Clinical evaluation of the removable partial dentures with implant fixed prostheses (임플란트 고정성 보철물을 이용한 가철성 국소의치의 합병증에 관한 임상적 평가)

  • Purpose: The purpose of this study was to identify clinical complications in removable partial denture (RPD) with implant-supported surveyed prostheses, and to analyze the factors associated with the complications such as location of the implant, splinting adjacent prostheses, the type of retentive clasps, Kennedy classification, and opposing dentition. Materials and Methods: A retrospective clinical study was carried out for 11 patients (7 male, 4 female), mean age of 67.5, who received RPD with Implant-supported surveyed prostheses between 2000 and 2016. The mechanical complications of 11 RPDs and 37 supporting implant prostheses and the state of natural teeth and peripheral soft tissue were examined. Then the factors associated with the complications were analyzed. Results: The average of 3.4 implant-supported prostheses were used for each RPD. Complications found during the follow-up period of an average of 42.1 months were in order of dislodgement of temporary cement-retained prostheses, opposing tooth fracture/mobility, screw fracture/loosening, clasp loosening, veneer porcelain fracture, marginal bone resorption and mobility of implant, artificial tooth fracture. Complications occurred more frequently in anterior region compared to posterior region, non-splinted prostheses compared to splinted prostheses, surveyed prostheses applied by wrought wire clasp compared to other clasps, and natural dentition compared to other removable prostheses as opposing dentition. There were no significant differences in complications according to the Kennedy classification. Conclusion: All implant-assisted RPD functioned successfully throughout the follow-up. However, further clinical studies are necessary because the clinical evidences are still not enough to guarantee the satisfactory prognosis of implant-assisted RPD for long-term result.

Effects of Oral Hygiene Improvement of the Elderly Patients by Caregiver's in Rural Long-term Care Hospital (농촌지역 노인요양병원 입원환자의 간병인을 통한 구강건강관리 효과)

  • Objectives: This study was conducted to be identify the effects oral hygiene improvement of the elderly by caregiver in a rural. Methods: It recruited Fifty three elderly patients were recruited who admitted at a long-term hospital located in Mokchon city, Chungchungnam-do province. as the intervention group. Fifty two elderly patients group were selected another one long-term care hospital in Daegu city as control group. study was conducted for seven months. long-term care hospital. for seven months from Dec. 2007 to June 2008. For three months, the caregivers provide tooth brushing and cleaning artificial teeth once a day, to intervention group. once a day for three months. Results: Before the program there was no significant difference between the control and the intervention groups in general characteristics, prevalence rates of diseases, oral sanitary condition. Dental plague score was decreased significantly(p<0.001) before and after intervention in the intervention group. Using analysis of covariance for evaluation of the effect of the intervention, a significant difference was observed between the intervention group and the control group(p<0.01). Conclusions: These results were thought to be used as important basic data to develop oral health management program for elderly patients who needed long-term care.

DETECTION OF EARLY PROXIMAL CARIES WITH LASER FLUORESCENCE (레이저 형광법을 이용한 인접면 우식증의 진단)

  • Artificial carious lesions in various depths were observed with visual examination using light transillumination, bite-wing radiography, laser fluorescence, and dye-enhanced laser fluorescence to determine the reproducibility, correlation of each diagnostic method, diagnostic sensitivity and diagnostic specificity. And optical densities according to demineralized times were measured whether laser fluorescence could be used as a quantitative diagnostic method. The following results were obtained whether laser fluorescence could be used for diagnosis of initial proximal caries. 1. Tau-c values of visual examination was 0.08 which showed lowest reproducibility and those of bite-wing radiography, laser fluorescence, dye-enhanced laser fluorescence were 0.60, 0.48, and 0.64, respectively which showed relatively high reproducibility. 2. The correlation between demineralization time and each examination was the highest in dye-enhanced laser fluorescence$({\gamma}=0.51)$ followed by laser fluorescence$({\gamma}=0.43)$, bite-wing radiograph$({\gamma}=0.35)$, and visual examination$({\gamma}=0.33)$. Dye-enhanced laser fluorescence and laser fluorescence showed significant correlation with demineralization time. 3. The sensitivity of laser fluorescence and dye-enhanced laser fluorescence for diagnosing approximal caries based on bite-wing radiography were 67%, 100% and those of specificity were 57%, 11% which showed diagnostic specificity was relatively lower than sensitivity. 4. The difference in optical density(DFR) between sound teeth and carious lesions according to lesion depth was high with dye-enhanced laser fluorescence compared with laser fluorescence. DFR measured with laser fluorescence according to changes in lesion depth was statistically significant but was not statistically significant with dye-enhanced laser fluorescence. Based on these results, laser fluorescence and dye-enhanced laser fluorescence have comparable diagnostic power as bite-wing radiography in early diagnosis of proximal caries.

THE CHANGE OF THE CONFIGURATION OF HYDROXYAPATITE CRYSTALS IN ENAMEL BY CHANGES OF PH AND DEGREE OF SATURATION OF LACTIC ACID BUFFER SOLUTION (유산 완충용액의 pH 및 포화도 변화에 따른 법랑질 내 수산화인회석 결정 형태의 변화)

  • Since it was reported that incipient enamel caries can be recovered, previous studies have quantitatively evaluated that enamel artificial caries have been, remineralized with fluoride showing simultaneously the increase of width of surface layer and the decrease of width of the body of legion. There is, however, little report which showed that remineralization could occur without fluoride. In addition, the observations on the change of hydroxyapatite crystals also have been scarcely seen. In this study, enamel caries in intact premolars or molars was induced by using lactic acidulated buffering solutions over 2 days. Then decalcified specimens were remineralized by seven groups of solutions using different degree of saturation(0.212, 0.239, 0.301, 0.355) and different pH(5.0, 5.5, 6.0) over 10 days. A qualitative comparison to changes of hydroxyapatite crystals after fracturing teeth was made under SEM(scanning electron microscopy) and AFM(atomic force microscopy). The results were as follows: 1. The size of hydroxyapatite crystals in demineralized area was smaller than the normal ones. While the space among crystals was expanded, it was observed that crystals are arranged irregularly. 2. In remineralized enamel area, the enlarged crystals with various shape were observed when the crystals were fused and new small crystals in intercrystalline spaces were deposited. 3. Group 3 and 4 with higher degree of saturation at same pH showed the formation of large clusters by aggregation of small crystals from the surface layer to the lesion body than group 1 and 2 with relatively low degree of saturation at same pH did. Especially group 4 showed complete remineralization to the body of lesions. Group 5 and 6 with lower pH at similar degree of saturation showed remineralization to the body of lesions while group 7 didn't show it. Unlike in Group 3 and 4, Group 5 and 6 showed that each particle was densely distributed with clear appearance rather than crystals form clusters together.
