생애 첫 발열성 요로 감염 환아에 대한 평가에서 배뇨성 방광 요도 조영술을 대체하기 위한 검사로서 $^{99m}technetium$ dimercaptosuccinic acid(DMSA) scan의 의의 (The Significance of $^{99m}Technetium$ Dimercaptosuccinic Acid(DMSA) Scan as a Substitute for Voiding Cystourethrography(VCUG) in Evaluating Children with first Febrile Urinary Tract Infection)

  • 목 적: 환아의 임상 양상과 검사실 소견 및 DMSA scan 검사 결과를 이용하여 방광 요관 역류에 대한 예측성을 연구하였다. 방 법 : 생애 첫 요로 감염이 진단되어 입원 치료를 받았던 환아 중, 요로 감염 급성기에 DMSA scan 및 배뇨성 방광 요도 조영술을 모두 시행한 84명의 환아를 대상으로 후향적 연구를 하였다. 환아들은 배뇨성 방광 요도 조영술 결과에 따라 역류가 없는 집단, 경증 역류가 있는 집단(grade I-II), 중증 역류가 있는 집단(grade III-V)으로 분류되었고, 환아의 성별, 나이, 발열 기간, 혈중백혈구 수치, 혈장 CRP수치, 원인 균주 및 DMSA scan 결과와 방광 요관 역류 사이의 연관성을 조사하였다. 결 과 : 84명의 환아 중, 6명은 경증, 17명은 중증 역류를 보였다. DMSA scan 결과 급성 신장손상 소견은 38명에서 관찰되었다. 방광 요관 역류를 갖는 환아는 역류가 없는 환아에 비해 유의하게 나이가 많은 결과를 보였다(P<0.01). 요로감염 원인균주로서의 E. coli 감염의 빈도는 중증역류 환아에서 나머지 두 집단에 비해 유의하게 낮았다(P<0.01). DMSA scan에서의 급성 신장손상 소견은 역류 현상 및 역류의 중증도와 유의한 연관성을 보였다(P<0.05). DMSA scan에서 정상 소견을 보였던 환아 중 10.9%는 중증의 방광요관 역류를 보였다. 방광 요관 역류를 진단하는데 있어서 DMSA scan은 민감도 69.6%, 특이도 63.9%, 양성예측도 42.1%, 음성예측도 84.8%의 결과를 보였다. 결 론 : 방광 요관 역류 및 그 중증도는 DMSA scan에서의 급성 신장 손상 소견과 연관성이 있으나 낮은 민감도, 특이도, 양성 예측도를 볼 때, 급성기 DMSA scan에서 이상 소견을 보인 환아에서는 배뇨성 방광 요도 조영술을 시행하여 방광 요관 역류의 정도를 진단하여야 하며, DMSA scan에서 이상 소견이 없더라도 10.9%에서 중증의 방광 요관 역류가 동반되므로 배뇨성 방광 요도 조영술을 시행해야 할 것으로 생각한다.

소아 요로감염에서 배뇨성 방광 요도 조영술의 결정 (The Decision of Voiding Cystourethrography in Children with Urinary Tract Infection)

