Anatomical analysis of the resected roots of mandibular first molars after failed non-surgical retreatment

  • Objectives: Understanding the reason for an unsuccessful non-surgical endodontic treatment outcome, as well as the complex anatomy of the root canal system, is very important. This study examined the cross-sectional root canal structure of mandibular first molars confirmed to have failed non-surgical root canal treatment using digital images obtained during intentional replantation surgery, as well as the causative factors of the failed conventional endodontic treatments. Materials and Methods: This study evaluated 115 mandibular first molars. Digital photographic images of the resected surface were taken at the apical 3 mm level and examined. The discolored dentin area around the root canal was investigated by measuring the total surface area, the treated areas as determined by the endodontic filling material, and the discolored dentin area. Results: Forty 2-rooted teeth showed discolored root dentin in both the mesial and distal roots. Compared to the original filled area, significant expansion of root dentin discoloration was observed. Moreover, the mesial roots were significantly more discolored than the distal roots. Of the 115 molars, 92 had 2 roots. Among the mesial roots of the 2-rooted teeth, 95.7% of the roots had 2 canals and 79.4% had partial/complete isthmuses and/or accessory canals. Conclusions: Dentin discoloration that was not visible on periapical radiographs and cone-beam computed tomography was frequently found in mandibular first molars that failed endodontic treatment. The complex anatomy of the mesial roots of the mandibular first molars is another reason for the failure of conventional endodontic treatment.

Piroxicam, Mitoxantrone, and Hypofractionated Radiation Therapy with Volumetric Modulated Arc Therapy for Treating Urinary Transitional Cell Carcinoma in a Dog: A Case Report

  • A 12-year-old spayed female beagle dog was presented with pollakiuria and stranguria. Abdominal ultrasonography identified irregular a marginated, hyperechoic mass in the urethra and trigon area of the bladder. Computed tomography (CT) revealed a heterogeneous mass in the trigone area leading to a urethra. There was no evidence of regional or distant metastasis. Cytologic analysis suspected transitional cell carcinoma (TCC). The patient was treated with piroxicam, mitoxantrone, and once weekly fractionated radiation therapy (RT) with volumetric modulated arc therapy (VMAT). A follow-up CT scan at 6 months after RT revealed a reduction in tumor size. At 17 months after the start of RT, the patient became severely anorectic and lethargic. Ultrasound examination revealed a hyperechoic mass in the apex area of bladder while the trigone area of the bladder and urethra appeared normal. Multiple hypoechoic nodules of various sizes were found in the liver and spleen. The patient was humanely euthanized at the request of the owner. A combination of piroxicam, mitoxantrone, and hypofractionated RT with VMAT protocol was well tolerated. This case described tumor response and survival time of a canine TCC treated with piroxicam, mitoxantrone, and once weekly palliative RT using computer-assisted planning and VMAT.

협설만곡치아의 파노라마방사선영상소견에 대한 연구 (A study of the panoramic radiographic images of the buccolingual dilaceration)

  • Purpose : We want to identify the appearance of the buccolingual root dilaceration teeth in the panoramic views and specify the characteristics of these teeth. Materials and Methods : One thousand-six patients were examined on the basis of both panoramic and CT image criteria. We diagnosed and excluded certain teeth from the samples; both prosthodontic or pathologic lesion appearing teeth and mesiodistally dilacerated ones. We meticulously discerned buccolingually dilacerated teeth in the CT images and total 48 samples were selected. The degree of severity in dilaceration was standardized by 2 types of criteria. The samples were differentiated into 3 groups and again categorized into six types showing from the panoramic views: irregular view on the root apex area, clear blunt on the root tip, stepping on root tip, double lamina dura or double tip, arrow-target shaped root, bull's eye, normal view. Results : The types of teeth selected from total 48 buccolingual root dilaceration samples were mandibular first and second molar, premolars, canines, and lateral incisors. The direction of dilaceration was an even percentage to each buccal and lingual side for most selected teeth, however, that of both canines and lateral incisors were directed in almost a buccal side. In the panoramic views, the root types of the buccolingually dilacerated teeth were irregular view on the root apex area, clear blunt on the root tip, stepping on root tip and normal types were almost always normal view. The more severity in dilareated degree, the more chances of observation in the panoramic views were clear blunt on the root tip and stepping on root tip. Conclusion : As observed in the shape of stepping on root tip or double lamina dura in the panoramic views, there can be much more probability to diagnose as a buccolingually dilacerated root.

