비알콜성 지방간 환자에 대한 베르베린 투여 치료의 임상적 효능 연구: 체계적 문헌 고찰과 메타분석 (Clinical Efficacy of Coptidis Rhizoma for Non-Alcoholic Fatty Liver Disease: A Systematic Review and Meta-Analysis)

  • Objectives: The purpose of this meta-analysis was to evaluate the effects of berberine on non-alcoholic fatty liver disease (NAFLD) through a systematic review and meta-analysis. Methods: We searched seven electronic databases for studies through October 2022. All randomized controlled trials (RTCs) of berberine were included. Data extraction and risk of bias assessments were performed by two independent reviewers. The RevMan 5.4.1 program was used for meta-analysis. Results: A total of 279 relevant studies were identified, and 6 eligible RCTs were included to study the efficacy berberine on NAFLD. The six selected trials are studies on the effect between berberine and conventional treatment combined treatment versus conventional treatment. Liver function tests, blood lipid levels, and blood glucose levels were assessed. Combination treatment with berberine, ursodeoxycholate acid, metformin, and monascus purpureus showed statistically significant improvements in liver function levels, blood lipid levels, and blood glucose levels compared to conventional treatment alone. However, there was no significant efficacy of berberine combination dietary, exercise than control group on NAFLD. The meta-analysis results of examining 4 RCTs comparing the therapeutic efficacy of berberine showed statistically significant improvement in the liver function test, blood lipids levels, blood glucose levels. Conclusions: This study suggests that berberine has positive efficacy on blood lipids, blood glucose liver function, fatty liver condition of NAFLD. However, the level of evidence is low because of small effect size,so further investigation is needed.

Anti-obesity Effect of Berberine in Mice Fed a High Fat Diet

  • We investigated the anti-obesity effect of berberine in mice fed a high fat diet and focused on the analysis of adipogenesis in epdidymal adipose tissue. Male C57BL/6J mice were divided into three groups, which were fed either a normal diet (Nor), a high fat diet (HFD), or a high fat diet plus orally administered berberine (0.2 g /kg body weight) (HFD+B) for 8 weeks. Relative to mice in the HFD group, mice in the HFD+B group showed significant reductions in weight gain and adipose tissue weight. Serum triglyceride levels in mice from the HFD+B group were significantly lower than those of the HFD mice, as were the levels of serum insulin and leptin. An effect of berberine to reduce epididymal adipose mass was revealed by H&E staining. Berberine inhibited the high fat diet-induced increase in levels of the proteins CD36 and CCAAT/enhancer-binding protein $\alpha$ ($C/EBP{\alpha}$) observed in epididymal adipose tissues of mice from the HFD group. These results suggest that berberine has an anti-obesity effect in mice and that the effect is mediated by inhibition of adipogenesis.

Therapeutic Effects of Coptidis Rhizoma and Berberine in Streptozotocin-induced Diabetic Rats

  • Objectives: We performed this study to compare the antidiabetic effects of Coptidis Rhizoma (CR) and its major component berberine with gliclazide. Materials and Methods: Diabetic rats induced by injection of streptozotocin (STZ) 55mg/kg were treated with CR 100, 200, 400mg/kg and berberine 100mg/kg. After rats were treated for 5 days, serum glucose, total cholesterol, triglyceride, BUN, creatinine and antioxidant levels were determined. Results: The cytotoxic effects of CR (0.1, 0.01, and 0.001mg/mL), berberine and gliclazide ($0.1{\mu}M$, $1{\mu}M$, and $10{\mu}M$) were tested in rat insulinoma (RIN) cells induced with 5mM STZ. The levels of fasting blood glucose, total cholesterol, triglyceride, BUN and creatinine of CR and berberine treated groups were reduced as much as that of gliclazide group in comparison to control groups, whereas total antioxidant levels increased. In vitro experiments showed that CR and berberine have a cytoprotective effect on RIN cells.

