역충전재 및 치근단 절제 각도와 와동 형성 기구에 따른 역충전물의 변연 누출에 관한 전기화학적 연구 (AN ELECTROCHEMICAL STUDY ON MICROLEAKAGE OF RETROGRADE FILLING USING DIFFERENT RETROGRANE FILLING MATERIALS, ROOT RESECTION ANGLE AND CAVITY PREPARATION INSTRUMENTS)

  • The purpose of this study was to evaluate the microleakage electrochemically using different retrograde filling materials, different root resection angle, and different cavity preparation instruments. 104 extracted single-rooted teeth were selected for this study. 100 teeth were used as experimental groups and four was used as controls. Anatomical crowns were resected, root canals were prepared, and the apical 2 mm of roots were removed. The experimental roots were randomly divided into five equal groups. Experimental groups : Group 1. no bevel, cavity preparation with ultrasonic instruments, amalgam filling Group 2. no bevel, cavity preparation with ultrasonic instruments, SuperEBA cement filling Group 3. no bevel, cavity preparation with ultrasonic instruments, desiccated ZOE filling Group 4. $45^{\circ}$ bevel, cavity preparation with ultrasonic instruments, amalgam filling Group 5. no bevel, cavity preparation with conventional bur, amalgam filling Microleakage was measured once a day for 30 days using electrochemical method and were analyzed statistically. The results were as follows : 1. The group with Super EBA cement filling showed the least marginal leakage from second to fourth day(p<0.05), there was no significant difference between the group with amalgam filling during eighth to eighteenth day(p>0.05), but after the nineteenth day here was a higher marginal leakage than the group with amalgam filling(p<0.05). 2. The group with desiccated ZOE filling demostrated that the highest marginal leakage, started on the eighth day(p<0.05). 3. The group using ultrasonic instrument showed lower marginal leakage than the group using bur until the nineteenth day(p<0.05), but there was no significant differnce with the group using bur after twentythird day(p>0.05). 4. The group without bevel showed lower marginal leakage than the group with bevel (p<0.05). 5. Whether bevel or nor had much more effect on marginal leakage than with cavity perparation instrument when the cavity was retrogrdefilled with amalgam(stepwise regression).

치근 우식 수복에 사용되는 심미성 수복물의 변연누출에 관한 연구 (A STUDY ON THE MARGINAL LEAKAGE OF ESTHETIC RESTORATIVE MATERIALS ON ROOT CARIES RESTORATION)

  • The purpose of the study was to evaluate the degree of the marginal leakage of esthetic restorative materials on root caries restoration. 120 cavities were prepared to $90^{\circ}$ butt joint on all margins on the crown and root portion, and divided into 4 groups. The four groups of cavity were filled with Amalgam(Dongmuyung Dental Alloy Co., Ltd, KOREA), Silux$^{(R)}$(3M Co., USA)-Scotch Bond 2$^{(R)}$(3M Co., USA), Silux$^{(R)}$-All Bond$^{(R)}$(BISCO USA), and GC Fuji II$^{(R)}$(G-C Co., JAPAN) respectively. The apical margin of the preparation was finished to leave a flash of restorative material. The coronal margin of the preparation was finished not to leave a flash of restorative material. All specimens were sectioned longitudinally with Isomet Low speed saw(Buether Ltd, USA). The degree of dye penetration was evaluated as the parameter of marginal leakage under the stereoscope. The results were as follows. 1. At the enamel and dentin/cementum margins, the margin were finished to leave a flash of material showed less marginal leakage than that were finished not to leave a flash of material (P<0.001). 2. The enamel margins showed less marginal leakage than the dentin/cementum margins(P<0.001). 3. There was no significant difference in the degree of the marginal leakage between Silux$^{(R)}$-Scotch Bond 2$^{(R)}$ group and Silux$^{(R)}$-All Bond$^{(R)}$ group.

