A Case Report of Nerve Entrapment Syndrome with Lymphedema (림프부종에 의한 신경포착증후군: 증례 보고)

  • Purpose: One of the most common cause of upper extremity lymphedema is breast cancer surgery. We experienced the nerve entrapment syndrome which was associated with postmastectomy lymphedema. To the best of our knowledge, this is the first case report of lymphedema induced nerve entrapment syndrome on upper extremity in Korea. Methods: A 54-year-old woman presented with a tingling sensation on her right hand, which had been present for 1 year. On her history, she had a postmastectomy lymphedema on her right upper extremity for 20 years. Initial electromyography (EMG) showed that the ampulitude of the median, ulnar, and dorsal ulnar cutaneous nerve were decreased, and conduction block was also seen in median nerve across the wrist. In needle EMG, incomplete interference patterns were observed in the muscles innervated by median and ulnar nerves. In conclusion, electrophysiologic study and clinical findings suggested right median and ulnar neuropathy below the elbow. Therefore, we performed surgical procedures, which were release of carpal tunnel, Guyon's canal, and cubital tunnel. Results: The postoperative course was uneventful until the first two years. The tingling sensation and claw hand deformity were improved, however, the motor function decreased progressively. In 7 years after the operation, patient could not flex her wrist and thumb sufficiently. EMG which was performed recently showed that ulnar motor response was of low ampulitude. Moreover, median, ulnar, dorsal ulnar cutaneous, lateral antecubital cutaneous and median antebrachial cutaneous sensory response were unobtainable. Abnormal spontaneous activities were observed in upper arm muscles. In conclusion, multiple neuropathies were eventually developed at above elbow level. Conclusion: On treating nerve entrapments associated with lymphedema, medical professionals should be fully aware of the possibility of unpredictable results after the surgery, because of the pathophysiologic traits of chronic lymphedema.

Arthroscopic Subacromial Decompression for Chronic Impingement (견관절 만성 충돌 증후군의 관절경적 견봉하 감압술)

  • The purpose of this study was to assess the results of arthroscopic subacromial decompression in patients with chronic impingement and to evaluate the results according to the rotator cuff pathology. We evaluated the clinical results of treatment for chronic impingement syndrome in 28 patients from Feb 1996 to Feb 1997. There were twenty men and eight women in age from 24 to 72 years (mean age 51) with dominant arm involvement in sixteen patients. Follow up evaluations averaged 15(range 12-24)months. The average duration of symptoms were 15(range 6­60)months. The final diagnoses which were based on the physical examination, plain radiographs and arthroscopic findings, were stage II impingement in 16 patients and stage ill impingement in 12 patients. We excluded the patients with acromioclavicular arthritis or glenohumeral instability in this study. All patients were managed non-operatively a minimum of six months. During the operation we performed contouring and smoothing the acromial undersurface and only resecting of the anterolateral band of the coracoacromial ligament. The clinical results were quantitated using UCLA shoulder rating score. Satisfactory results were obtained in 23(80%) patients. Unsatisfactory results were obtained in 5(18%) patients with posterior cuff tear. The average UCLA pain score showed significant improvement from 2.8(constant pain) to 7.2(present during heavy activities) at final follow up. The function and active forward flexion scores also increased from their preoperative value. There was no significant differences according to the surface and severity of tear and NeeI' stage (P>0.05). These results compared favorably with those reported following open acromioplasty. While arthroscopic subacromial decompression is a demanding technique with a learning curve, it is a reliable treatment for chronic impingement syndrome. A less aggressive approach to subacromial decompression and preserving the posteromedial band of the coracoacromialligament does not appear to compromise results.

A Case Report of Acute Hepatitis B (급성 B형간염 환자에 대한 치험 1례(例))

  • A 36 year-old female patient with acute hepatitis B was treated with modified Chunggangunbi-tang. The patient complained of right arm and numbness, chest pain and discomfort, pain in the right chest, fever, athalpia, indigestion, fatigue, pruritus, jaundice and other minor problems. Two weeks later, symptoms has gone from severe to mild and transaminase level was lower. The medicine was prescribed for four more weeks and symptoms disappeared. The transaminase level fell to within normal range with no side effect. The Chunggangunbi-tang showed desirable effect on indigestion and more rapid recovery of liver function than previous reports on treatment for hepatitis. Finally results from clinicopathological examinations(about AST, ALT, total bilirubin, direct bilirubin, etc.) were promising. o we hope that this clinical study is helpful in treating patients with hepatic disease. Result suggests that oriental medical therapy is useful in treating acute hepatitis type B. More study and development of approach and application of this treatment for acute hepatitis type B are necessary.

