A case of a patient with forcible closure of the eyelids diagnosed as blepharospasm treated with Traditional Korean Medicine (안검경련을 호소하는 여환 한방 치험 1례)

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Traditional Korean Medicine(TKM) on a patient with forcible closure of the eyelids due to blepharospasm. ■ Methods A patient with forcible closure of the eyelids diagnosed as blepharospasm was treated with herbal medication, acupuncture, electro-acupuncture, cupping and moxibustion. Then we evaluated the improvement with length of eye opening and Blepharospasm Disability Index. ■ Results Increase of length of eye opening and Blepharospasm Disability Index were observed after the TKM treatment. ■ Conclusion This study proved the effect of TKM treatment on forcible closure of the eyelids due to blepharospasm.

Differences Between Facial Skin Temperature of the Paralyzed Side and Those of the Normal side in Essential Blepharospasm patients. (포륜진도 환자의 건측-환측 안면부 피부온도차이에 관한 연구)

  • Objectives Benign essential blepharospasm is a progressive neurological disorder characterized by involuntary muscle contractions and spasms of the eyelid muscles. The aim of this study is to provide evidence of differences between facial skin temperaure of the paralyzed side and normal side in Essential Blepharospasm patients. Methods The author studied 13 patients with Essential Blepharospasm. We measured skin temperature of the forehead, zygoma area of the paralyzed side and those of the normal side with The Digital Infrared thermal image. Results There were no significant facial skin temperature differences between the paralyzed side and normal side. Conclusions Thermography is a useful diagnostic tool. But we expect that it is essential to diagnose a disease correctly at an early stage and to find a prompt treatment by introducing and utilizing a simple nonivading diagnofic method other than Thermography.

  • Blepharospasm and apraxia of lid opening(ALO) are non-paralytic causes of involuntary eyelid closure. Clinically it is difficult to differentiate blepharospasm and ALO, and these two conditions are sometimes associated. We report a case of pretarsal blepharospasm presenting as apraxia of lid opening. 55-year-old woman was noted to have voluntary eye opening difficulty. We synchronously record the electromyographic(EMG) from the levator palpebrae superioris and the orbicularis oculi muscles in this patient. This results suggested that she has a variant of blepharospasm due to abnormal contraction in the pretarsal orbicularis oculi.

Does a Blepharospasm mean the Presymptom of Stroke? (안검경련(眼瞼痙攣)을 중풍 전조증상이라 할 수 있는가?)

  • Objectives : The purposes of this study were to investigate that a blepharospasm means the presymptom of stroke based on the traditional hypothesis in the oriental medicine and to compare a blepharospasm with warning signs of stroke in the western medicine. Methods : In the time period Oct. 2005 to Oct. 2006, 409 patients with a first-ever stroke admitted in the department of Internal Medicine of Kyungwon University In-cheon Oriental Medical Hospital, Kyunghee University Oriental Medical Hospital, Dongguk Il-san Oriental Medical Hospital were included. Patients were hospitalized within 14 days after the onset of stroke. Stroke patients were interviewed by residents who studied standard operation procedures in Fundamental Study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. A questionnaire was completed by a question-and-answer form between patients and residents after explanation details to patients and the agreement of patients. Results : Age, care of patients or grandson, diabetes mellitus, and smoking were higher among cerebral infarction group, while smoking was higher among hemorrhage group. Female, young age, hyperlipidemia, hypertension in the family history and fat body in waist-hip ratio were higher among patients undergoing the blepharospasm before stroke onset. And the incidence of blepharospasm was lower in patients who dislike the fast food. Finally, when we compared a blepharospasm with warning signs of stroke in the western medicine, the incidence of blepharospasm in this study were the most frequent. Conclusion : In this study, the incidence of blepharospasm in patients before stroke onset was more frequent than that of warning signs in the western medicine. But more data from prospective cohort studies should be collected to be accepted that the blepharospasm is the presymptom of stroke as like warning signs in the western medicine.

The Clinical Observation on 47 Cases of Patients with Blepharospasm Treated by Wisunghanggyeok(胃勝寒格) (위승한격(胃勝寒格)을 시술한 안검경련환자 47례에 대한 임상적 고찰)

  • Objectives : This study was performed to observe 47 cases of patients with blepharospasm treated with Wisunghanggyeok(胃勝寒格). Methods : For 28 months from Sep. 2006 to Dec. 2008, we researched 47 blepharospasm patients treated with Wisunghanggyeok(胃勝寒格) who visited the author's clinic. Results & Conclusion: 1. It shows more female patients than male patients, and mostly patients in their fifties. 2. The lesion is shown more on the right side than on the left side, and the left side blepharospasm is cured more effective. 3. The causes of hemifacial spasm are mainly stress and overwork, and there is a 85% improvement of 39 cases from these two causes. 4. The disease period is mainly within 1 week, and that period is the most effective time. 5. The number of treatment is mainly under 5 times, and the effect of treatment over 4 times for week is more available. 6. There are 59.57% of excellent improvement and 23.4% of good improvement, and these make 82.97% improvement of 47cases.

