대동맥판막협착증 환자에서 판막치환 후 좌심실심근비후의 변화 (Regression of Left Ventricular Hypertrophy after AVR in Aortic Valvular Stenosis)

  • 배 경: 대동맥판막협착증 환자에서 판막치환후 좌심실심근비후감소의 정도는 치환된 판막의 적절성을 고려하는 중요한 인자로 생각된다. 방 법: 1990년 7월부터 1997년 7월까지 서울중앙병원 흉부외과에서는 심근비후의 감소정도를 분석하기위해 St. Jude 판막을 치환한 대동맥판막협착증 환자 36명(남녀 각각 22명과 14명, 평균나이 54세, 평균체표면적 1.61m2)에서 수술전과 수술후 조기(7.5$\pm$2.1일)와 만기(10.7$\pm$1.8개월)에 심초음파와 심전도를 시행하였다. 사용된 판막에 따라 3개의 군(19, 21 그리고 23이상)으로 나누었다. 결 과: 19 mm 판막군에서의 평균 체표면적(1.48$\pm$0.13)은 타군(1.63$\pm$0.12)에 비해 체표면적이 작았다(p<0.05). 심박출량은 모든 군에서 수술전에 비해 수술만기에 차이가 없었다. 좌심실심근량지수는 21 mm와 23mm이상 판막군에서 수술후 만기에 유의하게 감소하였으나 19 mm 판막군은 유의한 감소를 보여주지 못했다. 심전도상 Scott의 기준에 의한 전위의 크기는 모든 크기의 판막에서 술후 만기에 감소되었다. 결 론: 19 mm 판막군에서는 임상증세의 호전에도 불구하고 좌심실심근비후의 감소가 원할하지 못해 대동맥판륜이 작은 환자에서는 판륜확장술 또는 동종이식판막치환과 같은 대책들이 필요할 것으로 사료된다.

선천성 사엽성 반월형 판막 - 1예 보고 - (Congenital Quadricuspid Semilunar Valve - A case report -)

  • 호흡곤란을 주소로 내원한 17세 환자가 대동맥 및 폐동맥 사엽성 판막을 보이며 심방중격결손과 폐동맥 판막 협착증이 발전되어 소심막을 이용한 심방중격결손 봉합 및 폐동맥 판막 교련절개술을 시행하였다. 대동맥 판막과 폐동맥 판막이 모두 사엽성 판막을 보이는 경우는 매우 드문 선천성 기형으로 폐동맥 사엽성 판막이 대동맥 사업성 판막보다 9배정도 많다. 본 증례는 Hurwitz and Roberts 분류법상 대동맥판막은 A형, 폐동맥판막은 B형이었으며 대동맥 판막기능은 정상적이었고 폐동맥 판막 협착증을 보였다.

  • Double outlet right ventricle (DORV) and transposition of the great arteries (TGA) with ventricular septal defect (VSD) and pulmonary stenosis (PS) are complex heart diseases, the treatment of which remains a surgical challenge. The Rastelli procedure is still the most commonly performed treatment. Aortic root translocation including an arterial switch operation is advantageous anatomically since it has a lower possibility of conduit blockage and the left ventricle outflow tract remains straight. This study reports successful aortic root transpositions in two patients, one with DORV with VSD and PS and one with TGA with VSD and PS. Both patients were discharged without postoperative complications.

Low Contrast and Low kV CTA Before Transcatheter Aortic Valve Replacement: A Systematic Review

  • BACKGROUND: Minimizing contrast dose and radiation exposure while maintaining image quality during computed tomography angiography (CTA) for transcatheter aortic valve replacement (TAVR) is desirable, but not well established. This systematic review compares image quality for low contrast and low kV CTA versus conventional CTA in patients with aortic stenosis undergoing TAVR planning. METHODS: We performed a systematic literature review to identify clinical studies comparing imaging strategies for patients with aortic stenosis undergoing TAVR planning. The primary outcomes of image quality as assessed by the signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were reported as random effects mean difference with 95% confidence interval (CI). RESULTS: We included 6 studies reporting on 353 patients. There was no difference in cardiac SNR (mean difference, -1.42; 95% CI, -5.71 to 2.88; p = 0.52), cardiac CNR (mean difference, -3.83; 95% CI, -9.98 to 2.32; p = 0.22), aortic SNR (mean difference, -0.23; 95% CI, -7.83 to 7.37; p = 0.95), aortic CNR (mean difference, -3.95; 95% CI, -12.03 to 4.13; p = 0.34), and ileofemoral SNR (mean difference, -6.09; 95% CI, -13.80 to 1.62; p = 0.12) between the low dose and conventional protocols. There was a difference in ileofemoral CNR between the low dose and conventional protocols with a mean difference of -9.26 (95% CI, -15.06 to -3.46; p = 0.002). Overall, subjective image quality was similar between the 2 protocols. CONCLUSIONS: This systematic review suggests that low contrast and low kV CTA for TAVR planning provides similar image quality to conventional CTA.

