A Morphometric Study of the Obturator Nerve around the Obturator Foramen

  • Objective : Obturator neuropathy is a rare condition. Many neurosurgeons are unfamiliar with the obturator nerve anatomy. The purpose of this study was to define obturator nerve landmarks around the obturator foramen. Methods : Fourteen cadavers were studied bilaterally to measure the distances from the nerve root to relevant anatomical landmarks near the obturator nerve, including the anterior superior iliac spine (ASIS), the pubic tubercle, the inguinal ligament, the femoral artery, and the adductor longus. Results : The obturator nerve exits the obturator foramen and travels infero-medially between the adductors longus and brevis. The median distances from the obturator nerve exit zone (ONEZ) to the ASIS and pubic tubercle were 114 mm and 30 mm, respectively. The median horizontal and vertical distances between the pubic tubercle and the ONEZ were 17 mm and 27 mm, respectively. The shortest median distance from the ONEZ to the inguinal ligament was 19 mm. The median inguinal ligament lengths from the ASIS and the median pubic tubercle to the shortest point were 103 mm and 24 mm, respectively. The median obturator nerve lengths between the ONEZ and the adductor longus and femoral artery were 41 mm and 28 mm, respectively. Conclusion : The obturator nerve exits the foramen 17 mm and 27 mm on the horizontal and sagittal planes, respectively, from the pubic tubercle below the pectineus muscle. The shallowest area is approximately one-fifth medially from the inguinal ligament. This study will help improve the accuracy of obturator nerve surgeries to better establish therapeutic plans and decrease complications.

Usefullness with 3D CT Angiography in Microsurgical Reconstruction (미세수술을 이용한 하지의 재건시 삼차원 컴퓨터 혈관조영술의 유용성)

  • Preoperative angiography is frequently used in the planning of microsurgical reconstruction for identification of vascular abnormality that influence the planning of operation. But, recently 3D CT angiography is considered as new technique that can provide detailed information about vascular anatomy as well as soft and bony tissue without the risks of invasive angiography. 3D CT angiograms were performed in 19 patients before microsurgical reconstruction for the lower extremity and hand between May of 2003 and Oct of 2004. Sixteen of the studies were of the donor site and all of 19 studies were of the recipient site. No complications were found from the 3D CT angiograms. In one case of the bone exposed open wound, the injury of anterior tibial artery was identified and the zone of injury was adequately demonstrated. With the improvement in quality of CT imaging, 3D CT angiograms may provide a favorable alternative to invasive angiography. It is capable of providing high-resolution, three dimensional vascular imaging without the need for arterial puncture and prolonged post-procedure observation. The relation among blood vessels, bones, and soft tissue is well demonstrated in 3D CT angiogram. Also The acquisition time and examination cost were considerably lower in comparison with invasive angiography. In conclusion, this study demonstrates that 3D CT angiography may provide accurate, safe, and cost-effective preoperative imaging. The 3D CT angiography with relatively low morbidity, low cost, ease of image acquisition can have an broader role in microsurgical reconstructive surgery.

The Anatomy and Histology of the Olfactory Organ in the Korean Sand Goby Favonigobius gymnauchen (Pisces, Gobiidae) (한국산 날개망둑 Favonigobius gymnauchen (Pisces, Gobiidae) 후각기관의 해부 및 조직학적 연구)

  • The anatomy and histology of the olfactory organ in Favonigobius gymnauchen was investigated using a stereo microscopy, light microscopy and scanning electron microscopy. The paired olfactory organs in the dorsal snout are set in between the upper lip and the eyes. These organs are composed of two openings (anterior nostril with a tubular structure and posterior nostril), a single olfactory cavity, two nasal sac (ethmoidal and lacrimal sacs), olfactory nerve and olfactory bulb. The distributional pattern of the sensory epithelium is a only one type (continuous type). This epithelium is made up of the receptor cell, supporting cell and basal cell. The receptor cell has a only one type (ciliated receptor cell with 3~4 cilia). The non-sensory epithelium is built of the stratified epithelial cells and has mucous openings on the surface. Such an olfactory organ in F. gymnauchen may be considered to reflect its ecological habitat as a shallow water or tidal pool in the coastal zone.

