  • Crown fractures are relatively common trauma to anterior teeth, and should be restored immediately in most cases. For those who suffer from unfortunate traumatic episode, the best treatment option should be minimally invasive approach. In the presence of fractured tooth fragment, reattachment procedure creates positive emotional response in the patient and simplifies the procedure and maintenance of the patient's original tooth anatomy and occlusion. Without fractured tooth fragment, next conservative option could be direct composite restoration which is based on minimal invasion concept. This article proposes simple and very conservative techniques that anyone can do in daily practice.

Esthetic Restoration of Deciduous Teeth Using Prefabricated Zirconia Crown in Children with Early Childhood Caries: A Report of Three Cases

  • Early childhood caries is a widespread condition that requires attention; however, its treatment remains a challenge in terms of child behavior management. This study describes the usefulness of customized zirconia crowns for the restoration of deciduous teeth through the evaluation of some cases. Three cases are described: a 29-month-old girl who presented with severe early childhood caries affecting anterior tooth, a 50-month-old boy who presented with extensive caries of his anterior tooth, and 70-month-old girl who presented with extensive caries of his primary posterior dentition. These prefabricated, zirconium-based ceramic crowns ($Nusmile^{TM}$ NuSmile) are available in various sizes, shapes and colors. Before the treatment, radiographs and intraoral photographs were taken to evaluate the appropriateness of treatment. Our findings indicate that customized zirconia crowns may be appropriate for restoring the deciduous teeth. Further long-term clinical studies are required to clarify the usefulness of this restorative method.

Triplication of upper right primary anterior teeth : a case report (상악 우측 유전치부에 발생한 삼중치 : 증례 보고)

  • The fusion is an anomalous union of two or more tooth germ at some stage in the development process resulting in a decreased number of teeth in the dental arch. Fusion is common odontogenic anomaly but triplication of primary teeth is a rare phenomenon. We report a case of a 3 - year - old girl who visited our clinic for the pain on maxillary anterior tooth. The patient was diagnosed by the fusion of a the maxillary primary right central and lateral incisor with a supernumerary tooth and caries of dentine. And then, Endodontic treatment and composite resin restoration were performed on the triple teeth. After follow-up of 6 months period, there was no marked complications.

  • Supernumerary tooth develops as a result of abnormal proliferation of the dental lamina during the initiation stage of dental development. It could be a sporadic occurrence or a hereditary transmission. Supernumerary tooth occurs with a frequency of 1 to 3%. Generally, there is a 2:1 preference for boys. It is usually found in the anterior portion of the maxilla and may be associated with complications such as impaction, malposition of permanent teeth, formation of diastema, cysts and eruption into nasal cavity, The position of supernumerary tooth found in the anterior portion of the maxilla is determined by the axis of the tooth. One third of supernumerary tooth in the anterior portion of the maxilla has no possibility of eruption due to its invertedly impacted position. However, as long as the coronal part of the follicle remains intact, migration of supernumerary tooth is possible. The migration may occur into the palate, the premolar region, the sinus or the nasal cavity. Also, growth of vertical dimension of maxilla could make surgical approach difficult as time goes by For this reason, we have found invertedly impacted mesiodens moved to the vault of the palate in the two cases, and extracted supernumerary tooth surgically.

  • A tooth solely can not be an exclusive factor to attain our goal, esthetics. Other consideration, such a proportion of individual tooth, harmonized arrangement of the dentition and surrounding tissues including gingiva, lip and face should be taken into account. Esthetic gingival pattern and incisal level in the anterior area are the very important factors for esthetic restoration of the smile. We often propose adequate periodontal and prosthodontic approaches in order to improve the esthetics of the anterior area. Here, I would like to share some cases which include soft tissue management for ovate pontic placement, osteoplasty, resin filling for severe cervical caries, and bleaching approach to discolored root caused by endodontic treatment, and so on.

