Takayasu씨 대동맥염에 의한 양경동맥협착과 우관상동맥개구협착의 동시수술치험 -1례 보고- (One Sage Operation for Bilateral Carotid Arterial Obstruction and Right Coronary Artery Ostial Stenosis in Takayasu's Arteritis -Report of One Case-)

  • We report a case of recurred Takayasu,s arteritis.The patient was 28-year-old female underwent aorto-left common carotid and aorto-left subclavian bypass graft replacement 1 year ago.Unfortunately, she was readmitted because of newly developing angina and both eye claudication severe headache. Aorto-coronary angiogram showed complete obstruction of left common carotid artery ,stenosis of right carotid artery bifurcation and ostial stenosis of right coronary artery.Bilateral carotid arteries bypass graft with great saphenous vein and right coronary artery bypass graft with right internal mammary artery were done at same the time and she discharged after 21 days without any problem.

Downbeat Nystagmus Associated With Brainstem Compression by Vertebral Artery

  • Rarely, downbeat nystagmus can occur due to compression of the lower brainstem by the ectatic vertebral artery and be resolved by microvascular decompression. We present a case of a 67-year-old man with downbeat nystagmus associated with brainstem compression by ectatic vertebral artery. He presented with oscillopsia and vertigo. When he turned his head upward, his symptoms were aggravated and a gait disturbance occurred. Magnetic resonance imaging and computed tomographic angiography demonstrated compression of the medulla oblongata by the left ectatic vertebral artery and other medical causes of downbeat nystagmus were ruled out. Retromastoid craniotomy was performed and after lifting the vertebral artery off the medulla, a trough-shaped indentation in the lower brainstem was identified. The ectatic vertebral artery was repositioned and a Teflon was inserted between the brainstem and the ectatic vertebral artery. Postoperatively, downbeat nystagmus had disappeared.

폐동맥에서의 우관상동맥 기시이상증 -1례 치험- (Anomalous Origin of Right Coronary Artery from Pulmonary Artery)

  • Anomalous origin of right coronary artery from pulmonary artery is a rare congenital cardiac anomaly. Anomalous right coronary artery often be a incidental finding without serious cardiovascular sequale. The only characteristic physical finding is a continuous murmur with diastolic accentuation. There are no diagnostic EKG or chest X-ray changes. Diagnosis is made best by selective left coronary arteriography showing retrograde filling of right coronary artery from collateral vessels. Here, we present a case of twenty-four months old aged girl with anomalous origin of right coronary artery combined with pulmonary stenosis. This is the first pediatric patient with anomalous right coronary artery and the first patient to have surgical correction for this malformation.

  • 우측 폐동맥이 대동맥에서 이상기시되는 기형은 선천성 심장질환의 드문 형태이고 치명적이다. 주로 동맥관개존과 같이 동반되어 나타나며, 대개 출생후 이른 유아기에 사망한다. 우폐동맥의 이상기시는 좌폐동맥의 이상기시에 비해 더욱 많은 빈도를 나타내며, 비정상적인 우폐동맥은 주로 대동맥판막에 인접한 상행대동맥의 뒤쪽편에서 주로 기시한다.

  • The common carotid artery generally bifurcates into the internal and external carotid arteries at the level of C3-4. Injury to the common carotid artery during anterior cervical discectomy is a complication that is very much feared but encountered rarely. Knowing the anatomic variations of the common carotid artery and using an operating microscope during the anterior cervical approach for cases with low-lying bifurcation of the common carotid artery would prevent injuries to this artery. We present a 42-year-old female who has successfully undergone anterior cervical discectomy at the level of C5-6 and C6-7. She had a low-lying bifurcation of the common carotid artery.

Takayasu 동맥염;치험 1례 (Takayasu`s Arteritis; A Case Report)

  • Recently we experienced a case of Takayasu`s arteritis involving the major aortic branches. A 30 year-old female patient admitted with the complaints of dizziness, visual disturbance, headache and tingling sensation of upper extremities. Aortogram revealed nearly complete obstruction of the origin site of both common carotid arteries and right vertebral artery, and irregular luminal narrowing of the origin site of innominate artery and left subclavian artery, but opacification of right subclavian artery and left vertebral artery. Successful surgical treatment was accomplished with a bypass from the ascending aorta to the left common carotid artery using a tube graft. The left subclavian artery and right axillary artery were revascularized distal to the stenosis with tube grafts that extended from the aortic graft. Postoperative complications were atelectasis, lymph leakage and left phrenic nerve palsy. She discharged uneventually at postoperative 22 days and most of symptoms were relieved.

Cervical Radiculopathy Caused by Vertebral Artery Loop Formation : A Case Report and Review of the Literature

  • Vertebral artery loop formation causing encroachment on cervical neural foramen and canal is a rare cause of cervical radiculopathy. We report a case of 61-year-old woman with vertebral artery loop formation who presented with right shoulder pain radiating to her arm for 2 years. Plain radiograph and computed tomography scan revealed widening of the right intervertebral foramen at the C5-6 level. Magnetic resonance imaging and angiogram confirmed the vertebral artery loop formation compressing the right C6 nerve root. We had considered microdecompressive surgery, but the patient's symptoms resolved after conservative management. Clinician should keep in mind that vertebral artery loop formation is one of important causes of cervical radiculopathy. Vertebral artery should be visualized using magnetic resonance angiography in suspected case.

  • 관상동맥-페동맥루는 관상동맥 기형 중 드문 질환으로 그중에서도 양측성은 매우 드물다. 흉통 및 호흡곤란을 주소로 내원한 두 환자에서 심도자술 및 관상동맥 조영술로 양측성 관상동맥-폐동맥루를 진단하였고 이들 중 한 환자에서는 좌전하행지 협착증, 다른 환자에서는 낭성 종양을 동반하였다. 이두 환자에 대해 수술적 교정을 시행하여 좋은 결과를 얻었기에 보고하는 바이다.

A Giant Popliteal Artery Aneurysm Treated with Exclusion and Bypass Using a Saphenous Vein

  • While popliteal artery aneurysm is the most common form of peripheral artery aneurysm, it is a rare condition in the general population. Furthermore, a giant popliteal artery aneurysm has not previously been reported in Korea. A 67-year-old male presented with left thigh pain that had begun 4 months earlier and was aggravated when in a sitting position. We found a giant aneurysm on the left popliteal artery and performed a bypass from the common femoral artery to the distal popliteal artery below the knee, using the autologous greater saphenous vein, and excluded the aneurysm at the sites of anastomoses.

  • 1970년대에 요골동맥은 관상동맥우회로술의 이식편으로 제안되었으나 적출시 나타나는 경직에 의한 이식편의 폐쇄 때문에 금지되었다. 새로운 항경직성 약물의 등장과 외상을 최소화시키는 기술의 발전은 관상동맥 우회로술의 이식편으로 요골동맥을 되살리게 되었다. Harmonic Scalpel(Ultracision Inc, Smitweld, Rl)은 요골동맥을 덜 손상을 주고, 경직이 없고, 더 빨리 적출할 수 있게 한다.
