양성전립선비대증 증상 조절을 위해 투여되는 알파차단제의 골절위헙 (The Risk of Fracture with Taking Alpha Blockers for Treating Benign Prostatic Hyperplasia)

  • Objectives : We evaluated the risk of fracture associated with hypotension-related adverse drug reaction caused by taking alpha blockers to treat benign prostatic hyperplasia (BPH). Methods : We used the Health Insurance Review and Assessment Service database from January 1st 2005 to June 30th 2006 for this study. The male patients with BPH and who had a prescription for alpha blockers following any fractures were defined as the cases. We set the 20 day long hazard period prior to the index date and the four control periods whose lengths were same with hazard period. After 1:4 matching of the hazard and control periods, conditional logistic regression was used to calculate the odds ratios for the risk of fractures as related to the alpha blocker exposure. Results : Doxazosin and tamsulosin showed the increased risk of fractures, whereas terazosin did not. After stratification using the defined daily doses, a protective effect was shown for the patients who took terazosin at the doses lower than 0.4 DDD and the hazardous effect at the doses higher than or equal to 0.4 DDD. There was no significant difference for the risk of patients taking tamsulosin at the doses higher than 1.0 DDD but there was a statistically significant increase in the risk at the doses higher than or equal to 1.0 DDD. Conclusions : Alpha blockers for BPH may increase the risk of fracture in elderly patients who have comorbidities and take the concomitant medications. Alpha blockers need to be prescribed with caution, although some have high prostate specificity.

지역사회 성인남성에서의 하부요로증상 및 전립선비대증 의료이용과 사회경제적 요인의 관련성 (Relationship of Socioeconomic Factors with Medical Utilization for Lower Urinary Tract Symptoms and Benign Prostatic Hyperplasia in a South Korean Community)

  • Objectives : We wanted to evaluate the medical underutilization for benign prostatic hyperplasia (BPH) and lower urinary tract symptoms (LUTS) among Korean elderly men and we wanted to determine their associated factors. Methods : This study was conducted on 239 men with LUTS and 116 men with BPH who were compatible with the diagnostic criteria from a total of 641 participants. These participants were over 50 years old and they were randomly chosen in a community-based study for estimating the prevalence of BPH. Using a self-reported questionnaire, we surveyed the sociodemographics, health status, quality of life, lower urinary tract symptoms, medical utilization and reasons for not seeking treatment. Results : Only 27.6% of the men with LUTS and 31.0% of the men with BPH reported having visited a doctor for urinary symptoms. The reasons for not visiting a doctor were, in order of responses from the group with LUTS: 'considered the symptoms as a part of the normal ageing process', 'not enough time to visit a doctor', 'financial difficulty' and 'the symptoms were not severe or bothersome'. Regarding BPH, the responses were the same as those of the group with LUTS however, 'financial difficulty' placed second. Among the men with experience of visiting a doctor for urinary symptoms, 33.3% of those with LUTS and 28.1% of those with BPH were not treated. The most common reason in both groups was 'the symptoms were not severe to be treated'. On a multiple logistic regression analysis, the larger size household (odds ratio (OR) 3.03, 95% confidence interval (CI)=1.40-6.54) and an unsatisfactory quality of life related with urinary symptoms (OR 2.98, 95% CI=1.23-7.21) were associated with medical utilization in the group of LUTS. For BPH, the current employment status was related with the medical utilization (OR 2.80, 95% CI=1.10-7.11), in addition to the larger size household (OR 3.24, 95% CI=1.14-9.21). Conclusions : Many men with urinary symptoms do not visit a doctor. This medical underutilization for people with LUTS and BPH may be associated with economic status in Korea.

전립선 비대증에 대한 Houttuynia cordata Thunberg 에탄올 추출물의 최적용량 결정 (Determination of Optimal Dosage of Ethanol Extract of Houttuynia cordata Thunberg Against Benign Prostatic Hyperplasia)

  • Houttuynia cordata Thunberg는 전통의학에서 다양한 약리학적 작용에 대해 연구되어 왔다. 본 연구에서, 거세 및 테스토스테론 프로피오네이트(TP) 주입에 의해 유도된 양성 전립선 비대증(BPH) 모델에 대한 Houttuynia cordata 에탄올 추출물(HCE)의 영향을 조사했다. 총 30마리의 쥐를 6개의 그룹으로 나누었다. 한 그룹은 정상 대조군으로 사용되었고 다른 그룹은 거세 후 14일 동안 복강내 TP를 주입하여 BPH를 유도하였다. 양성 대조군은 BPH 모델에 매일 피나스테라이드(5 mg/kg)를 투여했다. 피나스테라이드 대신 HCE (0.5, 1, 2mg/kg)를 투여한 군은 실험 군으로서 대조군과 비교하였다. 체중 100 g 기준으로 전립선 무게에 대한 통계적 유의성은 없었다. ELISA 검사로 5-α 환원효소 및 디하이드록시 테스토스테론(DHT) 농도를 측정하였으며, 모든 실험군에서 유의미한 감소가 있었다. 5-α 환원효소의 경우 HCE (0.5 mg/kg) 군이 가장 낮았고, DHT는 HCE (2 mg/kg) 군에서 가장 낮은 수치를 보였다. 전립선에 대한 조직병리학적 관찰에서 대조군과 HCE (2 mg/kg) 투여 군은 정상 세포 형태를 보였고 세포팽창이 없었다. 그러나 음성 대조군과 HCE (1 mg/kg) 군에서는 세포가 부풀어 오르고 세포 사이의 간격이 좁아졌다. 특히 HCE (0.5 mg/kg) 군에서는 일부 세포가 터지는 현상이 나타났다. 따라서 2 mg/kg 이상의 HCE를 투여하는 것이 BPH의 보호 효과에 적합할 것이다.

