It has been known that the eczema herpeticum is the one of most severe skin manifestation of external herpetic infection. It is developed by skin infection of Herpes simplex Virus(HSV) that superimposed on children with atopic dermatitis, and this could be fatal. The secondary bacterial infection happens frequently in eczema herpeticum. Staphylococcus aureus is the most frequently isolated aerobic strain. The important point of treatment of eczema herpeticum is the immediate start of systemic antiviral treatment and use of antibiotics to decrease the secondary infection. We report two cases of severe eczema herpeticum after herb medical treatment in atopic dermatitis. They have been improved after systemic acyclovir and antibiotic therapy with burn dressing.
A field trial of norfloxacin on bovine 206, swine 222 and poultry 205 with respiratory diseases and diarrhea was conducted from Sept 1, 1990 to Jan 30, 1991 in Inchon and Kyonggi-do. The results are as follows. 1. Guinea pigs and rabbits were dosages 10 and 20 times normal. Ten days of observation saw no side effect. 2. Oral treatment on bovine, swine and poultry showed a curing rate of 82.00∼89.06%(Mean 86.00%), compared with 87.27∼96.36%(Mean 92.24%) by Injection. 3. The curing rate was 81.92% by the fourth treatment. 4. The curing rate with and without the use of adjuvant was 80.00∼100% and 83.60∼93. 10%, respectively, for swine, and 81.82∼100% and 82.00∼97.14% for bovine. 5. The curing rate was more dependent on seriousness of illness, environment and time than age. 6. The main disease causing bacteria of bovine respiratory diseases: Haemolytic Sta 27.62%, Haemolytic Str 33.33%, sum of this bacteria is 60.95%, diarrhea: Haemolytic coliform 80.20%, swine respiratory diseases; Haemophilus Spp 20.95%, Haemolytic Sta 72%, Haemolytic Str 21.90%, Sum of these bacteria is 68.57%, diarrhea; Haemolytic coliform 57.26%, poultry respiratory diseases; mycoplasma Spp 29.52%, Haemolytic Sta 13.33%, Sum of this bacteria is 42.85%, diarrhea; Haemolytic coliform 53.00%. 7. The curing rate of norfloxacin by species of strain, the unidentified bacteria on bovine respiratory diseases was the lowest with 77.78%, the others ranged from 84.21% to 100% with most of them over 90%. 8. The loss of clinical sign on bovine and swine was the highest when after 2∼4 application. 9. In terms of antibiotic sensitivity test, norfloxacin was the highest with 84.62∼100% (mean 90.09%) among eight bacterias, only 5 of 15 antibiotics had a sensitivity of over 56.25%, showing a high degree of tolerance.
Recently, a total of 74 Staphylococcus pseudintermedius isolates were collected from clinical cases of canine pyoderma and otitis externa in Korea. In this study, we examined in vitro fluoroquinolone resistance among those isolates using a standard disc diffusion technique. The results demonstrated that, except for one isolate, approximately 18.9% to 27.0% of the isolates possessed bacterial resistance to both veterinary- and human-licensed fluoroquinolones including moxifloxacin (18.9% resistance), levofloxacin (20.3% resistance), ofloxacin (24.3% resistance), ciprofloxacin (25.7% resistance), and enrofloxacin (27.0% resistance). Most surprisingly, 14 out of 74 (18.9%) isolates were resistant to all the five fluoroquinolones evaluated. Moreover, a PCR detection of the methicillin resistance gene (mecA) among the 74 isolates revealed that 13 out of 25 (52.0%) mecApositive isolates, but only 7 out of 49 (14.3%) mecA-negative isolates, were resistant to one or more fluoroquinones. Taken together, our results imply that bacterial resistance to both veterinary- and human-use fluoroquinolones becomes prevalent among the S. pseudintermedius isolates from canine pyoderma and otitis externa in Korea, as well as that the high prevalence of the mecA-positive S. pseudintermedius isolates carrying multiple fluoroquinolones resistance could be a potential public health problem.