  • 목 적 : 본 연구에서는 소아에서의 연령과 성별, CRP, 그리고 혈액 내 백혈구 등의 독립적 요소들을 방사선학적 검사와 각각 비교평가하고 배뇨성방광 요도 조영술의 시행이 필요한지에 대하여 평가를 해보고자 한다. 방 법 : 본 연구에는 2002년 1월부터 2005년 1월까지 OO대부속병원 소아청소년과에서 $38^{\circ}C$ 이상의 발열을 주소로 내원하여 요로감염으로 처음 진단받은 98명이 포함되었다. 요로감염의 진단은 소변 배양 검사상 단일 균주가 105 CFU/mL 이상 자랐을 때로 정의하였으며, 신경학적 신장 기능이상이 보이는 환자들은 검사에서 제외하였다. 이들 모두에게 치료시작 전후의 발열기간을 기록하고 CRP, 백혈구 등을 검사하였으며, $^{99m}Tc$-DMSA 신장스캔과 복부-신장초음파, 퇴원 2근주 후 방광요로조영술 등을 시행하였다. 성별에 따른 방사선학적 이상의 빈도와의 관계는 chi-square test로 검정하였고, 연령에 따른 방사선학적 이상의 빈도와의 관계는 logistic regression model로 검정하였다. 방사선학적인 이상을 보인 환자군에서 진단당시의 발열기간, 백혈구, CRP 수치의 차이가 있는지를 알아보기 위하여 t-test를 사용하여 검증하였고 방사선 검사들을 chi-square test로 비교하여 양성간의 상관관계를 분석하였으며 유의 수준은 P<0.05로 하였다. 결 과 : 요로감염이 진단된 98명의 환아 중에서 52명이 남자 환자였고 46명이 여자 환자였다. 총98명의 환자들 중 배뇨성 방광 요도 조영술 검사상 이상소견을 보인 경우가 총18명이었고, 신장-복부초음파 결과에서 이상소견을 보인 경우는 총17명이었으며, $^{99m}Tc$-DMSA 신장 스캔에서 부분적 결손이나 미만성 섭취감소를 보인 경우는 총20명이었다. 각 연령과 성별에서의 방사선검사 결과들과의 상관관계는 통계학적으로 유의 한 차이를 보이지 않았다(Table 1, 2). 치료 전의 발열기간에서 신반흔을 보인 군이 음성인 군보다 발열기간이 길었다(Table 3). CRP, 백혈구는 방사선학적 이상소견이 있는 군에서 통계학적으로 유의하게 높았다(Table 4, 5). $^{99m}Tc$-DMSA 신장 스캔과 신초음파 검사의 양성유무는 배뇨성 방광 요도 조영술 검사의 양성유무와 통계적으로 유의한 상관관계를 보였다(Table 6, 7). 결 론 : 치료 전 발열기간, 입원 당시 백혈구, CRP에 기초한 임상적 평가는 첫 요로감염 질환에 걸린 환아들의 방광 요관 역류 여부를 예측하는데 유용하며, 추가적인 방사선학적 검사가 필요한지 에 대해 지침을 제공할 수 있다. 소아의 첫 요로감염시 초음파나 $^{99m}Tc$-DMSA 신장 스캔상에서 양성소견이 있을 경우 배뇨성 방광 요도 조영술 검사를 시행하는 것이 좋으며, 초음파와 $^{99m}Tc$-DMSA 신장 스캔상에서 양성소견이 없을 경우라도 CRP 또는 백혈구 등의 임상자료들을 평가하여 배뇨성 방광 요도 조영술 검사를 시행유무를 결정하는 것이 잔존하는 방광요관역류를 찾는데 도움이 될 것으로 생각된다.

Urine endothelin-1의 소아 요로 감염 환아에서의 진단적 유용성 (Urine endothelin-1 as a Biomarker for Urinary Tract Infections in Children)

