  • Because of increasing demand for safety of the power transformer, the seismic qualification process in accordance with the standard of IEEE Std 693-1997 is essentially required by customer. Dynamic analysis method and static analysis method were used to qualify the seismic withstanding of the power transformer at high seismic level. Maximum stress was detected at the connection between the main structure and appendages, and maximum displacement was detected at the point of appendage's end tip. Load path and substructure system can be considered as important elements to prevent over stress and over displacement.

  • Container vessels are required to have a large KMT to load many containers which requires a wide transom stern form. The wide transom stern generates large stern waves particularly at the scantling draft. This means that reducing the stern wave leads to resistance reduction. Numerical analyses and Model tests for duck-tail of the stern part have been performed to reduce the resistance of the container vessel having the wide transom on the scantling draft and optimize the form of duck-tail with the change of the design parameter i.e. length and edge height. The optimized duck-tail increases the speed by 0.8 % at scantling draft.

Total Correction of Mixed Type Total Anomalous Pulmonary Venous Return 1 Case (혼합형 총폐정맥환류이상 수술치험 1례)

  • Total anomalous pulmonary venous return (TAPVR) is very rare congenital heart disease. 25-year old male was admitted our hospital with dyspnea, headache and syncope as chief complaint. He was confirmed as mixed type TAPVR by echocardiography and cardiac catheterization. In this case, mixed type TAPVR was consisted with supracardiac type connection of left pulmonary vein and cardiac type of right pulmonary vein. Supracardiac type of left pulmonary common channel was anastomosed to the left auricular appendage during total cardiopulmonary bypass with fibrillating heart. Cardiac type of right pulmonary vein was operated during moderate hypothermia and aortic cross clamping. Coronary sinus septum was incised into ASD and closed with Gore-Tex patch so that right pulmonary blood flow directed to the left atrium. The patient's post-operative course was uneventful.

  • The pressure drop of twelve fuel bundle string in the CANDU-6 fuel channel is equal to the sum of the eleven junction pressure losses, the bundle string entrance and exit pressure losses, the skin friction pressure loss, and other appendage pressure losses, where the junction loss is dependent on the bundle end faces and angular alignments of the junctions. The results of the single junction pressure drop tests in a short rig show that the most probable pressure drop of the eleven junctions was analytically equal to the eleven times of average pressure drop of all the possible single junction pressure drops, and also that the largest and smallest junction pressure drops across the eleven junctions probably occurred only with BA and BB type junctions, respectively, where A and B denote the bundle end sides with an end-plates on which a company monogram is stamped and unstamped, respectively.

  • Tricuspid atresia is the third most common cause of cyanotic heart disease, following T.O.F. and T.G.A. It is seen in about 30% of patients with congenital heart disease on postmortem examination. Recently, we experienced a case of tricuspid atresia, who had received Blalock-Taussig Shunt operation 10 years ago. The Patient was 15 year-old girl with the complaints of persistent cyanosis and exertional dyspnea. Cineangiography revealed Keith type lib tricuspid atresia, so, RA appendage was anastomosed to the right pulmonary artery and ASD was also closed using patch. Postoperative course was very difficult because of persistent right heart failure and weak respiratory power, but from postoperative 15th day, all cardiac and respiratory problems were resolved. We followed up this patient for about 1 year, and her condition is excellent up to now.

Successful Surgical Correction of Tricuspid Atresia with Complete Transposition of Great Arteries [S.D.D.]. (Modified Fontan 씨 수술법을 이용한 삼첨판막 폐쇄증과 완전대혈관전위증)

  • A 18-year-old female underwent surgical correction of tricuspid atresia and complete transposition of great arteries combined with atrial 8 ventricular septal defect and pulmonary stenosis. After the transection of main pulmonary artery just above the pulmonic valve, proximal portion of main pulmonary artery was closed with running suture and distal portion of main pulmonary artery anastomosed with right atrial appendage without valve insertion. Atrial septal defect was closed with running suture. Postoperative course was uneventful and she discharged on 18th postoperative day. Her condition is in very good until present. Modified Fontan`s operation without valve placement [in the condition of low pulmonary vascular resistance and good left ventricular function] may has a good result.

Report of Eight Cases of Cor Triatriatum (삼중방심 치험 8례보고)

  • Cor triatriatum is a rare congenital heart disease that is often lethal in children if not correctly identified and properly managed, Characteristically an anomalous membrane divides the left atrium into two chambers one located posterosuperiorly which is connected to the common pulmonary venous trunk and the other anteroinferiorly which is connected to the left atrial appendage and the mitral valve. Eight patients with Cor triatriatum were been seen at our hospital from 1984 to 1999. The clinical presentation diagnostic evaluation and surgical results are outlined in this retrospective review. Resection of the obstructing anomalous atrial membrane was performed using a hypothermic cardiopulmonary bypass in all cases. Right atriotomy was performed in all patients and left atriotomy was performed in a patient who had poor preoperative general conditions and serious cardiac defects(TAPVR & hypoplastic left ventricle) died of low cardiac output during the immediate postoperative period. The postoperative course has been excellent in the remaining. Cor triatriatum is amenable to surgical repair with excellent results when diagnosed early and in those who are not complicated by other complex cardiac anomalies.

Some practical design aspects of appendages for passenger vessels

  • The hydrodynamic effect of appendages for high-speed passenger vessels, such as Ro-Pax, Ro-Ro and cruiser vessels, is very severe and, therefore, it is essential to carry out the design of appendages for high-speed passenger vessels from the preliminary design stage to the final detail design stage through a full survey of the reference vessels together with sufficient technical investigation. Otherwise, many problems would be caused by mismatches between the appendages and the hull form. This paper investigates the design characteristics of some appendages, such as the side thruster, the shaft-strut, and the stern wedge, based on the design experience accumulated at Samsung, on CFD, and on model test results for high-speed passenger vessels. Further to this investigation, some practical and valuable design guidelines for such appendages are suggested.

  • Tumor of follicular infundibulum (TFI) is a rare benign cutaneous appendage tumor that does not have characteristic clinical features. It is mainly present in the head, neck, and trunk as a solitary lesion. In particular, TFI typically manifests as a plate-like proliferation with multiple thin epidermal connections comprise of monomorphic cells. TFI do not represent cutaneous characteristics, but have clinical significance because TFI is associated with basal cell carcinoma and Cowden's syndrome. We report a case of TFI in parietal scalp with a review of literatures.