Effect of Berberine on MUC5AC Mucin Gene Expression and Mucin Production from Human Airway Epithelial Cells

  • We conducted this study to investigate whether berberine signifi cantly affects MUC5AC mucin gene expression and mucin production induced by epidermal growth factor (EGF), phorbol 12-myristate 13-acetate (PMA) or tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) from human airway epithelial cells. Confl uent NCI-H292 cells were pretreated with varying concentrations of berberine for 30 min and then stimulated with EGF, PMA or TNF-${\alpha}$ for 24 h. MUC5AC mucin gene expression and mucin production were measured by reverse transcription-polymerase chain reaction (RT-PCR) and enzyme-linked immunosorbent assay (ELISA), respectively. Berberine was found to inhibit the expression of MUC5AC mucin gene induced by EGF, PMA or TNF-${\alpha}$. Berberine also inhibited the production of MUC5AC mucin protein stimulated by the same inducers. This result suggests that berberine can regulate the expression of mucin gene and production of mucin protein, by directly acting on human airway epithelial cells.

황백의 주요 구성 화합물에 의한 약물대사효소 및 약물수송단백 저해능 평가 (Inhibition of Drug-metabolizing Enzyme and Drug Transporter by Major Components of Phellodendri cortex)

  • 본 연구는 황백에 함유되어 있는 주요 화합물인 berberine, palmatine, limonin 및 rutaecarpine의 CYP2D6 및 p-glycoprotein 활성에 대한 저해정도를 탐색함으로써, 황백을 다른 양약과 병용시 약물상호작용을 유발할 수 있는 가능성을 평가하고자 하였다. 인체 간 마이크로좀 시료에 CYP2D6 동효소의 기질약물인 dextromethorphan과 NADPH 재생성계 및 저해제 ($200{\mu}M$)를 첨가한 후 반응시켜 생성된 대사물을 LC/MS/MS를 이용하여 정량하여 CYP2D6 동효소 활성의 변화를 평가하였다. 또한 약물수송단백인 p-glycoprotein의 활성은 L-MDR1 세포주를 이용한 calcein AM 축적 실험을 통하여 평가하였다. 그 결과 식물 알카로이드인 berberine에서 강력한 CYP2D6 활성 저해능을 관찰하였으며, 저해 효과는 농도 의존적으로 증가하였으며, mechanism-based 저해 기전을 나타내었다. 그러나 limonine과 rutaecarpine은 CYP2D6 저해 활성을 보이지 않았고, p-glycoprotein 기능에 대해서는 평가한 어떤 화합물도 저해 활성을 나타내지 않았다. 황백의 주요 성분인 berberine의 CYP2D6 활성 저해능을 고려할 때, 황백을 CYP2D6 기질약제와 병용시 약물상호작용을 유발할 가능성을 보여준다. 이러한 황백의 CYP2D6를 매개로한 임상적인 약물상호작용 가능성은 임상시험을 통하여 추가적인 검정이 필요할 것으로 사료된다.

황련(Coptis chinensis Franch) 추출물의 항충치효과 (Anticariogenic effects of Coptis chinensis Franch Extract)

