Vocal sulcus accompanying with benign vocal cord disease (양성 성대질환을 동반하는 성대구 (Vocal Sulcus))

  • A retrospective review of 158 patients who had microlaryngeal surgery for benign vocal cord disease, at the Department of Otolaryngology, Kang-Nam St. Mary Hospital, over 2-year period from Mar. 1991 to Feb. 1993, was completed. Of 158 patients, 16 patients(10.1%, male 8 & female 8, most common in fifth decade) had concomitant vocal sulcus unilaterally (10) or bilaterally (6). Vocal sulcus was accompanied with 7.5% (7/93) of vocal polyp, 15.8% (6/38) of vocal nodule, 11.1% (1/9) of Reinke's edema and 8.3% (1/12) of vocal cyst. Vocal sulcus located above the associated disease in 13(59.1%), below the disease in 7(31.8%) and in the lesion in 2(9.0%). Vocal sulcus was found mainly on the midportion of the membranous vocal cord and the length was less than half of membranous vocal cord in 16(72.7%), more than half in 6(27.3%). In 12 cases (54.5%), vocal sulcus was deep enough to attach to the vocal ligament. We thought it is important to consider the concomitant vocal sulcus during the microlaryngeal surgery for benign vocal cord disease.

A Study for Acoustic Features of Benign Laryngeal Disease (양성 성대 점막 질환의 음향학적 특성에 관한 연구)

  • Background and Objectives:The purpose of this study is to find features in acoustics and to learn useful features of parameters in order to distinguish laryngeal diseases through many acoustic variables. Materials and Methods:The subjects of this study were 125-male patients who had been diagnosed with vocal nodule, vocal polyp, vocal cyst, Reinke's edema, leukoplakia. To research the features of each disease in acoustics, they are measured 34 parameters by using MDVP. Results:It is clear that in order to see a meaning result when distinguishing laryngeal diseases, $F_0$, $MF_0$, $T_0$, Fhi, Flo, PER variables are significant (p<.05). It means that variables related to fundamental frequency are important to anticipate which group will be diagnosed with Reinke's edema and leukoplakia. vAm had an effect on getting a significant result in terms of amplitude perturbation parameters, which is useful to distinguish between laryngeal polyp/cyst and other laryngeal disease (p<.05). ATRI made a significant result in related to tremor parameters, which is useful to distinguish between laryngeal polyp and other laryngeal disease (p<.05). Conclusion:$F_0$, $MF_0$, $T_0$, Fhi, Flo, PER, vAm, ATRI might be meaningful parameters distinguishing pathologic from benign laryngeal diseases. Especially, the vAm and ATRI are an important factor when forecasting which group would be diagnosed with vocal polyp.

Requests for Tumor Marker Tests in Turkey Without Indications and Frequency of Elevation in Benign Conditions

  • Aims: To investigate the incidence of ordering tests for tumor markers which are used in cancer diagnosis, follow-up treatment and detection of recurrence, the rate of elevation in benign diseases and which clinics order them frequently. Materials and Method: Data for the tumor markers carbohydrate antigen 19-9 (CA 19-9), carcinoembryonic antigen (CEA), cancer antigen 125 (CA 125), cancer antigen 15-3 (CA 15-3) and alpha-fetoprotein (AFP) that were ordered by all the clinics in our Hospital between 2010 and 2011 were screened. When excluding repeated orders the results of 3,416 patients were available. It has been determined that in which benign diseases were the tumor markers frequently ordered and which of these conditions had high levels of them. Results: CA 19-9 was ordered for 1,858 patients 191 (10.3%) were malignant while 1667 (89.7%) were ordered in benign diseases. For CEA the total was 1,710, 226 (13.2%) malignant and 1484 (86.8%) benign, and for CA 125 1267, 111 (8.8%) malignant and 1156 (91.2%) benign. AFP was ordered for 1687 cases, 80 (4.7%) malignant but 1607 (95.3%) benign. CA 15-3 was ordered 1449 times, 174 (12%) for malignant and 1275 (88%) for benign diseases. In all cases, considerable proportions were positive. Conclusions: It was shown that clinicians frequently order tumor markers for benign conditions. The findings of this study has shown that tumor markers are used widely without indications as cancer screening tests.