A Study on the Development of Dance Sportswear with Cool-touch Function (냉감 기능성 댄스스포츠 웨어 개발에 관한 연구)

  • This study helps develop cool-touch functional dance sportswear. We suggest a draft design for dance sportswear that chooses appropriate cool-touch functional materials based on an investigation of the changes of body surface temperature before and after exercise, the physical properties of cool-touch materials on the market, and the preference for cooling tools. The results are as follows. First, cool-touch functional sportswear products on the market utilize materials such as PCM, Delta fabric, high gauge fabric, and ice chips as well as incorporate functions such as UV block and eyelets for enhanced breathability. Polyester and polyurethane fibers are mainly used for cool-touch functional sportswear. Second, the neck area showed the highest surface temperatures (32.7℃ and 32.1℃) before and after exercise. Body surface temperatures measured after exercise were also lower than temperatures measured before exercise when wearing dance sportswear. Third, as for the physical properties of cool-touch materials, material 1 showed amaximum drying speed (130 min), material 3 the best moisture absorption speed (122 × 132 min), and material 4 the best thermal conductivity (0.013 7 w/m·K). Fourth, a draft design for a cool-touch functional dance sportswear was suggested, including a neckband made of removable soft PVC material on the neck area and applying material 4 in F1, B4, S2 and lower arm areas and material 1 in the armpit area. Deodorant tape was also attached to the armpit area for added comfort and antibacterial deodorant effect.

The Treatment for Mandibular Condyle Fracture of Children by a Threaded Kirshcner Wire and External Rubber Traction (Threaded Kirschner Wire와 외부 고무줄 견인을 통한 소아 하악골 관절돌기 골절의 치료)

  • Purpose: The treatment of children mandibular condyle fracture that is severely displaced is controversial. The conservative treatment of it may lead to complications - mandibular deficiency, asymmetry, malocclusion and temporomandibular joint dysfunction. Moreover, open reduction carries risks for growth retardation, facial nerve injury, scarring and joint stiffness. The aim of this article is to present an alternative technique of the treatment by using a threaded Kirschner wire and external rubber traction. Materials: From November 2005 to May 2008, three patients underwent the management by using a threaded Kirschner wire and external rubber traction. A threaded Kirschner wire was inserted in the condylar segment by using a C-arm. We applied the external rubber traction, and we reducted the segment progressively until complete reduction. The mandibular - maxillary fixations were removed after 3 weeks, and patients went into training for mouth opening. Results: The technique didn't result in complications - joint dysfunction, facial nerve injury, sore, infection and nonunion during follow - up period. Radiologic follow - up examinations revealed correct reduction in all patients. In all cases, we found restoration of preinjury occlusion and temporomandibular joint function. Conclusions: Closed reduction of children mandibular condyle fracture by using a threaded Kirschner wire and external rubber traction did achieve anatomic reduction and restore mandibular height. This alternative technique is simple, effective, inexpensive, easy to apply and minimally invasive.

The Incidence of Venous Thromboembolism Following Shoulder Surgery: A Pilot Study

  • Background: To assess the incidence of venous thromboembolism (VTE) following shoulder surgery and to evaluate the role of postoperative duplex ultrasonography. Methods: The study comprised a total of 224 patients who underwent shoulder surgery, including 180 shoulder arthroscopic surgeries, 28 shoulder arthroplasties, and 16 plate fixations for proximal humerus fracture between January 2014 and December 2014. The mean age of patients was 59.0 years, and there were 81 men and 143 women. Clinical data, including body mass index, blood tests, metabolic work-up for liver and renal function, previous, and present medical history, were evaluated. Duplex ultrasonography in the operative arm was performed on 2 to 4 days after surgery. Results: The overall incidence of VTE following shoulder surgery was 0.45% (1/224). One patient with open reduction and plate fixation for proximal humerus fracture had asymptomatic deep vein thrombosis that showed complete remission after anticoagulant medication during a 2-month period. Four patients had asymptomatic superficial cephalic vein thrombosis and complete remission without any treatment. There was no case of pulmonary embolism. Conclusions: The incidence of VTE following shoulder surgery was extremely low in Asians. Duplex ultrasonography may be not considered a routine follow-up of shoulder surgery and can be selectively performed in high-risk or symptomatic patients for VTE.

