  • The purpose of this study is to analyze a Characteristics of block formation and commercialization of residential books commercial area along urban arteries on Teheran-ro in Seoul. 'Teheran-ro' is selected as a case site to analyze the characteristics of residential blocks commercial area along urban arteries. Echocardiograpyhic Assessment and Clinical Application of Cardiac Disease in Korea Jin-do Dog III. Detection of Heartworm (진돗개에서 심장초음파 측정치와 평가와 임상적 응용)

  • Echocardiography was performed to obtain the optimal planes for diagnosis of canine heartworm disease in 11 dogs. Imaging planes were taken the left ventricular outflow tract long-axis view, 4-chamber long-axis view, chordae tendineae level short-axis view and modified pulmonary arteries level short-axis view in the right parasternal window, and aptical 4-chamber long-axis view in hte left parasternal window. After echocaridography, the pulmonary artery valve pat was ligated with double 1-0 silk under the general anesthesia to prevent heartworm moving from pulmonary artery to right ventricle. The dogs were euthanized and examained for heartworms in heart and great vessels. Many adult heartworms were identified ultrasonographically within main pulmonary artery, bifurcation of pulmonary artery and right pulmonary artery, but not identified within right ventricle and atrium. At necropsy, adult heartworms were mainly found in pulmonary artery in 11 days, and also found in right ventricular outflow tract and right ventricle in 2 of 11 dogs. The modified pulmonary arteries level short-axis view was the best imaging plane for identifiying heartworms because the main pulmonary artery, branches of pulmonary artery and right pulmonary artery could be showed. The aortic root internal dimension (AOID) and right pulmonary atery internal dimension (RPAID) were measured from the modified pulmonary ateries level short-axis view and left ventricular outflow tract long-axis view. The RPAID and RPAID/AOID was higher in heartworm infected dogs than normal Korea Jin-do dogs in modified pulmonary arteries level short-axis view, and the AOID, RPAID and RPIAID/AOID was higher than in left ventricular outflow tract long-axis view, respectively. These results indicate that the pulmonary arteries were the major habitat of adult heartworm in canine heartworm disease and the modified pulmonary arteries level short-axis view was the best imaging planes for identifying heartworms because the main pulmonary arteries could be showed. Therefore the modified pulmonary arteries level short-axis view can be used for diagnosing heartworm disease and for monitoring dilation of pulmonary artery.

Expression of Nitric Oxide Synthase and Endothelin-1 in Human Uterine Artery from Full-Term Pregnancies

  • The aim of this study was to determine the roles of ET-1 and NO on uterine blood flow in pregnancy. Uterine arteries were isolated from 17 nonpregnant and 12 pregnant women. Nonpregnant group included patients with median age of $48.6{\pm}2.3$ years who underwent hysterectomy, because of myoma. Pregnant group included patients with median age of $31.3{\pm}1.4$ years undergoing cesarean delivery. ET-1 and ET-2 induced concentration-dependent contraction in isolated nonpregnant and pregnant uterine arteries. The contractile response and maximal contraction were increased in pregnant uterine arteries. In nonpregnant uterine arteries, there was no contraction in response to ET-3, whereas pregnancy induced concentration-dependent contraction by ET-3. Tissue nitrite/nitrate level and immunohistochemical staining of eNOS and iNOS were increased in pregnant uterine arteries, compared with nonpregnant uterine arteries. In addition, the expressions of eNOS and iNOS mRNA were significantly increased in pregnancy. Moreover, contractions by ET isopeptides, including ET-1, were enhanced, and immunohistochemical staining of ET-1 and ET-1 mRNA expression was increased in pregnant uterine arteries. These results suggest that NO production by increased NOS activity, especially eNOS activity, is related to placental and uterine blood flow. Furthermore, ET-1 appears to play a pathophysiological role in pregnant complications such as hypertension.

  • An eight-day-old neonate was diagnosed with dextro-transposition of the great arteries, atrial septal defect, patent ductus arteriosus, and a single sinus origin of the coronary arteries. The single coronary artery originated from the left sinus (sinus 2), had a proximal left circumflex arterial branch, and passed anteriorly to the right side of the aorta, further branching into the right coronary and left anterior descending arteries. We successfully performed an arterial switch operation and coronary transfer by tube graft reconstruction with autologous aortic tissue to treat the dextro-transposition of the great arteries and atrial septal defect with a single-sinus origin of the coronary arteries.

