Injection technique에 의한 retroseal의 변연폐쇄능에 관한 실험적 연구 (A COMPARATIVE ANALYSIS BETWEEN INJECTION-MOLDED THERMOPLASTICIZED GUTTA-PERCHA AND SILVER AMALGAM AS RETROSEAL)

  • The purpose of these study was to compare the sealing ability of a injection-molded thermoplasticized gutta-percha and silver amalgam as retroseal material in vitro. Sixty two upper and lower extracted human teeth with single root were randomly selected and instrumented in a conventional method with H-file. After instrumentation the root canal was obturated with gutta-percha by lateral condensation technique with AH26 and an apicoectomy was performed by beveling the root tip 45 angle. In the experimental group 1 and 2, a class I preparation was made and filled with silver amalgam or gutta-percha and in the experimental group 3 an apicoectomy only was performed. All specimens were immersed in black Indian ink, decalcified and cleared. The depth of dye penetration into the canals were evaluated by califer. The results were as follows ; 1. The experimental group 1 displayed the smallest mean dye penetration as 0.45mm. 2. The experimental group 3 displayed the greatest mean dye penetration as 0.65mm. 3. There was the difference in dye penetration between each group, but the difference was not statistically significant(P>0.05).

  • This type of neuropathic pain(atypical odontalgia) is seen most often in middle-aged women or men after dentoalveolar operation. Atypical odontalgia probably is caused by deafferentation leading to intraneural changes in the medullary dorsal horn. Treatment of this problem is difficult, but some success has been reported in uncontrolled, open-labeled studies using high doses of tricyclic antidepressants. This is the management report of a patient case, that had a neuropathic atypical odontalgia recognized with the right maxillary lateral incisor. The patient was consulted to the Department of Pain Clinics, ENT & Neurology and diagnosed the adenoid cystic carcinoma on left cerebellum and right paranasal sinus with extension to the cavernous sinus. In spite of the osteoplastic craniotomy, neurosurgical mass removal and radiation therapy were done with chemotherapy, atypical odontalgia was continued. In addition to the consultation to Psychology, stress management and antidepressant medication were done and improved slowly.

  • This study was undertaken to access the effect of toothash combined with plaster of Paris in the filling of jaw defect and the substitution as new bone during the follow up period. We used the toothash and plaster after the cyst enucleation, the apicoectomy, the extraction of supenumerary tooth with ratio of 2 : 1 by weigh. 15 consecutive patients were evaluated retrospectively. Complications were swelling, perforation, infection and treated without problems using incision & drainage, aspiration, antibiotic treatment, 2ndary buccal flap. The follow-up period ranged from 28 to 35 months. Based on radiographic and clinical observation, it may be concluded that toothash and dental plaster of Paris($CaSo_4\;{\cdot}\;1/2H_2O$) are useful for bone substitute.

  • The bilateral periodontal cysts in the 36 years old korean male's upper both anterior regions between the central and lateral incisors have been observed. The authors have treated the bilateral periodontal cysts by means of the apicoectomies of the involved both central and lateral incisors.

치근단절제술 실패의 일례 (A FAILED CASE OF ROOT RESECTION)

  • The author experienced a failed ase of root resection which qas performed on the abnormal upper right lateral incisor. The patient was 16year old female. Clinically the effected tooth had neither a carious cavity nor periondontal diseases, ut, a fistula was present on the gum tissue near the root apex. The roentogenogram revealed a pathological area around the apex of the tooth. The results of failed apicoectomy lead to following conclusions. 1. The operated tooth was a case of dense invaginatus which had two pulp cavities and two canals seperated along their entire length. 2. It was the main cause of failure that the second pulp cavity and root canal were not obturated.

Progression of periapical cystic lesion after incomplete endodontic treatment

  • We report a case of large radicular cyst progression related to endodontic origin to emphasize proper intervention and follow-up for endodontic pathosis. A 25 yr old man presented with an endodontically treated molar with radiolucency. He denied any intervention because of a lack of discomfort. Five years later, the patient returned. The previous periapical lesion had drastically enlarged and involved two adjacent teeth. Cystic lesion removal and apicoectomy were performed on the tooth. Histopathological analysis revealed that the lesion was an inflammatory radicular cyst. The patient did not report any discomfort except for moderate swelling 3 days after the surgical procedure. Although the patient had been asymptomatic, close follow-ups are critical to determine if any periapical lesions persist after root canal treatment.

