A Case Report of Avascular Necrosis of the Femoral Head (대퇴골두 무혈성 괴사로 진단받은 고관절 통증 1례에 대한 증례보고)

  • Objective : The etiology and treatment of the avascular necrosis of the femoral head continiue to be controversial and the pathogenesis remains obscure. And its clinical study and report about oriental treatment are absolutely insufficient. Therefore, we report a case of avascular necrosis of the femoral head. Methods : The patient was treated with Bee-Venom therapy, Electro Acupuncture and Sopungwhalhyul-tang(疎風活血湯). Results : After 26days of Oriental treatments, there was remarkable improvement in condition of the patient. Conclusions : In this case, oriental treatments for avascular necrosis of the femoral head was effective. Futher studies are required to prove effectiveness of oriental treatments for treating avascular necrosis of the femoral head.

  • Objectives : The Purpose of this study is to investigate the clinical application of conservative treatment for femoral neck stress fracture developed in avascular necrosis of the femoral head. Methods : Patient is hospitalized at Dept. of Oriental Rehabilitation Medicine, Bu-Chun Jaseng Oriental Medicine Hospital, diagnosed as femoral neck stress fracture developed in avascular necrosis of the femoral head and treated by herbal medicine, acupuncture and moxibustion. This study was measured by Visual Analogue Scale(VAS) score, range of motion and walking time. Results : After conservative treatment, the patient's pain was controlled and VAS score was decreased. Range of motion and walking time were improved. Conclusions : As seen in this one case, Oriental conservative treatment has a positive effect to control pain with femoral neck stress fracture developed in avascular necrosis of the femoral head.

Superior Gluteal Artery-pedicled Iliac Crest for the Treatment of Avascular Necrosis of Femoral Head (상둔 동맥 혈관경 후방 장골릉 골 이식을 이용한 대퇴골 두 무혈성 괴사의 치료)

  • Introduction: To evaluate the efficacy of superior gluteal artery-pedicled iliac crest for the treatment of avascular necrosis of femoral head. Material & Method: From January 2001 to October 2001, we used the superior deep branches of superior gluteal artery for the pedicled posterior iliac crest bone graft to revascularize the avascular femoral head in 4 patients. They were 1 man and 3 women, and the mean age of the patients was 34 years (range, 27 to 60). The average follow-up after surgery was over 57 months (range, 15 to 82). We analyzed the clinical results by the Harris hip score, and evaluated the vascularity of the femoral head by radiographic methods. Results: All cases showed no evidence of collapse on femoral heads and good revascularizations on the radiographic images. The average Harris hip score was 88.5 points. There was no complication. Conclusion: The revascularization procedure using the superior gluteal artery-pedicled posterior iliac crest was thought to be one of the effective and promising techniques for the treatment of the avascular necrosis of femoral head.

  • In this paper, we propose effecient methods to extract the femoral head region in MR images. The femoral head area in MRI is approximated using Hough transform and the anatomical features of the femoral heads. Then, modified region growing method is applied to extract the femoral head region. We measured the parameters for the diagnosis of the avascular necrosis of the femoral heads from the segmented femoral head region. The proposed methods are proved very effective to extract the femoral head of healthy volunteer and of the patient having heavy avascular necrosis. The measured parameters can be used very efficiently for the quantitative analysis and the diagnosis of the avascular necrosis of the femoral heads.

  • Avascular necrosis of the femoral head is a debilitating disease that leads to destruction of tho hip joint in patients who are in the third, fourth, or fifth decade of life. The total societal cost is so high because these patients are generally young, hold jobs, and have a considerable life expectancy. Therefore, we have to delay or eliminate the need for total hip replacement as possible as we can. The purpose of this review is to provide a current perspective of avascular necrosis of the femoral head.

  • Non traumatic aseptic necrosis of the head of the femur is well documented with respect to its clinical roentgenographic and pathological aspect, but not with respect to etiology. A number of reports have associated various diseases and conditions with necrosis of the femoral head. Prominent among these are alcoholism, steroid therapy and hemostatic disorder. The increased incidence of avascular necrosis of bone associated with long-term corticosteroid therapy has aroused much interest. The pathogenesis of cortisone-induced avascular necrosis is as yet unproved. But cortisone-induced fatty liver with hyperlipidemia, leading to systemic embolization of fat and avascular necrosis of bone has much experimental support. We present a case of bilateral avascular necrosis of the femur head following several epidural steroid injections in a 30 year old male patient who had low back pain with radiation to both legs for 1 year and who had Cushinoid appearance for several months. One week later, he had a total replacement of both hip operation.