압입자 첨단마모에 따른 나노압입곡선의 변화 및 이의 보정기법 (Variation of Nanoindentation Curve due to Wear of Indenter Apex and Its Correction Method)

  • 나노압입시험기의 힘교정과 압입자에 대한 3차원 형상 관찰 및 분석이 본 연구에서 진행되었다. 표준분동으로 교정한 마이크로밸런스로 나노압입시험기에서 발생시킨 하중을 측정하여 측정치와 발생치의 비로 압입하중을 교정하였고, 나노압입시험의 시작점인 초기 접촉 하중도 확인할 수 있었다. 삼각뿔 압입자를 원자현미경으로 관찰하여 분석한 결과 비교적 사용이력이 없는 압입자 A와 마모된 압입자 B의 첨단곡률반경은 각각 $19.71{\pm}3.03$ nm와 $1043.94{\pm}50.91$ nm로 결정되었다. 완벽한 삼각뿔 압입자 형상과 중첩하여 압입자 A와 B의 첨단무딘깊이(bluntness depth)를 1.22 nm와 64.56 nm로 결정하였고, 용해실리카 기준시편에 수행한 나노압입시험 결과를 살펴본 결과 두 압입자의 압입하중-변위곡선들이 무딘깊이 차이만큼 수평축으로 서로 어긋나 있음을 확인할 수 있었다. 수평 이동을 통해 보정된 압입곡선의 분석을 통해 개별 압입자 면적함수에 대한 고려없이 1.11 % 이내에서 동일한 용해실리카의 나노경도를 결정할 수 있었다.

Denta $Scan^R$을 이용한 즉시 임플랜트 시술시 최적의 식립 위치 대한 통계적 연구 (STATISTICAL STUDY ON OPTIMAL PLACEMENT OF IMMEDIATE DENIAL IMPLANTATION USING DENTA $SCAN^R$)

  • Purpose : The purpose of this study was to determine proper position and angulation of an implant for immediate implantation. Materials and Method : From the years 1997 to 2000. 52 Denta $scan^R$ views, 22 upper and 32 lower jaw with an average age of 43 and 40 respectively, were investigated, which comprise intact upper and lower 6 anterior teeth and premolars. On the Denta $scan^R$, the optimal placement for the immediated implantation was simulated. The measuring methods included 1) Angulation difference between tooth long axis and alveolar bone process. 2) Angulation difference of long axis between tooth and installing fixture 3) Distance between center of tooth at cervical area and center of fixture. 4) Distance from root apex to the bone limit of vital structure. One sample t-test was used for statistical analysis. Result : The results were as follows. 1) At the maxillary central incisor and lateral incisor, angulation difference of long axis between tooth and installing fixture was respectively 0.5 and 3.2 degrees with the fixture center's palatally positioned 2mm apart from tooth center. 2) At the lower anterior 6 teeth, that was about $-2.8^{\circ}\;to\;-4.6^{\circ}$ with the fixture center's lingually positioned 1mm apart from tooth center. 3) At the maxillary canine and premolar, that was respectively $11.8^{\circ}\;and \;7.2^{\circ}$ with the fixture center palatally positioned $2\sim2.4mm$ apart from tooth center. 4) At the lower premolar area, that was about $0^{\circ}\;to\;2^{\circ}$ with the fixture center's lingually positioned $0.5{\sim}1mm$ apart from tooth center. 5) Distance from root apex to the bone limit of vital structure, at the maxillary anterior and premolars. was the range of 10 to 12mm, and at the mandibular anterior teeth and the 1st premolar, that was the range of 18 to 20mm. Conclusion : The proper implant position of maxillary anterior and premolar teeth is as paralleled as or more buccally angulated than long axis of tooth with the fixture center's palatally positioned. In mandiblular anterior region, long axis of implants is lingully angulated compared with long axis of tooth and in premolar, almost parelleled with long axis of tooth and alveolar process.