  • Benign symmetric lipomatosis was initially described in 1846 by Sir Benjamin Brodie. In 1888, Otto Madelung presented 33 cases of benign symmetric lipomatosis and described the classic“horse collar”cervical distribution of the lipomatous tissue. Launois and Bensaude described benign symmetric lipomatosis as a distint syndrome characterized by a diffuse, symmetric, fatty accumulation in the cervical region. This disease is rare condition affecting mostly middle aged alcoholic men and associated with many systemic diseases such as diabetes mellitus, hyperuricemia, renal tubular acidosis, liver enzyme abnormality etc. The condition does not spontaneously involute and surgical excision is the only proven method of treatment, and recurrence is frequent. We experienced six patients of benign symmetric lipomatosis who underwent surgical excision via collar incision which afford wide exposure of the entire cervical area. We report them with the review of literature.

  • Angiolymphoid hyperplasia with eosinophilia(ALHE) is an uncommon, benign vascular proliferation with the background of a stroma which is heavily infiltrated by lymphocytes and eosinophils and includes lymphatic follicles with prominent germinal centers. ALHE primarily involves the skin and subcutis in head and neck as various forms of nodules. There has been considerable controversy about the relationship between ALHE and Kimura's disease. Kimura's disease, originally reported by Kimura et el., is an unusual granulation with proliferation of lymphoid tissue. Wells and Whimster published the first report describing a condition that resembled Kimura's disease and designated it as ALHE. For a time being two lesions are thought to be same lesion, but recently they are considered as two different entities, histopathologically. The cause of this disease remains unknown, and physicians have used a variety of treatment modalities including cryosurgery, steriod therapy, electrodesiccation, curettage, radiotherapy, laser therapy and surgical excision. But any treatment modality leaves problem of recurrence because the lesion is not well encapsulated. Being poorly encapsulated, the lesion's remnants are apt to be left and this markes some problems : recurrence and possible adjacent organ injury. In this case we misdiagnosed the lesion as well encapsulated benign mass. We performed excisional biopsy and experienced prolonged operation time and unwanted mentalis muscle injury. We think that the importance of poor encapsulation of ALHE should be stressed. So we report our experience with literature review.

Differential diagnosis of peripheral vertigo (말초성 현기증의 감별진단)

  • Dizziness can be classified mainly into 4 types: vertigo, disequilibrium, presyncope, and lightheadedness. Among these types, vertigo is a sensation of movement or motion due to various causes. The main causes of peripheral vertigo are benign paroxysmal positional vertigo (BPPV), acute vestibular neuritis (AVN), and Meniere's disease. BPPV is one of the most common causes of peripheral vertigo. It is characterized by brief episodes of mild to intense vertigo, which are triggered by specific changes in the position of the head. BPPV is diagnosed from the characteristic symptoms and by observing the nystagmus such as in the Dix-Hallpike test. BPPV is treated with several canalith repositioning procedures. AVN is the second most common cause of peripheral vertigo. Its key symptom is the acute onset of sustained rotatory vertigo without hearing loss. It is treated with symptomatic therapy with antihistamines, anticholinergic agents, anti-dopaminergic agents, and gamma-aminobutyric acid-enhancing agents that are used for symptoms of acute vertigo. Meniere's disease is characterized by episodic vertigo, fluctuating hearing loss, and tinnitus. It is traditionally relieved with life-style modification, a low-salt diet, and prescription of diuretics. However, diagnosis and treatment of the peripheral vertigo can be difficult without knowledge of BPPV, AVN, and Meniere's disease. This article provides information on the differential diagnosis of peripheral vertigo in BPPV, AVN, and Meniere's disease.

  • Madelung's disease, or benign symmetric lipomatosis, is an uncommon disorder that is characterized by massive symmetrical deposits of adipose tissue in the upper trunk, neck and head, and is usually associated with alcohol abuse; as such, patients usually complain of cosmetic issues. Historically, Madelung's disease is usually encountered in men between 30 and 60 years of age, and is more prevalent in the Mediterranean population. In this case study, we describe a rare case of Madelung's disease, in an Asian patient who presented with symmetrically located bilateral masses in the anterior neck, which grew progressively larger over a period of seven years.

