Suitability of Measuring a Kidney Depth with Assessment of Glomerular Filtration Rateusing 99mTc-DTPA in the Ectopic Kidney and Pediatric Patients (99mTc-DTPA를 이용한 사구체여과율 검사에서 이소성 신장과 소아 환자의 신장 깊이 측정방법의 적절성)

  • A glomerular filtration rate (GFR) study is a test that uses radioactive materials or tracers (radiopharmaceuticals) and a computer to see how well the kidneys are working. Asan Medical Center analyzed and compared data between kidney depth, acquired from kidney donors' CT image and acquired from Gates method's GFR value that are calculated by Tonnesen equation. This study was able to confirm that kidney depth measured from CT image was higher than the Gates Method's GFR value, which was calculated by Tonnessen equation; the direct relationship among pathologic results is confirmed. Particularly, kidney donor whose kidney was at the pelvic area had direct relationship with other clinical results. During the GFR test, it is necessary to confirm the location of kidney has no change with reference of CT image. If kidney depth is manually corrected using CT image when we measures GFR of deformed or horse-shoe kidney, it would be possible to acquire the compatible value which is equivalent to clinical result. There would be a possible issue of appropriateness that whether the applied GFR using CT image's kidney depth has clinical validity. In case of a pediatric patient, the GFR derived from Tonnesen was quiet underestimated while manual method and Gordon stay in normal range. Which results may be correct among them? There have been many reports about kidney depth, to be an accurate index of GFR in children. As one of the study performers, we should contemplate what the best option for pediatric patients would be.

Analysis of Misconceptions on Oceanic Front and Fishing Ground in Secondary-School Science and Earth Science Textbooks (중등학교 과학 및 지구과학 교과서 조경 수역 및 어장에 관한 오개념 분석)

  • Oceanic fronts, which are areas where sea water with different properties meet in the ocean, play an important role in controlling weather and climate change through air-sea interactions and marine dynamics such as heat and momentum exchange and processes by which properties of sea water are mixed. Such oceanic fronts have long been described in secondary school textbooks with the term 'Jokyung water zone (JWC hereafter) or oceanic front', meaning areas where the different currents met, and were related to fishing grounds in the East Sea. However, higher education materials and marine scientists have not used this term for the past few decades; therefore, the appropriateness of the term needs to be analyzed to remove any misconceptions presented. This study analyzed 11 secondary school textbooks (5 middle school textbooks and 6 high school textbooks) based on the revised 2015 curriculum. A survey of 30 secondary school science teachers was also conducted to analyze their awareness of the problems. An analysis of the textbook contents related to the JWC and fishing grounds found several errors and misconceptions that did not correspond with scientific facts. Although the textbooks mainly uses the concept of the JWC to represent the meeting of cold and warm currents, it would be reasonable to replace it with the more comprehensive term 'oceanic front', which would indicate an area where different properties of sea water-such as its temperature, salinity, density, and velocity-interact. In the textbooks, seasonal changes in the fishing grounds are linked to seasonal changes in the North Korean Cold Current (NKCC), which moves southwards in winter and northwards in summer; this is the complete opposite of previous scientific knowledge, which describes it strengthening in summer. Fishing grounds are not limited to narrow coastal zones; they are widespread throughout the East Sea. The results of the survey of teachers demonstrated that this misconception has persisted for decades. This study emphasized the importance of using scientific knowledge to correct misconceptions related to the JWC, fishing grounds, and the NKCC and addressed the importance of transferring procedures to the curriculum. It is expected that the conclusions of this study will have an important role on textbook revision and teacher education in the future.

Real Option Analysis to Value Government Risk Share Liability in BTO-a Projects (손익공유형 민간투자사업의 투자위험분담 가치 산정)