Objectives & methods; In order to evaluate the in vivo synergic effect of Mawhangyounpye-tang, a traditional poly-herbal formula used in the treatment of respiratory diseases in Korea, with the quinolone antibiotic ciprofloxacin (CPFX), the viable bacterial number and histopathological changes were monitored after experimental respiratory infection with Streptococcus Pneumoniae ATCC 6303. Results: 1. In CPFX groups, the viable bacterial numbers were significantly decreased compared to that of the control group, and were even more dramatically decreased in concomitant group treated with Mawhangyounpye-tang. 2. In the control group, severe infiltration of inflammatory cells, hemorrhage and hypertrophy of alveolar linings were demonstrated at microscopic levels. However, these abnormal histopathological changes were significantly decreased compared. to that of the control group in CPFX groups, and were even more dramatically decreased in concomitant groups treated with Mawhangyounpye-tang. 3. In CPFX groups, the LSA (Iuminal surface of alveoli $\%$) were significantly increased compared to that of the control group, and more dramatically in concomitant groups treated with Mawhangyounpye-tang. Conclusions: According to these results, it is considered that the in vivo antibacterial activity of CPFX against Streptococcus Pneumoniae ATCC 6303 infection of respiratory tract was dramatically increased by concomitant use of Mawhangyounpye-tang.
Purpose: We investigated the clinical features and factors affecting the choice of treatment modality and the course of pediatric gallstone (GS) disease. Methods: We retrospectively analyzed the medical records of 65 patients diagnosed with GS using imaging studies between January 2009 and December 2017 were included. Results: This study included 65 patients (33 boys and 32 girls; mean age, 8.5±5.3 years; range, 0.2-18 years) who primarily presented with abdominal pain (34%), jaundice (18%), and vomiting (8%). Idiopathic GS occurred in 36 patients (55.4%). The risk factors for GS included antibiotic use, obesity, hemolytic disease, and chemotherapy in 8 (12.3%), 7 (10.8%), 6 (9.2%), and 4 patients (6.2%), respectively. We observed multiple stones (including sandy stones) in 31 patients (47.7%), a single stone in 17 (26.2%), and several stones in 17 (26.2%). GS with a diameter of <5 mm occurred in 45 patients (69.2%). Comorbidities included hepatitis, choledocholithiasis, cholecystitis, and acute pancreatitis in 20 (30.8%), 11 (16.9%), 11 (16.9%), and 4 patients (6.2%), respectively. Ursodeoxycholic acid (UDCA) was administered to 54 patients (83.1%), leading to stone dissolution in 22 patients (33.8%) within 6 months. Cholecystectomy was performed in 18 patients (27.7%) (mean age, 11.9±5.1 years). Most patients treated surgically had multiple stones (83%) and stones measuring <5 mm in size (89%), and 66.7% of patients had cholesterol stones. Conclusion: Cholecystectomy is feasible in patients with small-sized or large numbers of GS and those with persistent abdominal pain and/or jaundice. UDCA administration with close follow-up is recommended in patients with uncomplicated GS.

Tylosin has been used as a livestock feed additive and antibiotic growth promoter for many years. However, the mode of action by which tylosin enhances animal growth is unclear. We used high-throughput sequencing of 16S rRNA genes to investigate the effects of tylosin as a feed additive on swine gut microbiota. No significant difference in the rate of weight increase was observed between control and tylosin-treated pigs during a 10-week feeding trial. However, tylosin-treated pigs showed rapid increases in the relative abundance of the phylum Firmicutes. Increases in Firmicutes species are associated with (so-called) obese-type gut microbiota. The abundance of species of four families of the phylum Firmicutes (Streptococcaceae, Peptococcaceae, Peptostreptococcaceae, and Clostridiaceae) correlated positively with host weight gain. The abundance of Streptococcaceae family bacteria was least affected by tylosin treatment. Distribution analysis of operational taxonomic units (OTUs) showed that both control and tylosin-treated pigs exhibited similar OTU alterations during growth. However, the tylosin-treated group showed distinctive alterations in gut microbiota when the host weighed approximately 60 kg, whereas similar alterations occurred at around 80 kg in the control group. Our results suggest that use of tylosin accelerates maturation of swine gut microbiota rather than altering its composition.
The application of Bacillus sp. in the biological control of plant soilborne diseases has been shown to be an environmentally friendly alternative to the use of chemical fungicides. In this study, the effects of bioorganic fertilizer (BOF) fortified with Bacillus amyloliquefaciens SQY 162 on the suppression of tomato bacterial wilt were investigated in pot experiments. The disease incidence of tomato wilt after the application of BOF was 65.18% and 41.62% lower at 10 and 20 days after transplantation, respectively, than in the control condition. BOF also promoted the plant growth. The SQY 162 populations efficiently colonized the tomato rhizosphere, which directly suppressed the number of Ralstonia solanacearum in the tomato rhizosphere soil. In the presence of BOF, the activities of defense-related enzymes in tomato were lower than in the presence of the control treatment, but the expression levels of the defense-related genes of the plants in the salicylic acid and jasmonic acid pathways were enhanced. It was also found that strain SQY 162 could secrete antibiotic surfactin, but not volatile organic compounds, to suppress Ralstonia. The strain could also produce plant growth promotion compounds such as siderophores and indole-3-acetic acid. Thus, owing to its innate multiple-functional traits and its broad biocontrol activities, we found that this antagonistic strain isolated from the tobacco rhizosphere could establish itself successfully in the tomato rhizosphere to control soilborne diseases.