  • Objectives: Benign prostatic hyperplasia (BPH) is a progressive pathological condition characterized by excessive proliferation of the prostate. In this study, we evaluated the effect of Corni Fructus water extract (CF) on the promotion of prostate cell proliferation by dihydrotestosterone (DHT). Methods: The effect of CF on the proliferation of LNCaP prostate cells was evaluated, and DHT was treated to induce an in vitro BPH model. To study the mechanism of inhibition of cell proliferation and BPH by CF, changes in the expression of key factors related to cell cycle and BPH were investigated. We further investigated the effect on the production of reactive oxygen species (ROS) and nitric oxide (NO) to evaluate the antioxidant and anti-inflammatory efficacy of CF. Results: Inhibition of LNCaP cell proliferation by CF was associated with decreased expression of cyclin D1 and cyclin A and increased expression of cyclin-dependent kinase inhibitor p21. CF also suppressed expression of BPH inducing factors such as 5α-reductase type 2 and androgen receptor (AR) as well as prostate specific antigen (PSA). Furthermore, CF significantly blocked DHT-induced LNCaP cell proliferation and effectively attenuated DHT-induced expression of BPH mediators and cyclins. In addition, CF inhibited DHT-induced oxidative and inflammatory reactions by inhibiting production of ROS and NO. Conclusion: Our results demonstrated that CF probably acted as 5α-reductase type 2 inhibitor, preventing the 5α-reductase type 2-AR signaling pathway, thereby reducing the conversion of testosterone to DHT and the expression of PSA, which is at least correlated with the antioxidant and anti-inflammatory activities of CF.

피나스테라이드의 특성에 대한 이론적 연구 (Theoretical Study for the Characteristics of Finasteride)

  • Finasteride, ($5{\alpha}$, $17{\beta}$)-N-(1,1-dimethylethyl)-3-oxo-4-azaandrost-1-ene-17-carboxamide), is a 5a-reductase enzyme inhibitor. This enzyme converts testosterone to the more potent androgen, a-dihydrotestosterone. This molecules a logical medical treatment for benign prostatic hyperplasia (BPH), as it induced a reduction in serum dihydrotestosterone and prostatic specific antigen levels with a concomittant increase in blood testosterone concentration . Despite its widespread use, little has been published concerning its molecular properties. Therefore, in this study, in order to explain characteristics of finasteride, total energy, net charge, vibrational mode of melatonin are calculated by PM3 methods of HyperCam 8.0.

Prostate spindle cell neoplasm associated with early voiding difficulty after transurethral resection of the prostate

  • This report presents the case of 75-year-old men with spindle cell neoplasm. The patient underwent percutaneous nephrolithotomy and transurethral resection of the prostate (TURP) for renal stones and benign prostatic hyperplasia. One month postoperatively, the patient was able to void without any difficulty. Five months later, the patient experienced difficulty voiding and presented to the emergency room with severe pelvic pain. Computed tomography (CT) showed regrowth of the prostate mass into the posterior bladder and penile root. The prostate-specific antigen level remained constant at 1.14 ng/mL during the pre-and postoperative periods. Five months before the TURP operation, the patient's CT scan showed a soft and mildly enlarged prostate with no protrusion into the bladder. Biopsy of the prostate, however, showed a protruding mass, indicative of a spindle cell neoplasm. The patient was subsequently treated with the chemotherapeutic drug adriamycin. Unfortunately, treatment was unsuccessful, and the patient died 18 months later.

야콘을 포함한 수종 천연물의 남성호르몬 대사 억제효과 및 전립선비대 증상 개선효과 (Inhibition Effect of Testosterone Metabolism of Some Natural Products Containing Yacon and their Ameliorative Effect of Benign Prostatic Hyperplasia Symptom)

  • We reported previously that the extracts of Yacon tuber and leaf have spermatogenic activity and testosterone level increasing effect in men. The aim of this study was to investigate the mechanism of testosterone level increasing effect of Yacon tuber and leaf extracts. In this study, we found that Yacon leaf extracts and chlorogenic acid, a constituent of Yacon extracts showed significantly inhibition effect of testosterone degradation in rat liver homogenate. We suggest that this could be a part of direct mechanism about increasing effect of testosterone level. Futhermore, we evaluated the inhibition effect of testosterone metabolism of Torilis japonica and Coix lacryma-jobi have been reported as $5{\alpha}$-reductase inhibitor. The inhibition ratio of testosterone metabolism of these were Torilis japonica>Coix lacryma-jobi>Yacon. We also evaluated the benign prostatic hyperplasia (BPH) score with the preparation composed of these natural product, Yacon pill. It showed the decreased the BPH score in men. Rats with BPH showed significantly increased dihydrotestosterone (DHT) levels in the serum; however, Yacon pill extracts treated rats showed significant reduction of DHT levels compared with the BPH group. Conclusively, Yacon pill showed the possibility as ameliorable agents of BPH symptoms.

