폐결핵 치료중 발현된 척수내결핵종 및 다발성 뇌결핵종 1예 (A Case of Intramedullary Spinal Tuberculoma and Multiple Brain Tuberculoma Associated with Pulmonary Tuberculosis)

  • 폐결핵을 치료하는 중 갑작스럽게 생긴 하지의 근력저하와 배뇨장애를 호소하는 환자에서, MRI를 이용하여 척수내 결핵종이 진단되었다. 항결핵 제재와 대량의 스테로이드를 5일간 사용한 후 신경학적 증상은 호전되어 결핵치료를 지속하던 중 20일째 전신경련이 발생하여 다발성 뇌결핵종이 동반되어 있음이 MRI상에 나타났다. 이후 내과적 치료만으로 방사선 소견과 임상증상이 호전되어 가고 있어 9개월째 있는 증례이다. 우리나라는 결핵의 유병률이 높고 다제약제내성결핵이 늘고 있어, 신경학적 증상을 호소하는 환자에서는 반드시 중추신경계 결핵을 염두에 두고 MRI를 시행하면 중추신경계 결핵종의 발견율이 높아질 것으로 사료되어 이에 보고하는 바이다.

Recent Advances in Tuberculosis and Nontuberculous Mycobacteria Lung Disease

  • Tuberculosis (TB) is one of the largest health problems in the world today. And the incidence of nontuberculous mycobacteria (NTM) lung disease appears to be increasing worldwide. Recently, an automated, nucleic acid amplification assay for the rapid detection of both Mycobacterium tuberculosis and rifampin resistance was developed (Xpert MTB/RIF). And fixed-dose combinations of anti-TB drugs and linezolid have been introduced in the treatment of TB. And new NTM species, named Mycobacterium massiliense, which is very closely related to Mycobacterium abscessus was reported. In this review, these recent advances in the diagnosis and treatment of TB and clinical characteristics of M. massiliense lung disease are discussed.

혀에서 발생한 일차성 결핵 (Primary Tuberculosis of the Oral Tongue: A Case Report and Literature Review)

  • Tuberculosis is primarily a pulmonary disease and extrapulmonary involvement of the oral cavity is an infrequent occurrence accounting for 0.2 - 1.5% of all extrapulmonary sites. The authors report a case of primary tuberculosis of the oral tongue in a 60-years-old male patient who visited the clinic for ongoing tongue pain that he experienced for several months. The lesion was initially indistinguishable from the malignancy, but differential diagnosis was performed on the biopsy with Ziehl-Neelseen stain. After treatment with anti-tuberculosis, the oral lesion healed completely without surgery.

  • A mixed infection of Mycobacterium abscessus subsp. abscessus (Mab) and Mycobacterium tuberculosis (MTB) in the lung is an unusual clinical manifestation and has not yet been reported. A 61-year-old woman had been treated for Mab lung disease and concomitant pneumonia, and was diagnosed with pulmonary tuberculosis (PTB). Despite both anti-PTB and anti-Mab therapy, her entire left lung was destroyed and collapsed. She underwent left pneumonectomy and received medical therapy. We were able to successfully treat her mixed infection by pneumonectomy followed by inhaled amikacin therapy. To the best of our knowledge, thus far, this is the first description of a mixed Mab and MTB lung infection.

Fistula Formation between Right Upper Bronchus and Bronchus Intermedius Caused by Endobronchial Tuberculosis: A Case Report

  • Endobronchial tuberculosis is defined as a tuberculous infection of the tracheobronchial tree and has a prevalence of up to 50% in active pulmonary tuberculosis cases. The most common complication of endobronchial tuberculosis is bronchial stenosis; benign fistula formation by endobronchial tuberculosis is rare, especially inter-bronchial fistula formation. We reported a rare case of a 73-year-old woman with a fistula between the right upper bronchus and bronchus intermedius. A diagnosis of inter-bronchial fistula caused by endobronchial tuberculosis was based on the results of chest computed tomography scans, bronchoscopy, and microbiological and pathological tests. The patient was treated with anti-tuberculous medication, and her symptoms gradually improved.

