상악 전방견인시 악안면골의 초기반응에 관한 Laser Holography연구 (A LASER HOLOGRAPHIC STUDY ON THE INITIAL REACTION OF MAXILLOFACIAL COMPLEX TO MAXILLARY PROTRACTION)

  • In case of skeletal Class III malocclusion with underdeveloped maxilla, the extraoral orthopedic force for the stimulation of maxillary growth or anterior reposition of the maxilla has been used clinically for the improvement of facial skeletal relationship. The purpose of this investigation was to examine the initial reaction of maxillofacial complex to the maxillary protraction by using extraoral orthopedic force. The dried human skull was used and this investigation was done by means of double exposure holographic interferometry. The protraction forces placed on the canine or the first molar were parallel, $10^{\circ}$ downward, $20^{\circ}$ downward to the occlusal plane. Fringe pattern of each protraction condition was compared and analized. The results were as follows: 1. Each maxillofacial bone displaced saparately. 2. More displacement was shown at the area of the teeth and the alveolar bone. 3. A counterclockwise rotation of the maxilla wa decreased by downward protraction and especially 20 degree downward protraction from the canine showed least rotation. 4. On the zygomatic arch, outward bend was observed and this effect was decreased by downward protraction. 5. On the zygomatic bone, the counter clockwise rotation was increased by the downward protraction. 6. When maxillary expansion was applied at the same time, outward and upward displacement with counterclockwise rotation was observed on the maxilla. 7. The lateral pterygoid plate of sphenoid bone was affected by maxillary protraction.

  • Central odontogenic fibroma is an extremely rare benign tumor, accounting for less than 0.1% of all odontogenic tumors. The most recent literature review that only 69 cases have so far been reported. This tumor has a slow persistent growth that results in painless cortical expansion clinically, and well defined unilocular or multilocular radiolucent lesion. Root resorption of associated teeth is common, and lesions located between the teeth often cause root divergence. There is occurring tendency to female more than male, and occurring in the mandible and in the maxilla with equal frequency. The treatment is surgical excision with no tendency to undergo malignant transformation. We report a case of central odontogenic fibroma in the maxilla of a 27-year male with literatures review.

  • When we establish treatment planning of facial asymmetry, we must predict each asymmetrical element that will be changed upon coronal, axial, sagittal plane. At the visual point, prediction of the change of coronal plane is most important. It is important difference between Rt. and Lt. mandibular angle belonging to posterior coronal plane, as well as anterior coronal plane, such as upper and lower incisor, or midline of chin point. Several methods for control bulk of mandibular angle are additional angle shaving after osteotomy, grinding contact area between proximal and distal segment for decrease the volume, or bone graft for increase the volume. But, at the point of bimaxillary surgery, transverse position of posterior maxilla is an important factor for control it. So, we would report transverse movement of posterior maxilla for decrease asymmetry on the posterior coronal plane of face, that is, asymmetry of mandibular angular portion.

Color variation induced by abutments in the superior anterior maxilla: an in vitro study in the pig gingiva

  • PURPOSE. The aim of this work is to evaluate different types of materials used for making implant abutments, by means of an in vitro study and a review of the literature, in order to identify the indications for a better choice of an implant-supported restoration in the anterior section. MATERIALS AND METHODS. 5 implant abutments were tested in a random order in the superior anterior maxilla of pig gingiva (n = 8): titanium dioxide (Nobel Biocare); zirconium dioxide, Standard BO shade (Nobel Biocare, Kloten, Switzerland); zirconium dioxide, Light BI shade (Nobel Biocare); zirconium dioxide, Intense A 3.5 shade (Nobel Biocare); and aluminium oxide. Each abutment was tested for 2 mm and 3 mm thickness. To determine color variation, VITA Easyshade Advance spectrophotometer (Vita Zahnfabrik, Bad Sackingen, Germany) was used. RESULTS. Results showed that the color variation induced by the abutment would be affected by the abutment material and gingival thickness, when the gingival thickness is 2 mm. All materials except zirconium dioxide (Standard shade) caused a visible change of color. Then, as the thickness of the gingiva increased to 3 mm, the color variation was attenuated in a significant manner and became invisible for all types of abutments, except those made of aluminium oxide. CONCLUSION. Zirconium dioxide is the material causing the lowest color variation at 2 mm and at 3 mm, whereas aluminium oxide causes the highest color variation no matter the thickness.

