  • It has been reported that the operative treatment for the preservation of tooth having periapical lesion has shown over ninety percent of success whereas in the case where calcified canal is severe, canal is perforated, ledge is formed, tooth restored by post and core and cases where symptoms still exist after treatment, endodontic treatment may not all be the solution in attaining a successful treatment. Thus Apicoectomy would generally be used to prevent the loss of tooth having periapical lesion where treatment prognosis is poorly evaluated by endodontic treatment. In case 1 and 2, endodontic treatment was applied due to unilocular radiolucent lesion in periapical area but the symptoms somehow did not show improvement and therefore operated apicoectomy, and as in case 1, favorable results were attained but as in case 2, yet more observation has to be done. The followings are the results we have concluded : 1. In the process of endodontic treatment, absolute canal obturation has great effect on attaining successful treatment. 2. In cases where endodontic treatment is not highly recommended, for example, tooth having dental pulp and periapical lesion, favorable results can be attained by applying apicoectomy.

Apicoectomy of maxillary anterior teeth through a piezoelectric bony-window osteotomy: two case reports introducing a new technique to preserve cortical bone

  • Two case reports describing a new technique of creating a repositionable piezoelectric bony window osteotomy during apicoectomy in order to preserve bone and act as an autologous graft for the surgical site are described. Endodontic microsurgery of anterior teeth with an intact cortical plate and large periapical lesion generally involves removal of a significant amount of healthy bone in order to enucleate the diseased tissue and manage root ends. In the reported cases, apicoectomy was performed on the lateral incisors of two patients. A piezoelectric device was used to create and elevate a bony window at the surgical site, instead of drilling and destroying bone while making an osteotomy with conventional burs. Routine microsurgical procedures - lesion enucleation, root-end resection, and filling - were carried out through this window preparation. The bony window was repositioned to the original site and the soft tissue sutured. The cases were re-evaluated clinically and radiographically after a period of 12 - 24 months. At follow-up, radiographic healing was observed. No additional grafting material was needed despite the extent of the lesions. The indication for this procedure is when teeth present with an intact or near-intact buccal cortical plate and a large apical lesion to preserve the bone and use it as an autologous graft.

  • The surgical evaluation of the apicoectomy with various types of alveolar bone resorption was conducted in current study. The apicoectomy is the common and important treatment in endodontics. Finite element analysis was used for evaluation. The 3D solid model of the maxillary central incisor was reconstructed using CT images of a mature Asian female. Loading and boundary conditions were simulated in the normal mastication of maxillary central incisor. For evaluation of apicoectomy, lingual, labial and entire alveolar bone resorption models were developed. In the results, lingual alveolar bone resorption did not significantly influence stress distribution pattern of root dentin and labial alveolar bone had an important role for supporting structural stability in tooth system.

  • A 10-year-old, female mixed breed dog and a 3-year-old, female chihuahua which had swelling below lower eyelid were refered to Veterinary Medical Teaching Hospital, Seoul National University. The plain maxillary oblique view revealed radiolucency at periapical area in maxillary fourth premolar. The 2 cases were diagnosed as periapical abscess by clinical sign and radiographs, and apicoectomies were performed for resetting the lesions and preserving the teeth. Under general anesthesia with isoflurane, periapical abscess was resected by burs and the defect was filled with zinc phosphate cement. Radiograph was perfomed at 1 week and 1 year after apicoectomy and there was no evidence of recurrence.

  • The apicoectomy is an operation in which the root apex is removed and the adjacent periapical pathologic tissue is curetted. In this operation, there are several factors that may lead to the surgical wound infection, such as, local, systemic, environmental, endogenous and surgical factor. The systemic medical and surgical factor that may compromise patients defense are more important cause of the wound infection. The postoperative infection is likely to occur owing to poor systemic condition(especially geriatric cancer patient with operation, radiotherapy and chemotherapy), postoperative accumulation of hematoma & seroma and other contaminated factors. So, the authors established the immediate rubber drainage into the sutured wound of dental apicoectomy for the prevention of postoperative infection. The results are more favorable without the wound infection in total 31 cases of the dentistry of Dong San Medical Center & Wonju Christian Hospital.

Patients' perception and satisfaction with apicoectomy (치근단절제술에 대한 환자의 인식과 만족도 조사)

  • Objectives: This study was aimed to examine the patients' perception and satisfaction with the results of endodontic microsurgery which was apicoectomy with retrofilling. Materials and Methods: A questionnaire was given to 109 patients, who were recalled after a minimum of 3 months upon endodontic microsurgery in the Department of Conservative Dentistry, Yonsei University. A contingency table and correlation analysis were used to determine if there were any correlations between age/gender and the patients' responses (p = 0.05). Results: Approximately 60% of respondents answered they had never heard of surgical endodontic procedures. 63.3% of respondents chose the surgical option because they wanted to keep their natural teeth. If the patient required the same procedure on another tooth later, 100 out of 109 respondents answered they would choose microsurgery instead of extraction. Most patients (82.57%) appeared to be satisfied with the surgical procedure. Conclusions: Endodontic microsurgery consisting of apicoectomy and retrofilling seems to appeal to majority of patients as a satisfactory and valuable treatment choice.

  • The case of chronic alverolar abscess treated by apicoectomy and intubation technique is reported. The chronic alverolar abscess developed at the apex of the right lower first premolar in the 30-year old Korean female. And the lesion involved the root of the adjacent second premolar.