Rabbit maxillary sinus augmentation model with simultaneous implant placement: differential responses to the graft materials

  • Purpose: This study was performed to establish an experimental rabbit model for single-stage maxillary sinus augmentation with simultaneous implant placement. Methods: Twelve mature New Zealand white rabbits were used for the experiments. The rabbit maxillary sinuses were divided into 3 groups according to sinus augmentation materials: blood clot (BC), autogenous bone (AB), and bovine-derived hydroxyapatite (BHA). Small titanium implants were simultaneously placed in the animals during the sinus augmentation procedure. The rabbits were sacrificed 4 and 8 weeks after surgery and were observed histologically. Histomorphometric analyses using image analysis software were also performed to evaluate the parameters related to bone regeneration and implant-bone integration. Results: The BC group showed an evident collapse of the sinus membrane and limited new bone formation around the original sinus floor at 4 and 8 weeks. In the AB group, the sinus membrane was well retained above the implant apex, and new bone formation was significant at both examination periods. The BHA group also showed retention of the elevated sinus membrane above the screw apex and evident new bone formation at both points in time. The total area of the mineral component (TMA) in the area of interest and the bone-to-implant contact did not show any significant differences among all the groups. In the AB group, the TMA had significantly decreased from 4 to 8 weeks. Conclusions: Within the limits of this study, the rabbit sinus model showed satisfactory results in the comparison of different grafting conditions in single-stage sinus floor elevation with simultaneous implant placement. We found that the rabbit model was useful for maxillary sinus augmentation with simultaneous implant placement.

Step-down과 Balanced force 근관성형술식에 의한 근관 형태의 변화 (EFFECT OF "STEP-DOWN" AND "BALANCED FORCE" PREPARATION METHODS ON THE SHAPE OF THE ROOT CANAL)

  • This study was performed to investigate the effect of root canal shaping techniques on the change of the shape of prepared root canal. 40 mesiobuccal canals of recently extracted mandibular 1st and 2nd molars were divided into 4 groups and shaped by step-down/balanced force technique, step-down/step-back technique, step-back technique and conventional technique respectively. The change of the shape of root canal was traced by superimposing the radiographs obtained before and after shaping of each root canal. The results were as follows. 1. By the experimented techniques except conventional technique, the root canals were more shaped in convex side of apical area and in concave side of most curved and coronal area than in the other sides(P<0.05). By conventional technique, the root canals were more shaped in convex side than in convave side from apex to orifice(P<0.05). 2. By step-down/balanced force technique, the cancave sides at C and D points of proximal view and C point of clinical view were more shaped than the convex side(P<0.05). Through the entire canal, the concave side was more shaped than the convex side in proximal view(P<0.01). But there was no statistical difference between both sides in clinical view. 3. By step-down/step-back technique, the change of root canal shape was not statistically different in concave and convex sides at each point of both views(P>0.05). And through the entire canal in proximal view, there was no statistical difference in shaping percentage between both sides. But through the entire canal in clinical view, the concave side was more shaped than the convex side(P<0.1). 4. By step-back technique, the convex side at B point of clinical more shaped than the other sides(P<0.05). Through the entire canal in proximal and clinical views, there was no statistical difference in shaping percentage between both sides. 5. Comparing the total shaping percentage among techniques, that in conventional technique was the greatest numerically, and followed by the percentages in step-down/step-back, step-down/balanced force and step-back technique. But, in proximal view, shaping percentages were not statistically different among techniques(P>0.05, ANOVA test). In clinical view, shaping percentages in step-back and conventional techniques were statistically different(P<0.01, ANOVA test). * Proximal view: radiograph taken in mesiodistal direction. * Clincal view: radiograph taken in faciolingual direction. A point : 1mm point from radiographic apex B point : center point between A and C points C point : most curved point of root canal D point : center point between C point and canal oriffice.

Lingual sliding mechanics의 lever arm 효과에 대한 유한요소분석 (Finite element analysis of effectiveness of lever arm in lingual sliding mechanics)