초기치료 시행 후 치아동요도와 교합력의 변화에 관한 연구 (A STUDY OF THE CHANGES OF THE TOOTH MOBILITY AND MAXIMAL BITE FORCE FOLLOWING INITIAL THERAPY)

  • The purpose of this study was to evaluate the changes of the tooth mobility and maximal bite force over 4 weeks following initial therapy on the periodontal disease. Tooth mobility and maximal bite force due to change of viscoelastic property of periodontium were influenced by inflammation of periodontal tissue. 10 patients with the chronic adult periodontitis participated in this study. Each tooth was divided into anterior areas, premolar areas and molar areas. Tooth mobility was tested using Periotest(Siemens Co. Germany) and maximal bite force was evaluated with MPM-3000(Nihon kohden Co. Japan). Tooth mobility and maximal bite force were recorded at the initial examination, 1, 2, 3 and 4 weeks following initial therapy. All data were analyzed statistically. The obtained results were as follows ; 1. The changes of the tooth mobility following initial therapy were generally decreased in maxilla, showing the significant decrease at 1 and 4 weeks on premolar areas (p<0. 05). 2. The changes of the tooth mobility following initial therapy were generally decreased in mandible, however this changes were not statistically significant. 3. The changes of the maximal bite force following initial therapy in maxilla were significantly increased at 3 and 4 weeks on anterior areas, at 4 weeks on premolar areas (p<0. 05). These were decreased at 1 week on molar areas, but generally increasing with time. 4. The changes of the maximal bite force following initial therapy in mandible were significantly increased at 3 and 4 weeks on anterior areas (p<0. 05, p<0. 01). These were decreased at 1 week on premolar but molar areas, and generally increasing with time. 5. As tooth mobility increased, maximal bite force decreased with significance (p<0. 01), and they had high negative correlation on anterior areas but low negative correlation on premolar and molar areas.

Anterior maxillary defect reconstruction with a staged bilateral rotated palatal graft

  • Purpose: In the anterior maxilla, hard and soft tissue augmentations are sometimes required to meet esthetic and functional demands. In such cases, primary soft tissue closure after bone grafting procedures is indispensable for a successful outcome. This report describes a simple method for soft tissue coverage of a guided bone regeneration (GBR) site using the double-rotated palatal subepithelial connective tissue graft (RPSCTG) technique for a maxillary anterior defect. Methods: We present a 60-year-old man with a defect in the anterior maxilla requiring hard and soft tissue augmentations. The bone graft materials were filled above the alveolar defect and a titanium-reinforced nonresorbable membrane was placed to cover the graft materials. We used the RPSCTG technique to achieve primary soft tissue closure over the graft materials and the barrier membrane. Additional soft tissue augmentation using a contralateral RPSCTG and membrane removal were simultaneously performed 7 weeks after the stage 1 surgery to establish more abundant soft tissue architecture. Results: Flap necrosis occurred after the stage 1 surgery. Signs of infection or suppuration were not observed in the donor or recipient sites after the stage 2 surgery. These procedures enhanced the alveolar ridge volume, increased the amount of keratinized tissue, and improved the esthetic profile for restorative treatment. Conclusions: The use of RPSCTG could assist the soft tissue closure of the GBR sites because it provides sufficient soft tissue thickness, an ample vascular supply, protection of anatomical structures, and patient comfort. The treatment outcome was acceptable, despite membrane exposure, and the RPSCTG allowed for vitalization and harmonization with the recipient tissue.