Anatomic Consideration of the C1 Laminar Arch for Lateral Mass Screw Fixation via C1 Lateral Lamina : A Landmark between the Lateral and Posterior Lamina of the C1

  • Objective : To clarify the landmark for deciding the entry point for C1 lateral mass screws via the posterior arch by using 3-dimensional (3D) computed images. Methods : Resnick insisted that the C1 posterior arch could be divided into pure posterior and lateral lamina (C1 pedicle). Authors studied where this transition point (TP) is located between the posterior lamina and the C1 pedicle and how it can be recognized. The 3D computed images of 86 cadaver C1s (M : F=45 : 41) were used in this study. Results : The superior ridge of the C1 posterior arch had 2 types of orientation. One was in the vertical direction in the C1 posterior lamina and the other was in the horizontal direction in the C1 pedicle. The TP was located at the border between the 2 areas, the same site as the posterior end of the groove of the vertebral artery. On posterior-anterior projection, the posterior arch was sharpened abruptly at TP. We were unable to identify the TP in 6.4% of specimens due to complete or partial osseous bridges. A total of 93.8% of the TP were located between the most enlarged point of the spinal canal and the medial wall of the vertebral artery. Conclusion : The anatomic entry zone of C1 lateral laminar screws was clarified and identified based on the TP by using preoperative 3D computed images.

DIAGNOSIS AND TREATMENT PLAN OF MAXILLARY IMPACTED CANINE (상악매복견치의 진단 및 처치)

  • Upper canine is important because it protects and maintains the stability of the dental arch and also, joins the anterior with the posterior teeth. The incidence of impaction of upper canine is the second most frequent next to the third molar because it takes a long period of time to develop, and has a complicated path of eruption, and erupts lately. After the age of 10, clinical and radioglaphic examination can be used in revealing the possibility of impaction and efforts should be put to reduce the side effects. To prevent impaction, selective extraction of primary canine at the age of 8 to 9 could be considered and prolonged retention of primary canine in oral cavity should be avoided at this time. Once the impaction is iden, the first stage of the treatment is to lcocalize the lesion by radiographic examination and According to the severity, orthodontic traction or autotransplantation should be considered and comprehensive diagnosis and treatment plan of malocclusion should be established. Generally, labial impaction is due to arch length discrepancy and palatal impaction is due to malposition or morphologic pathosis of lateral incisors rather than arch length discrepancy. In surgical procedure, peridontal problems should be considered and the minimum amount of bone and soft tissue should be reduced and direct bonding method of many attachment methods should be recommended. Especially in traction of labially impacted canine, it should be guided to erupt through the keratinized zone and proper forced magnitude should be applied. The importance of periodontal condition should always be in mind following the patient education to mintain the good oral hygiene at each stage of treatment. Properly managed impacted canine can provide function and esthetic by proper diagnosis and treatment if extraction of canine is not indicated.

Reproductive Cycle of the Black-lined Limpet, Cellana nigrolineata (흑색배말, Cellana nigrolineata의 생식주기)

  • Gametogenesis, reproductive cycle, egg diameter composition and gonad index of the black-lined limpet, Cellana nigrolineata, were histologically examined. Specimens collected from the rocky intertidal zone of Hamdeok, Jeju from July 1997 to June 1998. The black-lined limpet has an unpaired gonad located at the ventro-anterior part, underneath the visceral mass. The gonad index increased from May when water temperature increased and reached maximum in August. It began to decrease from September thereafter, maintain a low value from January to April. The following reproductive cycle were classified based on the monthly changes of the histological features and sizes of oocytes in the gonad: recovery stage (November to April), active stage (March to June), ripe stage (May to October), spent stage (August to December). Spawning occur once a year between September and October. The black-lined limpet appeared to be gonochorism, neither sex change nor hermaphroditie.

The Pharmacological Effects of KR-30450 , A Potassium Channel Opener on Coronary Artery Occlusion / Reperfusion-Induced Myocardial Infarction in the Rat (흰쥐에서의 관상동맥 결찰/재관류도 유도된 심근경색에 대한 칼륨통로 개방제 KR-30450의 약리학적 효과)

  • The pharmacological effects of benzopyran potassium channel openers (lemakalim, KR-30450 and KR-30818) on the occlusion/reperfusion-induced myocardial infarction were investigat ed. In anesthetized rats, subjected to 45-min occlusion of the left anterior descending coronary artery (LAD) followed by 90-min reperfusion, the infarct size was measured by calculating the ratio of infarct zone to area at risk (IZ/AAR) with the Evans blue/TTC technique. Rats were intravenously given vehicle (1% DMSO), lemakalim, KR-30450, and KR-30818 alone or in combination with a selective K$_{ATP}$ blacker glibenclamide, 30 min prior to coronary occlusion. Compared to vehicle, lemakalim (30 ${\mu}$g/kg i.v.), the active enantiomer of cromakalim, had a tendancy to decrease infarct size. KR-30450(30 ${\mu}$g/kg, i.v.). the newly synthetized potassium channel openers (PCOs), caused a reduction of infarct size (from 70${\pm}$4%to 57${\pm}$5%). but KR-30818 (30 ${\mu}$g/kg, i.v.), a metabolite of KR-30450. did not modify infarct size. It seem ed likely that glibenclamide (0.3mg/kg, i.v.), given in combination, reduced the effects of these PCOs, especially KR-30450 (30 ${\mu}$g/kg, i.v.) on the infarct size. These results indicate that. in the coronary occluded rat model of ischemia, lemakalim and KR-30450 may exert cardioprotective activity through a reduction of infarct size, the effect being considered related to the opening of K$_{ATP}$ channel.

