A Case of Pedunculated Schwannoma of Aryepiglottic fold (피열후두개주름에 발생한 유경성 신경초종 1예)

  • Schwannoma, also known as a neurilemmomas or neurinomas, is benign peripheral nerve sheath tumors arising from the Schwann cells. About 25~45 % of all schwannomas are found in the head and neck region, and the majority are found in the parapharyngeal space. Schwannoma of the larynx is very rare with an incidence of 0.1~1.5% in all benign laryngeal tumors. Recently, we experienced a case of a pedunculated schwannoma arising from the left aryepiglottic fold in a 80-year-old patient. The tumor was completely excised under direct laryngoscopy with the use of a $CO_2$ laser and subsequently confirmed as schwannoma. So, we report this case with a review of the literatures.

A Case of Mixed Cavernous, Capillary Hemangioma Arising from Supraglottis (성문 상부에 발생한 혼합형 혈관종)

  • Laryngeal hemangioma occurs mostly in infantile patients and infantile hemangiomas can be frequently seen in the subglottic area. So, respiratory distress is a main symptom in this entity. But adult hemangiomas are rare and can be seen in different locations such as in epiglottis, aryepiglottic fold, arytenoids and false and true vocal folds. Authors experienced a case of mixed hemangioma at the right aryepiglottic fold in a 42 years-old-man who was presented with snoring, throat foreign body sense for 3 months. So, we report this case with a review of the literature.

  • 신경초종은 비교적 드문 질환으로서1908년Verocay가 이 종양의 특징적 조직소견인 Verocay body를 발견하고 이를 neurinoma라고 명명하였다. 이 종양은 양성종양으로서 뇌신경, 척수신경뿐 아니라 자율신경에서도 발생하여, 약 25%가 두경부에서 발생하고, 단일 부위로서는 청신경에서의 발생빈도가 가장 높으나 인후부 발생은 드물다. 저자들은 피열후두개추벽에 발생한 거대한 신경초종 1례를 치험하였기에 보고하는 바이다.

Clinical Characteristics and Treatment Result of Laryngeal Cysts (후두에 발생한 낭종의 임상양상 및 치료 결과)

  • Background and Objectives: The purposes of this study are to find out clinical causes, clinical characteristics and treatment outcomes in relation to anatomical location of laryngeal cyst. Subjects and Method: A retrospective study of medical records was carried out for 170 patients with cysts on vocal cord, epiglottis, vallecula, arytenoid and aryepiglottic fold. Results: There were 83 cases of epiglottic cysts, 41 cases of vallecular cysts, 35 cases of intracordal cysts, 3 cases of arytenoid cysts and 2 cases of aryepiglottic cysts. Laryngeal cysts were more common in men than in women, and the ratio between men and women was 2:1. The age of patients ranged from 7 to 90 years, with their average age being 52 years. The most common symptom was voice change at intracordal cysts and globus sensation at vallecular and epiglottic cysts. The most common cause of intracordal cysts were voice abuse. But other location of laryngeal cysts doesn't have common causes. The average size of cysts was 0.3cm at vocal cord, 1.43cm at epiglottis, 1.4cm at vallecula, 0.9cm at arytenoid and 1cm at aryepiglottis. Recurrence was observed in 7 cases from 1 months to 18 months following the operation. Size of all recurred cysts was over the average. Conclusion: Physicians should be aware of changes in clinical patterns of laryngeal cysts according to location and have long follow-up period at large cysts after operation.

Two Cases of Alocasia Intoxication (알로카시아 중독 2례)

  • Alocasia was originally distributed throughout subtropical and tropical areas. Recently, in Korea, it has been used in air cleaners and in control of humidity. Despite easy access in Korea, there are few reports on Alocasia toxicity. We report on two cases of Alocasia intoxication. One patient was a 16-month-old male, who was admitted with a complaint of irritability after biting leaves of Alocasia. Four hours later, he was discharged without any symptoms. Another patient, a 52-year-old female, complained of oral pain, numbness on the perioral area, dysphonia, swallowing difficulty, and chest and abdominal pain after eating root stuck of alocasia odora. She underwent gastrointestinal fibroscopy (GIF) due to lasting chest and abdominal pain. Finding on GIF showed erythema and swelling in the aryepiglottic fold and larynx. Her symptoms lasted 13 days; she was then discharged without any complications or sequelae.