수압 측정에 기반하는 요류검사의 정확도 검증 (Accuracy Validation of Urinary Flowmetry Technique Based on Pressure Measurement)

  • Uroflowmetry is a non-invasive clinical test useful for screening benign prostatic hyperplasia(BPH) common in the aged men. The current standard way to obtain the urinary flow rate is to continuously acquire the urine weight signal proportional to volume over time. The present study proposed an alternative technique measuring pressure to overcome noise problems present in the standard weight measuring technique. Experiments were performed to simultaneously acquire both weight and pressure changes during urination of 9 normal men. Noise components were separated from volume signals converted from both weight and pressure signals based on the polynomial signal model. Signal-to-noise ratio was defined as the ratio of the energies between signal and noise components of the measured volume changes, which was 8.5 times larger in the pressure measuring technique, implying that cleaner signal could be obtained, more immune to noisy environments. When four important diagnostic parameters were estimated, excellent correlation coefficients higher than 0.99 were resulted with mean relative errors less than 5%. Therefore, the present pressure measurement seemed valid as an alternative technique for uroflowmetry.

Effects of red ginseng oil(KGC11ℴ) on testosterone-propionate-induced benign prostatic hyperplasia

  • Background: Benign prostatic hyperplasia (BPH) is a disease characterized by abnormal proliferation of the prostate, which occurs frequently in middle-aged men. In this study, we report the effect of red ginseng oil (KGC11o) on BPH. Methods: The BPH-induced Sprague-Dawley rats were divided into seven groups: control, BPH, KGC11o 25, 50, 100, 200, and finasteride groups. KGC11o and finasteride were administered for 8 weeks. The BPH biomarkers, DHT, 5AR1, and 5AR2, androgen receptor, prostate-specific antigen (PSA), Bax, Bcl-2, and TGF-β were determined in the serum and prostate tissue. The cell viability after KGC11o treatment was determined using BPH-1 cells, and, androgen receptor, Bax, Bcl-2, and TGF-β were confirmed by western blotting. Results: In the in vivo study, administration of KGC11o reduced prostate weight by 18%, suppressed DHT (up to 22%) and 5AR2 (up to 12%) levels from administration of 100 mg/kg KGC11o (P < 0.05). PSA was significantly downregulated dose-dependently from at the concentration of 50 mg/kg KGC11o (P < 0.05). BPH-1 cell viability significantly reduced through the treatment with KGC11o. In vitro and vivo, AR, Bcl-2 TGF-β levels reduced significantly but Bax was increased (P < 0.05). Conclusion: These results suggest that KGC11o may inhibit the development of BPH by significantly reducing the levels of BPH biomarkers via 5ARI, anti-androgenic effect, and anti-proliferation effect, serving as a potential functional food for treating BPH.

Anti-inflammatory Effects in LPS-treated RAW 264.7 Cells and the Influences on Drug Metabolizing Enzyme Activities by the Traditional Herbal Formulas, Yongdamsagan-Tang and Paljung-san

  • Objectives: Yongdamsagan-tang (YST) and Paljung-san (PJS) in traditional medicine and finasteride in modern medicine are used to treat benign prostatic hyperplasia (BPH). In recent, the use of combination herbal remedies with conventional drugs has been increasing. Therefore, we investigated the anti-inflammatory effects of these drugs to treat BPH and the influence of herbal formulas on finasteride metabolism. Methods: The inhibitory effects of the herbal formulas and finasteride on the production of inflammatory mediators and cytokines were determined in lipopolysaccharide (LPS)-treated RAW 264.7 cells. Additionally, the influence of herbal formulas on activities of human drug metabolizing enzymes (DMEs) was assessed using human microsomal enzymes. Results: We observed that YST, PJS and finasteride inhibited the production of nitric oxide (NO), prostaglandin E2 (PGE2) and interleukin-6 (IL-6) in RAW 264.7 cells. The half maximal inhibitory concentration (IC50) of YST on PGE2 production was calculated to be below 25 ㎍/mL. YST inhibited the activity of uridine diphosphate-glucuronosyltransterase (UGT) 1A4 with an IC50 value of 49.35 ㎍/mL. The activities of cytochrome P450 (CYP) 1A2, CYP2B6, CYP2C19, CYP3A4, and UGT1A1 were inhibited by PJS (IC50 < 100 ㎍/mL, each). Although PJS and YST inhibited the activities of CYP3A4 and UGT1A4, respectively, these formulas may not influence the metabolism of finasteride because the IC50 values of herbal formulas on DMEs are too high to affect metabolism. Conclusions: Our results suggest that the combination of finasteride and YST or PJS might not influence their drug metabolism and that the drugs may have synergistic effects against BPH.