Comprehensive Korean Medicine Treatment for Benign Essential Blepharospasm in a Patient Responding Poorly to Botulinum Toxin Treatment

  • Benign essential blepharospasm (BEB) is a condition/disease which involves involuntary muscle contractions causing the eye to repetitively close, making it impossible for the patient to perform daily activities. This study reports Korean medicine treatment of a rare case BEB in a 34-year-old male patient whose symptoms showed minimal improvement following botulinum toxin injection (the standard treatment). Acupuncture, pharmacopuncture, and herbal medicine treatment for 11 days resulted in clinical improvement for all symptoms as assessed by using the Jankovic Rating Scale, Blepharospasm Disability Index, numerical rating scale, duration of spasms lasting more than 1 hour per day, and number of spasms lasting less than 1 minute per day. This case report suggests that comprehensive Korean medicine treatment could be a treatment option for BEB in patients who do not respond well to botulinum toxin injection.

A Case Report of a Patient with Cranial Dystonia Diagnosed as Meige Syndrome (메이그증후군으로 진단받은 안면 근육 긴장 이상 환자 증례 보고 1례)

  • Objective: Despite being the most common cause of cranial dystonia, Meige syndrome remains a rare clinical entity. It is characterized by blepharospasm and orofacial dystonia. This case study reports on the use of traditional Korean medicine for a patient who suffered from blepharospasm and oromandibular dystonia suspected to be the result of Meige syndrome. Methods: A patient was given a series of Korean medicine therapy treatments, including administration of the herbal medicine Gamiguibi-tang-gamibang, acupuncture, and cupping, over a 19 day hospitalization period. We measured the state and progress of this case using the numeric rating scale and dystonia frequency. Results: A decrease in subjective symptoms and reduction in the frequency of oromandibular dystonia was observed after Korean medicine therapy treatments. Conclusion: This case is useful for describing herbal medicine treatment for Meige syndrome, and further case reports and studies will be needed in the future.

  • The role of botulinum toxin as a therapeutic agent is expanding rapidly in otolaryngology. Botulinum toxin is a protease that blocks the release of acetylcholine from nerve terminals. Its effects are transient and nondestructive, and largely limited to the area in which it is administered These effects are also graded according to the dose, allowing for individualized treatment of patients and disorders. Botulinum toxin has been used primarily to treat disorders of excessive or inappropriate muscle contraction. In the field of otolaryngology, these include spasmodic dysphonia, oromandibular dystonia, and blepharospasm, vocal tics and stuttering, cricopharyngeal achalasia, various tremors and tics, hemifacial spasm, temporomandibular joint disorders and a number of cosmetic applications. Botulinum toxin treatment has recently begun to show some benefit in the control of pain from migraine and tension headache. It may also prove useful in the control of autonomic dysfunction, as in Frey syndrome, sialorrhea, and rhinorrhea. In over 20 yews of use in humans, botulinum toxin has accumulated a considerable safety record, and in many cases represents relief for thousands of patients unaided by other therapy.

The Histological Change of Orbicularis Oculi Muscle according to the Age (나이에 따른 안륜근의 조직학적 변화)

  • This study is to investigate the patients who had blepharoplasty for cosmetic purposes or facial nerve disorders. 21 patients were examined with an age from 17 to 63 years. Sixteen cases were classified as a normal group for investigation of the histopathologic change according to the age. The rest were patients of Bell's palsy(2), $Botox^{(R)}$ injection(2) and blepharospasm (1). We measured a mean diameter of myofiber and dimension's proportion of muscluar tissue per regular visual field. First, we measured a statistical significance according to the age in normal cases by grouping them into younger than 30-year-old, 30's, 40's, 50's and 60's. Second, we divided normal cases(16) into Group A and B by two bases for statistical analysis. In conclusion, although there were several differences as vacuolation, internalization of sarcolemnic nuclei between 35-year-old and 62-year-old patients in simple comparison, there was no uniform decrease according to the age. There were also no significant statistical changes in the value of a diameter of myofibers and dimension's proportion of muscular tissue according to the age in any of the two normal groups. But in $Botox^{(R)}$ injection and Bell's palsy, blepharospasm patients, two values showed remarkable difference compared to the normal group of same age.

Botulinum Toxin for other Head and Neck Lesions (기타 두경부 병변에서의 보툴리눔 독소의 이용)

  • The usages of botulinum toxin were most commonly for the treatment of spasmodic dysphonia in the otolaryngology field. It has been not only widely used in otolaryngology-Head Neck surgery but also plastic surgery, ophthalmology, rehabilitation medicine, and orthopedics. Now botulinum toxin is used such as blepharospasm (excessive blinking), strabismus, cosmetic, muscle spasms, upper motor neuron syndrome, severe primary axillary hyperhidrosis (excessive sweating), cervical dystonia (spasmodic torticollis), chronic migraine, bruxism, and achalasia. The indication of this drug still gradually expanding with the times. In this articles, the author will demontrate how to use the botulinum toxin for treating cricopharyngeal spasm, arytenoid dislocation, sialocele, Frey syndrome, contact granuloma, bilateral vocal fold paralysis, and mutaional falsetto instead of conventional surgical treatment.
