Surgery versus Nerve Blocks for Lumbar Disc Herniation : Quantitative Analysis of Radiological Factors as a Predictor for Successful Outcomes

  • Objective : To assess the clinical and radiological factors as predictors for successful outcomes in lumbar disc herniation (LDH) treatment. Methods : Two groups of patients with single level LDH (L4-5) requiring treatment were retrospectively studied. The surgery group (SG) included 34 patients, and 30 patients who initially refused the surgery were included in the nerve blocks group (NG). A visual analogue scale (VAS) for leg and back pain and motor deficit were initially evaluated before procedures, and repeated at 1, 6, and 12 months. Radiological factors including the disc herniation length, disc herniation area, canal length-occupying ratio, and canal area-occupying ratio were measured and compared. Predicting factors of successful outcomes were determined with multivariate logistic regression analysis after the optimal cut off values were established with a receiver operating characteristic curve. Results : There was no significant demographic difference between two groups. A multivariate logistic regression analysis with radiological and clinical (12 months follow-up) data revealed that the high disc herniation length with cutoff value 6.31 mm [odds ratio (OR) 2.35; confidence interval (CI) 1.21-3.98] was a predictor of successful outcomes of leg pain relief in the SG. The low disc herniation length with cutoff value 6.23 mm (OR 0.05; CI 0.003-0.89) and high baseline VAS leg (OR 12.63; CI 1.64-97.45) were identified as predictors of successful outcomes of leg pain relief in the NG. Conclusion : The patients with the disc herniation length larger than 6.31 mm showed successful outcomes with surgery whereas the patients with the disc herniation length less than 6.23 mm showed successful outcomes with nerve block. These results could be considered as a radiological criteria in choosing optimal treatment options for LDH.

Analysis of Changes in Stride Length, Time, and Electromyography Finding Depending on Athletic Crouch Start Method

  • Objective: The purpose of this study was to compare and analyze the kinematic variables and electromyography (EMG) findings that change with varying characteristics of crouch start and feedback provision, and to provide the fundamental data for record improvement in 400 mH. Method: Four short-distance runners participated in the experiment. The analyzed variables were elapsed time per interval, stride length, and muscle activities in three lower limb muscles. These variables were analyzed by using Kwon3d XP and Noraxon Myoresearch. The participants were subjected to three conditions, including two conditions that relate to the thrusting foot on the rear block and another condition pertinent to feedback provision. Results: In terms of a one-step interval, the elapsed time in condition A was longer than that in condition B, and the one after the feedback was the longest. The stride length of a one-step interval was the longest in condition A. The stride length of a two-step interval was the shortest in condition A. The muscle activity during a one-step interval showed differences in vastus medialis and medial gastrocnemius, with condition A being the highest. Conclusion: When the non-dominant left foot was located at the back, negative results were observed in terms of elapsed time and stride length. Moreover, an imbalance in muscle activity was observed between the left and right feet when the left foot was placed at the back. As a result, significant differences in elapsed time, stride length, and muscle activity were observed depending on the foot placed on the rear block. In conclusion, we identified the characteristics of crouch start in 400 mH, and a specialized program must be suggested.

  • This study is performed to examine the development of the plantable concrete using excellent soil compound, crushed stone, stone dust and superplasticizer. The following conclusions are drawn ; The void ratio is in the range of $25.10{\sim}31.40%$ and the compressive strength is in the range of $69{\sim}113kgf/cm^{2}$ at the curing age 28days, respectively. The plant length was showed higher block by 10% stone dust than 20%.

  • A new image coding technique has been proposed based on the wavelet transform. To achieve lower ceding rates and good qualities in reconstructed images, some of wavelet coefficients were removed by thresholding and quantized in accordance with the sensitivity of the human visual system(HVS). For each block of subimages in wavelet transform domain, block thresholding scheme has been used to remove the unimportant wavelet coefficients according to the frequency characteristic and statistical property of wavelet coefficients. The location information of quantized blocks and removed blocks were encoded using run-length coder which is effective for the exponential distribution. Quantized coefficients were encoded using variable length coder which matches well to their distribution. Simulation results show that the reconstructed images maintain high quality with the low bit rate, below 1.0 bits per pel.

  • Practical communication systems need to operate at various different rates. This paper describes and analyzes low-density parity check codes for various different rates. From a specific mother code, it allows LDPC codes for different rate. The advantage of this technique is that each different rate LDPC codes have a same block length as mother code though the rate changes so it can make up for the weak points of puncturing and shortening which reduce their block length as the rate changes. Row-splitting method is to split the row, so that the rate changes from a higher rate to lower rate and cause of its own property, it can overcome the defect of row-combining method.

  • The purpose of this study is to develop a bodice block suitable for women in their 20s with turtle neck syndrome. The problem of turtle neck syndrome body type was derived through the evaluation of bodice blocks. First, the length of the front bodice was longer and the length of the back bodice was shorter. Second, the front neck point, back neck point, and the shoulder line were not in place. Third, the side neck was pushed up. Fourth, the length of armhole was long and it floated. Fifth, the front hem was attached to the body and the back hem was away from the body. A finally developed pattern was designed by developing four test patterns to solve the problem that appeared in a selected bodice pattern, and a t-test was conducted to determine the difference in the appearance evaluation by virtual clothing between the two patterns, bodice block and the development pattern. According to the result, the fit was not good from the side body because the position of the side neck point was not in a place due to the characteristics of the body shape of turtle neck syndrome, but the problems of the anterior neck position, the position of the back neck point, the shoulder line position, and the armhole shape were improved in the developed pattern.

  • Objectives : Visual check and X-ray are commonly used by chiropractors to estimate ieg length inequality, This study have three categories: diagnosis for anatomic leg length inequality; difference between anatomic and functional leg length inequality; theraphies for anatomic or functional leg length inequality. Methods : We referred to a PubMed site by using word of 'leg length [JU] J Manipulative Physiol Ther', only items with abstracts. Results : We searched 26 articles in J Manipulative Physiol Ther with the key word-Ieg length. Conclusion : 1. Radiographs were most accurate and commonly used by chiropractors to measure anatomic leg length inequality, clinically wood block, tape measure, visual check are acceptable. 2. There was no article about difference between anatomic and functional leg length inequality. 3. Heel lift was commonly used with conservative theraphy for anatomic leg length Inequality. 4. Chiropractors have not yet proved that the supposed positive effects are a result of a reduction of subluxation, The detection of the manipulative lesion in the sacroiliac joint depends on valid and reliable tests, Because such tests have not been established, the presence of the manipulative lesion remains hypothetical. Great effort is needed to develop, establish and enforce valid and reliable test procedures.

  • Polyarylate(PAR)-nylon 6 block copolymers of various block lengths were prepared by the anionic polymerization of ${\varepsilon}$-caprolactam using the polymeric activator from hydroxy-difuncrtional PAR and toluene diisocyanate. Phase separated morphology of PAR-nylon 6 block copolymer was suggerted from the thermal properties measured by differential scanning calorometry(d.s.c.). Partial miscbility between PAR block and nylon 6 block of the block copolymers was more evident at shorter length of constituent blocks. In binary blends of PAR-nylon 6 block copolymer with PAR or nylon 6 homopolymer, molecular-level mixing of homopolymers with corresponding blocks of block copolymer was supposed from the thermal properties measured by d.s.c..
