여러 간외담도 질환에서 삼차원적 자기공명 췌담관 조영술과 내시경적 역행성 췌담관 조영술과의 비교: 진단적 정확성을 중심으로 (Comparison of Diagnostic Accuracy of Three-Dimensional MR Cholangiopanceatography and ERCP in Various Extrahepatic Biliary Lesions)

  • 목적: 다양한 간외담도질환에서 3차원적 자기공명췌담도조영술(MR cholangiopancreatography, MRCP)의 진단적 가치를 내시경적 역생성 췌담도조영술(ENDOSCOPIC RETROGRADE CHOLANGIOPANCREATOGRAPHY, ERCP)와 비교하여 알아보고자 하였다. 대상 및 방법: 간외담도질환이 의심되는 46명의 환자에 대하여 MRCP와 ERCP를 각각 시행하였다. MRCP는 reverse fast imaging with a steady-state free precession(reverse FISP:PSIF) 기법을 이용하여 얻어낸 후 MIP algorithm을 이용하여 재구성하였다. 담도확장여부와 폐쇄부위 진단에 대한 MRCP의 정확성을 ERCP를 최적기준으로 하여 평가하였다. 또한 악성 및 양성병변의 감별과 종합적인 진단정확도를 MRCP와 ERCP간에 비교하였다. 결과: MRCP의 담도확장여부 예측은 민감도 94.6%, 특이도 75.0%, 정확도 91.1%였다. 폐쇄부위 예측의 정확도는 87.0%였다. 악성과 양성병변의 감별에 있어 MRCP와 ERCP의 민감도, 특이도, 정확도는 각각 76.2%, 87.5%, 82.2%와 71.4%, 83.3%, 77.8%였다. 각각의 지롼에 대한 전체적 진단의 정확도는 MRCP가 60.0%, ERCP가 55.6%였다. 결론: 3D MRCP는 간외담도질환에 있어 ERCP와 비교하여 손색없는 진단적 가치를 가지며, 앞으로 ERCP를 대체할 수 있는 좋은 검사방법이라고 생각된다.

담도폐쇄증 진단에서 DISIDA 간담도주사의 진단적 의의 (Diagnostic Utility of Tc-99m DISIDA Hepatobiliary Scintigraphy in the Diagnosis of Biliary Atresia)

  • 목 적: 신생아 담즙정체증의 흔한 원인중 하나인 담도폐쇄증의 진단에 많이 이용되고 있는 Tc-99m DISIDA간담도 주사의 진단적 의의에 대하여 알아보고자 하였다. 방 법: 1995년 1월부터 1999년 8월까지 4년 8개월 동안 서울 중앙병원 소아과와 소아외과에서 신생아담즙정체증으로 Tc-99m DISIDA간담도 주사를 시행한 60명의 환자를 대상으로 그 결과를 분석하였다. 결 과: 전체 60명의 환자 중에서 담도폐쇄증으로 14례(23%), 신생아 간염으로 33례(55%), 간내 담도형성부전증으로 9례(15%), 경정맥 고영양법으로 인한 황달로 4례(7%)가 진단되었다. Tc-99m DISIDA 간담도주사의 민감도는 100%를 보였고 특이도는 80%였다. 결 론: Tc-99m DISIDA 간담도주사에서 장관내 방사능이 관찰되면 담도폐쇄증을 진단에서 제외할 수 있지만 관찰되지 않을 경우에는 경피간침생검이나 시험적 개복술 등의 적극적인 진단방법을 고려해야 할 것으로 생각된다.

속발성 담도부 종양에 의한 담도 폐쇄에서 고선량률 관내 근접치료 (High-Dose-Rate Intraluminal Brachytherapy for Biliary Obstruction by Secondary Malignant Biliary Tumors)