Modified Continuous Wave Technique을 이용한 근관충전시 형성된 치근단 크기가 치근단 폐쇄에 미치는 영향 (EVALUATION OF THE INFLUENCE OF APICAL SIZES ON THE APICAL SEALING ABILITY OF THE MODIFIED CONTINUOUS WAVE TECHNIQUE)

  • 본 연구에서는 근관형성의 치근단 크기가 근관 충전의 치근단 폐쇄에 미치는 영향에 대해 알아보고자 하였다. 36개의 단근관, 단근치를 대상으로 3가지 치근단 크기 (Master apical file : #25, #40, #60)에 따라 3가지 실험군으로 나누어 $Profile^{(R)}$$LightSpeed^{(R)}$를 이용하여 근관을 확대하고, Modified continuous wave technique으로 근관충전을 시행하였다. 미세누출의 측정을 위해 실험군당 10개의 치아를 1주일간 India ink에 넣어 색소 침투를 일으키고 치아를 투명화시켜 미세누출의 정도를 측정하였다. 미세누출에 치근단 크기가 미치는 영향을 분석하기 위하여 Kruskal-Wallis one-way ANOVA를 시행하였고, 사후 검정을 위하여 Student-Newman-Keuls method를 시행하였다. 또한, 치아 단면의 형태를 관찰하기 위하여 색소 침투까지 실행된 각 실험군당 2개의 치아를 근첨 1 mm에서 4 mm까지 1 mm간격으로 수평 절단하고 사진 촬영하였다. 각 실험군에서 측정된 미세누출양을 Krustal-Wallis one-way ANOVA에서 분석한 결과 치근단 크기는 유의성 있게 (P < 0.05) 미세누출에 영향을 미쳤으며 치근단 크기가 클수록 미세누출의 평균간과 표준편차 값은 감소하는 경향을 보였다. Student-Newman-Keuls method를 통한 사후 검정에서 치근단 크기가 #25 - #40, #25 - #60에서 유의 한 차이를 보였으며 #40 - #60간에는 통계적인 유의차는 없었다. 단면형태에서는 대부분의 치아에서 난원형의 근관 형태를 관찰 할 수 있었고, 치근단의 크기가 작을수록 성형된 근관에 불규칙한 내면이 존재하였고 gutta-percha나 sealer로 충전되지 않는 recess의 면적이 증가함을 알 수 있었으며 이러한 recess는 미세누출의 통로가 됨을 확인 할 수 있었다. 본 연구 결과, Modified continuous wave technique으로 근관충전시 근관 형성된 치근단 크기가 커질수록 보다 일관되고 성공적 인 결과를 예상할 수 있는 근관충전을 시행할 수 있음을 알 수 있었다.

수종(數種) 근관세척액(根管洗滌液)이 근관폐쇠성(根管閉鎖性)에 미치는 영향(影響)에 관(關)한 연구(硏究) (THE EFFECTS OF INSTRUMENTATION WITH SOME ENDODONTIC IRRIGANTS ON THE APICAL SEAL OF ROOT CANAL)

  • The purpose of this study was to examine the influence of canal irrigants and smear layer on the property of apical seal. Fourty-five canals from human teeth were randomly selected and divided into 5 groups according to the irrigants used in conjunction with instrumentation. The irrigants were RC-prep in combination with Naocl, Citric acid, Naocl, Naocl in combination with $H_2O_2$, and normal saline solution. After instrumention and dry the canal with paper point, the canals were obturated by lateral condensation of Zinc-oxide eugenol sealer and gutta percha cone. All the specimens were immersed in 2% methylene blue dye solution for 7 days and the depth of dye penetration into the canals was evaluated by macros cope. The following results were obtained. 1. All the canals experimented showed varying degree of dye penetration. 2. There was no significant difference in depth of dye penetration between the groups known to be effective and ineffective in the removal of the smear layer. 3. The canals irrigated with NaOCl alone showed the highest dye penetration and mean leakage was 7.7 mm. 4. In the canals irrigated with RC-prep in combination with Naocl and citric acid, the mean dye penetration was 5.5 mm and 4.8 mm respectively, and the canals irrigated with NaOCl in combination with $H_2O_2$ and normal saline solution revealed mean dye penetration of 3.9 mm.