  • The BTO-a projects is the types, which has a demand risk among the type of PPP projects in Korea. When demand risk is realized, private investor encounters financial difficulties due to lower revenue than its expectation and the government may also have a problem in stable infrastructure operation. In this regards, the government has applied various risk sharing policies in response to demand risk. However, the amount of government's risk sharing is the government's contingent liabilities as a result of demand uncertainty, and it fails to be quantified by the conventional NPV method of expressing in the text of the concession agreement. The purpose of this study is to estimate the value of investment risk sharing by the government considering the demand risk in the profit sharing system (BTO-a) introduced in 2015 as one of the demand risk sharing policy. The investment risk sharing will take the form of options in finance. Private investors have the right to claim subsidies from the government when their revenue declines, while the government has the obligation to pay subsidies under certain conditions. In this study, we have established a methodology for estimating the value of investment risk sharing by using the Black - Scholes option pricing model and examined the appropriateness of the results through case studies. As a result of the analysis, the value of investment risk sharing is estimated to be 12 billion won, which is about 4% of the investment cost of the private investment. In other words, it can be seen that the government will invest 12 billion won in financial support by sharing the investment risk. The option value when assuming the traffic volume risk as a random variable from the case studies is derived as an average of 12.2 billion won and a standard deviation of 3.67 billion won. As a result of the cumulative distribution, the option value of the 90% probability interval will be determined within the range of 6.9 to 18.8 billion won. The method proposed in this study is expected to help government and private investors understand the better risk analysis and economic value of better for investment risk sharing under the uncertainty of future demand.

Middle School Students' Perception of Body Image and Allowance for Plastic Surgery (중학생의 신체상 지각수준과 성형수술 허용도)

  • This study set out to investigate the relations between middle school students' perception of body image and their allowance for plastic surgery, to understand their perception of body image and desire for plastic surgery, and provide some data needed to warn against reckless plastic surgery and guide the students effectively. For those purposes, an examination was conducted of the relationships between the individual characteristics and perception levels of body image, the individual characteristics and allowance for plastic surgery, and perception levels of body image and allowance for plastic surgery. The subjects were drawn from sour middle schools located in two regions of Gyeonggi Province. Total 922 boys and girls were surveyed on a questionnaire, which was developed based on the pretest of previous literature, reviewed for appropriateness, and tested for reliability and reasonableness. The body image on the five scale was greater as the perception level was higher. The allowance for plastic surgery was also greater as the scores were more. The findings were as follows: First, the relationships between individual characteristics and perception levels of body image were examined. The third graders showed the highest perception level, being followed by the first and second graders. The girls were more perceptive than the boys, and those who were extrovert were more perceptive than those who were introvert. Those students whose parents earned 2 million won or more a month and who adapted themselves to the environmental changes had a higher perception level. In a word, the girls from the middle class that were well adapted, felt happy, and were extrovert had a higher perception level of body image. Second, the connections between individual characteristics and allowance for plastic surgery were investigated. The third graders were the most admissive of plastic surgery, followed by the second and first graders. That is, the upper graders were more admissive of plastic surgery. In addition, the girls were more admissive than the boys, and those who were extrovert were more than those who were introvert. There were no significant differences according to the monthly income of the parents, grades, adaptability to surroundings, and happiness, which results almost resembled the findings of a study conducted on adults. Third, there were negative correlations found between the perception levels of body image and the allowance for plastic surgery. To elaborate, the higher the perception levels were, the lower the allowance was, and vice versa. As for the items, the subjects showed more allowance for plastic surgery when they scored less in the item of caring about appearance, importance of looking pretty to others, and efforts to improve appearance. When they had a low value of body and easily felt tired, they were highly acceptive of plastic surgery. The allowance for plastic surgery was also great when their perception was much of how healthy they felt, how important they felt about their bodies, how they were satisfied with their current appearances, how they evaluated the appearance of others, how much they were satisfied physically, and how much demanding they were for physical changes. Meanwhile, there were no correlations between the allowance and physical attraction, the degree for one's activities to be hindered, and sickness. In short, the demand for plastic surgery was 41% for the girls and 20.2% for the boys. Just as the study on adults reported, those who had a low or negative perception of body image were more acceptive of plastic surgery. The middle school students were generally positive about their bodies with the lowest perception level at 2.91 and the highest at 3.21. Their individual allowance for plastic surgery was related to their individual body images, which were in turn affected by the mass communication, surrounding environments, and social values. Thus it's necessary for the entire society to try to improve or change the overall perception. Helping measures should be taken so that the students can form right sense of values about their bodies, avoid the obsession with appearance and appearance-based evaluation, and exercise righteous criteria against humans beings and things. In conclusions, the following suggestions were made: they need to develop such questionnaires or tools as can measure the body image of teens and fit the reality. Moreover, body image improvement programs should be more diverse and more applicable to teens. Despite the consistent reports that prove the correlations between body image and plastic surgery, there has been little effort to apply such factors as experience of the life of the disabled, volunteer activities for the disabled and at the hospitals, and others that can induce changes to body image to the body image improvement programs. In the future, comparative research should be carried out on body image and plastic surgery.

