주의력결핍 과잉행동장애에서 정신자극제를 이용한 약물치료 (Stimulants Medication of Attention-Deficit Hyperactivity Disorder)

  • Attention-deficit hyperactivity disorder (ADHD) is characterized by inattention, hyperactivity, impulsiveness and problems in other higher cognitive processes such as executive function deficits. Currently, there are many treatment modalities, of which pharmacotherapy is the most strongly supported by scientific and clinical evidence. Stimulants, which are first choice in the pharmacological treatment of ADHD, block dopamine reuptake by binding the dopamine transporter and so increasing the concentration of dopamine in synaptic clefts. Stimulants are effective in improving core ADHD symptoms, as well as the nonspecific symptoms, such as aggressiveness and oppositional behavior. Frequently reported short-term adverse effects are decreased appetite, sleep disturbance, headache, dizziness and irritability. Although questions have been raised about the long-term side effects of stimulants, including growth suppression, cardiovascular events, and abuse potential, there is no clear evidence to support these concerns.

주의력결핍 과잉행동장애 치료에 대한 문헌적 고찰 -최근 중의 잡지를 중심으로- (The Literature Study for Attention Deficit Hyperactivity Disorder(ADHD) in Traditional Chinese Medical Journals)

  • Objective : The aim of this study was to investigate treatment of Attention Deficit Hyperactivity Disorder(ADHD) in current Traditional Chinese Medical Journals. Methods : From January 2000 to December 2005, we searched Traditional Chinese Medical Journals. Results : 21 articles were selected and classified into two large groups, case report and comparison study. Totally 1137 cases, complete cures are 276, significant improvements and some improvement 1028, complete cure rate 24.27%, total improvement rate 90.41%. Conclusion : Oriental medical treatments for ADHD were investigated to be effective in most trials. But there remains a debate about outcome criteria. Further studies are needed. Treatment methods were herbal medicine, acupuncture, auricular acupuncture, family consult.

BDNF가 ADHD의 병인과 치료에 미치는 영향 (Association between the Brain-Derived Neurotropic Factor and Attention Deficit Hyperactivity Disorder)

  • Attention deficit hyperactivity disorder (ADHD) is a common childhood psychiatric disorder. Recently, it has been suggested that brain-derived neurotropic factor (BDNF) may play a role in the pathogenesis of ADHD. Our aim of this review is to understand the physiological functions of BDNF and its potential relationship with ADHD and therapeutic approaches of ADHD. Searches were conducted in Pubmed and Research Information Service System (RISS). In this review, we summarized important literatures for the physiological functions of BDNF in neurodevelopment, change of serum BDNF level in ADHD, association of BDNF polymorphism and ADHD and potential association of treatment of ADHD with serum BDNF level. Further studies are required to more clearly understand the source and the role of BDNF in ADHD and to develop BDNF based-ADHD treatement.

The Extent of Ordinary Classrooms Teachers' Knowledge and Perception of Attention Deficit Hyperactivity Disorder (ADHD)

  • The present study aimed to investigate the extent to which ordinary classrooms teachers are informed are familiar with Attention Deficit Hyperactivity Disorder 'ADHD'. The researcher adopted the descriptive approach. The sample of the study consisted of (89) male and female teachers who teach students from grade (1) to grade (6) at public primary schools in Northern Mazar Directorate of Education in Jordan. A questionnaire was employed as the primary tool for collecting the data. The findings of the current study indicated an average level of knowledge of ordinary classroom teachers about ADHD. The findings also indicated that there were no statistically significant differences in the extent of ordinary classroom teachers' knowledge of ADHD due to the variables (gender, qualification, professional experience, grade level being taught).