  • To improve esthetics and gain beautiful smile, the maxillary anterior dentition is crucial. Through alteration of height, arrangement or color of the maxillary incisor, we can rehabilitate the esthetic smile. The perception of dental esthetics is highly subjective. Personal perceptions or judgement of dental esthetics is highly related with each individual's experience and social and cultural environment. However, there have been many efforts to establish the criteria for generally accepted esthetic norm, in order to increase the predictability of restorative treatment. For maxillary anterior restoration, not only a single tooth, but also the compositions around tooth such as facial form, lips and gum and their relationships have to be considered to create harmonious smile. It can be determined as esthetic restorations when in consonance with facial form and structures that frame the restorations. In this review article, several guidelines that are generally accepted and useful to assess the esthetics and communicate with patients and technicians will be discussed.

Golden proportion assessment between maxillary and mandibular teeth on Indian population

  • PURPOSE. This study evaluated the existence of golden proportion between the widths of the maxillary and mandibular anterior teeth in Indian population. MATERIALS AND METHODS. The clinical tooth width measurements were recorded with the digital vernier calipers on 576 patients of both sexes in the age group of 21 - 30 years. Flexible ruler was used to determine the width of maxillary and mandibular anterior teeth on the patients by the same operator. The data obtained was statistically analyzed using paired student t-test (${\alpha}$=.05). RESULTS. The golden proportion was not found between the width of the right central and lateral incisors in 53% of women and 47% of men. The results revealed the golden percentage was rather inconstant in terms of relative tooth width. CONCLUSION. The golden proportion is an inappropriate method to relate the successive widths of the maxillary anterior teeth in Indian population.

  • Anterior set back segmental surgery has been used for shortening the period of orthodontic treatment in case of bimaxillary or maxillary protrusion. In most cases, it requires pre-operative orthodontic treatment. Through properly performed leveling and tooth aligning, the operative porcedure can be easier and post-operative occlusal stability can be increased. But it takes time for orthodontic treatment. Recently, we have been using anterior segmental surgery before orthodontic treatment and have reliable results from that. Therefore, we have to consider arch shape, curve of Spee, tooth selection to be extracted for obtaining of post-operative occlusal stability without pre-operative orthodontic treatment.

  • The more civilized, the more incidence had happened in our daily life, particularly in industrial factories, by traffic accident, and in sports, etc. in the dental field. The accidental trauma is usually involved the teeth and surronuding tissues. And the author intended to study of the traumatic injuries of anterior teeth by individual tooth, age, sex, cause of trauma, and how they were treated. The following results were obtained: 1. The patients with traumatic injuries of the anterior teeth were 1.8% among the total dental out-patients. 2. By the age group, the twenties was most frequent in both sexes, and the teenagers next. 3. The predominant causes of traumatic injuries in the dental field were traffic accident (23.4%), fighting (19.2%), falling (18.5%), and sports (7.3%). 4. In the classification of injuries of the teeth, uncomplicated crown fractures (31.2%), subluxations (14.7%), and the complicated crown fractures (13.2%) were presented. 5. By the individual tooth in the traumatic cases, the upper cental incisors were most frequently injuried (49.8%). 6. In the treatments fillings and endodontic treatments (33.2%), splints (13.5%), extractions (11.7%) were main after care method.

Palatal en-masse retraction of segmented maxillary anterior teeth: A finite element study

  • Objective: The aim of this finite element study was to clarify the mechanics of tooth movement in palatal en-masse retraction of segmented maxillary anterior teeth by using anchor screws and lever arms. Methods: A three-dimensional finite element method was used to simulate overall orthodontic tooth movements. The line of action of the force was varied by changing both the lever arm height and anchor screw position. Results: When the line of action of the force passed through the center of resistance (CR), the anterior teeth showed translation. However, when the line of action was not perpendicular to the long axis of the anterior teeth, the anterior teeth moved bodily with an unexpected intrusion even though the force was transmitted horizontally. To move the anterior teeth bodily without intrusion and extrusion, a downward force passing through the CR was necessary. When the line of action of the force passed apical to the CR, the anterior teeth tipped counterclockwise during retraction, and when the line of action of the force passed coronal to the CR, the anterior teeth tipped clockwise during retraction. Conclusions: The movement pattern of the anterior teeth changed depending on the combination of lever arm height and anchor screw position. However, this pattern may be unpredictable in clinical settings because the movement direction is not always equal to the force direction.