Analysis of utilization and profit for CT and MRI after implementation of insurance coverage for CT (CT 보험급여 전후의 CT 및 MRI검사의 이용량과 수익성 변화)

  • In order to analyze the shifts in the volume and profits of Computed Tomography(CT) and Magnetic Resonance Imaging(MRI) utilization for a year before and after the implementation of insurance coverage for CT, this study has been undertaken examining CT and MRI cost data from 'Y' University Hospital situated in Seoul, Korea. Following are the results of this study: 1. The medical insurance payment for CT, implemented on January 1, 1996, increased CT utilization from January 1996 to April 1996 due to low insurance premiums: however, from May 1996 the number of CT cases significantly decreased as a result of strengthened medical cost reviews and the new 'Detailed standards for approval of CT' announced near the end of April 1996 by the insurer. 2. Since the implementation of insurance coverage for CT, CT fee reduction rates for reimbursements by the insurer to the hospital were 50% and 40% for January and February, respectively, and 31% and 15% for March and April. A significant point in the lowering of the reduction rate was reached in May at 11%; furthermore, since June the reduction rate fell below the average reduction rate for reimbursements for all procedures. If the 'Detailed standards for approval of CT' had been announced before the implementation of insurance coverage for CT, CT utilization would not have been so high due to the need to meet those 'standards'. In addition, loss of hospital profits resulting from the reduction for reimbursements would not have occurred. 3. The shifts in MRI utilization showed that there was no particular change with the beginning of insurance coverage for CT, and the introduction of the 'Detailed standards for approval of CT' made MRI utilization increase because MRI is free of restrictions imposed by the insurer. 4. The relationship between CT utilization and MRI utilization showed that they were supplementary to each other before insurance coverage for CT, but that CT was substituted for MRI because of strengthened medical cost reviews after t~e beginning of insurance coverage for CT. 5. The shifts in volume by patient characteristics showed that the number of inappropriate case patients, according to the insurer's "Standards for approval", decreased more than the number of appropriate case patients after the introduction of insurance coverage for CT. Therefore, the health insurance fee schemes for CT have influenced patient care. 6. The shifts in profits from CT utilization showed a net profit decrease of 31.6%. In order to match the pre-coverage profit level, 5,471 more cases would need to be seen and productivity would need to be increased by 32.7%. This profit decrease resulted from a decrease of CT utilization and low reimbursements. With insurance coverage, net profits from CT were 24.4%, and a margin of safety ratio was 39.6%. Because of the net profits and margin of safety ratio, CT utilization fees for insured appropriate cases could not be considered inappropriate. 7. The shifts in profits from MRI utilization before and after the introduction of CT coverage showed that in order to match pre-CT coverage profit levels, 2,011 more cases would need to be seen and productivity would need to be increased by 9.2%. The reasons for needing to increase the number of cases and productivity result from cost burdens created by adding new MRI units. But with CT coverage already begun, MRI utilization increased. Combined with a minor increase in the MRI fee schedule, MRI utilization showed a net profit increase of 18.5%. Net profits of 62.8% and a 'margin of safety ratio' of 43.1% for MRI utilization showed that the hospital relied on this non-covered procedure for profits. 8. The shifts in profits from CT and MRI utilization showed the net profits from CT decreased by 2.33billion Won while the net profits from MRI increased by 815.7million Won. Overall, these two together showed a net profit decrease of 1.51billion Won. The shifts in utilization showed a functional substitutionary relationship, but the shifts in profits did not show a substitutionary relationship. From these results, We can conclude that if insurance is to be expanded to include previously uncovered procedures using expensive medical equipment, detailed standards should be prepared in advance. The decrease in profits from the shifts in coverage and changes in fees is a difficult burden that should be shared, not carried by the hospital alone. Also, a new or improved fee schedule system should include revised standards between items listed and the appropriateness of the fee schedule should constantly be ensured. This study focused on one university hospital in Seoul and is therefore limited in general applicability. But it is valuable for considering current issues and problems, such as the influence of CT coverage on hospital management. Future studies will hopefully expand the scope of the issues considered here.