Primary orbital tuberculosis on the lower eyelid with cold abscess

  • Orbital tuberculosis is a rare form of extrapulmonary tuberculosis, even in endemic areas. It may involve the soft tissue, lacrimal gland, periosteum, or bones of the orbital wall. We present a case of orbital tuberculosis on the lower eyelid. An 18-year-old woman with no underlying disease visited our clinic for evaluation of an oval nodule ($1.5{\times}1.2cm$) on the right lower eyelid. Incision and drainage without biopsy was performed 2 months ago in ophthalmology department, but the periorbital mass had deteriorated, as the patient had erythematous swelling, tenderness, and cervical lymphadenopathy. Visual acuity was normal; there were no signs of proptosis, diplopia, or ophthalmoplegia. Computed tomography revealed a small abscess cavity without bony involvement. We performed an excision and biopsy through a percutaneous incision under local anesthesia. Histological examination revealed a granuloma and was diagnosed as orbital tuberculosis. The patient was additionally treated with anti-tuberculosis therapy for 6 months and recovered without complication or recurrence by 7 months. Orbital tuberculosis occurs in patients with or without associated pulmonary tuberculosis, and should be considered as a differential diagnosis in patients with inflammatory orbital disease and an orbital mass. If recurrence occurs despite adequate initial treatment, we recommend an additional examination and excisional biopsy.

결핵환자에서 말초혈액과 흉막액내 ${\gamma}{\delta}$ T 림프구의 의의 (The Clinical Significance of ${\gamma}{\delta}$ T lymphocytes in patients with pleural tuberculosis)

  • 연구배경 : 최근 알려진 ${\gamma}{\delta}$ 수용체는 결핵균 감염의 초기에 제2형 주요 조직적합성 복합계(MHC class II)의 인식없이 결핵균 항원에 반응하여 세포성 면역반응을 나타냄이 보고되었다. 이에 연구자등은 페결핵환자와 결핵성 흉막염 환자, 그리고 다른 원인의 흉막염 환자사이에 T 림프구의 조성과 ${\gamma}{\delta}$ T-림프구 수의 차이를 관찰하였다. 방법 : 대상은 폐결핵환자 30예(이중 결핵성 흉막염환자 15예), 폐암 환자 12예(이중 악성 흉막염 환자 9예), 폐렴 7예(이중 폐렴성 흉막염 6예)등 모두 49예였다. 혈청 ADA(adenosine deaminase)활성도는 Hitachi 747 자동화학분석기에서 측정하였다. T 세포 림프구 아형의 분류는 lysed whole blood method로 anti-Leu4, anti-Leu3a, anti-Leu2a, anti HLA-DR 그리고 anti-TCR-${\gamma}{\delta}$-1를 이용하여 flow cytometer로 분석하였다. 결과 : 1. 말초혈액내 ${\gamma}{\delta}$-T 림프구의 평균치는 $4.8{\pm}4.6%$ 였고, 결핵군(29예) $5.5{\pm}4.5%$, 비결핵군(14예) $3.3{\pm}2.9%$(폐암군 $4.0{\pm}3.2%$, 폐렴군 $2.2{\pm}1.6%$로 유의한 차이는 없었다(p=0.24). 질병의 이환기간 1개월 이내의 환자중에서도 결핵군(20예) $6.4{\pm}6.6%$, 비결핵군(14예) $3.3{\pm}2.9%$ 으로 유의한 차이는 없었다(p=0.16)(Table 1). 2. 흉막액내 T 세포 림프구 아형중 CD4 림프구는 결핵성 흉막액에서는 $54.6{\pm}13.8%$, 비결핵성 흉막액에서는 $36.2{\pm}25.3%$(악성 흉막액 $38.4{\pm}23.8%$, 폐렴정 흉막액 $30.1{\pm}34.0%$ 결핵성 흉막액에서 의의있게 높았다(p=0.04)(Table 2). 3. 흉막염이 있던 환자에서 말초혈액내 ${\gamma}{\delta}$-T 림프구는 결핵성 흉막염군(14예)이 $7.0{\pm}9.0%$, 비결핵성 흉막염군(11예) $3.0{\pm}2.0%$ (악성 흉막염군 $3.1{\pm}2.2%$, 폐렴성 흉막염군 $2.7{\pm}1.7%$로 차이는 없었다(p=0.16). 흉막액내 ${\gamma}{\delta}$-T 림프구는 결핵성 흉막염군(15예)이 $3.9{\pm}2.9%$, 비결핵성 흉막염군(10예) $2.1{\pm}2.2%$(악성 흉막염군 $2.0{\pm}2.5%$, 폐렴성 흉막염군 $2.4{\pm}1.7%$ 로 유의한 차이가 없었다(p=0.12). 4. 환자의 연령이나 성별과 말초혈액내 ${\gamma}{\delta}$-T 림프구수와는 상관관계가 없었고, 폐결핵 환자에서 병변의 정도, 혈청 및 흉막액내 ADA와 ${\gamma}{\delta}$-T 림프구수와도 상관관계가 없었다. 결론 : 결핵성 흉막염환자에서 말초혈액 및 흉막액내 ${\gamma}{\delta}$-T 림프구수의 유의한 증가는 없어 다른 질환과의 감별진단에 도움이 되지 못할 것으로 생각되며, ${\gamma}{\delta}$-T 림프구의 증가는 결핵 초기 환자들을 대상으로 추가 연구가 필요할 것으로 생각된다.