Differences Between Facial Skin Temperature of the Paralyzed Side and Those of the Normal Side in Bell's Palsy Patients (Bell's Palsy 환자의 건측-환측 안면부 피부온도차이에 관한 연구)

  • Objectives : Bell's palsy in an acute peripheral facial nerve paralysis that usually affects only one side of the face. The seventh cranial nerve carries predominantly motor fibers, but also supplies some autonomic innervation, sensation to park of the ear, and taste to the anterior two thirds of the tongue. The aim of this study is to provide evidence of differences between facial skin temperature of the paralyzed side and normal side in Bell's palsy patients. Methods : the author studied 68 patients with Bell's palsy whose facial nerve function had been documented by the House-Brackmann grading system. We measured skin temperature of the forehead, zygoma, lower lip, temple and lower jaw area of the paralyzed side and those of the normal side. Results : there were significant facial skin temperature differences between the forehead area of paralyzed side and that of normal side of GrII(P<0.05) and III(P<0.05). The result also showed that the facial skin temperature difference according to the aflection period vanished when air temperature was calibrated (F=1.700, P=0.178). Conclusions : Thermography is a useful diagnostic tool in Bell's palsy if the air temperature is low enough to cool the facial skin temperature and the forehead area is evaluated as the sampling zone.

Gradual Reperfusion Lowers the Incidence of Reperfusion-Induced Ventricular Fibrillation in a Cat Model of Regional Ischemia

  • Blood flow restoration to ischemic zone of the heart is essential to salvage of ischemic tissue. However, there is a large body of evidence documenting that the reperfusion can induce reperfusion injury like reperfusion-induced malignant arrhythmias. In the present study, employing a cat model of regional cardiac ischemia, we examined if reperfusion rendered in a gradual fashion could lower the incidence of reperfusion-induced ventricular fibrillation (VF), which usually precipitated within a few to several tens of seconds after abrupt reperfusion. The experiments were conducted with male mongrel cats (n=46, 2.5-5 kg). The animals in the control and 30 MIN groups were subjected to an episode of 20- and 30-min left anterior descending coronary artery occlusion, respectively, followed by abrupt reperfusion. The animals in 5 G and 10 G groups received gradual reperfusion over a 5- and 10-min period, respectively, following a 20-min occlusion. The proportion of animals that exhibited VF during the reperfusion phase was 11/15 in the control, 7/10 in the 30 MIN, 5/10 in the 5 G and 2/11 in the 10 G groups. The incidence of VF in the 10 G group was significantly lower than that in the control or 30 MIN group subjected to abrupt reperfusion. These results suggest that the gradual reperfusion is a useful procedure against reperfusion-induced VF.

Common Trunk Anomalies Associated with Hemifacial Spasm (반얼굴연축과 관련된 공통줄기기형)

  • Background: The compression of 7th cranial nerve by arteries is one of the various causes of hemifacial spasm (HFS). A few previous studies were revealed the relation between the compression of 7th cranial nerve and common trunk anomaly. We evaluated the common trunk anomalies in patients with HFS using MRI and MRA. Methods: From January 2001 to December 2005, 41 consecutive patients (9 men, mean age $54.5{\pm}12.6$) with HFS underwent MRI and MRA. T2 axial images and time-of-flight angiographies were reviewed for identification of the compression at root exit zone by two neuroradiologists and one neurologist. Results: Thirty-seven patients showed neurovascular compression on the lesion side. Twenty patients of them were shown the compression of 7th cranial nerve by anterior inferior cerebellar artery (AICA), and seventeen patients of them were shown the compression by posterior inferior cerebellar artery (PICA). Twenty-four patients of the thirty-seven patients had common trunk anomaly. In control, twelve of twenty-one subjects had common trunk anomaly, that the frequencies of common trunk anomaly of two groups were 58.8% in HFS and 57.1% in controls. In the twenty-four patients with common trunk anomaly, eighteen patients had dominant-AICA, and six patients had dominant-PICA. The rate of nerve compression by common trunk anomaly in the HFS with unilateral common trunk, dominant-AICA was 76.5% and dominant-PICA was 100%. Conclusions: This study also revealed that AICA was most common compressive artery. There was no difference between the HFS groups and control groups in frequency of common trunk anomaly. Thus, we could not demonstrate the relationship between common trunk anomaly and HFS.