상악 구개측 중앙부에 매복된 과잉치 (MESIODENS IN THE VAULT OF THE PALATE)

  • 과잉치는 치배 형성기에 정상 치판의 과도한 증식의 결과로 발생하며 산발적일 수도 있고 유전적일 수도 있다 발생 빈도는 $1{\sim}3%$이며, 2:1의 비율로 남아에게 호발한다. 주로 상악 전방부에서 많이 발견되며 매복, 영구치의 위치 이상, 정중부 이개, 낭종, 비강으로의 맹출 등과 같은 다양한 합병증을 나타낼 수도 있다. 상악 전방부에서 발견되어지는 과잉치는 치축방향에 따라 위치가 결정되며 상악 전방부 과잉치의 1/3정도는 역위 매복되어 맹출의 가능성이 없지만, 치낭의 치관 부위가 정상적인 관계로 존재하는 한 과잉치의 이동이 일어날 수 있다. 치아의 이동은 경구개, 소구치 부위, 비강까지도 일어날 수 있다. 또한 전상악 수직고경의 성장으로 시간이 지남에 따라 외과적 접근이 어려워질 수 있다. 이에 본 두 증례에서는 역위 매복된 상악 전방부의 과잉치가 구개 측 중앙부까지 이동된 것을 발견하여 외과적으로 제거하였다.

선양치성낭 (Glandular odontogenic cyst) 3예 (GLANDULAR ODONTOGENIC CYST : REPORT OF THREE CASES)

  • Glandular odontogenic cyst(GOC) is a rare cyst of odontogenic origin, first described in 1988 by Gardner et al. Three glandular odontogenic cysts are presented which were experienced in the Dept. of Oral and Maxillofacial surgery, Yonsei University. The clinical characteristics, radiologic and histopathologic features, and method of treatment are discussed. One occured in the anterior maxilla, others in the mandible body area. One in the anterior maxilla showed swelling and tenderness, others not. All the lesion presented radiographically unilocular radiolucent lesion. Histopathologically, those were lined by nonkeratinizing stratified squamous epithelium of varying thickness showing plaque-like or spherical thickening. Partially, eosinophilic cuboidal cells lined the intraepithelial microcysts. Also, ciliated cuboidal cells and mucinous cells were observed. The cysts were treated by enucleation.

  • This study was undertaken to evaluate the cephalometric changes of the soft tissue and skeletal profile subsequent 10 the rapid palatal expansion in 25 Angle's Class III cases, ranging in age from six to fifteen years, with cross-bite of the anterior teeth, underdevelopment of maxilla and facial disharmony Following results were obtained: 1. ANS moved downward, Point A presented forward & downward movement increasing SNA and Point B presented backward & downward movement decreasing SNB. 2. Mandible was rotated to backward & forward and maxilla moved forward & downward with the bite opening and improvement of anterior teeth cross-bite. 3. Soft tissue on mandible was rotated to backward & forward following hard tissue changes causing the decrease of facial convexity angle and backward & downward rotation of Point B', Pog'. 4. Response of the upper lip was more significant in downward than forward direction, and correlated with the upper central incisor and mandible rotation. 5. Response of the lower lip was more significant in downward than backward direction, and correlated with the mandible rotation. 6. There was a rather high degree of correlation between skeletal profile and soft-tissue profile, 1 : LS, $\bar{1}$:Pog', Pog:LS, Pog:LI, Pog:Pog' in horizontal measurements and $\bar{1}$:Pog', Pog:LI, Pog:Pog' in vertical measurements.

  • The purpose of this report is to review vertical dysplasia such as openbite or deep bite in skeletal Class III malocclusion and their treatment modality and to present two cases treated with MEAW. The results obtained were as follows A. Open bite case 1. The treatment time was 3 year 8 months. 2. Upper and lower incisors showed extrusion and especially lower anterior alveolar process showed remodelling. 3. The mesially inclined upper and lower molars were uprighted and especially lower first molars showed extrusion that means remodelling of alveolar bone. 4. Normal overbite and overjet were established. 5. Mandible showed slight clockwise rotation. 6. Maxilla showed slight downward bending of ANS part. 7. Upper lip showed downward drop and lower lip showed retraction and touch between upper and lower lip was established. 8. Tongue posture of post-treatment was more raised than pretreatment. B. Deep bite case 1. The treatment time was 1 year 8 months. 2. Upper incisors showed intrusion and labioversion and lower incisors showed slight intrusion and linguoversion. 3. The lower molars showed distal uprighting and intrusion and upper molars showed mesial movement and extrusion. 4. Normal overbite and overjet were established. 5. Maxilla did not show downward movement. 6. Mandible showed slight clockwise rotation. 7. Lower lip showed retraction and downward drop and upper lip showed downward drop.