A Study on the Calculation of Evapotranspiration Crop Coefficient in the Cheongmi-cheon Paddy Field (청미천 논지에서의 증발산량 작물계수 산정에 관한 연구)

  • In this study, crop coefficients were calculated in two different methods and the results were evaluated. In the first method, appropriateness of GLDAS-based evapotranspiration was evaluated by comparing it with observed data of Cheongmi-cheon (CMC) Flux tower. Then, crop coefficient was calculated by dividing actual evapotranspiration with potential evapotranspiration that derived from GLDAS. In the second method, crop coefficient was determined by using MLR (Multiple Linear Regression) analysis with vegetation index (NDVI, EVI, LAI and SAVI) derived from MODIS and in-situ soil moisture data observed in CMC, In comparison of two crop coefficients over the entire period, for each crop coefficient GLDAS Kc and SM&VI Kc, shows the mean value of 0.412 and 0.378, the bias of 0.031 and -0.004, the RMSE of 0.092 and 0.069, and the Index of Agree (IOA) of 0.944 and 0.958. Overall, both methods showed similar patterns with observed evapotranspiration, but the SM&VI-based method showed better results. One step further, the statistical evaluation of GLDAS Kc and SM&VI Kc in specific period was performed according to the growth phase of the crop. The result shows that GLDAS Kc was better in the early and mid-phase of the crop growth, and SM&VI Kc was better in the latter phase. This result seems to be because of reduced accuracy of MODIS sensors due to yellow dust in spring and rain clouds in summer. If the observational accuracy of the MODIS sensor is improved in subsequent study, the accuracy of the SM&VI-based method will also be improved and this method will be applicable in determining the crop coefficient of unmeasured basin or predicting the crop coefficient of a certain area.

Development of Teaching-Learning Plans Applying ARCS Motivation Strategies for Food Safety Education (ARCS 동기유발 전략을 적용한 식품 안전 교육 교수·학습 과정안 개발)

  • The purpose of this study was to develop teaching-learning plans applying ARCS strategies for food safety education, focusing on units related to the 'food selection and safe cooking'. To achieve this purpose, this study was conducted in the following order of analysis, design, development, assessment I, revision I, assessment II, revision II, and completion. In the analysis stage, 2015 revised curriculum and middle school technology-home economics textbooks(12 kinds) contents of 'food selection and safe cooking' among content elements of core concept 'safety' were analyzed. In the design and development stages, teaching-learning plans, teaching-learning materials, and evaluation rubric for teaching-learning outcomes using the ARCS motivation strategy were developed. In the phases of assessment I and revision I, evaluation items were selected as open-ended questions about food safety education factors and ARCS strategies, and their validities were verified by four experts. The teaching-learning plans for nine lessons were revised based on the feedbacks such as evaluation plans considering the correspondence between instruction and evaluation, strategy to reinforce intrinsic motivation, correction of improper contents composition, and so on. In the phases of assessment II and revision II, the validity of the three items, including relevance of each teaching-learning to food safety education, suitability of learning goals, and appropriateness of motivation strategy, was verified by seven experts. The final teaching-learning plans for 10 lessons were developed by revising and supplementing the data by compiling opinions of the assessment II. It is expected that this study can contribute to food safety education for middle school students, and that teaching-learning plans which apply ARCS strategies for food safety education will be used as good references for school teachers and curriculum developers.

A study on developing a new self-esteem measurement test adopting DAP and drafting the direction of digitalizing measurement program of DAP (청소년 자존감 DAP 인물화 검사 개발 및 디지털화 측정 시스템 방향성 연구)

  • This is to develop a new way of testing self-esteem by adopting DAP(Draw a Person) test and to make a platform to digitalize it for young people in the adolescent stage. This approach is to get high effectiveness of the self-esteem measurement using DAP test, including some personal inner situations which can be easily missed in the large statistical analysis. The other objective of this study is digitalize to recover limits of DAP test in the subjective rating standard. It is based on the distribution of the figure drawing expressed numerically by the anxiety index of Handler. For these two examinations, we made experiment through 4 stages with second grade middle school 73 students from July 30th to October 31th in 2009 during 4 months. Firstly, we executed 'Self Values Test' for all 73 people, and divided them into two groups; one is high self-esteem group of 36 people, the other is low self-esteem group of 37 people. Secondly, we regrouped them following D (Depression), Pd (Psychopathic Deviate), Sc (Schizophrenia) scales of MMPI; one is high self-esteem group of 7 people, the other is low self-esteem group of 13 people. Thirdly, we conducted DAP test separately for these 20 people. We intended to verify necessity and appropriateness of direction of 'Digitalizing Measurement System' by comparing and analyzing relation between DAP and Self-esteem following evaluation criteria which has similarity in 3 tests, after executing DAP to reflect peculiarity of adolescents sufficiently. We compared and analyzed result abstracted by sampling DAP test of two groups; One is high self-esteem group of 2 people, the other is low self-esteem group of 2 people; to confirm whether we can improve limitation that original psychological testing has by comparing mutual reliance of measurement test. Finally, with DAP test gained from correlations between self-esteem and melancholia following as above-mentioned steps, we discovered possibility of realization to get a concrete and individual criteria of evaluation based on Expert System as a way of enhancing accessibility in quantitative manner. 'Digitalizing Measurement Program' of DAP test suggested in this study promote results' reliability based on existing tests and measurement.