근관충전 시 수분오염이 치근단 폐쇄성에 미치는 영향에 관한 연구 (A STUDY ON THE EFFECT OF MOISTURE ON APICAL SEALING PROPERTY OF ROOT CANAL)

  • The purpose of this study was to investigate the effect of moisture on apical sealing properties of root canal. Fifty five single rooted human teeth were selected from maxillary and mandibular teeth. After removing crown portion at the cemento-enamel junction, all teeth were routinely prepared with step-back method. And then, the canals were dried with paper point and the teeth were randomly divided into 3 groups of 15 teeth each, and remaining 10 teeth were used as positive and negative control teeth : Group 1 were irrigated with 1ml of 95% alcohol and dried with air and paper point. Group 2 and 3 were intentionally contaminated with 0.05ml of 3.5% NaOCl or saliva, respectively. All the teeth were obturated with sealapex and gutta percha cone by lateral condensation technique, and covered with two coat of nail varnish after 48 hours of obturation. The teeth were immersed in india ink for 1 week and cleaned with methyl salicylate and then the degree of dye penetration were measured with stereomicroscope. The data were analyzed statistically by one-way ANOVA. The results were as follows : 1. All experimental groups showed varying degrees of dye penetration, and the mean degree of dye penetration was 0.1mm to 0.7mm. 2. Saliva contamination group(group 3) showed the highest amount of dye penetration, followed by NaOCl contamination group, then alcohol dried group, but there was no significant difference among three experimental groups. * This results suggest that there was no significant differences of apical leakage after canal obturation between alcohol dried canal and moisture present canals and the use of alcohol instead of paper point is unnecessary to dry the canals prior to canal filling. But other factors such as bacterial contamination and sealer discoloration by moisture must be considered in application of this results to clinical practice.

상아질 접착제 사용 여부에 따른 근관 밀폐효과의 비교 (COMPARISON OF APICAL SEAL WITH OR WITHOUT THE USE OF DENTIN ADHESIVE SYSTEM)

  • 이 연구의 목적은 상아질 접착제의 사용 여부에 따른 근관 밀폐효과를 비교 평가하기 위해 40개의 단근치를 근관 성형 후 20개씩 두군으로 나누었다. 1군은 근관 충전시에 상아질 접착제를 사용하지 않았으며 2군은 상아질 접착제를 사용하였다. 근첨에서 1.5 mm (Level 1), 2.0 mm (Level 2), 2.5 mm (Level 3) 떨어진 부위의 평균 누출률을 측정하였다. 1. 두군의 평균 누출률은 치경부로 향하면서 감소하였다. 2. Level 1에서 1군과 2군의 평균 누출률은 차이가 없었다 (p > 0.05) .3. Level 2와 3에서 2군의 평균 누출률은 1군보다 낮았으며, 통계학적으로 유의차를 나타내었다 (p < 0.05) .연구결과 상아질 접착제를 사용하여 근관 충전하는 경우 치근단 누출을 감소시킬 수 있을 것으로 판단된다.

측방가압충전시 Nickel-Titanium spreader의 사용유무가 근관충전효과에 마치는 영향 (INFLUENCE OF NICKEL-TITANIUM SPREADER ON THE SEALING ABILITY IN LATERAL CONDENSATION TECHNIQUE)

  • Lateral condensation with gutta-percha and sealer has been shown to provide an excellent apical seal; however, the lateral condensation technique has demonstrated less favorable apical leakage results in curved canals when compared with straight canals. Placement of endodontic spreaders to within 1 to 2mm of the root canal working length has been advocated for optimum gutta-percha obturation. Due to their stiffness, stainless-steel(SS) spreaders will often fail to achieve this position in curved canals. Newly marketed nickel-titanium(NT) spreaders may offer an advantage in this regard due to the increased flexibility of these instruments. The purpose of this study was to evaluate the effect of NT finger spreader on the sealing ability in lateral condensation technique, compared with conventional SS finger spreader. Twenty four standardized resin models simulating curved canals(30 degree) were randomly placed into 2 groups and instrumented to a #30 master apical file size with Ni-Ti Profile .04 taper series using step down technique. Each groups was obturated with standardized gutta-percha cone by standard lateral condensation technique using SS finger spreader, NT finger spreader. And then, each model was sectioned horizontally with microtome at 1, 2, 3, 4, 5mm levels from the apex. At each of 5 levels, ratio of the area of gutta-percha was obtained by calculating the area of gutta-percha to the total area of the canal. The data collected were then analyzed statistically using a t test for independent samples. The results as follows ; 1. The total mean ratio of area of gutta-percha was 89.20${\pm}$7.00(%) for SS spreader group. 92.20${\pm}$5.17(%) for NT spreader group. There was statistically significant difference between each group(p<0.05). 2. At 3mm level, the mean ratio of area of gutta-percha was 88.32${\pm}$5.41(%) for SS spreader group, 95.25${\pm}$2.60(%) for NT spreader group. There was statistically significant difference between each group(p<0.05). At 1,2,4mm levels, NT spreader group showed greater mean ratio of area of gutta-percha than SS spreader group, too. But there was no statistically significant difference. 3. At 5mm level, the mean ratio of area of gutta-percha was 91.83${\pm}$3.42(%) for SS spreader group, 87.91${\pm}$3.68(%) for NT spreader group. There was statistically significant difference between each group(p<0.05). This study concluded that the NT spreader demonstrated somewhat favorable apical sealing effect than SS spreader in prepared curved canals. The clinical use of NT spreaders may enhance our ability to create better apical seals in curved canals, but further studies in this area will help clarify some of the remaining areas with which practitioners are concerned, such as compaction forces exerted by NT spreaders.