A Case of Neurilemmoma of the Larynx (후두에 발생한 신경초종 1예)

  • Neurilemmoma is a relatively rare benign tumor which may be derived from nerve sheath of the peripheral, sympathetic and cranial nerve except the optic nerve and olfactory nerve. And it is a characteristically solitary and well-encapsulated mass. About 25% of neurilemmoma occurs in the head and neck region and the acoustic nerve is most frequently involved, but neurilemmoma of the larynx is rare. Recently, we experienced a case of neurilemmoma occurred in the right aryepiglottic fold of the larynx. The tumor was successfully removed by transoral approach under direct laryngoscope. So we report this case with literature review.

Classification and Management in Patients with Laryngomalacia (후두연하증의 분류와 치료)

  • Laryngomalacia is the most common congenital anomaly that causes inspiratory stridor and airway obstruction in the newborn. Symptoms begin to appear after weeks of age, become worse at 4-8 months, improve between 8-12 months, and usually heal naturally at 12-18 months. Despite these common natural processes, the symptoms of the disease can be very diverse and, in severe cases, require surgical treatment. The diagnosis can be made by suspicion of clinical symptoms and direct observation of the larynx with the spontaneous breathing of the child. Typical laryngeal features include omega-shaped epiglottis, retroflexed epiglottis, short aryepiglottic fold, poor visualization of the vocal folds, and edema of the posterior glottis, including inspiratory supra-arytenoid tissue prolapse. In this review, we discuss the classification and treatment based on symptoms and laryngoscopic findings in patients with laryngomalacia.

A Case of Laryngeal Lipoma Arising at the Free Margin of the Vocal Cord (성대의 자유연에 발생한 후두 지방종 1례)

  • Lipomas are benign tumors and most commonly occurs in trunk, upper extremities and lower extremities. About 13-15% of lipomas are located in the head and neck area. However, lipomas of larynx are very rare and only about 100 cases have been reported. Laryngeal lipomas occur mainly in epiglottis, aryepiglottic fold and false vocal cords, which have adipose tissue. Author experienced an unusual presentation of laryngeal lipoma. Tumor seemed to be located in the supraglottis in the preoperative laryngoscopy, but it was found to be located at the free margin of the true vocal cord. To date, only one case has been reported in the world literature. We report this case with a review of the literature.

  • Solitary neurofibroma of the glottis is extremely rare and accounts for only 0.1-1.5% of benign laryngeal tumors. Aryepiglottic fold is the most frequent involved site followed by arytenoids and ventricular folds. There have been few reports of neurofibroma of the true vocal cord. We report a case of neurofibroma which was deeply embedded in the vocal cord and misdiagnosed as muscle tension dysphonia with a review of literatures.

Efficacy of laughing voice treatment (SKMVTT) in benign vocal fold lesions (양성성대질환의 웃음 음성치료(SKMVTT))

  • The purpose of this study was to evaluate the efficacy of a multiple voice therapy technique ($SKMVTT^{(R)}$) using laughter for the treatment of various benign vocal fold lesions. To achieve this, 23 female patients diagnosed with vocal nodules, vocal polyp, and muscle tension dysphonia through videostroboscopy were enrolled in vocal hygiene and $SKMVTT^{(R)}$. All of the patients were treated once a week for 4 to 12 sessions. The GRBAS scale was used to confirm the changes in voice quality before and after the treatment. Acoustic analysis was performed to evaluate jitter, shimmer, NHR, fundamental frequency variation, amplitude variation, PFR, and dB range. Videostroboscopy was performed to confirm the changes in the laryngeal features before and after the treatment. After the $SKMVTT^{(R)}$, the results of the perceptual evaluation demonstrated that the G, R, and B scales significantly improved. An acoustic evaluation also demonstrated that jitter, shimmer, NHR, vAm, vFo, PFR, and dB range also significantly improved after the $SKMVTT^{(R)}$. In comparison to the videostroboscopic findings, the size of the vocal nodules and vocal polyp decreased or disappeared after the treatment. In addition, the size of the cuneiform tubercles decreased, the length of the aryepiglottic folds became longer, and the laryngeal findings of the supraglottic compressions improved after the $SKMVTT^{(R)}$. These results suggest that the $SKMVTT^{(R)}$ is effective in improving the vocal quality of patients with benign vocal fold lesions. In conclusion, it seems that laughter and inspiratory phonation suppressed abnormal laryngeal elevation and lowered laryngeal height, which seems to have the effect of improving hyperfunctional phonation.
