Chronic Lymphocytic Leukemia and Prognostic Factors

  • Background: The clinical course of individual chronic lymphocytic leukemia (CLL) is highly variable and clinical staging systems do not help us to predict if and at what rate there will be disease progression in an individual patient diagnosed with early stage disease. Recently, several important observations related to other prognostic factors including lymphocyte doubling time (LDT), ${\beta}_2$-microglobulin (${\beta}_2$-MG), and percent of smudge cell in peripheral blood smears, cytogenetic and molecular analysis have been made. The aim of this study was to evaluate a range of prognostic factors in our CLL patients. Design and methods: Seventy patients with CLL were enrolled. Prognostic factors of disease including Binet staging, LDT, ${\beta}_2$-MG, ESR, LDH, percent of smudge cell in peripheral blood smear, absolute lymphocyte count, and conventional cytogenetic (CC) analysis were evaluated at diagnosis, and the patients were followed up to determine their outcome. We compared factors with each other and with Binet staging and prognosis. Results: Enrolled patients aged 37-85 years at diagnosis or during follow up. There was no relationship between serum LDH level (P=0.3), ESR (P=0.11), percent of smudge cells in peripheral blood smear (P=0.94), and absolute lymphocyte count (P=0.18) with the stage of disease and prognosis, but the ${\beta}_2$ macroglobulin level (p<0.0001), LDT (p<0.001) had direct and significant relation with staging and outcome. In 19% of patients cytogenetic alteration were seen. Conclusion: The detection of cytogenetic alteration only using the CC method is not sufficient and we need to use FISH, but because FISH study is an expensive method not available in all areas, instead we believe that ${\beta}_2$ MG can be applied in its place as a good prognostic factor for CLL at diagnosis and during follow up. We suggest to add it to Binet staging for prognostic subgrouping of CLL.

Effect of Peripheral Blood CD4 + CD25 + Regulatory T Cell on Postoperative Immunotherapy for Patients with Renal Carcinoma

  • Objective: To investigate the effect of peripheral blood CD4 + CD25 + regulatory T cell on postoperative immunotherapy in patients with renal carcinoma. Methods: 38 patients with renal cell carcinoma were recruited, and 20 patients from the operation group purely underwent the radical nephrectomy therapy, 18 patients from the combined group successively underwent the radical nephrectomy therapy and IFN-${\alpha}$ adjuvant immunotherapy. Additionally, 12 healthy subjects were recruited in the same period of time and regarded as the control group. Flow cytometry was used to detect CD4 +, CD8 +, CD4 + CD25+ T lymphocyte subset content and the ratio of all parts in the pre-operative period, in the first post-operative week and in the third post-operative month, compare and analyze its variation trend. Results: The CD4+CD25+ T lymphocyte subset content of individual renal carcinoma patients was significantly higher than that of the control group, also increases with the progression in the tumor stage (P<0.05). The post-operative CD4 + CD25+T lymphocytes of individual operation group and combined group patients showed different degrees of increment, but the increment of the combined group was significantly lower than that of the operation group (P<0.05). For the combined group patients with less pre-operative CD4 + CD25+T lymphocytes, their levels would increase after the immunotherapy, while the pre-operative patients with more CD4 + CD25+ T lymphocytes were the opposite situation. Conclusion: The detection of peripheral blood CD4+CD25+ regulatory T lymphocyte subset can reflect the anti-tumor immune status of renal cell carcinoma patient body. It can contribute to predict the prognosis of immunotherapy and provide reference for the choice of renal carcinoma post-operative adjuvant immunotherapy.