  • Benign symmetric lipomatosis is a rare disease. Its main characteristic is the symmetric massive deposits of adipose tissue, in unencapsulated form, predominatly on the neck, shoulder, back and upper exterimities. We report a case of a 61 year old man with benign symmetric lipomatosis. He had a history of alcoholic liver cirrhosis. Due to cosmetic defomity, he wanted surgical treatment . We did a cervical lipectomy. He was satisfied with the result, and there was no evidence of recurrence.

Comparison of $Na^+/I^-$ Symporter Expression Rate in Malignant and Benign Thyroid Diseases: Immunohistochemical Study (악성 및 양성 갑상선 질환의 조직에서 면역조직학적 검사법에 의한 $Na^+/I^-$ symporter의 발현율 비교)

  • Purpose: Previous studies have not showed consistent results for the level of expression of sodium/iodide symporter (NIS) in thyroid diseases, especially malignant tumor. We undertook this study to evaluate the distribution of NIS expression in malignant thyroid diseases and compare with that in benign thyroid disease. Materials and Methods: Total patients were 119 cases (Men 15, $48{\pm}13$ yrs). Total number of samples were 205 pieces. In malignant thyroid disease, there were 153 samples: 90 in papillary carcinoma, 4 in follicular carcinoma, 2 in medullary carcinoma and 57 in metastatic lymph node. In benign thyroid disease, there were 52 samples: 36 in goiter/cyst, 11 in thyroiditis and 5 in follicular adenoma. Using immunohistochemical methods, we probed 205 samples with monoclonal anti-NIS Ab. Grading of staining was stored as 0 (negative or absent), 1 (weakly positive), 2 (moderately positive) or 3 (strongly positive). Expression rate (ER) of NIS positivity in individual disease entity was expressed as percentage of total number divided by number in 2 plus 3 grade. Results: ERs of malignant thyroid diseases were 63% in papillary carcinoma, 81% in metastatic lymph node, 71% in follicular carcinoma and 100% in medullary carcinoma. ERs of benign thyroid disease were 53% in goiter/cyst, 64% in thyroiditis and 40% in follicular adenoma. ER of malignant thyroid diseases was higher than benign thyroid diseases (71% vs 54%). Grading of NIS expression in papillary carcinoma or goiter/cyst was heterogeneously distributed in considerable cases. Normal tissue also showed heterogeneous distribution of NIS expression, which was not correlated with that of primary lesion. Conclusion: In papillary thyroid carcinoma, distribution of NIS expression was heterogeneous and increased, and not different compared with that of benign thyroid disease.

The Clinical Implication of MAGE Gene Detection in Bronchial Washing Fluid in Routine Practice (기관지 세척액 검사를 이용한 MAGE 유전자 검사의 임상적 의의)

  • Background: Melanoma antigen genes (MAGE) are expressed in many human malignant cells and are silent in normal tissues other than in testis and in placenta. But MAGE expression in benign lung diseases, such as pulmonary tuberculosis or cases with severe inflammation, needs further evaluation to overcome false-positive findings. We evaluated detection rates of the melanoma antigen genes (MAGE) RT-nested PCR in bronchoscopic washing samples from patients with benign lung disease, as well as in patients with malignancies. Methods: Bronchial washing fluid from 122 patients was used for cytological examination and MAGE gene detection using RT-nested-PCR of common A1-6 mRNA. We compared the results from the RT-nested PCR and the pathologic or bacteriologic diagnosis. We also analyzed the expression rate and false positive rate of MAGE gene. Results: Among 122 subjects, lung cancer was diagnosed in 23 patients and benign lung disease was diagnosed in 99 patients. In patients with lung cancer, the positive rate of MAGE expression was 47.8% (11/23) and in benign lung disease group, the expression rate was 14.1% (14/99). Among benign lung disease group, the expression rate of MAGE gene (25.0%) in patients with pulmonary tuberculosis (11/44) was especially high. Conclusion: MAGE A1-6 RT-nested PCR of bronchial washing fluid can be used as a complementary method in lung cancer, but that test results in a high false positive rate in tuberculosis patients.