  • Urgent reduction is required in cases of traumatic hip dislocation to reduce the risk of avascular necrosis of the femoral head. However, in cases of femoral head fractures, the dislocated hip cannot be reduced easily, and in some cases, it can even be irreducible. This irreducibility may provoke further incidental iatrogenic fractures of the femoral neck. In an irreducible hip dislocation, without further attempting for closed reduction, an immediate open reduction is recommended. This can prevent iatrogenic femoral neck fracture or avascular necrosis of the femoral head, and save the natural hip joint.

  • Objectives This study was conducted to investigate a randomized controlled trial study on the clinical treatment of herbal medicines for avascular necrosis of the femoral head. Methods By March 2022, the China National Knowledge Infrastructure (CNKI) was reviewed about randomized controlled trial studies on the herbal treatment of avascular necrosis of the femoral head through. The publication year, evaluation criteria, treatment period, treatment method and effectiveness were analyzed for articles published from 2017 to 2022. Results 16 articles were selected and 14 kinds of herbal medicines were used. In most studies, Western medical treatments such as injection therapy, oral drug administration, traction therapy and mid-frequency electric therapy were treated alone in the control group, and a combination of Western medical treatment and herbal treatment was administered to the experimental group. Conclusions In China, there have been many studies on herbal treatment for avascular necrosis of the femoral head. In Korea, more clinical studies on the herbal treatment of avascular necrosis of the femoral head are needed. This study will be helpful for future research in Korean medicine on avascular necrosis of the femoral head.

A Review of Rat Models of Avascular Necrosis of the Femoral Head Treated with Natural Extracts

  • To determine the effect of Korean medicine treatment of avascular necrosis of the femoral head (ANFH) this study reviewed both single ingredients and bioactive compounds in the treatment of ANFH in a rat model. Literature was retrieved from PubMed and Google Scholar using the keywords "femur head necrosis," "natural extract," and "rat." The data from studies analyzed included: rats' characteristics, development methods of ANFH, natural extracts administration, observation methods, and outcome indicators. Two independent researchers screened all articles retrieved and 26 studies were chosen. The most used rat species was the Sprague Dawley rat (76.9%). To induce ANFH, steroid injections (46.2%), and oral gavage (53.8%) were typically used. Studies focused mainly on factors affecting bone formation (65.3%), and apoptosis (53.8%). Research on ANFH focused on using traditional natural substances mentioned in classical literature to confirm its effectiveness against anti-inflammation, osteogenesis, and cell death. ANFH has a diverse etiology, therefore research models such as genetic analysis of human-derived samples from ANFH patients may shed more light on the condition. Moreover, research into herbal medicines and pharmacoacupuncture treatment of ANFH should precede.

Analysis of Domestic and Foreign Research Trends on the Korean Medical Treatment of Avascular Necrosis of the Femoral Head (대퇴골두 무혈성 괴사의 한의학적 치료에 대한 국내·외 연구동향 분석)

  • Objectives This paper is an analysis of domestic and foreign trends for the Korean medical treatment of avascular necrosis of the femoral head. Methods Domestic and foreign databases were searched and analyzed for Korean medical treatment interventions, research types, and treatment effects for avascular necrosis of the femoral head. Results There were 10 case reports in Korea, 2 case reports and 3 randomized controlled studies abroad. A total of 12 treatment interventions were used, and the types include acupuncture, herbal medicine, phamacopunture, electroacupuncture, cupping, physical therapy, chuna, acupotomy, moxibustion, warm acupuncture, laser acupuncture, and motion style treatment. As for the evaluation index, visual analogue scale, range of motion and harris hip score were the most common with 7 cases each. Conclusions Through this study, it was possible to understand the trends of domestic and foreign research on the treatment of avascular necrosis of the femoral head with Korean medicine. However, due to the small number of literature and no randomized control studies in Korea, treatment evidence is insufficient. It is considered that further research is needed in the future.