원발성 자연 기흉의 흉강경 수술에서 폐첨부 흉막 박리술과 탈크 흉막 유착술의 비교 (Clinical Analysis of Video-assisted Thoracoscopic Surgery for Spontaneous Pneumothorax - Comparison of Apical Pleurectomy Versus Talc Powder Insufflation)

  • 원발성 자연 기흉의 치료에 있어서 단순히 기포절제술 또는 기낭절제술만 시행한 경우에 비해 흉막 유착술을 같이 시행한 경우, 수술 후 재발률을 줄일 수 있는 방법으로 인식되고 있다. 흉막 유착술의 방법에 있어 탈크 가루를 이용한 화학적 흉막 유착술과 폐첨부 흉막 박리술을 이용한 기계적 흉막 유착술의 장단점 및 재발률 등의 임상 결과를 비교하였다. 대상 및 방법: 2001년 1월에서 2002년 6월까지 원발성 자연 기흉으로 흉강경 수술을 받았던 환자 152명(흉막 박리술-68명, 탈크 유착술-84명)을 대상으로 하였으며, 평균 연령은 폐첨부 흉막 박리술 시행 군이 23.5$\pm$12.1세(14∼46세), 탈크 유착술 시행군이 26.2$\pm$9.0세(16∼44세)였다 결과: 추적 관찰 기간과 재발률은 박리 군에서 68명중 5∼17개월(12.56$\pm$ 2.98개월) 동안 1명(1.47%)이었고, 탈크 군은 84명 중 평균 5∼15개월(10.75$\pm$3.09개월) 동안 1명(1.19%)으로 나타나 통계적 유의성은 없었다(p=1.000). 이차 시술의 소요 시간은 박리군의 경우 평균 14.4$\pm$2.5분이었고, 탈크 군은 평균 4.4$\pm$1.5분으로 탈크 군보다 박리 군이 유의하게 길었다(p<0.001). 수술 후 통증의 변화는 박리 군에는 0이 48명(70.6%), 1이 12명(17.6%), 2가 4명(5.9%), 3이 4명 (5.9%)이고, 탈크 군에서는 각각 16명 (19.0%), 18명 (21.4%), 24명(28.6%), 26명(31.0%)으로 나타났으며, 통계검증 결과 수술 후 흥부 통증은 박리 군보다 탈크 군에서 유의하게 증가한 것으로 나타났다(p<0.001). 술 후 합병증으로는 박리 군의 경우 5일 이상의 공기 누출이 1명, 탈크 군에서는 공기 누출이 1명, 농흉이 1명 있었다. 결론: 탈크 유착술은 흉막 박리술과 달리 시술 시간이 거의 걸리지 않고 비교적 간단히 할 수 있으나, 흉강내 외부물질이 투여되는 것이어서 농흉과 같은 합병증이 생길 가능성이 있다. 흉막 박리술과 탈크 유착술에 따른 재발률은 유의한 차이가 없는 것으로 판단된다. 탈크 유착술은 흉막 박리술에 비해 술 후 통증이 심한 편이다. 이상으로 재발률에서 유의한 차이를 보이지 않는 이상 각각의 장단점이 있으므로 어느 것이 우수하다 단정 지을 수는 없으며, 두술식의 장단점을 고려하여 환자 개개인에게 적절히 사용될 수 있으리라 판단된다.