  • 목적: 요로감염은 특별한 가 없는 영아나 소아에서 흔한 세균 감염으로 요로감염은 소변배양검사를 통하여 진단되며, 빠르고 정확한 진단과 치료가 중요하다. 하지만 배뇨조절이 잘 안되는 소아에서 진단 과정에서 오류가 발생하기 쉽다. Urine Endothelin-1 (ET-1)은 사구체 혈관 손상 시 사구체간질 세포에서 나오는 물질로 이를 통하여서 요로감염의 조기 진단의 유용성을 알아보고자 하였다. 방법: 2012년 7월부터 2013년 7월까지 13개월간 발열을 주소로 중앙대학교 병원 소아청소년과에 내원한 18세 미만의 70명의 환자를 대상으로 전향적으로 비교 분석하였다. 소변배양검사상 요로감염으로 진단된 실험군과 요로감염으로 진단되지 않은 대조군으로 나누었으며, 0.3 mL의 소변을 이용하여 Enzyme-linked immunosorbent assay 방법을 통해 urine ET-1을 정량적으로 측정하였다. 결과: 실험군은 45명이었고, 대조군은 25명이었으며, 실험군의 소변배양검사에서 Escherichia coli 42명, Klebsiella pneumonia 2명, Enterococcus faecalis 1명이 배양되었으며, 상부요로감염은 19명, 하부요로감염은 26명이었다. Urine ET-1은 실험군에서 평균 $1.41{\pm}0.35$ pg/mL, 대조군에서 $0.33{\pm}0.07$ pg/mL으로 통계학적으로 유의한 차이를 보였으며(P=0.04), 상부와 하부 요로감염간의 정량적 수치에서 유의성은 없었다(P=0.552). Urine ET-1과 혈청 C-reactive protein, 혈청 내 백혈구 간의 연관성은 없었다(pearson 상관계수: 0.24, 0.19). 결론: Urine ET-1은 적은 양의 소변으로도 검사 할 수 있으며, 요로감염을 진단하는 데에 유의한 결과를 보였다.

결핵성 흉막염 치료 후 흉막비후의 예측인자 (Factors Associated with the Development of Pleural Thickening in Tuberculous Pleurisy)

  • 연구배경: 결핵성흉막염 환자중에서 일부에서 적절한 항결핵 약물치료 후에도 흉막비후가 남는 것으로 알려져 있다. 그러나 흉막비후를 예측할 수 있는 인자에 대해서 잘 알려져 있지 않다. 본 연구에서는 결핵성 흉막염 치료 후 흉막비후를 예측할 수 있는 인자들이 있는지 알아 보고자 하였다. 방 법: 1995년 3월부터 1998년 1월까지 단국대학교 부속병원에서 결핵성 흉막염으로 진단받고 6 개월간 적절한 항결핵 약물치료와 추적관찰이 가능하였던 58명의 환자들을 대상으로 6개월 치료 후 단순흉부사진상 흉막비후가 있었던 환자군(1군 : 27명)과 흉막병변이 소실되었던 환자군 (2군 : 31명)으로 나누고 진단당시 임상소견, 방사선소견, 흉수소견 등을 분석하여 두 군사이에 차이가 있는지 알아보았다. 결 과: 47%(27/58)의 환자에서 치료 후 흉막비후가 있었고, 남자에서 여자에 비해 흉막비후가 더 많이 발생하는 경향을 보였다(54% 대 29%, p=0.092). 2 군의 환자중에서 흉막 병변이 완전히 소실된 시기는 1-2개월째는 26%, 3-4개월째는 29%, 5-6개월째는 45%로 흉막병변은 비교적 늦게 소실되었다. 1 군에서 2군에 비해 진단시 방사선 소견상 소방화를 보였던 환자가 더 많은 경향을 보였고(26% 대 19%), 흉수에서 백혈구와 임파구 숫자가 많거나 락트산탈수소효소치가 높은 경향을 보였지만($3527\pm5652$$2467\pm2201/ml$, $2066\pm2022$$1698\pm1835/ml$, $1636\pm1143$$1441\pm923IU/ml$), 모두에서 통계적으로 유의한 차이는 없었다. 진단시 까지의 증상의 기간, 진단시의 흉수의 양, 폐실질의 결핵변변, 흉수에서의 총단백량, 포도당치, Adenosine deaminase 활성도는 두 군에서 비슷한 결과를 보였다. 결 론: 결핵성 흉막염의 53%에서 6개월 치료 후 흉막병변의 소실을 보였는데, 병변의 소실은 주로 후기에 많았다. 진단당시의 임상소견, 방사선학적 소견, 흉수검사소견 중에 흉막비후의 예측지표로 이용할 수 있는 인자는 없었다.