Association of the neutrophil-to-lymphocyte ratio and CA 125 with the endometriosis score

  • Objective: To evaluate the association between the severity of endometriosis and the preoperative neutrophil-to-lymphocyte ratio (NLR) and serum level of cancer antigen 125 (CA 125). Methods: Data were obtained from the medical records of 419 patients who underwent laparoscopic conservative surgery for ovarian endometrioma between April 2005 and March 2013. Each patient's preoperative complete blood count was recorded and the endometriosis score was assessed. Results: The endometriosis score was not associated with either the NLR or the serum level of CA 125. The endometriosis score was negatively related to preoperative hemoglobin, hematocrit, mean corpuscular volume, mean corpuscular hemoglobin, and mean corpuscular hemoglobin concentration. The only positive association was between NLR and the patients' age. NLR and preoperative serum anti-M$\ddot{u}$llerian hormone level were found to be negatively related. Conclusion: The severity of endometriosis was not associated with the serum level of CA 125 or the NLR. The presence of a negative correlation between the severity of endometriosis and red blood cell dynamics needs further investigation.

Clinico-Hematological Findings for Classical Hodgkin's Lymphoma: an Institutional Experience

  • Background: Classical Hodgkin's lymphoma (cHL) is a B-cell lymphoid neoplasm characterized by a distinctive biological behavior with potentially curable disease characteristics. It is an uncommon hematological malignancy which primarily affects younger individuals. The rationale of this study was to determine its clinico-hematological profile along with stage stratification in Pakistani patients. Materials and Methods: In this descriptive study, adult patients with Hodgkin's lymphoma were enrolled from January 2010 to December 2014. Results: Sixty two histopathologically confirmed cases of cHL were identified. There were 42 males and 20 females, with a male to female ratio of 2: 1. The mean age was $29.7{\pm}13.8$ years with the median age of 30 years. B symptoms were present in 72.5% of patients. Histopathologically, the mixed cellularity type constituted 62.9% of cases, followed by nodular sclerosis in 25.8%, lymphocyte predominant in 9.6% and lymphocyte depleted in 1.6%. Stages I and II were present in 43.5% of patients at disease presentation, with 56.4% in stages III and IV. Conclusions: Our analysis shows that clinico-pathological features of Hodgkin's lymphoma in Pakistan are comparable to published data. Mixed cellularity is the commonest histological variant and advanced stage at presentation are common findings in our patients.

The Effect of Red Ginseng for Postoperative Immune Response in Gastrointestinal Carcinoma (소화기계암의 수술후 면역기능에 대한 고려홍삼의 효과)

  • (Backgrounds) This study was performed to evaluate the usefulness of red ginseng ex rant as adjuvant therapeutic agent improving immune function in immune compromizing gas-trointestinal carcinoma patient. (Material and Methods) We were treated 72 patients with two groups after we were undertaken the curative resection for gastrointestinal carcinoma; 1) only chemotherapy and immunotherapy (control group) 2) chemotherapy and immunotherapy with 4500 mg (15 tablets) red ginseng for 6 months (study group). For investigating the immunologic alternations alongside the numerical changes in peripheral blood Iymphocyte and their subsets in the gastrolntestinal carcinoma patients, Iymphocyte surface markers were determined by monoclonal antibodies on the preoperative day, postoperative 1 months, 3 months, 6 months, 12 months and 18 months in 40 controls and 32 red ginseng groups In gastrointestinal carcinoma patients which was recruited at Korea diversity Hospital from March, 1995 to January, 1997. The patient was measured and compared in both groups with the body weight, total protein and albumin, blood hematocrit and hemoglobin, total leukocyte, lymphocyte and lymphocyte subsets count in peripheral blood through planed schedules. (Couclusion) This data suggests that red ginseng may be useful as a adjuvant therapeutic agent for improving the immune function after curative operation for immune compromizing gastrointestinal carcinoma patients. Side effects of treatment with busulfan at high doses in dogs

  • Busulfan is the most commonly used drug for preconditioning during the transplantation of hematopoietic stem cells and male germ cells. Here, we describe side effects of high doses of busulfan in male mongrel dogs. Busulfan was intravenously administered to three groups of dogs at doses of 10, 15, and 17.5 mg/kg body weight. The total white blood cell, neutrophil, eosinophil, lymphocyte, monocyte, and platelet counts steadily reduced in a dose-dependent manner following busulfan treatment. The white blood cell, neutrophil, and monocyte counts recovered after 6 weeks of busulfan treatment, however, the eosinophil, lymphocyte, and platelet counts remained unaltered. Additionally, there was one fatality in the each of the groups that were administered 15 and 17.5 mg/kg busulfan. The gross lesions included severe hemorrhage in the stomach, intestinal tracts, mesentery and urinary bladder. Microscopic investigation revealed severe pulmonary edema and hemorrhage in the lungs, and severe multifocal to coalescing transmural hemorrhage in the intestines and urinary bladder. These results indicated that treatment with busulfan at doses higher than 15 mg/kg initiates severe bleeding in the internal organs and can have fatal results.