애디슨병 환자에게 리팜핀 투여 후 발생한 급성 부신피질기능 저하증 1예 (A Case of Rifampin-Induced Recurrent Adrenal Insufficiency During the Treatment of Pulmonary Tuberculosis in a Patient with Addison's Disease)

  • Adrenal insufficiency during the treatment of pulmonary tuberculosis is a troublesome condition and can at times be lifethreatening if untreated. Rifampin is one of the most widely prescribed anti-tuberculosis agents. Furthermore, rifampin has been known to be capable of affecting the metabolism of various medications, including glucocorticoids. In this paper, a case of recurrent adrenal insufficiency induced by rifampin during the treatment of pulmonary tuberculosis is reported. The patient was a 63-year-old man who was diagnosed with Addison's disease 17 years earlier and had been undergoing glucocorticoid replacement therapy. Five months before, the patient manifested pulmonary tuberculosis and was immediately given anti-tuberculosis medication that included rifampin. After one week of medication, general weakness and hyponatremia occurred. Despite the increased dose of the glucocorticoid medication, the adrenal insufficiency recurred many times. Since the substitution of levofloxacin for rifampin, the episodes of adrenal insufficiency have not recurred so far.

Effect of tuberculosis treatment on leptin levels, weight gain, and percentage body fat in Indonesian children

  • Purpose: Tuberculosis (TB) remains a problem in the community. TB patients usually experience malnutrition, which is characterized by both decreased body weight (BW) and body fat percentage (BFP). Leptin, an important regulator of BW, also plays an important role in cellular immunity, which is integral to defense against Mycobacterium tuberculosis infection. We analyzed the effect of an anti-TB treatment regimen on the leptin level, BW, and BFP of children with TB. Methods: The design of this study was a group interrupted time series. The subjects were children with probable TB according to clinical criteria based on an Indonesian scoring system adopted from the Consensus of Expert Panel. BW; BFP; energy intake; fat and protein intake; and leptin levels before, 2 months after (intensive phase), and 6 months after (continuation phase) anti-TB treatment, were measured. About 40 children, aged 5-14 years, participated in this study. Results: The BW, BFP and leptin level increased from before treatment to after completion of the intensive phase and still showed an increased during the continuation phase: BW 18.65 kg, 19.75 kg, and 20.85 kg; BFP 18.3%, 19.5%, and 20.2%; and leptin level 1.9 mg/dL, 3.07 mg/dL, and 3.4 mg/dL, respectively (P<0.01). Conclusion: Leptin level, BW, and BFP increased throughout the course of anti-TB treatment, compared with pretreatment values. Further research is needed to compare the results with data for healthy children.

경부 결핵성 림프절염의 임상양상과 치료 (Clinical Manifestations and Therapy of Tuberculous Cervical Lymphadenitis)

  • Background and Objective: The tuberculous lymphadenitis of neck is one of the most common extra-pulmonary tuberculosis in Korea. Although the incidence of pulmonary tuberculo-sis has decreased recently, that of cervical tuberculous lymphadenitis has not decreased. In spite of great efforts and diversity of study, the exact criterias of diagnosis and optimal therapeutic methods of cervical tuberculous lymphadenitis have been the subject of much debate and still remain unclear. So we intend to enucleate clinical manifestations and suggest the optimal therapeutic manners. Material : The 483 cases, diagnosed as cervical tuberculous lymphadenitis by fine needle aspiration biopsy during the past 10 years from Jan. 1987 to Dec. 1996 Method : Retrospective study Results 1) The overall rate of tuberculous cervical lymphadenitis was 23.4% of neck mass. 2) Incidence ratio of male to female was 1:2.7 3) The frequent location of tuberculous lymphadenitis was posterior cervical area, supraclavicular area, jugular chain in order. 4) The response rate of medical treatment in tuberculous cervical lymphadenitis was 84.9%. 5) The duration of medical treatment in remissioned group was 18.6 months in average. 6) Surgical intervention was needed in 15.1%. 7) The duration of post operative medical treatment was 18.4 months in average. Conclusion : Tuberculous cervical lymphadenitis is prevalent in women, age of 20-40 years and mainly involve posterior cervical area. Fine needle aspiration biopsy is a very useful method for early detection of cervical tuberculous lymphadenitis. After diagnosis is made, anti-tuberculosis medication is recommended for more than 18 months. Unless the size of neck mass is decreases inspite of the thorough anti-tuberculosis medication for more than 1 month or if complication like as abscess or fistula occurs, surgery is needed with post operative medical treatment for more than 12 months.
