each sample group and 3 times (baseline, after 30, and after 120min. (2hrs)) in the control group. Results : In a comparison of before-treatment BP and after-treatment BP in the sample group, SBP significantly decreased as time passed, while DBP slightly decreased but not significantly. In comparison of BP differences in each sample subgroup, BP slightly increased in the Sample-Normal group, but significantly decreased in the Sample-Mild, Sample-Moderate, and Sample-Severe groups. In comparison of BP differences between the sample and control groups, SBP and DBP of the sample group definitely decreased more than those of the control group, but not significantly. Conclusions : This study suggests that wet-cupping at GV14 has significant decreasing effectson blood pressure in stroke patients. Accordingly, we hope that this measure will be used more widely as an emergent treatment for increased blood pressure.

A Vascular Characteristic Index of Blood Pressure Variation using the Pulse Wave Signal

  • Pulse waves continuously change with respect to the characteristics and status of the cardiovascular system and in relation to the blood pressure (BP) and the pulse wave velocity (PWV). Monitoring the vascular condition by analyzing the variations in pulse waveforms has been used to diagnose vascular disorders and in drug treatment of arteriosclerosis and peripheral circulatory obstruction. In this paper, we investigated the vascular characteristic index with regard to the BP and classified by pulse wave signals. The pressure pulse wave and photoplethysmography (PPG) were measured simultaneously while subjects exercised, producing changes in the BP, to analyze the variation in the vascular characteristic index. We investigated the correlation between the BP and vascular characteristic index with regard to the classification methods of the pulse wave. The reflection index (RI) and vascular stiffness index were correlated with the diastolic BP, but no correlation was found between these parameters and the systolic BP. These results suggest the possibility of estimating BP through simple measurements of pulse waves.

On Employing Nonparametric Bootstrap Technique in Oscillometric Blood Pressure Measurement for Confidence Interval Estimation

  • Blood pressure (BP) is an important vital signal for determining the health of an individual subject. Although estimation of mean arterial blood pressure is possible using oscillometric blood pressure techniques, there are no established techniques in the literature for obtaining confidence interval (CI) for systolic blood pressure (SBP) and diastolic blood pressure (DBP) estimates obtained from such BP measurements. This paper proposes a nonparametric bootstrap technique to obtain CI with a small number of the BP measurements. The proposed algorithm uses pseudo measurements employing nonparametric bootstrap technique to derive the pseudo maximum amplitudes (PMA) and the pseudo envelopes (PE). The SBP and DBP are then derived using the new relationships between PMA and PE and the CIs for such estimates. Application of the proposed method on an experimental dataset of 85 patients with five sets of measurements for each patient has yielded a smaller Cl than the conventional student t-method.

Disagreement between direct and indirect blood pressure measurements obtained from minipigs

  • Blood pressure (BP) measurement plays a pivotal role in veterinary medicine for diagnosing cardiovascular disorders and monitoring anesthesia of animals. Although indirect BP measurement has been widely applied to monitor BP because of its convenience and non-invasiveness, it is still unclear whether indirect BP measurement is compatible with direct BP measurement in minipigs. In addition, the effect of animal posture during BP measurement is not well understood in minipigs despite its importance to cardiovascular performance. Therefore, both systolic and diastolic arterial BPs in minipigs were measured via femoral artery catheterization for direct BP measurement and using a compressive cuff as an indirect BP measurement under the dorsal or right lateral recumbent postures. Numerical values were processed by the Bland-Altman method to calculate the bias ${\pm}$ SD and the limits of agreement (LOA). In accordance with the American College of Veterinary Internal Medicine guidelines, the results between direct and indirect BP measurements were determined as apparent disagreements in both systolic and diastolic arterial BPs under all postures because of large bias ${\pm}$ SD and wide LOA. The results of the present will help prevent misinterpretation of the anesthetized patient's condition during monitoring of BP by indirect measurement.

Consumption of Ultra-Processed Food and Blood Pressure in Korean Adults

  • Background and Objectives: There is growing evidence supporting the association between ultra-processed food (UPF) consumption and metabolic disease risk. However, little is known about the association between UPF consumption and blood pressure (BP). Thus, this study examined the association between UPF consumption and elevated BP in Korean adults. Methods: This study used data from the Korea National Health and Nutrition Examination Survey (2016-2018) and included 9,188 participants aged 30-79 years without a history of hypertension diagnosis. Food items reported in a one-day 24-hour recall were categorized on the basis of the NOVA (not an acronym) food classification criteria. UPF consumption was estimated as the contribution to total energy intake. Elevated BP was defined as systolic BP ≥120 mmHg or diastolic BP ≥80 mmHg. The independent association between UPF consumption and elevated BP was assessed by multivariable logistic regression analysis. Results: The upper tertile of UPF consumption was significantly associated with elevated BP compared with the lower tertile, after adjusting for potential confounders. A linear trend was observed for elevated BP across the tertiles of the dietary energy contribution of UPF. Similar results were found in stratified analyses by age group, smoking, obesity, and overall dietary quality. However, a marginal level of association was found in some subgroups, current smokers, and non-obese adults. Conclusions: The dietary energy contribution of UPF consumption was positively associated with increased prevalence of elevated BP, and these findings suggest that lowering UPF consumption might help prevent BP elevation.