  • 황련 추출물의 충치 예방효과 가능성을 확인하고자 S. mutans JC-2에 대하여 황련 추출물 및 그의 주요 활성성분의 항균효과, 산생성 억제효과, GTase 활성 억제효과, 균체부착 억제효과를 조사하였다. 메탄올 추출물은 물추물에 비하여 S. mutans에 대해 강한 항균효과를 나타내었으며, 이들 추출물의 S. mutans에 대한 최소저지농도(MIC)는 각각 $130\;{\mu}g/mL$, $200\;{\mu}g/mL$이었다. 황련의 주요 활성성분인 berberine과 palmatine의 MIC는 각각 $50\;{\mu}g/mL$, $110\;{\mu}g/mL$로 황련 추출물에 비하여 강한 항균활성을 나타내었다. 메탄올 추출물, berberine, palmatine을 $121^{\circ}C$, 15분간 열처리했을 때 항균효과가 전혀 감소하지 않아, 열에 매우 안정함이 확인되었다. S. mutans의 산생성에 대해서도 MIC 농도에서 메탄올 추출물, berberine, palmatine 모두 배양 24시간째에 산생성을 완전하게 억제하였다. GTase 활성 억제효과는 $300\;{\mu}g/mL$ 농도에서 메탄올 추출물, berberine, palmatine이 각각 23.2, 46.1, 17.1%의 억제율을 보였고, 물 추출물은 억제효과가 없었다. 반면에 S. mutans의 부착에 대해서는 생장 부분저해농도에서 물 추출물과 메탄올 추출물, beberine, palmatine의 경우 저해율이 각각 59.2%, 13.3%. 4.2%, 41.7%로써 물 추출물과 palmatine이 높은 저해율을 나타내었다. 이상의 결과로부터 황련 추출물의 충치억제효과는 주요 활성성분인 berberine과 palmatine의 S. mutans에 대한 항균효과, GTase 활성억제와 균체 부착 억제효과 등에 기인한 것이라 여겨지며, 황련 추출물이 충치예방을 위한 기능성 천연소재로서 매우 우수한 것으로 판단되었다.

Neuroprotective Effect of Root Extracts of Berberis Vulgaris (Barberry) on Oxidative Stress on SH-SY5Y Cells

  • Objectives: Oxidative stress plays a key role in chronic and acute brain disorders and neuronal damage associated with Alzheimer disease (AD) and other neurodegeneration symptoms. The neuroprotective effects of berberine and Berberis vulgaris (barberry) root extract against apoptosis induced by hydrogen peroxide (H2O2) in the human SH-SY5Y cell line were studied. Methods: The methanolic extraction of barberry root was performed using a maceration procedure. Oxidative stress was induced in SH-SY5Y cells by H2O2, and an MTT assay was applied to evaluate the neuroprotective effects of berberine and barberry root extract. The cells were pretreated with the half maximal inhibitory concentration (IC50) of each compound (including berberine, barberry root extract, and H2O2), and the anti-apoptotic effects of all components were investigated using RT-PCR. Results: The SH-SY5Y cell viability increased in both groups exposed to 75 and 150 ppm barberry extract compared with that in the H2O2-treated group. The data showed that exposing SH-SY5Y cells to 30 ppm berberine significantly increased the cell viability compared with the H2O2-treated group; treatment with 150 and 300 ppm berberine and H2O2 significantly decreased the SH-SY5Y cell viability and was associated with berberine cytotoxicity. The mRNA levels of Bax decreased significantly under treatment with berberine at 30 ppm compared with the control group. A significant increase in Bcl-2 expression was observed only after treatment with the IC50 of berberine. The expression level of Bcl-2 in cells exposed to both berberine and barberry extracts was also significantly higher than that in cells exposed to H2O2. Conclusion: The outcomes of this study suggest that treatment of SH-SY5Y cells with barberry extract and berberine could suppress apoptosis by regulating the actions of Bcl-2 family members.

  • 형광색소(螢光色素) berberine sulphate의 DNA 및 RNA-결합성질(結合性質)을 이용(利用)한 형광현미경기법(螢光顯微鏡技法)의 마이코플라스마(mycoplasma-like organism ; MLO) 감염진단(感染診斷)에 대(對)한 효용가치(效用價値)를 조사(調査)하였다. 이병(罹病) 대추나무, 오동나무, 뽕나무 및 일일초(日日草)의 조직절편(組織切片)을 berberine sulphate로 염색(染色)하여 형광현미경(螢光顯微鏡)으로 관찰(觀察)하였던 바, 사부조직(篩部組織)에서 MLO-특이형광반응(特異螢光反應)이 나타났으나, 건전조직(健全組織)의 사부(篩部)에서는 특이형광(特異螢光)이 관찰(觀察)되지 않았다. 이와 같은 berberine sulphate의 조직염색법(組織染色法)은 목본(木本) 및 초본식물(草本植物)의 MLO감염(感染)을 정확(正確)하게 진단(診斷)하는데 매우 유용(有用)한 방법(方法)임을 보여 주었으며, 사용(使用)이 간단(簡單)하고 비용(費用)이 저렴(低廉)하므로 대량(大量)의 시료(試料)에 대한 신속(迅速)한 검정(檢定)에 이용(利用)될 가능성(可能性)을 보여 주었다.