Differential Vasorelaxant Effects of KR-30075, a New Cyclic AMP-phosphodiesterase Inhibitor, on Guinea-pig Pulmonary, Bovine Coronary and Renal Arteries

  • The vasorelaxant effects of KR-30075 in guinea-pig pulmonary, bovine coronary and renal arterial strips contracted by either$K^+$depolarization, phenylephrine, or prostaglandin $F_{2a}$($PGF_{2a}$) were evaluated. KR-30075 was more potent than imazodan as a vasorelaxant against $PGF_{2a}$-induced contractions in bovine coronary and renal arteries, whereas against$K^+$induced contractions KR-30075 was less potent than imazodan in guieapig pulmonary arteries and more potent in bovine coronary arteries. KR-30075 was more potent against contractions induced by phenylephrine or $PGF_{2a}$ than the contractions induced by $K^+$ This profile of activity for KR-30075 was similar to that of imazodan and dissimilar from the calcium entry blocking agent nifedipine. There was no vascular selectivity of KR-30075 between bovine coronary and renal arterial strip preparations. In conclusion, this study shows that KR-30075 represents the vasorelaxant effects on guinea-pig pulmonary, bovine coronary and renal arteries without specific vascular selectivity. The vasorelaxant profile of KR-30075, with different sources of vascular smooth muscle, is unlike that of calcium entry blocking agent and more similar to the profile of the agent that inhibit cyclic nucleotide phosphodiesterase.

Pulseless Disease: Report of Two Cases (무맥증 수술치험 2례)

  • A patient was 29 year old house wife who was admitted to the Yonsei University Medical Center on 6 th of Nov., in 1970, with chief complaints of complete loss of bilateral visual acuity, generalized weakness, frequent palpitations, claudication of masseter muscles and intermittent fainting. These symptoms were developed 5 years prior to admission. Physical finding on admission revealed complete loss of bilateral visual acuity, absence of both radial and carotid pulse, but there was good femoral and popliteal pulse. She couldn't open her mouth as she desired and had weakness of mastication. Radiological findings of plane chest PA were not significant. Aortogram(Cineangiogram) showed non-visualization of both carotid and subclavian arteries. It showed only innominate and interal mammary artery preoperatively. The operative findings were as follows: There were complete obliterative changes in both common carotid and subclavian arteries, and periarteritis in the innominate artery. Tube Dacron Prosthesis Bypass with V-arm between innominate artery and both common carotid arteries was performed after thromboendarterectomy. Histopathological finding of the thromboend arterectomy specimen was compatible with pulseless disease, which showed marked fibrous thickening of intima and a diffuse inflammatory cell infiltration of the whole layers. Her postopererative course was uneventful. Follow up aortogram(Cineangiogram) was taken on 11th postoperative day, which revealed both common carotid arteries patent. Her preoperative Subjective symtoms disappeared remarkably, such as her visual acuity improved much, fainting and vertigo disappeared completely and she can go to bath room without difficulty and help. Another patient was 34 year old house wife who was admitted on August, 1964 with chief complaints of frequent fainting and progresive visual loss. She was operated only thromboendarterectomy of both common carotid arteries. Postoperative course was smooth and subjective symptoms were disappeared.

  • Congenitally corrected transposition of the great arteries is a congenital cardiac anomaly with ventriculoarterial discordant connection and atrioventricular discordant connection. This report describes a 17 year old male patient who had congenitally corrected transposition of the great arteries associated with ventricular septal defect and patent ductus arteriosus, underwent patch closure of the ventricular septal defect and suture closure of patent ductus arteriosus, and was discharged on 9th day after surgery with good condition.

  • Congenitally corrected transposition of great arteries is a congenital cardiac anomaly with ventriculoarterial discordant connection and atrioventricular discordant connection. A 8-year-old girl had congenitally corrected transposition of the great arteries with ventricular septal defect and pulmonary valvular stenosis. By transaortic approach, ventricular septal defect was closed and pulmonary valvotomy was performed by transpulmonary approach. No heart block or aortic insufficiency developed postoperatively.

  • Congenitally corrected transposition of great arteries is a rare congenital heart anomaly. We experienced two case of corrected transposition of great arteries, one [S,L,L] and one [I, D,D] associated with patent foramen ovale, ventricular septal defect and pulmonary stenosis. The patent foramen ovales were closed directly under right atriotomy, the ventricular septal defects were closed with Dacron patch under morphological left ventriculotomy and the pulmonary valvular and subvalvular stenosis were corrected under pulmonary arteriotomy. The postoperatively course was uneventful in case I, the permanent pacemaker was implanted in case ll.

  • In order to evaluate the effect of Polygoni Multiflori Radix on the atherosclerosis induced by high cholesteol diet, rabbits were fed with 4% cholesterol diet, 4% cholesterol-4% Polygoni Multiflori Radix diet during 12 weeks. After 12 weeks, histological changes in coronary artery of rabbits were studied. The results were obtained as follows; 1. At the area % of lumen in coronary arteries, Polygoni Multiflori Radix group showed significant inhibitory effect against decrease of area % in control group. 2. At the atheroma formation in coronary arteries, Polygoni Multiflori Radix group showed decrease effect as compared with control group. 3. At the mucoprotein formation in wall of coronary arteries, Polygoni Multiflori Radix group showed decrease effect as compared with control group. 4. At the lesion of elastic fibers in coronary arteries, Polygoni Multiflori Radix group didn't show effective changes as compared with control group.