Effect of levamisole, selenium and tocopherol on the lymphocyte blastogenesis and production of antibody in korean native goat (Levamisole, selenium 및 tocopherol이 한국재래산양의 혈중 림프구기능 및 항체생성에 미치는 영향)

  • In this study, effect of levamisole, selenium and tocopherol on the lymphocyte blastogenesis and antibody production in Korean native goat were evaluated in vitro and in vivo. Lymphocyte blastogenesis of goat blood increased significantly (p<0.01 and p<0.05) when the cells were treated in vitro with levamisole at the concentration of $50{\sim}500{\mu}g/ml$, with selenium at the concentration of $0.062{\sim}1.0{\mu}g$ and with tocopherol nt the concentration on $12.5{\mu}g$. Increased lymphocyte blastogenesis was detected from 2 to 24 hours after oral administration of levamisole (2.5mg/kg of body weight). After 7 days, increased mitogenic response of lymphocytes was not detected. Meanwhile increased blastogenesis of lymphocyte from goats given the selenium-tocopherol mixture (selenium $100{\mu}g$-tocopherol 200IU/head/day) was detected from 10 days after feeding, and the tendency continued throughout the entire experimental period. When immune responses of goats against PPD were subjected to test by ELISA, the mean IgG titers of levamisolc group (1 : 1,800) and selenium-tocopherol group (1 : 960) were higher than that of control group (1 : 600) at 2 weeks after lst inoculation. At 3 weeks after lst inoculation and 1 week after 2nd inoculation, the significant (p<0.05) differences in IgG titers were detected among the three groups. The mean IgG titers of levamisole group, selenium-tocopherol group and control at that time were 1 : 20,480, 1 : 5,120 and 1 : 2,640, respectively. The IgG production of levamisole group was significantly (p<0.01) higher than that of control group.

Impact Factors and Validity of Blood Variables on Death in COVID-19 patient: Using Data of Korea Disease Control and Prevention Agency

  • In this paper, we propose impact factors and validity of blood variables on death of COVID-19 patients. The clinical-epidemiological data of 5628 COVID-19 patients, provided from Korea Disease Control and Prevention Agency as day of 30th April 2020, were used. As results, impact factors of death were dementia, older age, high lymphocyte, cancer, dyspnea, COPD, change of consciousness, heart disease, high platelets, abnormal diastolic pressure and fever. The validities of blood variables for death were high in the order of lymphocyte, hemoglobin, hematocrit, platelet and WBC. Therefore, risk factors such as initial clinical characteristics, underlying disease and blood test results, could be regarded for efficient management of COVID-19 patients.

  • To evaluate radiation effect on the hematopoietic system, we analyzed 44 patients who were treated with conventionally fractionated radiation therapy (RT) at Chonbuk National University Hospital. According to the treatment sites, we classified them into three groups: group I as head and neck, group II as thorax, and group III as pelvis. White blood cell, lymphocyte, platelet and hemoglobin were checked before and during RT The results were as follows; 1. White blood cell (WBC) and lymphocyte count were declined from the first week of RT to the third week, and then slightly recovered after the third or fourth week. There was prominent decrease in lymphocyte counts than WBC. 2. Platelet counts were declined until the second week of the RT, showed slight recovery at fourth week in all groups. Hemoglobin values were slightly decreased in the first week and then recovered the level of pretreatment value, gradually. 3. Lymphocyte count were declined significantly on group III(p<0.05), WBC and platelet counts were decreased on group II but statistically not significant.