Growth Responses of seven Intestinal Bacteria Against Phellodendron amurense Root-Derived Materials

  • The growth responses of Phellodendron amurense root-derived materials against seven intestinal bacteria were examined, using an impregnated paper disk agar diffusion method and spectrometric method under $O_2$-free condition. The biologically active constituent of the P. amurense root extract was characterized as berberine chloride ($C_{20}H_{18}NO_{41}Cl$) using various spectroscopic analyses. The growth responses varied depending on the bacterial strain, chemicals, and dose tested. At 1 mg/disk, berberine chloride strongly inhibited the growth of Clostridium perfringens, and moderately inhibited the growth of Escherichia coli and Streptococcus mutans without any adverse effects on the growth of three lactic acid-bacteria (Bifidobacterium bifidum, B. longum, and Lactobacillus acidophilus). The structure-activity relationship revealed that berberine chloride exhibited more growth-inhibiting activity against C. perfringens, E. coli, and S. mutans than berberine iodide and berberine sulfate. These results, therefore, indicate that the growth-inhibiting activity of the three berberines was much more pronounced as chloridated analogue than iodided and sulphated analogues. As for the morphological effect caused by 1 mg/disk of berberine chloride, most strains of C. perfringens were damaged and killed, indicating that berberine chloride showed a strong inhibition against C. perfringens. As naturally occurring growth-inhibiting agents, the P. amurense root-derived materials described could be useful as a preventive agent against diseases caused by harmful intestinal bacteria such as clostridia.

황련해독탕(黃連解毒湯)과 그 주성분인 황련(黃連) 및 황련(黃連)의 주성분인 berberine이 니코틴 중독 흰쥐에 미치는 영향 (The Effect of Hwangryunhaedok-tang and its Major Component Coptidis Rhizoma, Berberine on Nicotine Addiction in Rats)

  • Objectives: The purpose of this study was to investigate the effect of Hwangryunhaedok-tang, its major component Coptidis Rhizoma, and the major component of Coptidis Rhizoma, berberine, on the expression of behavioral sensitization induced by the repeated administration of nicotine. Methods: Experimental ratswere divided into 3 groups and each group treated with saline, nicotine, and Hwangryunhaedok-tang. Experiments were performed on 4 further groups, each group treated with saline, nicotine, Coptidis Rhizoma, and berberine. To obtain rats addicted todrugs, nicotine was injected repeatedly. After 3 days of withdrawal phase, on the 11th day, rats were re-exposed to nicotine and thus able to induce the expression of the activity sensitization phenomenon successfully. To examine the change of c-Fos in the brain of experimental rats with the nicotine-induced activity sensitization, their neurological activation rate was assessed by c-Fos staining with Fos-like immunihistochemistry of the dopamine releasing area, the striatum and the nucleus accumbens. Results: The result demonstrate that Hwangryunhaedok-tang, its major component Coptidis Rhizoma and berberine,the major component of Coptidis Rhizoma, significantly reduced the walking activity rate in the drug addict models induced by nicotine administration, and Coptidis Rhizoma and its major component berberine reduce the marker of the neurological activation c-Fos, and thus revealed an effect of suppressing drug addiction. Conclusions: These results suggest the possibility of Hwangryunhaedok-tang, its major component Coptidis Rhizoma and the major component of Coptidis Rhizoma, berberine,as therapeutic for nicotine addiction.