근관 밀폐도에 미치는 도말층 및 상아질 접착강화제의 영향 (EFFECTS OF SMEAR LAYER AND DENTIN PRIMERS ON THE SEALING ABILITY OF ROOT CANAL)

  • The purpose of this study was to evaluate effects of smear layer and dentin primers on the sealing ability of root canals. 126 extracted human teeth with single, straight canals and mature apices were used. The Samples were first classified into six groups as follows: presence of smear layer; absence of smear layer; Scotchbond Bond Multi-Purpose; All Bond 2; Mac Bond 2; Clearfil Liner Bond 2. A Positive control was also established. All teeth except the control group were then obturated with thermoplasticized gutta-percha and AH26. Electrochemical and dye penetration technique were later used to evaluate the degree of micro leakage through the root canal. Seventy teeth were then immersed in a 1% potassium chloride solution and An external power supply(DC 10 V) was then applied to the circuit for the electrochemical microleakage test. The degree of Microleakage was determined over period of 28 days before being evaluated. In total, 48 teeth were submitted to the dye infiltration technique. All specimen were suspended in 2% methylene blue dye for 1 week before being longitudinally split. The degree of dye infiltration was measured under a stereo microscope at ${\times}10$ magnification and evaluated. The results were as follows: 1. Apical microleakage increased throughout the test period in all group and one group having a smear layer showed a dramatic increase under electrochemical test (p<0.05). In the group having smear layer, the degree of apical microleakage was the highest, and the micro leakage was much higher than in the smear layer removed group in electrochemical test (p<0.05). Scotchbond Multi-Purpose, All Bond 2, Mac Bond 2 and Clearfil Liner Bond 2 showed lower micro leakage than one group having smear layer. The All Bond 2 and Clearfil Liner Bond 2 treated groups showed the lowest microleakage in electrochemical test (p<0.05). 2. There was no significant difference between the experimental groups in dye penetration technique. These results suggested that the removal of the smear layer from root canal and concomitantly the application of dentin primer into root canal could improve the sealing ability of root canal obturation.

근관충전의 폐쇄효과 평가에서 색소침투의 정성적 및 정량적 측정의 상호관계 (RELATIONSHIP BETWEEN LINEAR AND VOLUMETRIC MEASUREMENTS OF DYE PENETRATION IN THE SEALING EFFECT OF CANAL OBTURATION)

  • 본 연구의 목적은 여러가지 근관충전법으로 충전된 근관의 폐쇄효과를 정선적 및 정량적으로 측정하여 각 충전법간의 폐쇄효과를 비교하므로써 정선적 측정법 및 정량적 측정법간의 상호 관계를 규명하는데 있다. 108개의 발거된 상악중절치 및 측절치와 상하악 견치에서 치관을 절제하고 통법대로 근관형성한 다음 4개군으로 나누어 제 I 군은 sealer를 사용하고 측방 가압근관충전법으로, 제 II 군은 sealer를 사용하고 저온연화 gutta-percha주입근관충전법으로, 제 III 군은 sealer를 사용하고 Thermafil근관충전법으로 그리고 제 IV 군은 sealer를 사용하지 않고 고온용융 gutta-percha주입근관충전법으로 충전하고 전 시편을 2 % methylene blue 용액 속에 근단부를 1mm담근 채 $37^{\circ}C$ 항온기에서 10일간 경과시켜 근단공을 통한 색소침투를 정선적 및 정량적으로 측정하여 다음의 결과를 얻었다. 정선적 측정에서는 II 군은 III군, I군 및 IV군보다, III군은 IV군보다 유의상 있게 색소침투가 적게 나타났으나 (P<0.05), 그 이외의 군들 상호간에는 유의한 차이가 없었다. 정량적 측정에서는 II 군이 가장 적게 나타나 I 군 및 IV군과는 유의한 차이가 있었으나 (P<0.05), III군과는 유의한 차이가 없었다(P>0.05). 정선적 측정과 정량적 측정과의 사이에는 미약하지만 유의한 상관관계가 있었다(r=0.3391, P<0.001).