  • Transcatheter aortic valve implantation (TAVI) has evolved from a challenging intervention to a standardized, simple, and streamlined procedure with over 350,000 procedures performed in over 70 countries. It is now a novel alternative to surgical aortic valve replacement in patients with intermediate surgical risk and its indications have been expanded to cohorts with bicuspid aortic valves, low surgical risk, and younger age and fewer comorbidities. Attention should be paid to further reducing remaining complications, such as paravalvular aortic regurgitation, conduction abnormalities, cardiac tamponade, and stroke. The aim of this review is to provide an overview on the rapidly changing field of TAVI treatment and to explore past achievements, current issues, and future perspectives of this treatment modality.

  • 심실 중격 결손과 폐동맥 협착을 동반한 완전 대혈관 전위를 갖는 환자에서 기존의 Rastelli수술법의 만족스럽지 않은 장기 결과가 보고되는 반면, 최근 들어 대동맥 전위술은 이 질환에서 유용한 수술방법으로 보고되고 있다. 본 증례는 우심실 유입부로 연장된 막성주위형 심실중격결손과 폐동맥 협착을 동반한 완전 대혈관 전위로 진단되어 과거에 체폐단락술을 받았던 2세 남자 환아(체중 9.6 kg)에서 우심실에서 분리한 대동맥 근부를 후방의 좌심실쪽으로 전위시켜 좌심실 유출로 재건술을 시행하고, 판막 도관을 이용하여 우심실 유출로 재건술을 시행하였다. 수술 후 시행한 심초음파 검사에서 양호한 혈역학적 소견을 보여 문헌 고찰과 함께 증례 보고를 한다.

Arch Reconstruction with Autologous Pulmonary Artery Patch in Interrupted Aortic Arch

  • Various surgical techniques have been developed for the repair of an interrupted aortic arch. However, tension and Gothic arch formation at the anastomotic site have remained major problems for these techniques: Excessive tension causes arch stenosis and left main bronchus compression, and Gothic arch configuration is related to cardiovascular complications. To resolve these problems, we adopted a modified surgical technique of distal aortic arch augmentation using an autologous main pulmonary artery patch. The descending aorta was then anastomosed to the augmented aortic arch in an end-to-side manner. Here, we report two cases of interrupted aortic arch that were repaired using this technique.

대동맥류로 인한 좌측 반회후두신경마비 2례 (Two Cases of Recurrent Laryngeal Nerve Palsy Related to Aortic Aneurysm)

  • After the first report of mitral stenosis as a cause of recurrent laryngeal nerve palsy by Ortner in 1897, many authors have described that some kinds of cardiovascular disease might contribute to the development of recurrent laryngeal nerve palsy. The estimated rate of aortic aneurysm related with recurrent laryngeal nerve palsy is about 5%. Aortic aneurysm is classified into 3 types according to the involving segment of aorta in which aneurysms develop, and the first class-aneurysm in ascending aorta and aortic arch-is known to be the only type related to recurrent laryngeal nerve palsy. Recently we experienced two cases of recurrent laryngeal nerve palsy each of which had aneurysm on aortic arch as a major contributing factor. We report these cases with brief review of the literature.

관상동맥 우회술을 병행한 대동맥판막 치환술 치험 1례 (Aortic valve Replacement Concomitant with Aorto-Coronary Bypass Surgery -One case report-)

  • Patient with aortic valvular disease have increased left ventricular work and greater myocardial oxygen demand, which may aggravate the effect of concomitant coronary artery disease. Thus in patient who repair aortic valve replacement, concomitant aortocoronary bypass surgery is often performed when angiographically significant coronary artery disease is present. This approach is supported by reports that revascularization does not increase operative risk when associated coronary artery disease is present and significantly reduce the occurrence of late sudden death. Recently we have experienced one case of aortic valve replacement concomitant with aorta-coronary bypass surgery. The patient was 56 year-old male and admitted with complaint of anterior chest pain especially during his exercise. He was diagnosed as aortic valve stenosis and regurgitation [GIII] with proximal right main coronary artery occlusion We performed aortic valve replacement with aorta coronary bypass surgery by use of saphenous vein. Post operative course was uneventful and chest pain was relieved. Post operative coronary angiogram disclosed good patency of grafted vessel.

Computed Tomography and Magnetic Resonance Imaging Findings of Bicuspid Aortic Valve and Related Abnormalities of the Heart and Thoracic Aorta

  • The bicuspid aortic valve (BAV) is the most common congenital cardiovascular malformation. Patients with BAV are at higher risk of other congenital cardiovascular malformations and valvular dysfunction, including aortic stenosis/regurgitation and infective endocarditis. BAV may also be related to aortic wall abnormalities such as aortic dilatation, aneurysm, and dissection. The morphology of the BAV varies with the presence and position of the raphe and is associated with the type of valvular dysfunction and aortopathy. Therefore, accurate diagnosis and effective treatment at an early stage are essential to prevent complications in patients with BAV. This pictorial essay highlights the characteristics of BAV and its related congenital cardiovascular malformations, valvular dysfunction, aortopathy, and other rare cardiac complications using multimodal imaging.