Retrospective Study of Cysts in the Oral and Maxillofacial Regions: Statistical and Clinical Analysis

  • Purpose: This study was designed for identification of the main clinicopathological features of cysts in the oral and maxillofacial regions. Methods: A retrospective observational study was conducted on 164 patients who had been diagnosed with cyst of the jaw, from the database of 168 histopathological diagnoses at the Department of Oral and Maxillofacial Surgery of Busan Paik Hospital at Inje University, from January 2009 to December 2011. The subjects were treated and the following variables were recorded: gender, age, clinical signs and symptoms, histopathological distribution, treatment methods, and complications. A descriptive analysis of the study variables was performed using a chi-square test. Results: Among the 164 patients, there were more male than female patients (male-female ratio: 1.7:1). The most predominant ages were the 20s and 40s. Among the classes of pathological cysts, radicular cysts and dentigerous cysts were the most common, with incidences of 56.0% and 35.1%. Thirty-six percent of the patients had no symptoms; and of those who had symptoms, the main signs and symptoms were swelling (24.4%) and pain (17.1%). The most frequent management method was the combination operation, such as enucleation with or without extraction and apicoectomy of the causative teeth. Of the 164 patients, 13 had complications; and one patient who had been treated with enucleation with apicoectomy had a recurrent cyst. Conclusion: Using a chi-square test, no significant differences in prevalence were observed in relation to gender according to age. Comparative analysis of radicular and dentigerous cysts showed a significant difference in their prevalence according to their anatomical location, however, no significant differences in were observed in their incidence rates according to age.

치근단 폐쇄방법에 따른 변연누출에 관한 실험적 연구 (A STUDY OF THE APICAL LEAKAGE OF VARIOUS RETROGRADE TECHNIQUES)

  • This study was conducted to evaluate and compare the apical leakage in the following retrograde techniques; apicoectomys, retrograde filling with zinc oxide eugenol cement, non-zinc and zinc amalgam, and cold-burnished gutta-percha method. One hundred twenty five upper anterior and lower premolar teeth were divided into five above mentioned groups and each tooth was individually prepared for its particular group. The specimens were incubated at $37^{\circ}C$ for 14 days and then were infiltrated by 2% methylene blue for 24 hours. The apical leakage was evaluated by measuring the degree of dye penetration between the filling material and the canal wall. The results were as follows: 1. The cold-burnished gutta-percha group showed the least amount of apical leakage and the apicoectomy group showed the greastest amount of apical leakage. 2. Statistics showed that there were significant differences among the groups (P<0.05), however there was no difference between the zinc oxide eugenol cement retrograde filling group and the non-zinc amalgam retrograde filling group (P>0.05).

  • When conventional root canal treatment is failed or contraindicated, retrograde root canal filling following apicoectomy is a valuable procedure, aimed at hermetically sealing the root canal against leakage of irritants from the canal into the periapical tissue. In this in vitro investigation, to analyze apical microleakage electrochemically in teeth with different retrograde filling materials and preparation types, single - rooted tooth was cut 2mm from the apex and each Class I and Slot preparation was prepared. Experimental groups : Group 1. Amalgam filling with cavity varnish in Class I preparation Group 2. Scotchbond 2+Silux filling in Class I preparation Group 3. Gutta percha filling with ZOE cement in Class I preparation Group 4. Amalgam filling with cavity varnish in Slot preparation Group 5. Scotchbond 2+Silux filling in Slot preparation Each specimens was immersed in 1% solution of KCl, and applied a potential of 9V external power supply. Measurements of the current flow were obtained at 1, 2, 3, 7, 9, 12, 14, 18, 21, 25 and 28 day after immerson. Marginal microleakage were compared and evaluated. The results were as follows ; 1. The group filled with composite resin with dentin bonding agent shows lower apical microleakage value than the group filled with amalgam following varnish application (P<0.01). 2. In the group filled with gutta percha, apical microleakage value was the hightest 3. There was no significant difference between Class I cavity and Slot type cavity regardless of the used materials.