학교 집단 요검사로 조기 진단된 IgA 신증 환아의 임상적 특징 (The Characteristics of IgA Nephropathy when Detected early in Mass School Urine Screening)

  • 목적: 면역글로불린A(IgA) 신증은 다양한 임상양상을 보이는 가장 흔한 원발성 사구체 신염으로, 적극적인 치료로서 단백뇨와 사구체손상을 줄여야한다. 저자들은 학교 집단 요검사를 통해 진단된 IgA 신증의 특징을 알아보고자 하였다. 방법: ${\bigcirc\bigcirc}$대학교병원 소아청소년과에서 조직검사를 받고 IgA 신증으로 진단받은 환자 64명을 내원 당시의 증상 유무에 따라 두 군으로 나누어 후향적으로 연구하였다. 결과: 증상 없이 학교신검에 의해 진단된 37명과 혈뇨 등으로 내원하여 진단된 27명을 대상으로 하였다. 각 군의 평균 연령은 각각 $10.8{\pm}2.7$세 및 $9.5{\pm}3.4$세였고, 각 군 모두에서 남아가 많았다. 증상 발현 후 내원까지의 기간과 조직 검사까지의 기간은 증상군에서 더 짧았다. 임상 증상은 증상군에서 육안적 혈뇨(P<0.001)와 부종(P=0.008)이 더 많았다. 혈액검사에서 증상군의 백혈구 수가(P=0.007) 많았고, 혈색소 값은(P=0.007) 더 낮았으며 BUN과 크레아티닌 값은 차이가 없었다. 혈청 알부민 값은 증상군에서 학교신검군에 비해 더 낮았고(P=0.006), 두 군에서 치료기간은 차이가 없었고, 치료약제는 ACEI가 가장 많았다. 전체 환아군을 대상으로 광학적 신조직 검사분석 시, 육안적 혈뇨기왕력이 있는 군에서 없는 군에 비해 좀 더 심한 병변을 보였으며, 단백뇨의 중증도와 병변의 중증도와는 상관이 있었다. 결론: 학교신검군 및 증상군 모두 조기 진단된 IgA 신증으로 임상적 및 조직학적인 큰 차이를 볼 수 없었다. 학교신검은 조기에 IgA 신증을 선별하는데 유용하지만, 조기진단이 치료나 예후에 어떤 영향을 주는지는 향후 보다 많은 대상자들의 장기간 추적관찰이 더 필요할 것으로 생각된다.

국소 마취하 배액관 삽입 및 세척을 통한 화농성 슬관절염의 치료의 효용성 (Effectiveness of Drain Insertion and Irrigation in the Treatment of Septic Arthritis of the Knee under Local Anesthesia)

  • 목적: 화농성 슬관절염은 조기 진단과 신속한 수술적 치료가 필요한 정형외과적 응급 질환이다. 본 연구는 국소 마취하에 배액관 삽입 및 세척을 통한 화농성 슬관절염 치료의 효용성에 대해서 알아보고자 하였다. 대상 및 방법: 2017년 9월부터 2020년 2월까지 화농성 슬관절염을 진단받고 국소 마취하에 배액관 삽입과 세척을 통해 치료를 시행 받은 총 9예(8명)의 환자를 대상으로 후향적 연구를 하였다. 상외측 삽입구에서 상내측으로 관통하는 방식으로 배액관을 넣은 후 약 3 L의 생리식염수를 이용하여 매일 세척하였다. 나이, 성별, 기저질환, 화농성 슬관절염의 원인, 골관절염의 정도, 진단 후 수술까지 소요된 시간, 재원기간, C-반응성 단백이 정상화되는 기간, 배양 균주 등을 확인하였다. 결과: 진단 당시 혈액검사에서 혈청 백혈구 수치는 평균 9,211±2,912개/mm3 (6,100-16,300개/mm3), C-반응성 단백은 평균 12.1±11.2 mg/dl (3.9-35.4 mg/dl)였으며, 초기 관절 천자액 검사상 평균 백혈구 수치는 71,472±51,667개/mm3 (32,400-203,904개/mm3), 평균 다형 백혈구 분율은 평균 91.1%±2.6% (86%-95%)였다. 진단 후 수술까지 소요된 시간은 평균 8.3±1.3 시간(6-10시간)이었고, 평균 세척 기간은 8.2±3.2일(4-15일), 평균 재원 기간은 20.8±8.7일(9-37일)였다. 도말 및 배양 검사로 원인균은 동정되지 않았다. 결론: 화농성 슬관절염의 치료에서 국소 마취를 통한 삽입관 삽입과 세척술은 비교적 빠르고 간단한 방법으로 조기에 관절 내 삼출물을 제거하여 통증을 줄이고 반복적으로 세척할 수 있어 전신 또는 척수 마취의 위험성이 높은 환자에게 치료의 대안으로 활용될 수 있다.