  • 전치의 후방 견인 시 적절한 치아 이동 상태 조절은 필수적이다. 설측 장치를 이용한 레버 암 길이의 조절을 통하여 치아 이동에 관한 연구는 있었으나 3차원적인 변위 양상에 대한 연구는 많이 이루어지지 않은 실정이다. 이에 본 연구는 상악 전치부의 레버 암(lever arm)의 길이를 5 mm 단위로 20 mm까지 증가시켰으며 대구치와 TPA (trans palatal arch) 상에 있는 견인 훅(hook)의 위치를 달리 하여 200 gm의 후방 견인력을 가했을 때 나타나는 치아 변위 양상과 응력분포를 3차원적 유한요소분석을 통하여 알아보고자 하였다. 이를 위하여 아시아 성인의 표본조사를 통해 제작된 치아모형(Nissan Dental Product, Kyoto, Japan)을 3차원적으로 스캐닝한 후 상악치아, 치주인대, 치조골에 대한 유한요소 모델을 제작하였다. 각 치아의 절단연과 치근첨의 이동량을 x, y, z 좌표에서 각각 계산하여 치열의 변위 양상을 분석하고 von Mises 응력 분포를 계측하였다. 연구 결과, 정상 교합 모형의 레버 암 길이가 15 mm, 20 mm인 경우 전치 절단연과 치근첨의 설측 변위가 유도되었다. 본 실험의 조건 중 20 mm에서 치근첨의 설측 변위는 최대로 나타났다. 구치부 견인 훅이 치근첨에 있을 때 대구치 치관은 원심 방향으로 변위되었다. 또한 레버 암의 길이가 20 mm인 경우 전치부의 정출은 미약하였고 견치 치관은 협측 방향으로 전위되었다. 이 때 구치부 견인 훅의 위치가 TPA의 구개 중앙 측에 있을 때보다 가장자리 측에 있을 때 견치 치관은 더 많이 전위되었다. 이상의 결과를 토대로 설측 장치를 이용한 상악 6전치의 후방 견인 시 레버 암의 길이가 길고 구치부 견인 훅의 위치가 구개 중앙부에 있을 때 전치부 절단연(incisal edge)의 정출 없이 견치의 측방 전위 및 전치부 절단연과 치근첨의 설측 변위가 공히 나타남을 알 수 있었다.

리본 형태의 근관에서 열연화주입법의 근관벽에 대한 적합도에 관한 연구 (A COMPARISON OF THERMOPLASTICIZED INJECIABLE GUTTA-PERCHA TECHNIQUES IN RIBBON-SHAPED CANALS : ADAPTATION TO CANAL WALLS)

  • The aim of this study is to compare the adaptability of thermoplasticized injectable gutta-percha technique to the canal walls in ribbon-shaped canals. Thirty resin models simulated ribbon-shape canals were instrumented to #40 using .06 taper Profile systems. Three groups of each 10 resin models were obturated by the lateral condensation technique(LC) and the two thermoplasticized injectable gutta-percha technique; Ultrafil Endoset+Obtura II(EO) and Ultrafil Firmset(UF), respectively. After resin model were kept at room temperature for 4 days, they were resected horizontally with micro-tome at 1, 2, 3, 4 and 5mm levels from apex. At each levels. image of resected surface were taken using CCD camera under a stereomicroscope at $\times$40 magnification and stored. Ratio of the area of gutta-percha was obtained by calculating area of gutta-percha cone to the total area of canal using digitized image-ana-Iyzing program. The data were collected then analyzed statistically using One-way ANOVA. The results were as follows. 1 At 1mm levels, there was no statistically significant difference in the mean ratio of gutta-percha among the groups. 2. At 2mm level, EO showed the highest mean ratio of gutta-percha (p<0.05) and there was no significant difference between LC and UF. 3. At 3, 4, 5mm levels, EO and UF had significantly greater mean ratio of gutta-percha than LC(p<0.05) and there was no significant difference between EO and UF. In conclusion, the thermoplasticized injectable gutta-percha techniques demonstrated relatively favorable adaptability to canal walls than lateral condensation technique in ribbon-shaped canals except for 1mm level.

교정용 미니임플랜트 식립 위치에 따른 dragon helix의 효과에 대한 유한요소해석 (Effects of orthodontic mini-implant position in the dragon helix appliance on tooth displacement and stress distribution: a three-dimensional finite element analysis)

  • Objective: The purpose of this study was to investigate the stress distribution on the orthodontic mini-implant (OMI) surface and periodontal ligament of the maxillary first and second molars as well as the tooth displacement according to the OMI position in the dragon helix appliance during scissors-bite correction. Methods: OMIs were placed at two maxillary positions, between the first and the second premolars (group 1) and between the second premolar and the first molar (group 2). The stress distribution area (SDA) was analyzed by three-dimensional finite element analysis. Results: The maximal SDA of the OMI did not differ between the groups. It was located at the cervical area and palatal root apex of the maxillary first molar in groups 1 and 2, respectively, indicating less tipping in group 2. The minimal SDA was located at the root and furcation area of the maxillary second molar in groups 1 and 2, respectively, indicating greater palatal crown displacement in group 2. Conclusions: Placement of the OMI between the maxillary second premolar and the maxillary first molar to serve as an indirect anchor in the dragon helix appliance minimizes anchorage loss while maximizing the effect on scissors-bite correction.
