A Rare Case of Primary Thymic Adenocarcinoma Mimicking Small Cell Lung Cancer

  • Primary thymic adenocarcinoma is a very rare malignancy of the anterior mediastinum with no standardized treatment. A 36-year-old male patient presented with hoarseness over the past 3 months. A chest computed tomography (CT) scan showed an infiltrative mass to the proximal vessels and aortic arch in left upper mediastinum ($4.1{\times}3.1{\times}5.4cm$). Brain magnetic resonance imaging (MRI) showed focal lesions, suggesting metastasis in the left frontal lobe. A thoracoscopic biopsy of the mediastinal mass confirmed a primary thymic adenocarcinoma forming a glandular structure with atypia of tumor cells. The patient received four cycles of systemic chemotherapy, consisting of etoposide and cisplatin, with concurrent radiotherapy (6,000 cGy/30 fractions) to the mediastinal lesion and the metastatic brain lesion (4,200 cGy/12 fractions). A follow-up chest CT scan and brain MRI showed a decrease in the size of the left upper mediastinal mass and brain lesion. We report a rare case of the primary thymic adenocarcinoma with a literature review.

Modified- Tribar의 수리특성에 관한 실험적 연구 (Experimental Study on Hydraulic Characteristics of Wave Dissipating Modified- Tribar)

  • Specially shaped concrete blocks are used for the armor layer of rubble structure for breakers, seawalls, or other shore protection work. In this study, the hydraulic characteristics of the Modified-Tribar(MTB), which addresses the shortcomings of the Arch-Tribar, and the most widely used Tetrapod(TTP) in Korea are examined through hydraulic model tests. The MTB are much more stable than the TTP, as shown through the stability model tests under non-breaking and non-overtopping condition. The value of the stability coefficient(KD) was obtained at around 30. The model tests show that the TTP random two layers and MTB uniform 1.5 layers have similar effects, but the MTB one layer shows slightly low effects in dissipating wave energy. The TTP random two layer model is the most effective in reducing wave overtopping rate, under overtopping condition, while the MTB uniform one layer and the MTB uniform 1.5 layer models follow respectively.

화재에 노출된 단층 돔의 후좌굴 거동에 관한 연구 (A Study on the Post-buckling Behaviour of Single-layer Domes exposed to Fire)

  • The lightweight structures such as domes are particularly vulnerable when it has been subjected to high temperature induced by the fire. It is therefore crucial to predict the possible instability path of structures exposed to the fire in structural design process. In this study, the instabilities of single-layer domes is investigated by using finite element technologies with the consideration of high temperature. The material properties of members under high temperature are considered by using the reduction factors which is provided in Eurocodes 3. Some damage patterns are assumed with use of a structural unit which is symmetric in radial direction. For numerical evaluations, the geometrically nonlinear truss element is implemented and the arch-length control method is employed to trace the post-buckling behaviour of domes. From numerical results, it is found to be that a significant change of post-buckling behaviour is detected in dome structures when structural members are exposed to the fire.

조선해양기자재 제작을 위한 SCM 기반 실시간 데이터 인터페이스 기술에 관한 연구 (A Study on the Real-time Data Interface Technology based on SCM for Shipbuilding and Marine Equipment Production)

  • The production and procurement of shipbuilding and offshore equipment is an important competitive factor in the shipbuilding and offshore industry. Recently, ICT-based digital technology has been rapidly applied to the manufacturing industry following the Fourth Industrial Revolution. Under the digital transformation, real-time data interface technology based on SCM (Supply Chain Management) is emerging as an important tool to improve the efficiency of the equipment manufacturing process. In this study, the characteristics and advantages and disadvantages of interface technologies of web-based data interface technologies were compared and analyzed. The performance was compared between theoretical evaluation based on technical features and practical application cases. As a result, it was confirmed that GraphQL is useful for selective data processing, but there is a problem with optimization, and REST API has a problem with receiving data due to a fixed data structure. Therefore, this study aims to suggest ways to utilize and optimize these data interface technologies.