유육종증 전국실태조사 (National Survey of Sarcoidosis in Korea)

  • 연구배경 : 전국 실태조사를 통하여 현재까지 확진된 전국의 유육종증 발생현황을 파악하고 이를 분석하고 외국의 보고와 비교하여 일반 개업의 및 내과 전문개업의 진료활동에 도움이 되도록 하고자 하였다. 방법 : 조사와 분석기간은 1991년 1월부터 1992년 12월까지 2년간으로 정하였으며 자료수집방법은 후향적 방법으로 입원실이 100병상 이상인 213개 종합병원의 내과, 안과, 피부과, 신경내과에 서신을 통한 경험증례를 수집 하였으며 확진예나 의심예가 있었던 경우에 유육종증 증례 기록지를 보내어 정확한 임상 및 검사정보를 수집하여 분석하였다. 결과 : 1) 제 1 차 설문엽서 발송은 입원실이 100병상 이상인 213개 종합병원의 내과, 안과, 피부과, 신경내과의 523개과 과장 및 호흡기 내과를 담당하는 전문의에게 발송하였다. 내과가 213개과로 41%, 피부과가 1087개과로 20%, 안과가 104개과로 20%, 신경과가 100개과로 19%를 차지하였다. 2) 제 1 차 설문조사에 대한 답신은 241개과에서 연락이 있었다(48% 회신율). 3) 확진예 및 의심예는 총 50개과 113예 및 10예가 있다고 보고되었으며 내과가 92예로 81%, 피부과 15예로 13%, 안과, 신경과가 각 3예로 3%를 차지하였다. 자료분석은 총 78예에서 시행되었고 내과 72예로 92%, 피부과 4예로 5%, 신경과 2예로 3%를 차지하였다. 4) 1980년 1에보고 이후 증례가 많은 수로 증가하여 1989년 13예, 19901건 17예, 1991년 18예가 보고되었다. 5) 진단 당시의 연령은 20대, 30대, 40대가 84.4%로 대부분을 차지하고 있었다. 6) 남녀비는 1 : 1.5의 비율로 여자 환자가 많았다. 7) 진단을 받게된 동기로 주소의 빈도로는 호흡기 증상이 48%로 가장 많았고, 피부의 병변, 전신증상, 시각장애, 관절통, 복통, 연하곤란의 순서이었고 무증상이 12예(16%)이었다. 8) 초증상에서 확진을 받을때까지 중앙치는 2개월이었다. 9) 증상의 반도로는 호흡기 증상, 전신증상, 피부증상, 눈 증상, 말초림프구 종창, 신경계증상, 심장증상의 순이었다. 10) 말초혈액검사 소견상 혈색소, 혈구용적치 및 혈소판수는 대부분 정상범위를 나타냈다. 58%의 증례에서 30%미만의 림프구 감소증을 나타냈으며, 임파구 아형으로 T4/T8비는 1.73{\pm}1.16$(0.43~4.62)였다. 43%에서 혈침속도가 상승하였다. 11) 혈청 생화학검사는 대부분 정상범위를 이었다. 혈청 ACE 농도의 평균치는 $66.8{\pm}58.6$(8.8~265) U/L로서 46.2%에서 증가되었다. 