Flexural Test of H-Shape Members Fabricated of High-Strength Steel with Considering Local Buckling (국부좌굴을 고려한 고강도 조립 H형강 부재의 휨성능 실험)

  • Depending on the plastic deformation capacity required, structural steel design under the current codes can be classified into three categories: elastic, plastic, and seismic design. Most of the current steel codes explicitly forbid the use of a steel material with a yield strength higher than 450 MPa in the plastic design because of the concerns about its low plastic deformation capacity as well as the lack of test data on local and lateral torsional buckling behavior. In this study, flexural tests on full-scale H-shape members built with SM490A (ordinary steel or benchmark material) and HSB800 (high-strength steel) were carried out. The primary objective was to investigate the appropriateness of extrapolating the local buckling criterion of the current codes, which was originally developed for normal-strength steel, to the case of high-strength steel. All the SM490A specimens performed consistently with the current code criteria and exhibited sufficient strength and ductility. The performance of the HSB800 specimens was also very satisfactory from the strength perspective; even the specimens with a noncompact and slender flange developed the plastic moment capacity. The HSB800 specimens, however, showed an inferior plastic rotation capacity due to the premature tensile fracture of the beam bottom flange beneath the vertical stiffener at the loading point. The plastic rotation capacity that was achieved was less than 3 (or the minimum level required for a plastic design). Although the test results in this study indicate that the extrapolation of the current flange local-buckling criterion to the case of high-strength steel is conservative from the elastic design perspective, further testing together with an associated analytical study is required to identify the causes of the tensile fracture and to establish a flange slenderness criterion that is more appropriate for high-strength steel.

A Study on Evaluation Method for Structural Suitability of Constructed Wetlands in Dam Reservoirs as an Ecological Water Purification System (생태적 수질정화시설로서 댐 저수지 인공습지의 구조 적정성 평가방안)

  • Many constructed wetlands have been installed in dam reservoirs nationwide for ecological purification of watershed pollutants, but aging and reduced efficiency are becoming issues. To improve the management of constructed wetlands, an objective evaluation of structural suitability is required. This study evaluated 39 constructed wetlands of 15 dams. First, through fogus group interview(FGI), survey analysis, and analytic hierarchy process(AHP), eight evaluation items in the physical and vegetative aspects were selected and the evaluation criteria applied with weights were prepared. Second, as a result of the structural suitability evaluation, the average score of the overall constructed wetlands was 80.8, with 10 sites rated as 'good grade(91~100)', 22 sites rated as 'normal grade(71~90)' and 7 sites rated as 'poor grade(70 or less)'. The average score of physical structure evaluation was 52.6, with 14 sites rated as 'good', 21 sites as 'normal' and 4 sites as 'poor'. The suitability of location was good level in most constructed wetlands, but the water supply system, depth of water, ratio of length-to-width, and slope of flow channel were evaluated as 'normal' or less in constructed wetlands of 50% or more. Therefore, it was found that overall improvement was necessary for stable flow supply and flow improvement in the constructed wetland. The average score of vegetative structure evaluation was 28.2, and about 84% of them were identified as 'normal' or lower. As a result of analyzing the Spearman's correlation coefficient between the physical structure evaluation score and the vegetation structure evaluation score, there was a significant correlation(r = 0.728, p < 0.001), and it was found that each evaluation factor also influences each other. As a result of the case review of 6 constructed wetlands, the appropriateness of the evaluation results was confirmed, and it was found that the location, flow rate supply, and type of wetland had a great influence on the efficiency and operation of the wetland. Through this study, it will be possible to derive structural weaknesses of constructed wetlands in dam reservoirs as a nature-based solution, to prepare types and practical alternatives for improved management of each constructed wetland in the future, and to contribute to enhancing various environmental functions.