열처리가 Elgiloy 선재의 기계적 성질에 미치는 영향 (THE EFFECTS OF HEAT TREATMENT ON THE MECHANICAL PROPERTIES OF THE ELGILOY WIRE)

  • Heat treatment which removes internal stress enhances the mechanical properties of the orthodontic arch wire. The main purpose of this experiment was to investigate the effects of the heat treatment on the mechanical properties of the Elgiloy wire. The Elfiloy wire, 0.016' X 0.022' and 0.018' X 0.025', were heat treated in an electric oven for 5, 10 and 15 minutes at selected temperatures between 300 and $900^{\circ}C$. Tensile strength and load deflection rate were measured to reveal the changes of mechanical property at various conditions, and each specimen was observed under metallurgic microscope. Also to trace the precipitation material due to overheat treatment, a qualitative analysis was carried out with EDS system. It was found that heat treatment at a low temperature caused an increase in the tensile strength and bending resistance, and a maintenance in the fibrous in the tensile strength and bending resistance, and a maintenance in the fibrous structure of both sizes of wire. The changes observed in properties and appearance were probably due to the relief of internal stresses incurred in the metal during cold working. In both sizes of wire the tensile strength and the bending resistance continued to decrease at high temperature, and the fibrous structure continued to disappear then was not observed at $900^{\circ}C$. The carbide precipitation founded in grain boundary at $750^{\circ}C$ probably was other elements carbide (Ni, Co) except Cr. The grain growth was observed at $1100^{\circ}C$. Optimum heat treatment for the 0.016' X 0.022' Elgiloy wire was 10 minutes at $500^{\circ}C$, and for the 0.018' X 0.025' Elgiloy wire it was 5 to 15 minutes at $500^{\circ}C$.

Position Change of the Neurovascular Structures around the Carpal Tunnel with Dynamic Wrist Motion

  • Objective : The purpose of this study was to determine the anatomic relationships between neurovascular structures and the transverse carpal ligament so as to avoid complications during endoscopic carpal tunnel surgery. Methods : Twenty-eight patients (age range, 35-69 years) with carpal tunnel syndrome were entered into the study. We examined through wrist magnetic resonance imaging in three different positions (neutral, radial flexion, and ulnar flexion) and determined several anatomic landmark (distance from the hamate hook to the median nerve, ulnar nerve, and ulnar vessel) based on the lateral margin of the hook of the hamate. The median nerve and ulnar neurovascular structure were studied with the wrist in the neutral, ulnar, and radial flexion positions. Results : The ulnar neurovascular structures usually passed just over or ulnar to the hook of the hamate. However, in 12 hands, a looped ulnar artery coursed 0.6-3.3 mm radial to the hook of the hamate and continued to the superficial palmar arch. The looped ulnar artery migrates on the ulnar side of Guyon's canal (-5.2-1.8 mm radial to the hook of the hamate) with the wrist in radial flexion. During ulnar flexion of the wrist, the ulnar artery shifts more radially beyond the hook of the hamate (-2.5-5.7 mm). Conclusion : It is appropriate to transect the ligament greater than 4 mm apart from the lateral margin of the hook of the hamate without placing the edge of the scalpel toward the ulnar side. We would also recommend not transecting the transverse carpal ligament in the ulnar flexed wrist position to protect the ulnar neurovascular structure.

터널 라이닝 구조체로서 활용을 위한 곡면 FRP-콘크리트 복합부재의 역학적 거동특성 분석 연구 (Study on mechanical behavioral characteristics of the curved FRP-concrete composite member for utilization as a tunnel lining structure)

  • 최근 장기 부식 등 일반적인 건설재료의 문제점을 보완할 수 있는 대체 재료로서, 섬유강화 복합재료의 활용이 증대되고 있다. 하지만 일반적으로 선형의 섬유강화 복합재료를 아치형인 터널구조물의 부재로서 활용하는 데는 많은 문제점이 대두된다. 본 연구에서는 FRP 복합부재의 거동특성 파악을 위한 수치해석적 사전검토 연구를 토대로, 터널 지보구조물로서 활용성 분석을 위한 FRP와 콘크리트 합성부재에 대한 하중재하 실험을 수행하였다. 또한 역학적 거동분석을 위하여 동일 조건에 대한 수치해석을 수행하였다. 하중재하 실험 및 수치해석결과, FRP와 콘크리트 합성부재는 두 부재 계면의 휨 인장에 의한 전단거동에 큰 영향을 받는 것으로 나타났다.