  • 목적 : 속발성 담도부 종양에 의한 담도 페쇄로 방사선치료를 시행한 환자 중 고선량률 관내 근접치료를 시행한 환자를 대상으로 생존기간, 고식적 치료 효과, 예후인자, 치료에 수반된 부작용을 조사하여 방사선치료의 유용성에 대해 분석하고자 하였다. 대상 및 방법 : 1992년 12월부터 20이년 8월까지 속발성 담도부 종양에 의한 담도 폐쇄가 발생한 24명의 환자를 대상으로 후향적인 연구를 하였다. 환자의 연령은 35세부터 82세(중앙 연령 58.5세)였다. 대상 환자를 원발 종양 부위에 따라 분류해 보면 위암이 12명, 담낭암이 6명, 간암이 3명, 췌장암이 2명, 대장암이 1명이었다. 환자의 중앙 추적조사기간은 7.2개월(3$\~$76개월)이었다. 외부방사선치료는 24명 중 18명에서 시행하였으며 매일 1.8$\~$2.5 Gy씩 주 5회 분할치료를 하였고 30$\~$61.2 Gy (중앙값 50 Gy)의 방사선량이 조사되었다. 고선량률 관내 근접치료는 주당 2회에서 3회를 치료하였고 16명의 환자는 일일분할선량을 3 Gy로, 8명의 환자에서는 분할선량을 2.5 Gy로 하루 2회 시행하는 방법으로 치료하였다. 고선량률 관내 근접치료의 총조사선량은 9$\~$30 Gy (중앙값 15 Gy)이었다. 결과 : 대상 환자 24명 중 22명은 추적기간 중 사망하였다. 환자의 중앙 생존 기간은 7.3개월이었고 6개월 이상 생존한 환자는 13명(54.2$\%$), 1년 이상 생존한 환자는 5명(20.8$\%$)이었다. 단일변량분석에서 1년 이상 생존에 영향을 준 유의한 인자는 총 방사선량50 Gy 이상)(p=0.0200)이었고 다변량분석에서도 총 방사선량(50 ey 이상)(p=0.0337)이 유의성을 보였다. 추적조사 기간 동안 5명의 환자에서 방사선치료로 인한 담도염이 의심되었다. 1명의 환자에서 치료후 7개월에 혈액담즙증(hemobllla)이 발생하였으나 이는 방사선치료에 의한 후기 합병증보다는 종양의 국소적인 진행에 의한 천공으로 생각되었다 결론 : 담도의 배액술을 시행한 후 고선량률 관내 근접치료를 시행할 경우 기존의 배액술만 시행한 환자에 비해생존기간의 연장이 기대되었고 고선량률 관내 근접치료를 외부방사선치료에 추가할 경우 추가 방사선량은 효과적으로 조사할 수 있었고 예후인자인 총 방사선량을 늘릴 수 있었다. 외부방사선치료 및 고선량률 관내 근접치료에 따른 부작용은 대증 요법에 의해 효과적으로 조절되어 안전한 치료였다

개에서 췌장염이 병발한 간외성 담관계 페쇄 치료를 위한 경피적 초음파 유도하 담낭 천자술 2예 (Percutaneous Ultrasound-Guided Cholecystocentesis for Treatment of Extrahepatic Biliary Tract Obstruction Concurrent with Pancreatitis in Two Dogs)

  • Two dogs were referred to Veterinary Medical Center, Chungbuk National University with the clinical signs of vomiting, diarrhea, anorexia, and jaundice. Both dogs were diagnosed as extrahepatic biliary tract obstruction(EHBO) with concurrent pancreatitis based on dilated common bile duct and hyperechoic or mixed-echoic pancreas in abdominal ultrasonographic examination and serum biochemical abnormalities, such as high serum bilirubin, cholesterol, and increased cholestatic enzyme activity. Percutaneous ultrasound-guided cholecytocentesis(PUCC) was performed to examine bile and decompress the gall bladder. After PUCC with medical therapy, both dogs were steadily improved clinical signs of jaundice and anorexia. Also, cholestatic enzyme activity and serum bilirubin concentration decreased. Any complications that have been described in previous studies, such as peritonitis resulting from bile leakage and hemorrhage, were not identified. It is assumed that percutaneous ultrasound guided cholecystocentesis may be an useful diagnostic and therapeutic tool in canine gallbladder disease and can be used easily and safely to gain bile for diagnosis of bacterial cholecytitis.

Prospective Multicenter Study of the Challenges Inherent in Using Large Cell-Type Stents for Bilateral Stent-in-Stent Placement in Patients with Inoperable Malignant Hilar Biliary Obstruction

  • Background/Aims: Although endoscopic bilateral stent-instent placement is challenging, many recent studies have reported promising outcomes regarding technical success and endoscopic re-intervention. This study aimed to evaluate the technical accessibility of stent-in-stent placement using large cell-type stents in patients with inoperable malignant hilar biliary obstruction. Methods: Forty-three patients with inoperable malignant hilar biliary obstruction from four academic centers were prospectively enrolled from March 2013 to June 2015. Results: Bilateral stent-in-stent placement using two large cell-type stents was successfully performed in 88.4% of the patients (38/43). In four of the five cases with technical failure, the delivery sheath of the second stent became caught in the hook-cross-type vertex of the large cell of the first stent, and subsequent attempts to pass a guidewire and stent assembly through the mesh failed. Functional success was achieved in all cases of technical success. Stent occlusion occurred in 63.2% of the patients (24/38), with a median patient survival of 300 days. The median stent patency was 198 days. The stent patency rate was 82.9%, 63.1%, and 32.1% at 3, 6, and 12 months postoperatively, respectively. Endoscopic re-intervention was performed in 14 patients, whereas 10 underwent percutaneous drainage. Conclusions: Large cell-type stents for endoscopic bilateral stent-in-stent placement had acceptable functional success and stent patency when technically successful. However, the technical difficulty associated with the entanglement of the second stent delivery sheath in the hook-cross-type vertex of the first stent may preclude large cell-type stents from being considered as a dedicated standard tool for stent-in-stent placement.