보건계열 대학생의 스마트폰 과다사용이 주의력결핍, 학습환경, 학업지연행동에 미치는 융합적 영향 (The Convergence Influence of excessive smartphone use on attention deficit, learning environment, and academic procrastination in health college students)

  • 본 연구는 보건계열 대학생의 스마트폰 과다사용이 주의력결핍, 학습환경과 학습지연행동에 미치는 융합적 영향을 파악하고자 2017년 3월 6일부터 6월 12일까지 부산광역시 보건계열 255명의 대학생을 대상으로 설문조사를 실시하였다. 스마트폰 사용 특성에 따른 스마트폰 과다사용정도, 주의력결핍, 학습환경, 학업지연행동 정도는 스마트폰의 하루 사용시간, 스마트폰 중독 인식과 수업시간 개인사용 경험에 유의한 결과를 보였다(p<0.001). 스마트폰 과다사용은 주의력결핍(r=0.870, p<0.01), 학습환경(r=0.812, p<0.01), 학업지연행동(r=0.772, p<0.01)과 정적상관을 보였고, 주의력결핍은 학습환경(r=0.918, p<0.01), 학업지연행동(r=0.798, p<0.01)과 정적 상관을 보였다. 학습환경은 학업지연행동(r=0.777, p<0.01)과 정적 상관을 보였다. 스마트폰 과다사용이 미치는 영향요인은 주의력결핍(p<0.001), 학업지연행동(p<0.01)순으로 나타났다. 따라서 스마트폰 중독예방과 올바른 스마트폰 사용으로 보건계열 대학생의 건강한 학습환경 조성이 이루어져야 할 것이다.

약물치료 중인 주의력결핍 과잉행동장애 아동 청소년에서 작업기억력 훈련이 임상증상과 신경심리소견에 미치는 부가적 효과 (Augmentative Effects of Working Memory Training on Clinical Symptoms and Neuropsychology in Medicated Children and Adolescents with Attention-Deficit Hyperactivity Disorder)

  • Objectives: Executive dysfunction including working memory deficit has been suggested to be one of the major neuropsychological etiologies of attention-deficit hyperactivity disorder (ADHD). The purpose of this study was to investigate the augmentative effects of Cogmed working memory training on the symptoms and neurocognitive functions in medicated children and adolescents with ADHD. Methods: Twenty-five children with ADHD, aged 7 to 19 years, taking ADHD medication participated in this study. The participants were trained for 5 weeks with a commercially available and computerized working memory program ($Cogmed^{(R)}$) without any changes to their medication. The Korean version of the ADHD Rating Scale, Clinical Global Impression Scale, and Comprehensive Attention Test were administered before training and 4 weeks and 7 months after training, respectively. Results: After completing the training, the clinical symptoms and function, rated by the parents and clinician, were improved. In addition, the level of commission errors was significantly reduced in the selective attention (visual/auditory) task, sustained attention to response task, and flanker task. The untrained visuospatial short-term memory and working memory were also improved. These effects were still observed 7 months after the training. Conclusion: Cogmed working memory training can be a promising training option for the additional improvement of the symptoms and deficits in working memory and response inhibition in medicated children with ADHD.

Affective Factors That Contribute to the Quality of Life of Juvenile Inmates with Attention-Deficit/Hyperactivity Disorder: A Focus on Items from the Korean Youth Self Report

  • Objectives: This study investigated quality of life in Korean juvenile inmates with attention-deficit/hyperactivity disorder (ADHD) and the impact of behavioral and emotional problems on quality of life. Methods: In total, 200 inmates were evaluated using the Korean version of the Mini-International Neuropsychiatric Interview (K-MINI) and the Korean version of the Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime (K-SADS-PL-K). We extracted the inmates with ADHD and evaluated their quality of life, behavioral problems, and emotional problems with the Pediatric Quality of Life Inventory (PedsQL) and the Korean Youth Self Report (K-YSR) scale. Descriptive statistics, Pearson correlation analysis, and multiple regression analysis were conducted. Results: Among the 200 total inmates, 68 were diagnosed with ADHD by the K-SADS-PL-K. Most of the correlations between PedsQL scores and K-YSR items were significant. Multiple regression analysis showed that PedsQL could be predicted by affective problems (among the DSM-oriented scales of the K-YSR) and attention problems (among the syndrome scales of the K-YSR). Conclusion: Our results demonstrate that, among juvenile inmates with ADHD, quality of life was negatively correlated with most behavioral and emotional problems. Meanwhile, the significant influence of affective and attention problems on inmates' quality of life suggests the necessity of comprehensive treatments for this group.