Study on bearing characteristic of rock mass with different structures: Physical modeling

  • In this paper, to study the stability of surrounding rock during roadway excavation in different rock mass structures, the physical model test for roadway excavation process in three types of intact rock mass, layered rock mass and massive rock mass were carried out by using the self-developed two-dimensional simulation testing system of complex underground engineering. Firstly, based on the engineering background of a deep mine in eastern China, the similar materials of the most appropriate ratio in line with the similarity theory were tested, compared and determined. Then, the physical models of four different schemes with 1000 mm (height) × 1000 mm (length) × 250 mm (width) were constructed. Finally, the roadway excavation was carried out after applying boundary conditions to the physical model by the simulation testing system. The results indicate that the supporting effect of rockbolts has a great influence on the shallow surrounding rock, and the rock mass structure can affect the overall stability of the surrounding rock. Furthermore, the failure mechanism and bearing capacity of surrounding rock were further discussed from the comparison of stress evolution characteristics, distribution of stress arch, and failure modes in different schemes.

CAD/CAM system을 이용한 하악 임플란트 피개의치 수복증례 (The implant overdenture on the edentulous mandible using CAD/CAM system: A case report)

  • 완전, 혹은 부분 무치악 환자의 가철성 보철물 수복치료에서 치아 상실에 따른 치조골 감소와 별형은 의치 사용에 어려움을 준다. 이런 문제를 극복하기 위해 임플란트를 이용하여 가철성 보철물에 지지, 유지, 및 안정을 제공할 수 있다. 임플란트를 이용한 피개의치에 사용되는 어태치먼트는 여러 종류가 있는데, 환자의 여러 조건을 고려하여 어태치먼트를 선택해야 한다. 그 중 milled bar의 경우 임플란트 간 거리가 좁아서 기성 bar를 사용할 수 없는 경우, 또는 기성 bar보다 상부 구조물의 확고한 안정성과 지지를 얻고자 할 때 선택 된다. Milled bar는 cross arch stabilization을 이룰 수 있고, 저작 시 의치의 움직임이 없어 환자에게 편안함을 제공하지만, 숙련된 기공과정을 요한다. 그러나 최근 CAD/CAM의 발전을 통해 milled bar의 제작이 간결해지고 적합성을 증진시킬 수 있었다. 본 증례는 8개의 임플란트가 식립된 하악 완전 무치악의 알츠하이머 환자로 milled bar와 magnet을 이용한 피개의치를 제작하였다. 전신병력이 양호해진 기간 동안 빠른 치료종결이 필요했기 때문에, 통법의 기공 작업을 CAD를 통해 시뮬레이션하여 복잡한 기공과정을 좀더 단순하게 하였고 이로써 치료 기간을 줄일 수 있었다. 이상과 같이 milled bar를 이용한 임플란트 지지 피개의치를 제작하는 과정 중 CAD/CAM system을 적절히 사용하여 심미적, 기능적으로 만족할 만한 결과를 얻었기에 보고하고자 한다.

리기다소나무 구조용 집성재를 활용한 아치 트러스 목조교량의 전과정평가 (Life Cycle Assessment of Timber Arch-Truss Bridge by Using Domestic Pinus rigida Glued-Laminated Timber)

  • 본 연구에서는 리기다소나무 구조용 집성재를 사용한 국내 최초 차량용 목조교량에 대한 지구온난화 영향을 평가하기 위해 전과정평가를 수행하였다. 교량연장 30 m, 교량폭원 8.4 m, 교량등급 1등급인 아치 트러스 형태의 대상 교량은 원료채취부터 제조, 수송, 시공, 사용, 해체, 건설폐기물 수송, 폐기 및 재활용까지 설계수명 50년간 총 192.56 ton $CO_2$ eq.의 온실가스를 배출한다. 전과정단계 중 원료채취 및 제조단계에서 총 온실가스 배출량의 81.14%를 배출하며 특히, 콘크리트 사용으로 인하여 82.84 ton $CO_2$ eq.의 온실가스가 배출된다. 그러나 대상 교량은 $116.57m^3$의 국산 리기다소나무 집성재를 사용하였으며, 교량을 구성하는 목재에서 104.72 ton의 이산화탄소를 저장하고 있어 이를 적용할 경우, 총 온실가스 배출량의 54.38%를 저감 가능한 것으로 도출되었다. 대상 교량과 동일한 수명과 구조를 갖는 타 교량의 철골자재를 구조용 집성재로 대체할 경우, 원료채취 및 제조단계의 온실가스 배출량을 최소 10.26%에서 최대 23.91%까지 저감 가능한 것으로 도출되었다. 본 연구의 결과는 향후 국산 목재 및 목조교량의 친환경적 우수성을 정량적으로 입증할 수 있는 기반자료로 활용 가능할 것이며, 목조교량의 친환경적 설계와 보급을 위해 활용 가능할 것으로 사료된다.