Vibrio metschnikovii strain RH530 is a non-pathogenic, industrially-important alkaline protease producer which has been isolated from wastewater. In this paper, we report on the transformation of this strain by using the method of electroporation. A field strength of $7.5\;kVcm^{-1}$ and $25\;{\mu}F$, and using a 0.2-cm cuvette, appeared to be the optimal conditions for electroporation of the cells with the recombinant pSBCm plasmid carrying the vapK alkaline protease gene and the ColE1 replicon. Cells were subjected to osmotic shock in order to remove extracelluar DNase, and adding 200 mM of sucrose to electroporation buffer cells showed an increased transformation efficiency. Maximum efficiency of transformation was obtained at an early exponential growth phase. Using all of the conditions mentioned above, we routinely obtained a transformation efficiency of more than $10^4{({\mu}g\;plasmid\;DNA)}^{-1}$. The stability of the plasmid pSBCm in V. metschnikovii RH530 was 25% after 18h of growth (27 generations) in the medium without antibiotic selection. The insertion of the par locus to the pSBCm increased the stability of the plasmid up to 42% without selective pressure. The increase in plasmid stability was accompanied by the increase in the productivity of alkaline protease in the recombinant V. metschnikovii strain RH530. Determining optimal conditions for the transformation of the industrially-important, nonpathogenic Vibrio strain, and the improvement of plasmid stability by introducing the par locus into the high copy number plasmid vector, will allow the development of procedures involved in the genetic manipulation of this strain, particularly for its use in the production of industrial enzymes such as alkaline protease.
Staphylococal infection still remains to be one of the most serious infections, having various complications in the clinical use of indwelling polymeric medical devices. However, there are a few promising systems showing a high antibacterial effect without causing any demage of polymer backbone under biological environments such as blood or body fluid. In order to resolve this problem, we have designed a new antibiotic releasing system via a hydrolysis mechanism. The surface of biomedical polyurethane (PU) was modified by using 1,6-diisocyanatohexane (HMDI) to immobilize the rifampicon. Also, the immobilized rifampicin was designed to be released by a selective cleavage of the unstable carbamate linkage that exists on the rifampicin-immobilized polyurethane (PHR). The immobilization of rifampicin on the surface of polyurethane was confirmed by the disappearance of the characteristics IR absorbance peak of the isocyanate (-NCO) group at $2,267\;cm^{-1}$. The PHR showed a continuous rifampicin release profile under an aqueous environment of 10 mM of PBS (phosphate-buffered saline) for ove 6 days. The rifampicin molecules, which are released from PHR under an optimal bacterial infection environment, had a higher antibacterial activity against both S. aureus and S. epidermidis than rifampicin-incorporated polyurethane (RIP). In addition, the PHR maintained a stable antibacterial effect under a blood-mimic aqueous environment such as bovine calf serum.

Background: The prevalence of extended-spectrum ${\beta}$-lactamase (ESBL)-producing Escherichia coli is increasing rapidly worldwide. Treatment options for ESBL-producing E. coli are limited, and infections caused by this organism are associated with improper antibiotic use, a long hospital stay, and increased mortality. Thus, the assessment and early recognition of the risk factors of nosocomial infections due to ESBL-producing E. coli are important for the infection control and proper treatment. Methods: A case-control study was performed that included nosocomial episodes of ESBL-producing E. coli bacteremia at a tertiary care hospital from January 2004 to December 2007. For each case patient, three controls were randomly selected and data on predisposing factors were collected. Results: Fifty-five cases of nosocomial ESBL-producing E. coli bacteremia were studied. Carbapenem usage (OR: 11.3, 95% CI: 1.1-115.9, p=0.041), quinolone usage (OR: 4.5, 95% CI: 1.1-18.8, p=0.042), biliary obstructive disease (OR: 11.8, 95% CI: 3.0-46.7, p<0.001) and the APACHE II score (OR: 1.3, 95% CI: 1.2- 1.5, p<0.001) were analyzed as independent risk factors of nosocomial ESBL-producing E. coli bacteremia. Conclusion: Our results showed that physicians caring for patients with risk factors of nosocomial bacteremia should consider ESBL-producing E. coli as the causative organisms of the disease.