주의력 결핍 과잉활동장애의 행동 특성 - 과잉활동성 유무에 따른 임상적 특성의 비교 - (BEHAVIORAL CHARACTERISTICS IN ATTENTION DEFICIT HYPERACTIVITY DISORDER - COMPARISONS OF ATTENTION DEFICIT DISORDER WITH AND WITHOUT HYPERACTIVITY -)

  • 저자들은 Attention Deficit Hyperactivity Disorder(ADHD) 환자에서 과잉활동증을 동반한 집단과 과잉활동증을 동반하지 않은 집단간에 행동 및 정서적 특성을 비교하고, 각 증상척도들 간의 임상적 관계를 알아보기 위하여 본 연구를 시행하였다. ADHD 환자 36명을 Werry-Weiss-Peters Activity Rating Scale(WWPARS) 평가기준(20점)에 따라 Attention Deficit Disorder with Hyperactivity(ADD+H) 집단 19명과 Attention Deficit Disorder without Hyperactivity(ADD-H) 집단 17명으로 분류하여 Conners Rating Scale(CRS), Home Situations Questionnaire(HSQ), School Situations Questionnair(SSQ), Child Behavior Check-list(CBCL) 등을 시행한 결과 다음과 같은 결과를 얻었다. 1) 두 집단간에 인구학적 특성은 차이가 없었다. 2) ADD+H 집단의 CRS 행동증상척도는 점수는 ADD-H 집단에 비하여 유의하게 높았으며, HSQ와 SSQ를 통하여 가정과 학교에서의 행동문제 상황을 비교한 결과, ADD+H 집단의 행동문제 상황의 유형이나 개수 및 증상의 심도가 ADD-H 집단에 비하여 유의하게 높았다. 3) 각 집단의 CBCL 8개 소척도 점수를 비교하였을 때, ADD+H 집단의 비행척도와 공격성 척도가 유의하게 높았다. 4) ADHD의 행동증상과 CBCL의 행동정서척도 간의 상관관계를 조사한 결과, CRS, WWWPARS의 행동증상척도와 CBCL의 공격성척도, 비행척도, 정서불안정척도 간에는 유의한 양성 상관관계가 있었으며, 특히 공격성척도와 과잉활동증 간에는 매우 유의한 상관성이 있음을 알 수 있었다. 본 연구결과로 미루어 ADHD는 과잉활동증의 유무에 따라 그 임상적 특징이 차이가 있음을 시사하며, 향후 ADHD의 여러 임상적 유형과 약물치료효과 간의 관련성에 대한 연구 및 ADHD 환자에서 행동 특성과 인지기능이 통합된 연구의 필요성이 요구된다.

  • 주의력결핍-과잉행동장애(Attention Deficit-Hyperactivity Disorder, ADHD)는 학령기 어린이의 가장 흔한 정신장애로 점차 증가하는 추세이다. ADHD 어린이들은 자극에 선택적으로 오래 집중하는 것을 어려워하고, 말이 많거나 허락 없이 자리에서 일어나고 뛰어다니며, 팔과 다리를 끊임없이 움직이는 등 활동 수준이 높다. 또한 충동적인 성향이 있어서 넘어지거나 사고를 당하는 경우도 많다. 이에 치과치료 시 행동조절, 구강위생, 외상 등의 문제를 예상할 수 있는 바, 치과의사의 관심이 증대되어야 할 필요가 있다. 본 증례는 ADHD로 진단받은 만 8세 여아로 약물진정 및 심리적인 행동조절법을 이용해 치과적 주소를 해결해 주었고, 문헌고찰을 통해 다소의 지견을 얻었다.