Effects of Exercise Program on Pain, Functional Level and Posture Characteristics for Professional Orchestra High String Musicians with Shoulder Pain: Case Series (어깨통증을 가진 전문 오케스트라 현악기 연주자에게 적용한 운동프로그램이 통증과 기능수준, 자세 특성에 미치는 효과: 사례군 연구)

  • Background: High string instrument (violin, viola) players are at high risk for developing musculoskeletal problems. In particular, a higher prevalence of shoulder problems has been reported. However, evidence of the effectiveness of therapeutic exercise for musicians with this condition is lacking until now. Objects: This case series study investigated the effect of a specific exercise program on shoulder pain, dysfunction, and posture of professional orchestral high string musicians. Methods: Five professional orchestral high string players with shoulder pain participated in this study. A six-week specific exercise program focus on scapular stability for shoulder pain consisted of scapular muscle exercise. Physical examinations for pain and dysfunction, posture were performed, were identified among all subjects before starting the exercise program. Results: The results revealed that shoulder pain decreased by 67.86% and the effect of pain on instrument play decreased by 63.33%. The shoulder pain and disability Index and the performing arts module-quick disability of the arm, shoulder and hand scale decreased by 71.72% and 51.61% respectively. The exercise program improved function and posture. Conclusion: A specific exercise program for high string players was effective in managing shoulder pain. This case series is intended to provide empirical data on the effectiveness of an exercise program aimed at assessing the effect of exercise on pain, dysfunction, and posture of professional orchestra musicians associated with specific instrument groups.

  • In this paper, a constant frequency phase shifting PWM controlled voltage source full bridge-type series load resonant high-frequency inverter using the IGBT power modules is presented for innovative consumer electromagnetic induction heating applications such as a hot water producer, steamer and super heated steamer. The full bridge arm side link passive quasi-resonant capacitor snubbers in parallel with the each power semiconductor device and high frequency AC load side linked active edge inductive snubber-assisted series load resonant tank soft switching inverter with a constant frequency phase shifted PWM control scheme is discussed and evaluated on the basis of the simulation and experimental results. It is proved from a practical point of view that the series load resonant and edge resonant hybrid high-frequency soft switching PWM inverter topology, what is called class DE type. including the variable-power variable-frequency(VPVF) regulation function can expand zero voltage soft switching commutation range even under low output power setting ranges, which is more suitable and acceptable for induction heated dual packs fluid heater developed newly for consumer power utilizations. Furthermore, even in the lower output power regulation mode of this high-frequency load resonant tank high frequency inverter circuit it is verified that this inverter can achieve ZVS with the aid of the single auxiliary inductor snubber.

Appropriate Ease of Men's Snowboard Upper Wear (남성 스노보드 상의의 여유량 설정에 관한 연구)

  • The purpose of this study were to develop snow-board upper wear with improved functionality. The study consisted of figure out appropriate ease amount of snow-board upper wear and make products by applying it to pattern design. The results of this study were as follows : First, The results of examined the difference between naked body measurements of and basic wear. According to motion, chest circumference(4.7cm, 5.1%), back width(8.9cm, 20.7%), and waist back length(8.0cm, 18.3%) increased. Around the elbow the length increased by 4.4cm(21.9%) when it was bent 90 degrees, while it increased by 6.0cm(29.8%) when it was bent to the full(145 degree). Second, 3 snow-board upper wear were selected and evaluated their appearance, comforts, and functionality, and that of the biggest margin proved to be the best. Eases necessary for motion proved to be insufficient especially in neck, arm and wrist parts of upper clothing. Third, on the basis of the analysis of snow-board upper wear on market, 2 pieces of experimental snowboard upper wear were designed, produced, and evaluated the patterns of with different ease, and snowboard upper wear B for experiment was proved to be better, Fourth, Snowboard upper wear was produced and evaluated for experiment 2 by giving additional ease to specific parts. Therefore experiment 2 was presented pattern, ease, and size of snowboard upper wear for with snowboard upper wear with improved functionality.

Study of Tolerance Suitability of Door Operation Mechanism on Mobile Air Handing Unit Using 3-DCS Analysis (3-DCS를 이용한 자동차 공기 분배장치의 도아 구동 기구의 공차 적합성 분석에 대한 연구)

  • Recent automakers are trying to be more precise with the dimension check and moving parts to guarantee high quality and satisfy customer requirements. The aim of this paper is to investigate the design tolerance suitability of door operating mechanism linked arms, lever, and cam-shaft in a mobile air handling unit. These parts are complicated because doors, arms, lever and cam-shaft are connected nonlinearly in 3D. The current tolerance analysis method poses problems in design analysis because the moving doors are reasonably suitable for the AHU function. The 3-DCS analysis method provided useful results not only in establishing the inspection criteria for the quality control of products but also in enabling economical production. As a result, the vent door had $1.62^{\circ}{\sim}1.72^{\circ}$ and the defrost door had $0.84^{\circ}{\sim}0.9^{\circ}$ for the directly connected arms operating-type. For the lever connected arm operating-type, the foot door had $2.0^{\circ}{\sim}2.24^{\circ}$ tolerance, while the tolerance values satisfied the air flow volume distribution rate criteria in the AHU. Finally, the results have confirmed the design's tolerance suitability by using 3-DCS analysis at the early design stages. Reliability can be achieved by analyzing accumulated tolerance during the sub-parts assembly process and the moving mechanism linked especially by arms, lever, and cam-shaft.