42.9%에서 하루 150 mg 이상의 단백뇨 소견을 나타냈고, 36.8%가 하루 300 mg이상의 단백뇨 소견을 나타냈다. 12) 면역학적 검사상 혈청 IgG는 43.5%, IgA는 45.5%, lgM은 59.1%, IgE는 46.7%에서 상승되었고, C3 및 C4는 대부분 정상범위 였으며, ANA는 11%에서 양성을 보였다. Kveim test는 시행된 3예에서 모두 양성을 나타냈다. 13) 폐기능검사중 FVC는 80% 미만인 경우가 17.3%, FEV1은 70% 미만인 경우가 11.5% FEV1/FVC는 70% 미만인 경우가 10%, TLC는 80% 미만인 경우가 15.2%, DLCO는 100% 미만인 경우가 64.7%이었다. 14) 동맥혈가스 분석소견은 PaO2가 90mmHg 미만인 경우가 48.6%, PaCO2는 평균 45mmHg보다 증가된 경우보다 5.7%이었다. 15) 기관지 폐포세척액 검사상 대식세포의 평균 백분율은 $51.4{\pm}19.2%$였고, 임파구의 평균 백분율은 $44.4{\pm}21.1$이였다. 임파구아형은 T4/T8 ratio는 $3.41{\pm}2.07$이였다. 16) 전 검사항목에 대한 남녀 성별간의 유의한 차이는 없었다. 17) 흉부방사선의 분석에서 폐문 임파절 비대를 보인례는 87.9%였고, 78.8%에서는 양측성병변을 9.1%에 일측성 병변을 보였다. 18) Siltzbach의 분류에 의한 병기 구분시 stage 0 : 5%, stage 1 : 58.3%, stage 2 : 28.3%, stage 3 : 8.3%의 분포를 보였다. 19) 흉부 CT를 시행한 54예에서 폐문임파절비대는 50예(92.6%)에서 보였고 이들 중 47예는 양측성 비대였다. 20) 고해상도 CT는 16예에서 시행하였는데, 2예에서는 병변이 없었으며, 폐실질의 병변은 결정성 병변, 소엽간 중격비후, 반점상 국소 폐음영의 순으로 나타났다. 21) 부수적으로 시행한 방사선학적 검사에서 골병변은 1예에서 수근골의 병변을 보였고, 초음파검사에서 2에에서 비장비대의 소견을 보였다. 22) 갈륨스캔은 12에에서 시행하여 폐문 및 종격동 임파절의 흡수증가 소견이 8예에서 보였고, 폐장의 흡수증가는 2예에서 보였다. 23) 조직검사상 가장 많이 시행한 진단방법은 경기관지 폐생검 47.3%이었고, 피부와 종격동 임파절 생검은 각각 34.5%, 말초임파절생검 23.6%, 개흉 폐생검 18.2%, 기관지생검이 16.2%이었다. 2가지 이상의 부위에서 조직검사를 시행한 경우는 52.7%이었다. 한 부위에서만 조직검사를 시행한 26예중 종격동 임파절 생검은 10예, 피부생검 6예, 말초임파절 생검 5예, 개흉폐생검 3예 그리고 경기관지 폐생검 2예이었다. 24) 병리소견상 비건락성 육아종은 100% 모두에서 관찰되었며, 다핵거대 세포는 47.3%, 봉입체는 10.9%, 초자질 섬유화는 34.5%, 괴사 9.1%, reticulin fibrils network 20%, lymphangitic distribution of granuloma 1.8%에서 관찰되었다. 결론 : 유육종 전국 실태조사에 따른 영상소견, 검사실소견, 방사선소견, 병리소견을 분석해 본 결과, 우리나라 유육종 환자들도 다소 차이는 있었지만 외국의 보고들과 유사한 소견들을 나타냈으며, 향후로는 이들에 대한 결과들을 추가분석하여, 유육종 진단에 도움을 줄 수 있고, 임상적 지표로 이용될 수 있는 검사항목을 찾아내는 것이 과제라 할 수 있겠다. 유육종증에 대한 자료는 대한 결핵 맺 호흡기학회에 보관이 되어 있음을 알려드립니다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure.