청훈화담탕가형개로 인해 변동된 국소뇌혈류량 및 평균혈압의 작용기전 (Mechanism on the Changed-Regional Cerebral Blood Flow and Mean Arterial Blood Pressure by CheonghunHwadam-tang Adding Schiznepetae Herba in Rats)

  • CheonghunHwadam-tang(CHT) have been used in oriental medicine for many centuries as a therapeutic agent of vertigo by wind, fire and phlegm. CHTS was CHT adding Schizonepetae Herba. The effects of CHTS on the cerebral blood flow and blood pressure is not known. The purpose of this Study was to investigate effects of CHTS on the regional cerebral blood flow(rCBF) and mean arterial blood pressure(BP), action-mechanism of CHTS-induced changed-rCBF and BP. The changes of rCBF and BP was determinated by Laser-Doppler Flowmetry(LDF). The results were as follows ; CHTS extract was increased significantly rCBF in a dose-dependent, but was not changed BP compared with CHTS non-treated group. Pretreatment with propranolol, indomethacin and methylene blue were inhibited CHTS induced increase of rCBF, propranolol(all CHTS-treated group) and indomethacin(CHTS 0.01 mg/kg) of them were significantly decreased. Pretreatment with propranonol and indomethacin were inhibited CHTS induced increase of BP, but pretreated with methylene blue was significantly accelerated BP in high dosage. This results suggest that CHTS increased rCBF by dilating pial arterial diameter and the action of CHTS is also mediated by adrenergic β -receptor and cyclooxygenase.

Self-Monitoring of Blood Pressure and Feed-back Using APP in TReatment of UnconTrolled Hypertension (SMART-BP): A Randomized Clinical Trial

  • Background and Objectives: Self-monitoring of blood pressure (SMBP) is a reliable method used to assess BP accurately. However, patients do not often know how to respond to the measured BP value. We developed a mobile application-based feed-back algorithm (SMBP-App) for tailored recommendations. In this study, we aim to evaluate whether SMBP-App is superior to SMBP alone in terms of BP reduction and drug adherence improvement in patients with hypertension. Methods: Self-Monitoring of blood pressure and Feed-back using APP in Treatment of UnconTrolled Hypertension (SMART-BP) is a prospective, randomized, open-label, multicenter trial to evaluate the efficacy of SMBP-App compared with SMBP alone. Patients with uncomplicated essential hypertension will be randomly assigned to the SMBP-App (90 patients) and SMBP alone (90 patients) groups. In the SMBP group, the patients will perform home BP measurement and receive the standard care, whereas in the SMBP-App group, the patients will receive additional recommendations from the application in response to the obtained BP value. Follow-up visits will be scheduled at 12 and 24 weeks after randomization. The primary endpoint of the study is the mean home systolic BP. The secondary endpoints include the drug adherence, the home diastolic BP, home and office BP. Conclusions: SMART-BP is a prospective, randomized, open-label, multicenter trial to evaluate the efficacy of SMBP-App. If we can confirm its efficacy, SMBP-App may be scaled-up to improve the treatment of hypertension.

PTT를 이용한 자전거 운동 중 지속적인 혈압의 예측 (Continuous Blood Pressure Prediction Using PTT During Exercise)

  • The purpose of this work is to predict the systolic blood pressure (BP) during exercise from pulse transit time (PTT) for warning of possible danger. PTT was calculated as the time between R-peak of ECG and the peak of differential photoplethysmograph (PPG). For the PTT-BP model, we used regress equations from previous studies and 3 kinds of new models combining linear and nonlinear regress equation. The model parameters were estimated with the data measured under low to middle intensity exercise, and then was tested with the data measured under high intensity exercise. Predicted BP values after high intensity exercise were compared with those measured by cuff-type sphygmomanometer. The results showed that the error between measured and predicted values were acceptable for the monitoring BP. We tested PTT-BP models 1 month after the identification without further calibration. Models could predict the BP and the errors between measured and predicted BP were about 5mmHg. The suggested system is expected to be helpful in recognizing any danger during exercise.

The correlation of blood pressure with height and weight in Korean adolescents aged 10-19 years; The Korean National Health and Nutrition Examination Surveys (2009-2011)

  • Purpose: Height-specific blood pressure (BP) is the standard parameter used to diagnose childhood hypertension. However, there has been some argument that weight may be a better variable than height in the reference BP standards. Therefore, before assessing the BP status using the reference BP standards, a basic understanding of the fundamental association of weight and height with BP is required. Methods: In the present study, we analyzed the correlation of BP with height and weight in Korean adolescents (age, 10-19 years), using data from the Korean National Health and Nutrition Examination Surveys (2009-2011). Results: Systolic BP (SBP) was more closely correlated with weight than with height in the normal weight (body mass index [BMI], ${\leq}85th$ percentile) and overweight (BMI, >85th percentile) groups and in the normal waist circumference (WC, ${\leq}90th$ percentile) and high WC (>90th percentile) groups in both sexes. Diastolic BP (DBP) had a higher correlation with height than with weight in the normal weight and normal WC groups, whereas weight was more closely associated with DBP than height in the overweight and high WC groups in both boys and girls. Conclusion: In Korean adolescents, weight had a greater effect on SBP than height in both the normal weight and overweight groups. DBP was mainly affected by height in the normal weight group, whereas weight was the major determinant of DBP in the overweight group. Therefore, it may be necessary to consider weight in the establishment of reference BP standards.