몇가지 간담도질환에서 간기능에 따른 Tc-99m EHIDA의 담도영상능 (Tc-99m EHIDA Scintigraphic Demonstrability of Biliary Elements and Liver Function Tests in Hepatobiliary Diseases)

  • In the present communication, the results will be reported on a clinical study of how well scintigraphic visualization of the hepatobiliary elements and several commonly used clinical liver function tests correlate each other in various diseases oft hepatobiliary system. The demonstrability of the biliary tract, gallbladder (GB) and duodenum was rather closely paralleled to serum bilirubin level and less closely to alkaline phosphatase and rather poorly to SGOT and SGPT. The biliary tree could not be visualized scintigraphically when bilirubin exceeded 10.5mg/dl. The usefulness of Tc-99m EHIDA [N-(2,6-diethylacetanilido) iminodiacetic acid, made by Amersham, England] hepatobiliary scintigraphy (Tc EHIDA HBS) in settling diagnostic controversy and ambiguity raised by oral cholecystography, intravenous cholangiography and ultrasonography in many hepatobiliary diseases is well known. The purpose of this investigation was to semiquantitatively evaluate the scintigraphic demonstrability of the hepatobiliary tract, GB and duodenum following intravenous injection of Tc-99m EHIDA in normal subjects and in patients with a disturbed liver function from various hepatobiliary diseases. The hepatobiliary scintigraphy was performed in 10 normal subjects and 39 patients with various hepatobiliary diseases (Table 1) at the Dept. of Radiology, St. Mary's Hospital Catholic Medical College, Seoul, Korea during 2 years period from September 1979. Scintigraphic examination was started at end of 3 minutes after intravenous injection of Tc-99m EHIDA in the amount of $50{\mu}Ci/kg$ and was continued until after 30 minutes at 5 minutes interval. The imaging was usually terminated when the tracer could be seen in the duodenum. Late scintigrams were obatained at 1 1/2, 3, 4 and 6 hours when reeded. Scintigrams were analyzed in terms of promptness and clarity of visualization of the biliary tree, GB and duodenum and demonstrability of these anatomical landmarks was correlated with the values of liver function tests. The demonstrability of the common hepatic duct, common bile duct, GB and duodenum was closely paralleled to the level of serum bilirubin when it is less than 10.5 mg/dl as shown in figure 1. However when the bilirubin exceeded 10.5 mg/dl the time of visualization between protracted reaching a flat curve or plateau around 10.5 mg/dl. The biliary tract could not be visualized when the bilirubin was higher than 10.5 mg/dl. The correlability between scintigraphic demonstration and serum alkaline phosphatase was less strong and between scintigraphic demonstration and SGOT and SGPT was rather poor. The present clinical study confirmed the usefulness and limitation of Tc-99m EHIDA hepatobiliary scintigraphy in visulizing and diagnosing the biliary system and duodenum when radiogrpahy and ultrasonography failed to provide useful informations. Scintigraphy was very helpful in the diagnosis of neonatal hepatitis, biliary atresia, cholecystitis and extrahepatic biliary obstruction. The hepatobiliary system and duodenum were visualized when serum bilirubin level was less than 10.5 mg/dl, SGOT 135 units, SGPT 114 units and alkaline phosphatase 52.2 KAU.

Feasibility of laparoscopic cholecystectomy for symptomatic gallstone disease with portal cavernoma: Can prior portal vein decompression be avoided?

  • Backgrounds/Aims: Biliary surgery in patients with extrahepatic portal vein obstruction with portal cavernoma (PC) is technically challenging, and associated with the risk of bleeding. Therefore, prior portal vein decompression is usually recommended before definitive biliary surgery. Only a few studies have so far reported the safety of isolated laparoscopic cholecystectomy. We aimed to evaluate our experience of laparoscopic cholecystectomy in patients with PC without prior portal decompression. Methods: Prospectively maintained data for patients with PC who underwent laparoscopic cholecystectomy for symptomatic gallstone disease without portal decompression were analyzed. Clinical features, imaging, intraoperative factors, conversion rate, complications of surgery, and long-term outcomes were assessed. Results: Sixteen patients underwent cholecystectomy without portal decompression from 2012 to 2021, of which interventions 14 were laparoscopic cholecystectomies. One patient required conversion (7.1%) to open surgery. Jaundice was present in 5 patients (35.7%), and underwent endoscopic stone clearance before surgery. Median intraoperative blood loss, operative time, and hospital stay were 100 mL (20-400 mL), 105 min (60-220 min), and 2 days (1-7 days), respectively. Blood transfusion was required in two patients (14.2%). Prior endoscopic or percutaneous intervention was associated with significant blood loss and prolonged intraoperative time. Conclusions: In centers with experience, prior portal decompression can be avoided in patients with PC requiring isolated cholecystectomy to treat gallstones or their complications. Laparoscopic surgery is safe and feasible for these patients, and gives excellent outcomes in the selected group.
